Abstract

Objectives: Cardiothoracic (CT) surgeons with National Institutes of Health (NIH) R01 funding face a highly competitive renewal process. The factors that contribute to successful grant renewal for CT surgeons remain poorly defined. We hypothesized that renewed basic science grants are associated with high research output and scholarly impact during the preceding award cycle. Methods: Using a database of academic CT surgeons (n = 992) at accredited training institutions in 2018, we identified basic science R01 grants awarded to CT surgeon principal investigators since 1985. Data for each award were obtained from publicly available online sources. Scholarly impact was evaluated using the NIH-validated relative citation ratio (RCR), defined as an article's citation rate divided by that of R01-funded publications in the same field. Continuous data are presented as medians and analyzed using the Mann–Whitney test. Results: We identified 102 basic science R01 award cycles, including 33 that were renewed (32.4%). Renewed and nonrenewed awards had a similar start year and funding period. Principal investigators of renewed versus nonrenewed awards were similar in surgical subspecialty, research training, attending experience, academic rank, and previous NIH funding. Renewed awards produced more publications per year over the funding cycle (3.4 vs 1.5; P = .0010) and exhibited a greater median RCR during the funding cycle (0.84 vs 0.66; P = .0183). Conclusions: CT surgery basic science R01 grants are associated with high research output and scholarly impact. At the 50th percentile among renewed grants, CT surgeons published 3.4 funded manuscripts per year with a median RCR of 0.84 during the previous award cycle.

37. National Institutes of Health R01 Grant Funding Is Associated With Enhanced Research Productivity and Career Advancement Among Academic Cardiothoracic Surgeons.

Abstract

National Institutes of Health (NIH) funding has declined among cardiothoracic surgeons. R01 grants are a well-known mechanism to support high-impact research, and we sought to clarify the association between NIH funding and academic achievement. We hypothesized that cardiothoracic surgeons who acquired R01 funding exhibit greater research output and faster career advancement. All cardiothoracic surgeons (n = 992) working at accredited United States cardiothoracic surgery training hospitals in 2018 were included. Institutional webpages, Scopus, and Grantome were utilized to collect publicly-available data regarding each surgeon's training and career history, research publications, and NIH funding. Seventy-eight (7.9%) surgeons obtained R01 funding as a principal investigator while 914 (92.1%) did not. R01-funded surgeons started their attending careers earlier (1998 vs 2005, P < 0.0001) and were more likely to have pursued dedicated research training (P < 0.0001). R01-funded surgeons authored 5.3 publications/year before their first R01 grant, 9.3 during the grant period, and 8.6 after the grant expired, all of which were greater than the publication rate of non-R01-funded surgeons at comparable career timepoints (2.0-3.0 publications/year, P < 0.0001). Among time-matched surgeons who completed medical school in 1998 or earlier (n = 73 R01-funded vs n = 602 non-funded), R01-funded surgeons have published more total publications (178.0 vs 56.5 papers, P < 0.0001) and exhibit a greater H-index (41.0 vs 19.0, P < 0.0001). These R01-funded surgeons have also advanced to higher academic ranks (P < 0.0001) and are more likely to be chiefs of their departments or divisions (42.5% vs 25.7%, P = 0.0035). AbstractIntroductionThere is growing concern about the declining physician-scientist workforce. NIH recently provided a national dashboard describing the biomedical research workforce, but local strategies are needed.MethodsWe used curated local and national data to develop a workforce dashboard.ResultsMany trends at Oregon Health & Science University (OHSU) were similar to those nationally, such as the increasing percentage of Research Project Grant (RPG)-holding PhDs and the aging RPG population, but differences were also apparent. At OHSU, nearly ¾ of physician-scientist RPGs hold MD-only, compared with nationally, where nearly half are MD/PhD. OHSU also lags in the percentage of RPGs held by women physician-scientists.ConclusionsOur analysis also permitted us to gain a more complete picture of research funding that has been done nationally. We used these data to develop a dashboard that allows our institution to develop policies to increase the numbers of physician-scientists. The data generation approaches and dashboard are likely to be useful at other institutions, as well.

Abstract

Introduction: There is growing concern about the declining physician-scientist workforce. NIH recently provided a national dashboard describing the biomedical research workforce, but local strategies are needed. Methods: We used curated local and national data to develop a workforce dashboard. Results: Many trends at Oregon Health & Science University (OHSU) were similar to those nationally, such as the increasing percentage of Research Project Grant (RPG)-holding PhDs and the aging RPG population, but differences were also apparent. At OHSU, nearly ¾ of physician-scientist RPGs hold MD-only, compared with nationally, where nearly half are MD/PhD. OHSU also lags in the percentage of RPGs held by women physician-scientists. Conclusions: Our analysis also permitted us to gain a more complete picture of research funding that has been done nationally. We used these data to develop a dashboard that allows our institution to develop policies to increase the numbers of physician-scientists. Abstract

This study aims to improve our understanding of handgun carrying among adolescents living in rural communities and provide a strong foundation for effective interventions that prevent firearm injury and promote safety in this population. We will collect new data on handgun carrying by conducting focus groups among adolescents currently living in rural communities, use existing data on handgun carrying during adolescence to young adulthood, and collect new data on handgun carrying with a new wave of survey data collection among these young adults who lived in rural communities during their adolescence.

Abstract

Objective: To determine whether there were gender differences in likelihood of receiving a first National Institutes of Health (NIH) R01 award among 5445 instructors and assistant professors at Harvard Medical School (HMS). Materials and Methods: Data on R01 award principal investigators were obtained from NIH ExPORTER and linked with faculty data. Using Cox proportional hazard regression, we examined the association of gender with receipt of first R01 award between 2008 and 2015 accounting for demographics, research productivity metrics, and professional characteristics. Results: Compared to males, females had fewer publications, lower h-index, smaller coauthor networks and were less likely to be assistant professors ( p < 0.0001). Four hundred and thirteen of 5445 faculty (7.6%) received their first R01 award during the study period. There was no gender difference in receipt of R01 awards in age-adjusted (hazard ratio [HR]: 0.87, 95% confidence interval [CI]: 0.70-1.08) or multivariable-adjusted models (HR: 1.07, 95% CI: 0.86-1.34). Compared to white males, there was a nonsignificant 10%, 18%, and 30% lower rate of R01 receipt among white, Asian or Pacific Islander, and underrepresented minority females, respectively. These differences were eliminated in the multivariable-adjusted model. Network reach, age, HMS start year, h-index, academic rank, previous K award, terminal degree, and HMS training were all significant predictors of receiving an R01 award. Conclusions: A relatively small proportion of HMS junior faculty obtained their first NIH R01 award during the study period. There was no significant gender difference in likelihood of award. Abstract

The National Institutes of Health (NIH) is the largest source of funding for biomedical research in the world. Funding decisions are made largely based on the outcome of a peer review process that is intended to provide a fair, equitable, timely, and unbiased review of the quality, scientific merit, and potential impact of the research. There have been concerns about the criteria reviewers are using, and recent changes in review procedures at the NIH now make it possible to conduct an analysis of how reviewers evaluate applications for funding. This study examined the criteria and overall impact scores recorded by assigned reviewers for R01 grant applications. The results suggest that all the scored review criteria, including innovation, are related to the overall impact score. Further, good scores are necessary on all five scored review criteria, not just the score for research methodology, in order to achieve a good overall impact score. [ABSTRACT FROM AUTHOR]

Introduction As the world's largest funding source for biomedical research, the National Institutes of Health (NIH) supports physician-scientists with a discipline-specific R01 grant. Recently, scholarly activity disparities regarding investigator degree and gender have been highlighted in the medical literature among allopathic and osteopathic investigators of various medical backgrounds. We aimed to assess trends in internal medicine NIH R01 grants over the past decade. Methodology Internal medicine R01 funding was retrospectively obtained from a centralized online NIH database encompassing 2008 through 2017. Principal investigators (PIs) were then categorized by gender and academic degree(s). Two-way analysis of variance was used to analyze NIH grant funding trends over the time period studied. Results A total of 5,089 NIH R01s were awarded to internal medicine PIs, with an average value per grant of $469,270. Awardees were predominantly male (71.5%, 3,639/5,089). Most awards were issued to PIs with an MD degree (62.4%, 3,173/5,089), followed by PhD degree (36.3%, 1,845/5,089). DOs accounted for five awards over the time period studied (0.15%). MDs were awarded higher funding than PhDs ($466,494 and $421,576, p < 0.001), and females were awarded higher amounts than males ($462,771 and $444,868, p < 0.001). Investigators who held a second degree received more funding than PIs with a single degree ($476,693 and $439,693, p < 0.001). Conclusion In the decade under investigation, both gender and degree disparities existed within NIH R01 funding for PIs in the field of internal medicine, and osteopathic representation accounted for a paucity of R01 funding.

45. Sustaining Independent Careers in Vision Research: Demographics and Success in Second R01 Attainment Among Clinician–Scientists from 1985 to 2019

Abstract

Purpose To evaluate the success of ophthalmology and optometry clinician-scientists in obtaining a second R01 (renewal or new) and factors associated with this success, including gender, clinical specialty, degree, institution, and bench versus non-bench research. Methods First-time National Eye Institute (NEI) R01 awardee data from 1985 to 2014 (N = 234) were analyzed to calculate second R01 success rates. Only R01 awards to ophthalmology or optometry clinician-scientists were included. Demographic data were obtained from clinicians with first-time NEI R01 funding spanning from 1962 to 2019 (N = 386). We obtained information regarding time span of the first R01, year of second R01, institution, and project title on the National Institutes of Health (NIH) Research Portfolio Online Reporting Tool, Expenditures and Results (RePORTER) database, and additional measures of gender, clinical specialty, and degree by performing Internet searches. Results Overall, from 1985 to 2014, 62.8% of ophthalmology or optometry clinician-scientists were awarded a second R01; at 5 years after receipt of the first R01 (the typical length of an R01), only 3.9% received their second R01. None of the factors examined (temporal cohort, gender, clinical specialty, degree, institution, or bench vs. non-bench research) was significantly associated with successful attainment of a second R01. Conclusions We found an overall success rate of 62.8% for receiving a second R01, but 5 years after the first R01 an attainment rate for a second R01 of only ∼4%. Translational Relevance Our study provides insight on significant leaks in the clinician-scientist pipeline and raises questions of how stakeholders should support this important group of individuals at the intersection of clinical medicine and biomedical research.

Introduction: Limited data currently exist regarding the demographics of principal investigators (PIs) in obstetrics and gynecology (OBGYN), who have received an R01 grant from the National Institutes of Health (NIH). Objective: This study investigated funding differences among gender and advanced degree for PIs in the disciplines of OBGYN. Methods: Retrospective data were collected from the NIH Research Portfolio Online Reporting Tools Expenditures and Results (RePORTER) tool to identify OBGYN PIs who received an R01 grant between 2008 and 2017. Results: Between 2008 and 2017, the NIH awarded 263 R01 grants totaling $113,326,883 in funding to investigators in OBGYN. Male PIs and PIs holding a non-medical degree were awarded the majority of R01 grants (52.47% and 55.51%, respectively). Zero osteopathic (Doctor of Osteopathic Medicine [DO]) physicians were awarded an R01 grant in OBGYN during this time period. Females were awarded larger dollar amounts than males ($449,556 vs $414,003, p=0.04). Abstract

This paper represents an attempt to reconcile some general intuitions provided by Daron Acemoglu and James A. Robinson in the book “Why Nations Fail” with the case of the deep regional disparities in the economic performances observed within the “Western” European Union during the period 2001-2015. By adopting an approach to growth analysis based on binary response models, this paper quantifies the extent to which the quality of government institutions has shaped regional economic performances in the European Union throughout the period comprising the Great Recession. Empirical results show that: 1) The higher is the quality of institutions, the higher is the probability that a region with high income per capita will grow above the levels of the European Union as a whole. 2) The higher is the quality of institutions, the lower is the probability that a low-income region will grow below the levels of European Union as a whole. 3) The higher is the quality of institutions, the higher (lower) is the probability that any region, regardless of its income per capita, will outperform (underperform) the European Union as a whole. 4) The higher is the quality of Institutions, the lower is the probability that a region will “fail” to grow.

Abstract

Introduction Despite a call for increased research by family-medicine physicians, there is no data on the demographics of those awarded a National Institutes of Health (NIH) R01 grant. Objective The purpose of the study was to assess recent NIH R01 funding trends over the last decade in family medicine. Methods A retrospective analysis of NIH R01 grant funding in family medicine was conducted by extracting demographic data from the NIH’s Research Portfolio Online Reporting Tools Expenditures and Results (RePORTER) database from 2008 through 2017. Demographics were reported as percentages, and comparisons of the groups were performed by the t-test. Results From 2008 to 2017, the NIH awarded 139 R01 grants to principal investigators (PI) in family medicine. Males comprised 51.80% of all awardees, and those holding a doctorate of medicine (MD) made up 43.88% of the awardees. No one holding a Doctorate of Osteopathic Medicine (DO) degree received an NIH R01 grant during the timeframe studied. A total of 81.97% of all MDs held a dual degree. When gender and degree were considered, no statistical difference was observed for the total amount of dollars awarded. Conclusion For the years studied, a disparity related to medical degrees was noted for those family-medicine physicians who received an NIH R01 grant. However, no gender disparity was observed.

Abstract

In this paper, we analyzed the spatial dependence of the efficiency of public-sector health services in Minas Gerais, taking as reference its 853 municipalities in 2010. We computed efficiency scores and rakings by employing the method of data envelopment analysis meta-frontier. Spatial analysis was carried through with exploratory data techniques. Our results highlighted problems with resource allocation, especially in smaller cities. In addition, we found clusters of high and low-efficiency scores in the state. When comparing groups, it was verified that municipalities classified as “high-high” had higher socioeconomic indicators than those in the “low-low” regions. Therefore, a proper understanding of local realities could represent a step toward the optimization of resource allocation in the public health system. Esta pesquisa analisa a relação espacial da eficiência municipal em saúde em Minas Gerais, tomando como referência os 853 municípios do estado em 2010. Para criar os indicadores de eficiência, utiliza-se a técnica de análise envoltória de dados meta-frontier. O estudo espacial é realizado por meio da análise exploratória de dados espaciais. Os principais resultados destacam problemas na aplicação dos recursos e elevado montante relativo de gastos, sobretudo nos pequenos municípios. Pela análise espacial, encontram-se aglomerados de alta e baixa eficiência no estado. Ao se comparar os agrupamentos, verifica-se que os municípios “alto-alto” apresentam indicadores socioeconômicos mais elevados em relação aos “baixo-baixo”. Estudar melhor a realidade dessas regiões poderia ser o primeiro passo para otimizar os recursos aplicados na saúde.
