Abstract

Background. The aim of this study was to identify trends in total, deceased donor (DD) and living donor (LD) kidney transplantation (KT) rates in European countries. Methods. The European Renal Association (ERA) Registry and the Global Observatory on Donation and Transplantation (GODT) databases were used to obtain the number of KTs in individual European countries between 2010 and 2018. General population counts were obtained from Eurostat or the national bureaus of statistics. The KT rate per million population (p.m.p.) and the average annual percentage change (APC) were calculated. Results. The total KT rate in the 40 participating countries increased with 1.9% annually [95% confidence interval (CI) 1.5, 2.2] from 29.6 p.m.p. in 2010 to 34.7 p.m.p. in 2018, reflecting an increase of 3.4 p.m.p. in the DD-KT rate (from 21.6 p.m.p. to 25.0 p.m.p.; APC 1.9%; 95% CI 1.3, 2.4) and of 1.5 p.m.p. in the LD-KT rate (from 8.1 p.m.p. to 9.6 p.m.p.; APC 1.6%; 95% CI 1.0, 2.3). The trends in KT rate varied widely across European countries. An East-West gradient was observed for DD-KT rate, with Western European countries performing more KTs. In addition, most countries performed fewer LD-KTs. In 2018, Spain had the highest DD-KT rate (64.6 p.m.p.) and Turkey the highest LD-KT rate (37.0 p.m.p.). Conclusions. The total KT rate increased due to a rise in the KT rate from DDs and to a lesser extent from LDs, with large differences between individual European countries.

Abstract

This section of the International Society for Heart and Lung Transplantation (ISHLT) Registry Report covers the pediatric lung and heart-lung transplantation experience from January 1986 through June 2011. During this time, 1,770 pediatric lung transplant and 660 pediatric heart-lung transplant procedures were reported to the Registry. In 2010, the last complete year included in this 15th official Pediatric Lung and Heart-Lung Transplantation Report, 126 pediatric lung transplant procedures were reported, compared with 97 procedures in 2005. The figures, however, contain only the pediatric lung transplant procedures that were reported to the ISHLT Registry and, thus, may not reflect numbers of pediatric lung procedures undertaken worldwide. This years Registry report incorporates a summary of data analyses performed by the Registry this year that is supplemented by further analyses and additional figures that are available on the ISHLT Registrys Web site (www.ishlt.org/ registries). Abstract

This 29th adult transplant report is based on data submitted by 394 transplant centers worldwide. In excess of 104,000 heart transplants have been registered in the database. Summary data are provided for the entire cohort of patients, whereas a number of additional analyses focus on cohorts of patients transplanted more recently. Detailed data analyses can be viewed on the slide set of the registry of the International Society for Heart and Lung Transplantation (ISHLT), which is available online (www.ishlt.org/registries). The report is divided into several sections. Baseline donor, recipient and medical center demographics are reviewed first. The second part focuses on immunosuppressive therapies and acute allograft rejection. Abstract

Context-All organ exchange organizations are challenged to maximize the utilization rate of all donors. Objective-To investigate the benefit of a rescue allocation policy and to study the impact of donor factors on the risk of kidney discard. Design and Setting-All 4057 donors with kidneys offered for allocation to Eurotransplant between 2006 and 2007 were included. Allocation was patientoriented, based on a point-score system including recipient and donor factors. If an organ offer was rejected 5 times for medical reasons, allocation was switched to rescue allocation (ie, the organ was then offered in a center-oriented way). A logistic regression model was built to test whether donor factors were predictors of rescue allocation or kidney discard. Results-Rescue allocation was used for 665 donors (16.4%); within this group, transplant rate was 54.3%, resulting in a donor discard rate of 304 donors (7.5% of total study group). The multivariate model showed that rescue allocation was used significantly more for kidneys from child donors and donors with a high creatinine level. Moreover, testing positive for hepatitis B surface antigen or antibody to hepatitis C virus was associated with an increased probability of rescue allocation. Kidney discard was significantly associated with donation after cardiac death, donor age, serum creatinine level, history of diabetes, and history of hepatitis. Conclusions-Rescue allocation is effective in lowering donor discard rates. Even with rescue allocation, several donor factors were significantly associated with a higher discard rate. Use of liberal donor criteria and a rescue allocation policy can reduce kidney discards and thus shorten the waiting list for kidney transplantation. [ABSTRACT FROM AUTHOR]

This tenth official pediatric report of the International Society for Heart and Lung Transplantation (ISHLT) covers the international pediatric lung and heart–lung transplantation experience from 1982 to 2006. For the first time this year, Registry data are analyzed by geographic region in addition to the usual aggregate analyses. Abstract

Background: Simultaneous pancreas kidney transplantation (SPK), pancreas transplantation alone (PTA) or pancreas transplantation after kidney (PAK) are the only curative treatment options for patients with type 1 (juvenile) diabetes mellitus with or without impaired renal function. Unfortunately, transplant waiting lists for this indication are increasing because the current organ acceptability criteria are restrictive; morbidity and mortality significantly increase with time on the waitlist. Currently, only pancreas organs from donors younger than 50 years of age and with a body mass index (BMI) less than 30 are allocated for transplantation in the Eurotransplant (ET) area. To address this issue we designed a study to increase the available donor pool for these patients., Methods/design: This study is a prospective, multicenter (20 German centers), single blinded, non-randomized, two armed trial comparing outcome after SPK, PTA or PAK between organs with the currently allowed donor criteria versus selected organs from donors with extended criteria. Extended donor criteria are defined as organs procured from donors with a BMI of 30 to 34 or a donor age between 50 and 60 years. Immunosuppression is generally standardized using induction therapy with Myfortic, tacrolimus and low dose steroids. In principle, all patients on the waitlist for primary SPK, PTA or PAK are eligible for the clinical trial when they consent to possibly receiving an extended donor criteria organ. Patients receiving an organ meeting the current standard criteria for pancreas allocation (control arm) are compared to those receiving extended criteria organ (study arm); patients are blinded for a follow-up period of one year. The combined primary endpoint is survival of the pancreas allograft and pancreas allograft function after three months, as an early relevant outcome parameter for pancreas transplantation., Discussion: The EXPAND Study has been initiated to investigate the hypothesis that locally allocated extended criteria organs can be transplanted with similar results compared to the currently allowed standard ET organ allocation. If our study shows a favorable comparison to standard organ allocation criteria, the morbidity and mortality for patients waiting for transplantation could be reduced in the future., Trial Registration: Trial registered at: NCT01384006.

Abstract

Lung donor recognition and treatment should be further improved. In 2007 only 22% of all donors in Eurotransplant were actual lung donors. During the past decade, Eurotransplant has increased the utilization of lung donors from 43% in 1999 to 54% in 2007. Increasing the number of lung transplants should be done by maximizing this utilization rate. Eurotransplant has 4 mechanisms in place in order to ensure that the highest degree of effectiveness is achieved with every reported lung donor. The Eurotransplant rescue allocation policy--which is one of these mechanisms--accounted for an additional 16.6% of the total transplant volume.
