Bispectral Index (BIS) is used to monitor anesthetic depth and is a useful instrument to keep a check on intraoperative awareness. But there are various situations in which it shows false readings. Our aim of the study was to observe the changes in BIS value with steep Trendelenburg position, which is usually done, in robotic pelvic surgeries.We included 100 patients in our study who underwent robotic prostatectomies and hysterectomies. After anesthetizing the patient, the patient's heart rate, systolic blood pressure, mean arterial pressure, end-tidal desflurane, end-tidal COWe found a significant increase in BIS values (p0.05) after the change of position from supine to steep Trendelenburg. Heart rate, systolic blood pressure, and mean arterial pressure were almost constant.Our study concluded that the BIS value increases when patients were shifted from the supine to Trendelenburg position, which might raise the concern of a decrease in anesthetic depth.

BACKGROUND Laparoscopic surgeries form an essence of today's surgical practice. It has been adapted as standard procedure for cholecystectomy at most centres. Haemodynamic perturbations associated with pneumoperitoneum during laparoscopic cholecystectomy may cause serious health hazard to the patient. The aim of the study is to compare the effect of oral clonidine and gabapentin premedication on intraoperative haemodynamic stability with a placebo. MATERIALS AND METHODS A prospective, randomized, double-blind, placebo-controlled study was conducted on 90 patients with ASA physical status I and II aged between 20-60 years undergoing elective laparoscopic cholecystectomy. They were randomly allocated to one of the three groups containing 30 patients each. Group C (n=30) received 150? g clonidine, group G (n=30) received 600 mg gabapentin and group P (n=30) received a multivitamin tablet 2 hrs. prior to the surgical procedure. We then assessed haemodynamics of the patients with parameters HR, SBP, DBP, MAP at different points of time and any postoperative adverse events. RESULTS Demographically all the three groups were statistically comparable. The rising cost of newer inhalational anaesthetic agent like desflurane has influenced increasing number of anaesthesiologist to use minimal flow anaesthesia. We did a randomised prospective study on total of sixty patient, who were divided into two groups of thirty patients each. Two volatile inhalational anaesthetic agents were compared: group I received desflurane ( n =30) and group II isoflurane ( n =30) in minimal flow anaesthesia. Recovery time was 5.70 ± 2.78 minutes in desflurane group and 8.06 ± 31 minutes in isoflurane group ( P value 0.004). Desflurane was found costlier than isoflurane but it has many inherent quality which make it superior to other inhalational agent in use. A further saving by desflurane is due to more rapid recovery and patient remain alert and clear headed permitted more economical use of recovery facilities and discharge of patient. Summary To conclue, we can use desflurane more efficiently with minimal flow anaesthesia with quicker and clear headed recovery with economical use of recovery staff and facilities.
