Few data are available on clinically diagnosed vertebral fracture. Information about osteoporotic vertebral fracture has mainly been obtained via inferences from epidemiological studies of vertebral deformity. We evaluated the characteristics of patients with osteoporotic vertebral fracture diagnosed in a rheumatology department over a 10 year period.Patients with back pain and vertebral fracture diagnosed between January 1990 and December 1999 were recruited from our data base. Patients with high energy trauma, malignancies, and metabolic bone diseases other than osteoporosis were excluded. These variables were analyzed: sex, age at diagnosis, type of osteoporosis (primary vs secondary), number of fractures at diagnosis (single vs multiple), and percentage of admissions and length of stay.Of the 669 patients, 534 (80%) were women and 135 (20%) were men. Age at diagnosis ranged from 30 to 91 yrs, mean 67.1 +/- 9.1. Secondary osteoporosis was diagnosed in 177 (26%) patients and the frequency was significantly higher in men than women (55% vs 19%; p0.001); the most common associations for secondary osteoporosis were oral corticosteroids, chronic obstructive airway disease, and rheumatoid arthritis. At diagnosis, half of the patients presented with multiple fractures. One hundred twenty (18%) patients were admitted; length of stay ranged from 5 to 56 days, mean 15.9 +/- 7.7. The frequency of admissions was higher in men than women (27% vs 16%; p0.001), higher in patients with secondary osteoporosis than in those with primary osteoporosis (33% vs 12%; p0.001), and higher in patients with multiple fractures than in those with single fractures (27% vs 8%; p0.001).Characteristics of patients recruited from a clinical setting differ significantly from those of subjects included in the epidemiological studies. In a rheumatology practice, frequency of secondary osteoporosis, mainly associated with corticosteroid treatment, is notably high. Admission is by no means a rare event.

Objective - Vertebral deformity is associated with back pain and disability. The aim of tiffs analysis was to determine whether location within the spine influences the strength of association between vertebral deformity, back pain and disability. Methods - Men and women aged 50 years and over were recruited from population registers in 30 European centres. Subjects were invited for an interviewer administered questionnaire, and for lateral spinal radiographs. The questionnaire included questions about back pain, general health and functional ability. The spinal radiographs were evaluated morphometrically and vertebral deformity defined according to the McGloskey-Kanis method. Results - 756 (11.7%) men and 885 (11.8%) women had evidence of one or more vertebral deformities. Among women with a single deformity, after adjusting for age and centre, those with a lumbar deformity were more likely than those with a thoracic deformity to report back pain, both currently (OR = 1.4; 95% CI 1.0, 2.0) and in the past year (OR = 1.5; 95% CI 1.0, 2.3). No association was observed in men. Among women with two deformities, those with adjacent deformities were more likely than those with non-adjacent deformities to report poor general health (OR = 2.2; 95%CI 0.9, 5.6), impaired functional ability (OR = 1.9; 95%CI 0.8, 4.7) and current back pain (OR = 2.1; 95%CI 0.9, 4.9), though none of these associations were statistically significant. By contrast, among men, non-adjacent deformities were associated with impaired functional ability compared with those with adjacent deformities. Conclusions - Location within the spine influences the strength of association between self reported health factors and vertebral deformity.

Prevotella bivia is a gram-negative anaerobic bacteria traditionally classified in the genus Bacteroides, and usually reported in obstetric and gynecologic infections. To date, there has been only one description of infectious arthritis secondary to this germ. We report the first case of septic arthritis due to Prevotella bivia in a patient with severe, long-lasting rheumatoid arthritis (RA) treated with low doses of corticosteroids. RA is a well-known predisposing factor to septic arthritis, whose causes are multifactorial (general immunosuppression induced by the disease and its therapy, presence of prosthetic joints, etc.). However, infectious arthritis due to anaerobic bacteria is rarely observed. In general, clinical presentation varies widely: insidious onset, apyrexia, and lack of inflammatory signs or systemic disturbance are frequent features. Joint infection is generally secondary to hematogenous spread, the Bacteroides fragilis group being the most commonly isolated pathogens. Early diagnosis and prompt treatment, with drainage and debridement of the affected joint and specific antimicrobial treatment with Metronidazole are essential for a successful outcome.

A Behçet's disease patient developed sudden cochlear hearing loss during a flare of her disease. Prednisone and cyclosporin therapy was ineffective, probably because it was initiated late. Sensorineural hearing loss is a rare but underrecognized complication of various forms of vasculitis such as Wegener's granulomatosis, polyarteritis nodosa, giant cell arteritis and Behçet's disease. Its importance lies in the need for an early diagnosis, since prompt treatment with steroids and immunosuppressive agents may lead to restoration of hearing.

To assess the influence of corticosteroids in the mineral content of patients with giant cell arteritis (GCA), 56 patients, 28 with polymyalgia rheumatica (PMR) and 28 with temporal arteritis (TA) were studied.The bone mineral density (BMD) in the lumbar spine and the femoral neck was measured by dual photon X-ray absorptiometry. A control group (48 people) comparable in age and sex was also evaluated.Compared with the controls, the patients with GCA had lower values of BMD in the femoral neck (men: p0.03; women: p0.001). In the lumbar spine differences were significant in women (p0.05) but not in men (p0.1). Multiple regression analysis showed that the BMD at L2-L4 in men correlated with height and weight and was inversely related to the cumulative dose of corticosteroids and the duration of treatment. The BMD at L2-L4 in women correlated with height and weight, but not with corticosteroids. Multiple regression analysis in men showed that age and the total dose of corticosteroids were significant independent predictors of femoral BMD. In women BMD in the femoral neck was correlated with age and weight, but not with corticosteroid treatment.The total dose and duration of corticosteroid treatment have been shown to be determinant factors of bone mass in patients with GCA who received corticosteroids.

To assess limitation of joint mobility in patients with type I diabetes mellitus and to evaluate its relation with retinopathy, joint mobility was prospectively assessed in 96 diabetic patients and 68 healthy controls.Joint mobility was explored with the praying maneuver and the measurement in mobility degrees of the third and fifth metacarpophalangeal joints, wrists and elbows. The degree of metabolic control was assessed with the mean glycosylated hemoglobin in the last two years. Retinopathy was investigated with direct funduscopy. The results were statistically evaluated with chi-square and Student's t tests and the linear coefficient.A reduced joint mobility was found in 41 diabetics and 5 controls (p less than 0.0001). The reduction in joint mobility was related with the patients' age but not with the degree of metabolic control. 85% of diabetics with joint involvement had retinopathy of some degree.A limited joint mobility is a common complication of type I diabetes mellitus. The demonstration of this abnormality in diabetic patients might represent a first and simple marker of microangiopathy.

Se estudia una serie de 82 prótesis totales de cadera cementadas tipo Muller implantadas en enfermos afectos de coxitis reumáticas, fundamentalmente artritis reumatoide. Los casos tienen un seguimiento medio de cuatro años y una edad media de cincuenta y un años; la mitad de ellos tenían afectados entre siete y 10 articulaciones y en la cadera el 28 por 100 presentaban protrusión acetabular. En el postoperatorio consta un 2,5 por 100 de complicación séptica y 2,5 por 100 de tromboembolismos. El análisis clínico de los resultados se basa en el dolor, la movilidad y la marcha según el baremo de Merle d'Aubigne y en la capacidad funcional segiin los criterios de STEINBROCKER. En grupo aparte se ha estudiado el resultado en los casos con un seguimiento mínimo de cinco años, en los que tenían dolor importante y en los que presentaban protrusión. En conjunto obtienen resultado muy bueno el 52,5 por 100 y bueno o aceptable el 30 por 100. El aflojamiento aumenta con el paso del tiempo y es del 18 por 100 en la serie global, pero alcanza al 30 por 100 en los casos con seguimiento de cinco años o más y las profusiones. El resultado funcional depende de las varias intervenciones practicadas en las extremidades y con el programa quirúrgico complejo conseguimos que algo más de la mitad de los enfermos recuperen una autonomía para la vida diaria fuera de su casa.

Little information is available on the occurrence of generalized osteopenia in psoriatic arthritis. The only two published studies of bone mass in psoriatic arthritis produced conflicting results.We compared bone mineral density measured at the lumbar spine and femoral neck using dual-energy X-ray absorptiometry in 52 patients with active peripheral psoriatic arthritis and in 52 controls. The psoriatic arthritis group included 19 males, 14 premenopausal women, and 19 post-menopausal women. Controls were matched to the patients on sex, age, and menopausal status.In the overall study population no significant differences were found between psoriatic arthritis patients and controls. Postmenopausal psoriatic arthritis patients had a lower femoral neck bone mineral density than the relevant subgroup of controls. No significant differences in lumbar spine bone mineral density were found in the analyses of the male, premenopausal female, and postmenopausal female subgroups. Neither was femoral neck density significantly different between male or premenopausal female psoriatic arthritis patients and controls.These results suggest that peripheral psoriatic arthritis is not associated with significant generalized bone loss.

Lipoma arborescens is a rare intraarticular lesion characterized by diffuse replacement of the subsynovial tissue by mature fat cells, producing villous transformation of the synovium. The etiology of this benign condition is unknown. The most typical site of involvement is the knee, most notably at the suprapatellar pouch, although other joints can be affected. Symptoms consist of gradual joint swelling, variable pain, motion range restriction, and intermittent joint effusions or bleeding. We report a case of lipoma arborescens and discuss the clinical features, diagnosis, and treatment of this disorder based on a literature review. Although it is rare, lipoma arborescens should be included in the differential diagnosis of patients with chronic joint swelling or hemarthrosis.
