4. Marguerite Duras: A Thematic and Technical Study

- Abstract
This thesis attempts to examine the works of Marguerite Duras up to the year 1970. Duras has written novels and plays, and more recently film-scenarios; she has also directed films based on her own works. All these aspects of her output are discussed. After a brief introduction outlining Marguerite Duras's life and artistic development, the thesis divides into two parts: "Thematic Continuity" and "Technique and Style". In the first part Duras's works are studied for their close thematic similarities. In her outlook on the human condition, Duras wavers between optimism and pessimism. She writes principally about love, which according to her can offer self-fulfilment and spiritual release; it is an intensely passionate experience; yet its survival is never guaranteed and it may lead to unhappiness (ch.l). Duras is obsessed by the idea of loneliness, though she always holds out the possibility that men and women will enter into contact (ch.2). Human existence is presented as claustrophobic, often painful, yet Duras shows how ordinary people are capable of escaping from it, or of rebelling against it (chapters 3» 4). Time, too, can be both friend and enemy, fulfilling an individual's aspirations, but more frequently bringing change for the worse (ch. 5 ). A further theme in her work, related to that of time, is the way in which individuals are linked with their past through the processes of memory (ch.6). Since 1964, the majority of Duras's works have centred on madness, and the next two chapters cover what she considers at present her most important ideas (chapters 7, 8). Madness is seen as a means of self-liberation. The idea is crucial to Duras's present political concerns, since madness offers the key to transcendence and a new fora of Communism, as Duras conceives it. The ninth chapter covers two minor themes: the relationship between parent and child, and the effect of sexuality upon personal development. The second part of the thesis studies Duras's experiments in form, which throw light on meaning and the writer's attitude to her material. Duras's novel-techniques have been dictated by her rejection of authorial omniscience and the novel "S. la Balzac". The angle of vision is severely restricted. Duras has dispensed with psychological analysis of all characters save the central figure. Conversation therefore assumes capital importance in her aesthetic; it is skilfully fashioned to hint at psychic states and unarticulated thoughts (ch.10). Chapter 11 deals with technique in the plays, which again are totally dependent upon conversation; the tempo of the dialogue gives the plays their shape. Both as script-writer and film-director, Duras exploits the resources of the cinema for dramatic effect and to underline theme (ch.12). In chapter 13, the essential aspects of Duras's style are examined. The discussion is primarily concerned with style in the novels, but shows how stylistic devices have been transferred from one genre to another. The thesis concludes by describing Duras's position as a writer today.

10. Prognostic and Predictive Factors in Early Alzheimer's Disease: A Systematic Review.

- Abstract
Background: Alzheimer's disease (AD) causes progressive decline of cognition and function. There is a lack of systematic literature reviews on prognostic and predictive factors in its early clinical stages (eAD), i.e., mild cognitive impairment due to AD and mild AD dementia., Objective: To identify prognostic factors affecting eAD progression and predictive factors for treatment efficacy and safety of approved and/or under late-stage development disease-modifying treatments., Methods: Databases were searched (August 2022) for studies reporting prognostic factors associated with eAD progression and predictive factors for treatment response. The Quality in Prognostic Factor Studies tool or the Cochrane risk of bias tool were used to assess risk of bias. Two reviewers independently screened the records. A single reviewer performed data extraction and quality assessment. A second performed a 20% check. Content experts reviewed and interpreted the data collected., Results: Sixty-one studies were included. Self-reporting, diagnosis definition, and missing data led to high risk of bias. Population size ranged from 110 to 11,451. Analyses found data indicating that older age was and depression may be associated with progression. Greater baseline cognitive impairment was associated with progression. APOE4 may be a prognostic factor, a predictive factor for treatment efficacy and predicts an adverse response (ARIA). Elevated biomarkers (CSF/plasma p-tau, CSF t-tau, and plasma neurofilament light) were associated with disease progression., Conclusions: Age was the strongest risk factor for progression. Biomarkers were associated with progression, supporting their use in trial selection and aiding diagnosis. Baseline cognitive impairment was a prognostic factor. 11. Atezolizumab in Combination with Bevacizumab for the Management of Patients with Hepatocellular Carcinoma in the First-Line Setting: Systematic Literature Review and Meta-Analysis.

- Abstract
Background: In 2020, atezolizumab-bevacizumab became the new standard of care (SOC) for first-line unresectable hepatocellular carcinoma (HCC) patients, following a decade where sorafenib was the preferred first-line treatment. In the last few years, a number of novel systemic treatments with non-inferiority and superiority to sorafenib have been approved as first-line treatments., Objectives: The objective of this systematic literature review (SLR) and network meta-analysis (NMA) was to compare randomised controlled trial evidence for atezolizumab-bevacizumab with globally relevant pharmacological comparators for first-line treatment of patients with unresectable HCC., Methods: Randomised controlled trials investigating first-line treatment of HCC in adults with no prior systemic treatment were eligible for inclusion into the SLR and were retrieved from Embase, MEDLINE, and Evidence-Based Medicine (EBM) Reviews. Interventions of interest for the NMA included atezolizumab-bevacizumab, sorafenib, lenvatinib, durvalumab (including in combination with tremelimumab), cabozantinib (including in combination with atezolizumab), camrelizumab (including in combination with rivoceranib), pembrolizumab (including in combination with lenvatinib), and tislelizumab. Random effects NMA was conducted for survival endpoints within a Bayesian framework with an informative prior distribution for between-study heterogeneity. The hazard ratios for relative treatment effect were estimated with 95% credible intervals (CrIs)., Results: The SLR identified 49 studies, of which eight formed a connected evidence network permitting the indirect treatment comparison of atezolizumab-bevacizumab with comparators of interest. The indirect comparisons suggested an improved overall survival (OS) with atezolizumab-bevacizumab versus most comparators. All indirect treatment comparison results for atezolizumab-bevacizumab included the null value within the 95% CrI ( n = 1) for OS and progression-free survival (PFS)., Conclusions: The results of the NMA indicate atezolizumab-bevacizumab is associated with superior or comparable OS and PFS together with a manageable safety profile compared with globally relevant comparators in the unresected HCC indication. 13. 'A British Problem Affecting British People': Sickle Cell Anaemia, Medical Activism and Race in the National Health Service, 1975-1993.

- Abstract
Recent historiography has explored a contradiction at the heart of the British welfare state-it was founded on and supported by migrant and non-white labour, whose own healthcare and broader welfare state entitlements were neglected. This article explores how this contradiction was exposed and challenged by some of the health service's own workforce, who witnessed and contested racism in the National Health Service (NHS). This is discussed through the lens of the treatment of sickle cell anaemia (SCA), a genetic trait and disease more common in people of African, South Asian, Middle Eastern and Mediterranean descent, which has been highly racialized as affecting black people in particular. By pushing for improved responses to pain in sickle cell disease, and demonstrating the need for SCA screening in urban areas, healthcare professionals within the NHS-many of whom were black or migrant nurses, health visitors or doctors-articulated the status and entitlements of Black British citizenship., (© The Author(s) [2021]. - Abstract
The UK national sperm donor shortage is well known. We aimed to analyse the trends in various aspects of the sperm donor programme at Newcastle Fertility Centre (NFC) between 2000 and 2010. A retrospective review of the assisted conception treatments with donor sperm was performed. A decline in the numbers of donors recruited alongside a declining trend in the number of patients treated with donor sperm and donor insemination (DI) treatment cycles carried out was apparent. There was an accompanying rising trend in donor IVF cycles and in same-sex couples and single women coming for treatment. The transfer of sperm to local peripheral centres ceased during this time and an increasing number of patients imported sperm from overseas commercial sperm banks. A waiting list for treatment was set up in 2007 with a gradual increase in waiting time to 18 months in 2010. Overall, there was a significant change in the sperm donor programme at NFC with fewer donors recruited, fewer patients receiving treatment, increasing sperm import and longer waiting times for treatment over the study period.

- Abstract
We test between four separate hypotheses (social gossip, social contracts, mate advertising and factual information exchange) for the function(s) of language using a recall paradigm. Subjects recalled the social content of stories (irrespective of whether this concerned social behavior, defection or romantic events) significantly better than they did ecological information. Recall rates were no better on ecological stories if they involved flamboyant language, suggesting that, if true, Miller's "Scheherazade effect" may not be independent of content. One interpretation of these results might be that language evolved as an all-purpose social tool, and perhaps acquired specialist functions (sexual advertising, contract formation, information exchange) at a later date through conventional evolutionary windows of opportunity.
