Abstract

Recombinant fluorescent probes allow the detection of molecular events inside living cells. Many of them exploit the intracellular space to provide positional signals and, thus, require detection by single cell imaging. We describe here a novel strategy based on probes capable of encoding the spatial dimension of intracellular signals into 'all-or-none' fluorescence intensity changes (differential anchorage probes, DAPs). The resulting signals can be acquired in single cells at high throughput by automated flow cytometry, (i) bypassing image acquisition and analysis, (ii) providing a direct quantitative readout, and (iii) allowing the exploration of large experimental series. We illustrate our purpose with DAPs for Bax and the effector caspases 3 and 7, which are keys players in apoptotic cell death, and show applications in basic research, high content multiplexed library screening, compound characterization, and drug profiling. 10.1021/ac9015227

Abstract

A randomized trial was carried out to compare imatinib with interferon alfa plus cytarabine in the chronic phase of chronic myeloid leukemia (CML) to demonstrate the significant superiority of imatinib in all standard indicators of the disease within a median follow-up of 19 months. After 5 years of follow-up, continuous treatment of chronic-phase CML with imatinib as initial therapy was found to include durable responses in a high proportion of patients.

The drug Gleevec is more effective than interferon and cytarabine for treating patients with newly diagnosed chronic myeloid leukemia, according to a study of 1,106 patients. Gleevec inhibits an abnormal enzyme that is produced by a chromosomal abnormality. It is the abnormal enzyme that causes the symptoms of this type of leukemia.

Abstract

Granulocyte colony stimulating factor (G-CSF) does not appear to reduce mortality rates in elderly patients with acute myelogenous leukemia (AML). G-CSF is often given to AML patients after chemotherapy because it reverses the drop in white blood cells called neutrophils that occurs during chemotherapy. Of 173 AML patients 65 years or older who had received chemotherapy, 88 received daily intravenous infusions of G-CSF and 85 received placebo infusions. Although G-CSF treatment significantly hastened the recovery of neutrophils, it did not reduce the infection rate compared to placebo. Seventy percent of those receiving G-CSF went into remission compared to 47% of those receiving placebo. However, 12-month survival rates were similar: 45% in those receiving G-CSF and 40% in those receiving placebo. Eight percent of those in each group experienced a re-growth of their leukemic cells, but G-CSF caused no other side effects.

Abstract

Multiple myeloma results from the tumor-like growth of bone marrow cells. At present, the condition is generally fatal. However, there are indications that chemotherapy may be effective if greater doses and more aggressive drug combinations are used. However, the doses of chemotherapeutic agents that can be used are limited by destruction of normal bone marrow cells, a dangerous side effect. One method of circumventing this problem is to use a dose of drugs that virtually ablates the bone marrow, followed by bone marrow transplantation from a closely matched donor. This method has now been used to treat 90 patients with multiple myeloma. After a follow-up period of 79 months, 47 patients were still alive, and the remainder had died. Ten patients died soon after the bone marrow transplantation. Of the remaining patients, 37 had no signs of graft-versus-host disease (GVHD), in which the bone marrow of the donor attacks the tissues of its new host. Twenty-two patients had grade I GVHD, 10 had grade II, four had grade III, and four had grade IV GVHD. Patients with grade I GVHD or without GVHD were more likely to survive than patients with more extensive GVHD. Achieving a complete remission after bone marrow transplantation was also a significant predictor of long-term survival. The results of this study indicate that bone marrow transplantation from a carefully matched sibling may be an important treatment for some patients with multiple myeloma.
