Cutaneous leishmaniasis in Venezuela has been associated with different Leishmania species and the visceral form is frequently related to L. chagasi. However, in this study we have characterized as L. colombiensis by molecular techniques some isolates of Leishmania from patients presenting either cutaneous or visceral leishmaniasis. Three different molecular markers (two DNA and one protein) were used to investigate the diversity among the L. colombiensis isolates. The analysis revealed the isolates to be similar although distinct from the reference strain of L. colombiensis. Slight variation among the isolates could not be correlated with any epidemiological features of the samples however the visceral isolates were observed to cluster on a phenetic tree derived from one DNA marker. The results confirm previous findings that L. colombiensis can cause both the cutaneous and visceral forms of the disease and that there exists at least two distinct etiological agents of American visceral leishmaniasis (AVL). The report also shows the necessity of careful differentiation of leishmanial isolates in any clinical or epidemiological study of human leishmaniasis.

Background. While studying cutaneous leishmaniasis in the central part of western Venezuela, we found four cases of disseminated American cutaneous leishmaniasis, three from the Lara State and one from Portuguese State. Methods. A clinical history was taken for each of these patients, followed by microscopic examination of the Giemsa-stained smears from their cutaneous lesions and by a Montenegro skin test. Serum from a skin lesion were grown in Novy-MacNeal-Nicolle medium (nnn). Hamsters were inoculated with suspension of tissues taken from the patient's lesions. Biopsies were taken for histopathologic examination. Isolates from cultures on nnn medium and from hamsters were subcultured in Schneider's medium for parasite identification, using molecular techniques. Treatment with injections of N-methyl glucamine antimonate, 25 mg/kg/day was precribed for each patient for 20 consecutive days and, after a week of rest, a second course of injections was administered. Results. Patients had disseminated papular, ulcerous, nodular, and ulceronodular lesions on the skin. Smears of the skin lesions from all of the patients showed abundant amastigotes within histiocytes or free in the tissues. The skin test was negative in two patients. On histopathologic examination of skin lesions, mainly numerous vacuolated histiocytes filled with amastigotes were observed. Isolates from all the patients were identified as Leishmania venezuelensis. One of the patients healed after treatment with N-methyl glucamine antimonate. The others were resistant to this therapy. Conclusions. Diffuse cutaneous leishmaniasis can be caused also by Leishmania venezuelensis. Patients with nodular lesions who presented a negative Montenegro skin test were more resistant to treatment with specific pentavalent antimonials.

Se realizó un despistaje serológico y recolección de vectores en cuatro comunidades rurales del municipio Andrés Eloy Blanco, Estado Lara, Venezuela. La muestra fue escogida en forma sistemática y aleatoria basada en conglomerados familiares. Se muestrearon 869 habitantes para determinar anticuerpos anti-Trypanosoma cruzi y anti-Leishmania sp. por inmunofluorescencia indirecta, aceptando como positivo diluciones > a 1:32 para anticuerpos anti-T. cruzi no reactivos para antígenos de Leishmania sp., obteniendo una frecuencia de anticuerpos en la muestra de 6,9% (n = 60); de los cuales 46,66% son femeninos, 53,33% masculinos y 60% mayores de 40 años. Se observó que 5 (8,33%) de los seropositivos eran menores de 10 años y 10 (16,66%) menores de 20 años. Rhodnius prolixus y Panstrongylus geniculatus fueron los triatominos capturados, con índice de infestación de 1,9 y 10,54%, índice de colonización, del 0 y 18,18% en las viviendas infestadas e índice de infección a T. cruzi del 20 y 5,07%, respectivamente. Los resultados sugieren que existe una transmisión activa de la enfermedad de Chagas en el Municipio Andrés Eloy Blanco en las últimas dos décadas y que P. geniculatus está substituyendo a R. prolixus como vector de la enfermedad de Chagas. RESUMENSe realizo un estudio seroepidemiologico en281 individuos con edades comprendidas entre 3y 75 anos en la localidad rural de Caballito,municipio Simon Planas, estado Lara, Venezuela.Ciento treinta y uno fueron varones (46,6%) y150 mujeres (53,4%). Se tomaron muestras desangre por puncion venosa a cada uno de lospacientes y se separo el suero. Se determinocuantitativamente la presencia de anticuerposespecificos circulantes por la tecnica deinmunofluorescencia indirecta, utilizando comoantigenos epimastigotes de una cepa local de Trypanosoma cruzi y, como control, promastigotesde Leishmania braziliensis , para detectarreacciones cruzadas. Se utilizo como conjugadoanti globulina humana IgG marcada confluoresceina. La reaccion se considero positivacuando presentaba titulos de 1: 8 o mas. Sesentay ocho personas (24,2%) tuvieron anticuerposcirculantes a T. cruzi y 34 (12%) a L. braziliensis .La mayor parte de los casos positivos fueronencontrados en personas mayores de 20 anos,pero los mayores resultados fueron encontradosen el grupo entre 6 y 10 anos. Between 1975 and 1987, epidemiological studies were carried out in several rural and urban communities in the central part of western Venezuela, especially in the state of Lara. 115 positive cultures were obtained from human cases and identified by their reactivity patterns to a cross-panel of specific monoclonal antibodies using a radioimmune binding assay; 53 were Leishmania venezuelensis and 62 were L. braziliensis. Most of these stocks were also characterized by isoenzyme electrophoresis, which confirmed the identification of the L. venezuelensis isolates. The enzyme electrophoretic profiles of the L. braziliensis isolates, however, revealed two populations with distinct electromorphs, one related to the World Health Organization L. braziliensis reference strain while the other population appeared to be a hybrid between L. braziliensis and L. guyanensis. L. braziliensis variants showed the widest geographical distribution, and were found in 7 states: Districto Federal (Caracas); Lara (Barquisimeto, Crespo, Iribarren, Jimenez, Moran, Palavecino, Torres, Urdaneta); Nueva Esparta (Margarita); Portuguesa (Las Cruces, Rio Amarillo); Trujillo (Cuicas); Yaracuy (Agua Fria, Cambural, Guaremal); and Zulia (Zipa-Yare). L. venezuelensis was found in the following endemic regions: Lara (Barquisimeto, Iribarren, Jimenez, Moran); Merida (Zea); and Yaracuy (Campos Elias), showing that this parasite has a much wider geographical distribution than was initially recognized and that both these species can occur simultaneously within the same endemic region. Five isolates of L. braziliensis were made from infected donkeys (Equus asinus) in Urdaneta, Lara State, suggesting a possible domestic reservoir of L. braziliensis.

A survey of the vaginal cavity of 170 pregnant women was undertaken to ascertain the presence ofCandida in the genital organs of these patients. Most of them had vaginitis and/or cervicitis. Cultures were made from samples swabbed from the vaginal wall and the cervix. The genusCandida was isolated from 46.5 % of the patients;C. albicans, 37.1 %;C. krusei, 3.5 %;C. parapsilosis, 2.9 %;C. tropicalis, 1.8 %; andC. guilliermondii, 1.2 %.
