Background: Tranexamic acid (TXA) has been widely used in various orthopaedic subspecialities to decrease blood loss, transfusions, and wound complications. However, the role of TXA in foot and ankle surgery is not clearly delineated. This meta-analysis aims to report the efficacy and safety of TXA in relation to foot and ankle surgical procedures. Methods: Database searches were conducted for eligible studies from data inception through January 2022. Clinical studies on the use of TXA in foot and ankle procedures reporting the desired outcomes were included. Outcomes were estimated blood loss, change in hemoglobin, and overall complications. Risk of bias was assessed using the Newcastle-Ottawa quality assessment scale and the Joanna Briggs Institute (JBI) critical appraisal tool. Results: Nine studies met the inclusion criteria. A total of 752 foot and ankle procedures were included, in which 511 (67.95%) procedures received TXA whereas 241 (32.05%) served as controls and did not receive TXA. The pooled data of change in hemoglobin and overall complications showed no difference between the TXA and control group. Estimated blood loss was significantly lower in the patients who received TXA. Conclusion: In conclusion, TXA use was associated with lower estimated blood loss in foot and ankle surgeries without increased risk of thromboembolic events, wound complications, or changes in hemoglobin. Rationale & Objective Persons with chronic kidney disease (CKD) are often unaware of their disease status. Efforts to improve CKD awareness may be most effective if focused on persons at highest risk for progression to kidney failure. Study Design Serial cross-sectional surveys. Setting & Participants Nonpregnant adults (aged ≥20 years) with CKD glomerular filtration rate categories 3-4 (G3-G4) who participated in the National Health and Nutrition Examination Survey from 1999 to 2016 (n = 3,713). Predictor 5-year kidney failure risk, estimated using the Kidney Failure Risk Equation. Predicted risk was categorized as minimal ( Outcome CKD awareness, defined by answering “yes” to the question “Have you ever been told by a doctor or other health professional that you had weak or failing kidneys?” Analytical Approach Prevalence of CKD awareness was estimated within each risk group using complex sample survey methods. Associations between Kidney Failure Risk Equation risk and CKD awareness were assessed using multivariable logistic regression. CKD awareness was compared with awareness of hypertension and diabetes during the same period. Results In 2011 to 2016, unadjusted CKD awareness was 9.6%, 22.6%, 44.7%, and 49.0% in the minimal-, low-, intermediate-, and high-risk groups, respectively. In adjusted analyses, these proportions did not change over time. Awareness of CKD, including among the highest risk group, remains consistently below that of hypertension and diabetes and awareness of these conditions increased over time. Limitations Imperfect sensitivity of the “weak or failing kidneys” question for ascertaining CKD awareness. Background: Supination adduction ankle fractures are unique among rotational ankle fractures as plate constructs are more commonly used than independent screws for medial malleolar fixation. The purpose of this study was to compare fracture displacement between plate fixation to a novel screw-only construct using a cadaveric biomechanical early-weightbearing model for the treatment of vertical medial malleolus fractures. Methods: Six nonosteoporotic fresh-frozen cadaver shanks and feet in matched pairs underwent a vertical osteotomy of the medial malleolus to simulate the supination adduction type injury. Osteoporosis was measured using DEXA scans. One specimen from each pair was fixed with a one-third tubular buttress plate and the other with screw-only fixation. The specimens were then axially loaded for 100 000 cycles to simulate protected weightbearing, and subsequently loaded to failure in supination. Stiffness, fracture displacement, and load to failure were recorded. Statistical significance was set at P Results: There were no measurable differences in displacement between the 2 constructs during axial cyclic loading after 100 000 cycles (plate, 0.74 ± 0.09 mm; screws, 0.79 ± 0.18 mm; P = .225). During supination and axial load to failure, the plate outperformed the screw construct. For load to failure (2 mm displacement) at the fracture site, the plate group failed at 716 ± 240 N, whereas the screw group failed at 567 ± 237 N ( P = .015). During load to catastrophic failure, the plate group outperformed the screw group (plate, 6011 ± 1646 N; screws, 4578 ± 1837 N; P = .002). Conclusion: For vertical medial malleolar fractures, the screw-only construct demonstrated no statistical difference when compared to buttress plating for cyclical axial loading, simulating early weightbearing in a boot. However, buttress plating is 21% to 24% stronger than the screw-only fixation construct in overall strength and prevention of catastrophic failure when loading in a supinated position. Clinical Relevance: The screw-only construct is biomechanically similar to a buttress plate when simulating early protected weightbearing. This suggests that early weightbearing as tolerated in a controlled ankle motion boot beginning 2 weeks postoperatively is mechanically safe for this fracture pattern and does not result in unacceptable amounts of fracture displacement. This construct may be useful as a less invasive treatment modality for the treatment of vertical medial malleolus fractures in select patients.

ObjectivesIn lupus nephritis the pathological diagnosis from tissue retrieved during kidney biopsy drives treatment and management. Despite recent approval of new drugs, complete remission rates remain well under aspirational levels, necessitating identification of new therapeutic targets by greater dissection of the pathways to tissue inflammation and injury. This study assessed the safety of kidney biopsies in patients with SLE enrolled in the Accelerating Medicines Partnership, a consortium formed to molecularly deconstruct nephritis.Methods475 patients with SLE across 15 clinical sites in the USA consented to obtain tissue for research purposes during a clinically indicated kidney biopsy. Adverse events (AEs) were documented for 30 days following the procedure and were determined to be related or unrelated by all site investigators. Serious AEs were defined according to the National Institutes of Health reporting guidelines.Results34 patients (7.2%) experienced a procedure-related AE: 30 with haematoma, 2 with jets, 1 with pain and 1 with an arteriovenous fistula. Eighteen (3.8%) experienced a serious AE requiring hospitalisation; four patients (0.8%) required a blood transfusion related to the kidney biopsy. At one site where the number of cores retrieved during the biopsy was recorded, the mean was 3.4 for those who experienced a related AE (n=9) and 3.07 for those who did not experience any AE (n=140). All related AEs resolved.ConclusionsProcurement of research tissue should be considered feasible, accompanied by a complication risk likely no greater than that incurred for standard clinical purposes. In the quest for targeted treatments personalised based on molecular findings, enhanced diagnostics beyond histology will likely be required.

The Dietary Approaches to Stop Hypertension (DASH) diet lowers blood pressure, an important risk factor for chronic kidney disease (CKD) and end-stage renal disease (ESRD). However, it is unclear whether adherence to a DASH diet confers protection against future ESRD, especially among those with pre-existing CKD and hypertension. We examined whether a DASH diet is associated with lower risk of ESRD among 1,110 adults aged ≥ 20 years with hypertension and CKD (estimated glomerular filtration rate, eGFR 30-59 ml/min/1.73 m2) enrolled in the National Health and Nutrition Examination Survey (1988-1994). Baseline DASH diet accordance score was assessed using a 24-hour dietary recall questionnaire. ESRD was ascertained by linkage to the U.S. Renal Data System registry. We used the Fine-Gray competing risks method to estimate the relative hazard (RH) for ESRD after adjusting for sociodemographics, clinical and nutritional factors, eGFR, and albuminuria. Over a median follow-up of 7.8 years, 18.4% of subjects developed ESRD. Compared to the highest quintile of DASH diet accordance, there was a greater risk of ESRD among subjects in quintiles 1 (RH=1.7; 95% CI 1.1-2.7) and 2 (RH 2.2; 95% CI 1.1-4.1). Significant interactions were observed with diabetes status and race/ethnicity, with the strongest association between DASH diet adherence and ESRD risk observed in individuals with diabetes and in non-Hispanic blacks. Low accordance to a DASH diet is associated with greater risk of ESRD in adults with moderate CKD and hypertension, particularly in non-Hispanic blacks and persons with diabetes.

Nonionic surfactant-activated microgels (SAMs), composed of hydrophobic alkyl acrylates and hydrophilic hydroxyalkyl esters that utilize the effects of surfactant mediated swelling and interaction to provide pH-independent rheological properties, were previously reported as a new pathway to the rheology modification of surfactant solutions. Crosslinking was shown to play an important role in the properties of these soft microgel systems. To understand the impact of crosslinking chemistry on SAM polymers, we have compared two types of SAM polymers: a conventionally crosslinked SAM polymer via allyl pentaerythritol and a novel SAM polymer, where the surface is self-crosslinked via a reactive surfactant. We have systematically characterized the polymer's swelling, rheology and microstructure in a model system containing the polymer, sodium dodecyl sulfate (SDS) and water. Surface self-crosslinking is demonstrated to be a more effective crosslinking approach to create surfactant-mediated interactions between the microgel particles, resulting in more effective rheology modification. Internal crosslinking hinders both the full swelling of the SAM polymer as well as inter-particle bridging interactions, and is therefore less effective. To our best knowledge, this is the first report on creating a novel surface self-crosslinked microgel via a dual-functional reactive surfactant that interacts with a non-reactive surfactant to create a yield stress fluid.

Background Poor access to food among low-income adults has been recognized as a risk factor for chronic kidney disease (CKD), but there are no data for the impact of food insecurity on progression to end-stage renal disease (ESRD). We hypothesized that food insecurity would be independently associated with risk for ESRD among persons with and without earlier stages of CKD. Study Design Longitudinal cohort study. Setting & Participants 2,320 adults (aged ≥ 20 years) with CKD and 10,448 adults with no CKD enrolled in NHANES III (1988-1994) with household income ≤ 400% of the federal poverty level linked to the Medicare ESRD Registry for a median follow-up of 12 years. Predictor Food insecurity, defined as an affirmative response to the food-insecurity screening question. Outcome Development of ESRD. Measurements Demographics, income, diabetes, hypertension, estimated glomerular filtration rate, and albuminuria. Dietary acid load was estimated from 24-hour dietary recall. We used a Fine-Gray competing-risk model to estimate the relative hazard (RH) for ESRD associated with food insecurity after adjusting for covariates. Results 4.5% of adults with CKD were food insecure. Food-insecure individuals were more likely to be younger and have diabetes (29.9%), hypertension (73.9%), or albuminuria (90.4%) as compared with their counterparts ( P P =0.05). Food-insecure adults were more likely to develop ESRD (RH, 1.38; 95% CI, 1.08-3.10) compared with food-secure adults after adjustment for demographics, income, diabetes, hypertension, estimated glomerular filtration rate, and albuminuria. In the non-CKD group, 5.7% were food insecure. We did not find a significant association between food insecurity and ESRD (RH, 0.77; 95% CI, 0.40-1.49). Limitations Use of single 24-hour diet recall; lack of laboratory follow-up data and measure of changes in food insecurity over time; follow-up of cohort ended 10 years ago. Conclusions Among adults with CKD, food insecurity was independently associated with a higher likelihood of developing ESRD. Innovative approaches to address food insecurity should be tested for their impact on CKD outcomes.

Alkali swellable microgels are widely used to control rheology of formulated products containing surfactants. However, formulations based on these pH-responsive polymers show undesirably large changes in yield stress in a range of pH close to the pKa of the acid group. Analysis of the behavior of a cross-linked copolymer of ethyl acrylate and methacrylic acid in the nonionized form (at pH below the pKa of methacrylic acid) in the presence of sodium dodecyl sulfate shows surfactant-mediated swelling (an increase in particle diameter by over 2.5×) and a peak in zero-shear viscosity versus surfactant concentration indicating surfactant-mediated interaction of the swollen microgels. On the basis of these results, we demonstrate a new class of nonionic microgels composed of hydrophobic alkyl acrylates and hydrophilic hydroxyalkyl esters that utilize the effects of surfactant-mediated swelling and interaction to provide pH-independent rheological properties.

Films of poly(vinyl chloride) have been cast from solutions of varying thermodynamic quality and the film properties studied by inverse phase gas chromatography. The solvents included tetrahydrofuran, cyclohexanone, and mixtures of toluene with both tetrahydrofuran and cyclohexanone. The solvent composition was followed during evaporation and film formation for the mixed solvents, and it was shown that the slower evaporating solvent component has the greatest effect on the surface properties of the films. The porosity of the polymer film surfaces is interpreted in terms of the mobility of the polymer chains during film formation. At high concentrations, polymer chain–polymer chain interactions become stronger when thermodynamically less powerful solvents are used or are last to leave during film formation; therefore, the solvent balance has a critical influence on the film surface properties. Annealing the poly(vinyl chloride) above the glass transition temperature reduces the surface porosity of the cast polymer.
