La infección por virus sincitial respiratorio constituye una significativa carga de morbilidad y mortalidad en niños de alto riesgo. La profilaxis con palivizumab es considera la principal estrategia para prevenir las infecciones graves en este grupo poblacional. Con el objetivo de establecer recomendaciones para su uso en Colombia, se llevó a cabo un consenso de expertos siguiendo un proceso basado en las recomendaciones de la National Institutes of Health (NIH). En este pro-ceso participó un panel de seis expertos, quienes revisaron estudios que incluyeron revisiones sistemáticas, metaanálisis, ensayos clínicos controlados, estudios observacionales y guías clínicas, enfocados en niños menores de dos años tratados con palivizumab. La evaluación de la calidad de la evidencia y la formulación de recomendaciones se realizaron siguiendo las metodologías propuestas por el Centro de Medicina Basada en la Evidencia de Oxford y el sistema GRADE. Se desarrollaron recomendaciones para la profilaxis con palivizumab en subgrupos de pacientes de alto riesgo, incluyendo neonatos prematuros, prematuros moderados-tardíos, pacientes con displasia broncopulmonar, fibrosis quística, síndrome de Down y trastornos innatos de la inmunidad. Este proceso permitió generar recomendaciones clínicamente relevantes y fundamentadas en la evidencia, con el propósito de mejorar la práctica clínica en el país.

4. Hallazgo incidental de hernia de Amyand en un lactante de 10 meses de edad con antecedente de prematuridad extrema: Presentación de caso y propuesta de manejo.

Introduction. Amyand’s hernia is a condition in which the cecal appendix is contained within the inguinal hernia sac, its diagnosis and management continue to be a challenge as it is a rare pathology. Case report. A 10-month-old male infant with a history of extreme prematurity presented a right Amyand’s hernia and a normal appendix. The appendix was returned to the cavity and the hernia was repaired. Result. After one year of follow-up, the patient had no complications. Conclusions. Has been reported that the risk of incarceration in full-term children is 12% and in premature babies it is 39%, therefore, surgical repair of an inguinal hernia is always necessary. There is no standard treatment for Amyand hernia in children, therefore the CiX classification could be considered for management. A treatment proposal is presented in the pediatric population, where three types were included, considering the evolutionary stage of the cecal appendix and that almost all inguinal hernias in children are due to persistence of the vaginal process; therefore, surgical treatment only includes herniotomy. By not using prosthetic material, handling is facilitated and the risk of infection is reduced. [ABSTRACT FROM AUTHOR]

5. Feeding Intolerance in Late Preterm Infants: Single Center Experience.

OBJECTIVE: To determine the frequency and clinical features of the late preterm infants with feeding intolerance. MATERIASL AND METHODS: Medical records of 426 infants with gestational age of 340/7- 366/7 weeks were retrospectively reviewed for the study. Clinical and demographic features of late preterm infants and episodes of feeding intolerance were recorded. RESULTS: A total of 54 late preterm infants with feeding intolerance were evaluated and compared with 178 infants without feeding intolerance. Baseline demographics were similar between groups. Mean duration of intolerance period was 2.3 ± 1.2 days. Time to full enteral feeding was significantly longer in late preterm infants with feeding intolerance when compared with infants without feeding intolerance (8 ± 2.3 vs 5.2 ± 1.7 days, respectively, p<0.001). Breastfeeding rates were similar between the groups. Rate of prokinetic use in the feeding intolerance group was 37%. There were no differences between the time to full enteral feeding and the duration of parenteral nutrition between prokinetic users and non-users. CONCLUSIONS: Although the gestational ages of late preterm infants are close to term and their size is relatively large, they are not like term infants. These infants should be followed closely in terms of feeding problems as well as many morbidities. [ABSTRACT FROM AUTHOR]

6. Suplementación probiótica con Lactobacillus y Bifidobacterium para la prevención de enterocolitis necrosante en recién nacidos pretérmino de muy bajo peso.

BACKGROUND: The high morbidity of necrotizing enterocolitis (NEC) in very low birth weight infants (<1500 grams) (VLBW) requires the search for prevention strategies that are more effective than treatment. Current evidence supports the use of probiotics as protective factors. OBJECTIVES: To determine if probiotic supplementation in VLBW infants represents a preventive factor for the development of NEC. METHODS: Retrospective cohort study conducted in 145 VLBW infants, where the presence of NEC was evaluated in populations supplemented with L. reuteri, B. lactis or not supplemented. The comparison among groups was performed with the chi-square test for maximum likelihood-ratio. RESULTS: Of 66 supplemented patients, 86.4% did not present NEC (RR 0.23 [95% Ci: 0.12-0.43]; p<0.0001). In those not supplemented, the incidence of NEC was 59.5%. An NNT of 1.7 to 3.1 was obtained. CONCLUSIONS: Despite the heterogeneity in clinical studies of probiotic supplementation, the evidence is strongly in favor of probiotic treatment for the prevention of NEC. This study found that supplementation from the first days of life at doses of 600 million CFUs/ day was more effective in preventing the presentation of NEC than higher doses. [ABSTRACT FROM AUTHOR]

Introducción: La literatura científica ha documentado varias secuelas sistémicas y orofaciales en bebés prematuros, incluido el retraso en el desarrollo dentario. Objetivos: Determinar si hay retraso en el desarrollo dentario en pacientes prematuros usando el método de Demirjian. Material y métodos: Se estudiaron 14 pacientes prematuros de 4 a 19 años de edad atendidos en la Facultad de Odontología de la Universidad Complutense de Madrid, quienes previamente se habían realizado una radiografía panorámica. Se calculó la edad dentaria con el método de Demirjian y se comparó con la edad cronológica al momento de la radiografía. Se usaron SPSS 25.0, los test de Wilcoxon y de la U de Mann-Whitney, y el coeficiente de correlación de Spearman para el análisis estadístico, asumiendo un nivel de significación del 95%. Resultados: No se encontraron diferencias estadísticamente significativas entre la edad cronológica y la edad dentaria según el método de Demirjian. Además, no se observó relación entre el desarrollo dentario y variables como el sexo o la edad gestacional. Conclusiones: Los resultados indican que en los casos estudiados los pacientes prematuros no presentan retraso en el desarrollo dentario. El método de Demirjian es aplicable a esta población. Tampoco se halló relación con la edad gestacional o el sexo. Se necesitan estudios adicionales con muestras más grandes.

Introduction: there is a lack of evidence with objective measurements of respiratory muscle tone in premature infants with bronchopulmonary dysplasia BPD. Objective: to assess the muscle tone of accessory inspiratory muscles in premature infants with and without BPD from birth to 36 gestational weeks. Material and method: a longitudinal observational clinical study with 37 premature infants of less than 36 weeks of gestational age and weighing less than 1500 grams, hospitalized at the neonatal intensive care unit. Premature infants underwent assessments of muscle tone with surface electromyography every two weeks after birth, on the muscles: pectoralis major, anterior serratus, trapezius, and erector of the spine. Those with severe complications, deaths and fewer than three surface electromyography measures were excluded. Premature infants were allocated into two groups: premature infants with bronchopulmonary dysplasia (with BPD, defined by the need for supplemental oxygen for 28 days or more) and control group, without BPD. Results: premature infants with bronchopulmonary dysplasia presented higher muscle tone in trapeziusin the 1st, 2nd, 3rd and 4th evaluations, in anterior serratus only in the 1st evaluation and inerector of the spine in the 2nd and in the 4th evaluations, compared to CG (p < 0.05 for all). In the intragroup analysis, the muscle tone of erector of the spine decreased over time in both groups (p < 0.05). In addition, infants with BPD required more invasive and non-invasive ventilatory support compared to CG (p < 0.05 for all). Conclusion: premature infants with BPD exhibit heightened muscle tone in accessory inspiratory muscles and elevated requirement for ventilatory support during the hospitalization in the neonatal intensive care unit. Introduction: Maternal recognition of special health needs of premature babies is essential, given the importance of early stimuli after discharge to strengthen child development. Objective: To identify premature newborns with potential factors for the development of special health needs and describe maternal recognition of these factors. Materials and methods: Qualitative and quantitative study, performed using a triangulation method, with concomitant incorporation of data, included to analysis Chi-square test (p<0.05) for quantitative data and interpretation of meanings for qualitative. Variables: Birth weight, gestational age, Apgar score, complications, and length of hospital stay from 951 records of hospitalized premature infants; 18 mothers were interviewed during hospitalization, 15 days after discharge at home, 30 days after this at-home visit and at the age of 6 months of the child, by telephone, in Southern Brazil. Results: Identified as potential factors: Extreme prematurity and/or very premature, low birth weight, complications, and prolonged hospitalization. With the incorporation of the data, the relevance of these factors for children at risk of changes in development and behavior was verified. Mothers described their children as having healthy development but recognized prematurity is a factor that predisposes to new hospitalizations, and perinatal factors and those related to hospitalization contribute to the development of special health needs. Conclusion: The importance of early identification of factors that enhance the development of special health needs is understood to initiate the necessary stimuli to strengthen child development. Introducción: La lactancia materna (LM) hace parte fundamental del cuidado integral que ofrecen los profesionales de enfermería en la Unidad de Cuidados Neonatales (UCN). Debido a la carga laboral y la delegación de diferentes acciones de cuidado, las dinámicas en estas unidades se centran en la innovación por parte del uso tecnológico, y postergan la humanización del servicio perjudicando el apoyo a la LM. Objetivo: Diseñar un programa de apoyo a la LM en la UCN. Materiales y métodos: Investigación cualitativa acción participación. Se realizaron 5 entrevistas semiestructuradas a profesionales de la salud expertos en LM, unos que trabajan en la unidad de la institución en que se realizó el estudio y otros externos, indagando, desde su experiencia y conocimiento, los elementos clave para el diseño de un programa de apoyo a la LM en la UCN. Resultados: Se exponen en las categorías: a. Cultura en lactancia, comprende: política institucional en lactancia y estandarización del proceso; b. Calidad, comprende: registro y control de actividades y; c. Seguimiento de indicadores y auditoría constante. Discusión: Se diseñó un programa de LM el cual será aplicado por la institución con el fin de que la LM exclusiva no se vea frustrada, por medio del acompañamiento, la capacitación, la estandarización y la promoción. Conclusión: El personal de enfermería está interesado en que se estandarice un programa que permita que la información brindada a los pacientes sea veraz y concisa, plantean la educación como pilar fundamental para el desarrollo de un programa exitoso en LM.

Objective: to describe the follow-up of the preterm newborn in the third stage of the kangaroo method from the perspective of Primary Care nurses. Method: a qualitative, descriptive and exploratory study conducted in the basic health units of the city in the interior of Bahia, with nine nurses. The semi-structured interview and content analysis of Bardin were used. Results: nurses understand what a premature infant is superficially, but do not understand exactly how the Kangaroo Method works. Among the facilities, the help of Community Health Agents stand out, and the difficulties that prevailed most were the fragility in the reference and counter-reference. Final considerations: in view of this, it is evident the need for continuing education for nurses in Primary Care, the systematization of the third stage of the Method and professional training for nursing in order to improve information about the premature child, to disseminate knowledge that favors the service to this public. Introduction: Preterm birth and low birth weight constitute a health problem worldwide, since they represent an important predictor of development and postnatal growth, as well as an indicator of newborn morbidity and mortality. Objective: To characterize mothers with preterm and low weight infants according to clinical and epidemiological variables. Methods: A descriptive, cross-sectional study was carried out from January to December, 2022, of 35 mothers with preterm and low weight infants, belonging to the East Area Teaching Polyclinic in Camagüey city. Age, nutritional state, risk classification, prenatal control, preconcepcional reproductive risk, multidisciplinary visits, diseases characteristic of pregnancy, birth state, detected deficiencies and causes were among the analyzed variables. Results: In the series there was prevalence of the 35 and over age group (25.7%), obesity (37.1%), high risk mothers (45.7%) who received among 8-14 prenatal controls, and bad obstetric history (31.4%). On the other hand, 22.9% of the mothers had low weight children and 31.4% preterm children; as long as there were difficulties in follow up due to absences (57.1%). Conclusions: The mothers presented unfavorable clinical and epidemiological characteristics, such as the 35 and over years, obesity, high risk, bad obstetric history and vaginal infection as a disease characteristic of pregnancy, which influenced the infants to be preterm or low weight, or both. Introducción. La hernia de Amyand es una condición en la que el apéndice cecal está contenido dentro del saco herniario inguinal. Su diagnóstico y su manejo continúan siendo un reto al tratarse de una patología poco frecuente. Caso clínico. Lactante varón de 10 meses con antecedente de prematuridad extrema, quien presentó hernia de Amyand derecha con apéndice normal. Se retornó el apéndice a la cavidad y se procedió a la reparación de la hernia. Resultado. Después de un año de seguimiento, el paciente no presentó complicaciones. Conclusiones. Se ha reportado que el riesgo de incarceración en niños nacidos a término es del 12 % y en prematuros del 39 %, por tanto, la reparación quirúrgica de una hernia inguinal siempre es necesaria. No existe un tratamiento estándar para la hernia de Amyand en niños, por tanto, la clasificación CiX podría considerarse para el manejo. Se presenta una propuesta de tratamiento en la población pediátrica, donde se incluyeron 3 tipos, considerando el estadio evolutivo del apéndice cecal y que casi la totalidad de hernias inguinales en niños son debidas a persistencia del proceso vaginal, por tanto, el tratamiento quirúrgico solo incluye herniotomía. Al no utilizar material protésico, se facilita el manejo y se reduce el riesgo de infección.

Background: The electroencephalogram (EEG) in the new-born period is highly superior to the clinical exam in the detection and prognosis of brain dysfunctions, since it allows continuous functional documentation of the brain at the patient's bedside in a non-invasive way. However, there is still some disagreement about these findings. Objective: To describe the electroencephalographic findings in newborns with a history of prematurity. Material and methods: Cross-sectional, descriptive, retrospective study. The inclusion criteria were: newborns with a history of prematurity, regardless of gender, who underwent an EEG from June 2017 to June 2021. Patients with incomplete electroencephalographic records or clinical records without complete data were excluded; patients using sedatives (thiopental, fentanyl, midazolam, diazepam) were eliminated from the study. Results: 107 patients (37 women and 70 men) with a history of prematurity were included, with a mean gestational age at birth of 30.9 WOG ± 3.25. Electroencephalographic findings were normal in 40%, abnormal in 32%, and immature in 28%. The most frequent abnormal finding was focal paroxysmal activity in 86%. 93.4% of the participants presented comorbidities, the most frequent being neurological. Conclusion: Preterm neonates are at high risk of neurologic sequelae, and EEG is a sensitive method for assessing neuromotor and cognitive prognosis. In our study population, one-third had abnormal findings. Early postnatal screening is helpful, but additional records are usually needed to detect high-risk newborns. Introduction: Breastfeeding (BF) is an essential part of the comprehensive care nurses provide in the Neonatal Care Unit (NCU). Due to the workload and delegation of different care activities, the dynamics in these units focus on innovation using technology and postpone humanization of service, undermining BF support. Objective: To design a BF support program at NCU. Materials and methods: Qualitative participatory action research. Seven semi-structured interviews were conducted with health professionals with expertise in BF. Some of them worked in NCU of the health facility where the study was conducted; others were external. Their experience and knowledge were used to identify the key elements for designing a BF support program at the UCN. Results: They are presented in the following categories: a) breastfeeding culture, including institutional policy on breastfeeding and standardization of the process; b) quality, including registration and control of activities; and c) monitoring of indicators and continuous auditing. Discussion: A BF program has been designed and it will be implemented by the health facility to prevent the interruption of exclusive BF through support, training, standardization, and promotion. Conclusion: Nurses are interested in standardizing a program that allows them to provide patients with truthful and concise information, and they view education as a fundamental pillar in the development of a successful BF program. Objective: to explore the potential use of the “Baby Diary” from the perspective of mothers with premature babies hospitalized in the intensive care unit. Method: descriptive and exploratory study, with a qualitative approach. The collection took place in a neonatal intensive care unit in Southern Brazil from February to June 2022 and consisted of implementing the emotional support tool “Baby Diary”, so that mothers of premature babies could record their experiences during hospitalization. After delivery, the document became the possession of the mother and family. After discharge to the ward, the tool was evaluated through audio-recorded interviews regarding its use. Data processing and text analysis were carried out using the IRaMuTeQ software. Result: most mothers were primigravidae, with intensive care stays between 22 and 65 days. Four classes were identified: The use of the “Baby Diary” and initial resistance; Emotional support and demonstration of feelings; Use of the “Baby Diary” as an intermediary in organizing thoughts for conversation with the family; Empowerment in issues related to care. Aim: to describe the feeding and growth patterns of preterm-born children at preschool age, considering feeding problems based on behaviours and skills, diet quality and parental feeding practices. Methods: a cross-sectional study was performed on preterm children born in Asturias (Spain) in 2016 (n = 94). When preterm-born children reached 3-4 years of age their families were asked to complete the Behavioral Pediatrics Feeding Assessment Scale and a food frequency questionnaire to identify possible feeding problems and assess diet quality (KIDMED index), respectively. Self-reported anthropometric data were also collected to assess weight growth. Electronic health records were reviewed to gather gestational and neonatal clinical data. Results: feeding problems were found in 7.4% of children and 20.2% of parents. According to the KIDMED index, 25.5 % of children had a high-quality diet. Feeding problems were higher in children born before 32 weeks of gestation, and decreased in frequency as the gestational age increased (p = 0.030). No differences were found in parental feeding practices (p = 0.455) or diet quality according to gestational age (p = 0.399), but body weight at 3-4 years was lower in preterm-born children (p = 0.015). Conclusions: feeding patterns of preterm-born children were suitable at the age of 3-4 years, but diet quality was moderate to poor in the majority of children. Follow-up of very preterm children beyond 3-4 years of age becomes necessary as they displayed more feeding problems and lower body weight. [ABSTRACT FROM AUTHOR]

Contexto: los nacimientos prematuros y el bajo peso al nacer se han descrito como factores de riesgo para el desarrollo de enfermedad renal crónica. Las madres son susceptibles de desarrollar alteraciones psicosociales ante el diagnóstico de sus hijos. Objetivo: evaluar el impacto psicológico de las madres canguro ante el riesgo de enfermedad renal crónica en sus hijos prematuros o de bajo peso al nacer, así como describir sus experiencias frente a esta situación. Metodología: estudio mixto con componente cuantitativo de corte transversal y cualitativo de tipo fenomenológico. Se incluyeron madres con hijos que presentaron bajo peso al nacer o prematurez en riesgo de enfermedad renal crónica. Los desenlaces principales fueron la frecuencia de ansiedad, depresión y estrés parental medidos con instrumentos estandarizados. También se describen las categorías emergentes de la experiencia reportada por las madres. Resultados: se incluyeron 27 madres en el periodo de análisis, cuyo promedio de edad fue de 27,11 ± 6,93 años. El 51,85?% eran inmigrantes y la totalidad pertenecían al estrato socioeconómico bajo. Para el 100?% de las madres se identificó algún nivel de ansiedad, donde el 44,44?% tenía depresión moderada y el 29,63?% depresión mínima, además, la mediana de puntaje en la escala de estrés parental fue de 89 puntos. Dentro de las categorías emergentes del análisis fenomenológico se encontró el afrontamiento, la culpa, la confianza, la fe y la esperanza, el apoyo, la interacción social y el rol familiar, entre otras. Conclusiones: la condición de riesgo de enfermedad renal crónica en hijos impacta la condición emocional y psicológica de las madres, generando en la mayoría de los casos estrés, ansiedad o depresión. La identificación oportuna de estas alteraciones permitirá brindar a las madres consejería y acompañamiento de salud mental y orientación en el cuidado de sus hijos.

El Programa Madre Canguro (PMC) permite un seguimiento a prematuros con comorbilidades como displasia broncopulmonar (DBP). El objetivo fue caracterizar los pacientes con DBP, describir su compromiso nutricional, neurológico, respiratorio en un PMC. Se realizó un estudio de cohorte retrospectivo en el PMC de un Hospital Universitario con aprobación del comité de ética. Incluyó pacientes con DBP entre 2015-2018, sin malformaciones mayores. Evaluamos el estado nutricional, neurológico y respiratorio a las 40 semanas, 6 y 12 meses de edad corregida mediante escalas recomendadas en Colombia. Realizamos análisis descriptivo y comparación según severidad de la DBP. Describimos la evolución en pacientes con seguimiento completo. Introducción. En este artículo se presenta una reflexión sobre la necesidad de tener una visión holística en la problemática de la prematurez, para que los equipos del área de salud puedan tener una comprensión de la multiplicidad de factores presentes en el parto prematuro, y sus consecuencias para el menor, la madre, los familiares y el personal de salud involucrado. Tema de reflexión. El tema se sustenta en dos investigaciones realizadas con las madres, los padres y los niños beneficiarios del Programa Madre Canguro en un hospital del Estado, en dos momentos, 2012 y 2014, con algunos de los autores del presente artículo. Conclusiones. Además de la revisión de la literatura científica relacionada con el tema y la problemática, se concluye que, bajo una mirada holística, se comprende de manera integral la problemática y se propicia una mejor comunicación con los padres; con el equipo de la atención en salud, se logra un manejo transdisciplinario, superando los objetos de estudios aislados. Se da así un abordaje integrador y significativo en la cotidianidad de cada actor, desde la piel, con el Programa Madre Canguro, hasta la racionalidad de la ciencia. Introduction. This article presents a reflection on the need for a holistic view of the problem of premature births, so that healthcare teams can understand the multiplicity of factors present in premature births, and its consequences for the minor, the mother, the family members, and the healthcare staff involved. Topic of reflection. The topic is based on two investigations carried out with mothers, fathers, and children’s beneficiaries of the Mother Kangaroo Program in a State hospital, at two moments, 2012 and 2014, with some of the authors of this article. Conclusions. In addition to the review of the scientific literature related to the topic and the problem, it is concluded that, under a holistic view, the problem is understood in a comprehensive way and better communication with the parents is encouraged. With the healthcare team, cross-disciplinary handling is achieved, overcoming the objects of isolated studies. As such, there is an integrative and significant approach in the daily life of each actor, from the skin, with the Mother Kangaroo Program, to the rationality of science. Introdução. Este artigo apresenta uma reflexão sobre a necessidade de se ter uma visão holística da problemática da prematuridade, para que as equipes da área da saúde possam ter uma compreensão da multiplicidade de fatores presentes no parto prematuro, e suas consequências para a criança, a mãe, os familiares e o pessoal de saúde envolvido. Tópico de reflexão. O tema é baseado em duas pesquisas realizadas com mães, pais e filhos beneficiários do Programa Mãe Canguru em um hospital estadual, em dois momentos, 2012 e 2014, com alguns dos autores deste artigo. Conclusões. Além da revisão da literatura científica relacionada ao tópico e ao problema, conclui-se que, sob uma visão holística, o problema é plenamente compreendido e incentiva-se uma melhor comunicação com os pais; com a equipe de saúde, consegue-se uma gestão transdisciplinar, superando os objetos de estudos isolados. Isto proporciona uma abordagem integradora e significativa no cotidiano de cada ator, desde a pele, com o Programa Mãe Canguru, até a racionalidade da ciência.

Objetivo: describir el proceso de diseño e implementación de un protocolo de atención para la primera hora de vida del recién nacido prematuro. Método: investigación participativa, que utilizó el marco de la ciencia de la implementación y los dominios del Consolidated Framework for Implementation Research. Estudio realizado en un hospital escuela del sureste de Brasil, con la participación del equipo multidisciplinario y de los gestores. El estudio se organizó en seis etapas, mediante del ciclo de mejora continua (Plan, Do, Check, Act): diagnóstico situacional; elaboración del protocolo; capacitaciones; implementación del protocolo; relevamiento de barreras y facilitadores; seguimiento y revisión del protocolo. Los datos fueron analizados mediante estadística descriptiva y análisis de contenido. Resultados: el primer protocolo de la Hora Dorada de la institución fue organizado por el equipo multidisciplinario a partir de un enfoque colectivo y dialógico. El protocolo priorizó la estabilidad cardiorrespiratoria, la prevención de hipotermia, hipoglucemia e infección. Después de cuatro meses de capacitación e implementación, el protocolo fue evaluado como una intervención de calidad, necesaria para el servicio, de bajo costo y de poca complejidad. La principal sugerencia de mejora fue realizar actividades educativas frecuentes. Conclusión: la implementación generó cambios e inició un proceso de mejora de la calidad de la atención neonatal, es necesario que la capacitación sea continua para lograr mayor adherencia y mejores resultados.

Objetivo: evaluar la autoeficacia materna para el cuidado de recién nacidos prematuros en la unidad de cuidados intensivos neonatales y después del alta hospitalaria, y relacionarla con la duración de la lactancia materna en el hogar. Método: estudio longitudinal que empleó escalas de evaluación de autoeficacia de 38 madres de recién nacidos prematuros en el período de noviembre de 2020 a enero de 2022 en la ciudad de Cascavel – PR – Brasil. Los datos fueron analizados por estadística descriptiva e inferencial. Resultados: La autoeficacia materna durante la hospitalización mostró ser elevada, y se mantuvo, así, en la evaluación tras el alta. La autoeficacia para la lactancia no presentó diferencias estadísticamente significativas durante la hospitalización (p=0,335) ni tras el alta (p=0,640). Sin embargo, las madres con alta autoeficacia en la hospitalización y en el hogar mantuvieron la lactancia materna exclusiva durante más tiempo. Conclusión: Identificar la autoeficacia materna debe ser una rutina en la práctica clínica de la enfermería en la hospitalización y seguimiento post alta, para potencializar el mantenimiento de la lactancia materno exclusivo.

Resumen El objetivo fue determinar las estrategias y necesidades educativas de padres de bebés prematuros en un hospital de Cali, Colombia. El estudio fue cualitativo de sistematización de experiencias centrada en un proceso de intervención mediada. Los ejes de la sistematización fueron: las necesidades educativas y las estrategias de mejoramiento. La muestra estuvo conformada por 11 madres y padres quienes recibieron educación en el contexto de un programa de seguimiento, seleccionados a través de un muestreo opinático por criterios, entrevistados en profundidad. Se realizó análisis de contenido temático. Se encontró que las necesidades educativas se agrupan en los cuidados del bebé: conocimientos básicos, conductas y emociones, condición de salud y alimentación y cuidados del cuidador. Las estrategias se enfocaron en el uso y aprovechamiento de las tecnologías de la información, la escuela de padres y la integración del grupo familiar.

Background: Promoting physical contact, especially skin-to-skin contact, immediately or as soon as possible in preterm newborns is still a challenge. Objective: To determine the prevalence of physical contact between mothers and preterm newborns at birth and to analyze the association between the type of physical contact between them in the delivery room and the prevalence of exclusive breastfeeding at hospital discharge. Methodology: Cross-sectional study with a nonprobability sample of 78 mothers and 79 preterm newborns. Data were collected from July to November 2017, through interviews and medical records, and analyzed through descriptive statistics and Poisson Regression with robust variance. Results: The prevalence of any type of physical contact in the delivery room was 51.9%, without statistical difference between the type of physical contact between mothers and infants and exclusive breastfeeding at discharge. Conclusion: Physical contact by touching and kissing after birth did not influence the prevalence of exclusive breastfeeding at hospital discharge. [ABSTRACT FROM AUTHOR]

- Abstract
Parenteral nutrition is a nutrient providing method necessary for growth and development of preterm newborns. The aim of this study is to compare the nutritional recommendations for macro and micronutrients in different international guidelines. PubMed, EBSCO and Google Scholar databases were used to collect relevant articles (observational, guidelines and reviews) written in English and Spanish, published between 2016 and 2022. In total, 28 articles were selected, 12 belonging to international guidelines and 16 with dosage recommendations of one or more macronutrients in relation to some condition or result obtained. When comparing the international guidelines, it has been shown that the macronutrient recommendations are similar to guarantee an ideal nutritional contribution in the preterm newborn. However, the ESPGHAN/ESPEN/ESPR/CSPEN 2018 guide has more complete information for each nutrient, supporting its suggestions with evidence articles and recommendations, being the only guide with a more focused section on vitamins and trace elements. OBJECTIVE: To characterize clinical and epidemiological aspects of transfused neonates in Intensive and Special Care Units of a high-complexity Institution in Medellín, Colombia. MATERIALS AND METHODS: Retrospective descriptive study that included hospitalized neonates in intensive or special care units between 2017 and 2018 who received transfusions during their hospital stay. For data collection information from medical records was used. The data analysis was done in the SPSS software, average and standard deviation were calculated for quantitative variables, and for qualitative variables, proportions were estimated. This study had ethical approval. RESULTS: 175 patients were included, with an average gestational age at birth of 31 weeks and an average weight of 1610 grams. 52.9% of the population was female. The main diagnoses were prematurity 127 (72.6%), respiratory distress syndrome 93 (53.1%), sepsis 75 (42.8%), and unspecified anemia 53 (30.3%). 94.4% of the neonates were hospitalized in the intensive care unit, with a median hospital stay of 35 days. Discharge was the main outcome with 83.7%, followed by death with 11.6%. CONCLUSIONS: Red blood cell transfusion in the neonatal population was frequently used in those admitted to intensive and special care units, this intervention showed favorable results but the lack of data in the medical records as a retrospective study limits the establishment of further associations.
