Abstract

Introduction and objectives The epidemiology of heart failure (HF) that complicates ST-segment elevation myocardial infarction (STEMI) is not well defined. Our aim was to analyze the epidemiology and prognosis of HF that complicates STEMI treated with primary percutaneous coronary intervention (PCI). Methods Multicentre registry of 14 070 patients with STEMI treated with primary PCI from January 2010 to December 2015. Results Patients with HF were older, more frequently female, and diabetic and had chronic ischaemic heart disease. Only 10.3% of patients with STEMI treated with primary PCI had HF on admission and the majority of them had the mildest form (Killip–Kimball II 77.8%). HF was associated with high mortality (30-day mortality was 2.9% of patients in Killip–Kimball I, 9.5% of Killip–Kimball II and 17.4% in Killip–Kimball III, P Conclusions HF complicates 10.3% of patients with STEMI treated with primary PCI and it was associated with a higher risk of developing cardiogenic shock and with 30-day and 1-year mortality in 30-day survivors. In this high-risk population, primary PCI and posterior medical treatment should be prioritized to avoid worsening of HF.

En el marco de un proyecto transnacional, la red de montañas emblemáticas mediterráneas (EMM, por sus siglas en inglés) está desarrollando el diseño y estructuración de nueve ecoviajes como producto turístico basado en rutas por territorios situados alrededor de áreas de montaña cercanas a la costa mediterránea. Los ecoviajes nacen con la intención de generar una alternativa al turismo de masas que soporta tanto el litoral como —en ocasiones— las cimas de las montañas. En el presente artículo, describimos el modelo y el proceso de diseño y desarrollo sobre el terreno de estos ecoviajes. Además, interpretamos algunos resultados de dicho proceso; que muestran la complejidad que supone el trabajo en red, además de poner de manifiesto los numerosos retos que plantea la práctica del turismo sostenible en territorios rurales y de montaña, tanto para actores públicos como privados. Within the framework of a transnational project, the Emblematic Mediterranean Mountains network (EMM) has developed nine ecojourneys as a tourist product based on routes through areas located in the hinterland of mountain regions near the Mediterranean coast. The ecojourneys intend to become an alternative to mass tourism that affects both the coastline and —sometimes— mountain summits. In this article, we describe the design process and development on the field of the ecojourneys. In addition, we interpret the results of the process. They show the complexity of networking, in addition to highlighting the numerous challenges that applying the principles of sustainability in tourism pose in rural and mountain territories, both for public and private actors.

Abstract

[EN] Traditionally, glass fibre has been used as plastic reinforcement whenever mechanical properties of a matrix, like stiffness, do not meet the specifications. However, current tendencies try to replace glass fibres by more sustainable fibres to obtain eco-friendlier products. Natural fibres show comparatively good physical and mechanical properties and, unlike glass fibres, come from renewable resources and are recyclable and sustainable. In this work, hemp straw discarded from hemp manufacturing was used as reinforcement in polypropylene composites. One drawback associated to hemp straw is its high lignin content that reduces its reinforcing potential. Therefore, a soft alkaline treatment was employed to adjust the lignin contents. In this work, the evolution of the Young's modulus with the NaOH treatment is assessed and discussed. Intrinsic Young's moduli of hemp straw fibres at different alkaline conditions were determined by Hirsch model. Finally, Tsai-Pagano and Halpin-Tsai equations allowed the prediction of the theoretical Young's modulus of the composites. The results showed the competitiveness of a by-product reinforced composite in front of commodity materials.

Abstract

Objectives: This study aimed to assess the real use of cefazolin for methicillin-susceptible Staphylococcus aureus (MSSA) infective endocarditis (IE) in the Spanish National Endocarditis Database (GAMES) and to compare it with antistaphylococcal penicillin (ASP). Methods: Prospective cohort study with retrospective analysis of a cohort of MSSA IE treated with cloxacillin and/or cefazolin. Outcomes assessed were relapse; intra-hospital, overall, and endocarditis-related mortality; and adverse events. Risk of renal toxicity with each treatment was evaluated separately. Results: We included 631 IE episodes caused by MSSA treated with cloxacillin and/or cefazolin. Antibiotic treatment was cloxacillin, cefazolin, or both in 537 (85%), 57 (9%), and 37 (6%) episodes, respectively. Abstract

The formulation of greener composite materials by substituting glass fibers with natural fibers is a current field of research. If such natural fiber reinforcements come from industrial side streams, as hemp core fibers (HCFs) come from the extraction of hemp strands for the textile industry, an additional advantage can be identified. Nonetheless, such by-product fibers show some drawbacks, such as high lignin contents, which can make it difficult to obtain a good interphase between the fibers and the matrix and to obtain a good fiber individualization. A digestion treatment at different NaOH contents is proposed to eliminate soluble lignin and extractives from the surface of the fibers. At the same time, the use of a coupling agent solves incompatibilities between the fibers and the matrix. The composites were tensile tested and the impact of the proposed treatments is evaluated and discussed. Later, the Kelly-Tyson modified equation and a modified rule of mixtures—the micro-mechanic models—is used to study the impact of such treatments on the quality of the interphase between the polymer and the reinforcement.
