Abstract

One hundred patients with infrarenal abdominal aortic aneurysms were treated by surgery. Fifteen deaths occurred postoperatively. Eleven patients died during the follow-up of a medium period of 13 months. Three patients with a ruptured aneurysm died intraoperatively before the graft was implanted. Three patients were lost to follow-up. Sixty-four patients were examined with colour-coded Duplex sonography after surgical repair. Colour-coded Duplex sonography was able to image the aortic prostheses with the adjacent major vessels. Fifty-eight patients showed a normal prosthesis, 3 dilatation proximal of the prosthesis, 1 dilatation distal of the prosthesis, 1 mild stenosis, 1 haematoma. Colour-coded Duplex sonography imaged the intraluminal space of the prosthesis more easily than ultrasound. Colour-coded Duplex sonography was able to differentiate between intraluminal and extraluminal lesions.

Abstract

ZusammenfassungFokale Leberläsionen sind unabhängig vom Vorhandensein eines Malignoms ein häufiger Befund. Für ein eventuelles therapeutisches Management ist es dabei erforderlich, die Läsionen eindeutig zu identifizieren und zu charakterisieren. Zur nicht-invasiven Diagnostik stehen die Sonographie, Computertomographie (CT), Magnetresonanztomographie (MRT) und nuklearmedizinische Verfahren zur Verfügung. Die modernen chirurgischen wie auch minimalinvasiven ablativen Verfahren erfordern eine hohe diagnostische Präzision, die insbesondere von der CT und MRT gewährleistet wird. Darüber hinaus gewinnt die Positronenemissionstomographie (PET) gerade beim Staging und der Therapiekontrolle zunehmend an Bedeutung, was umso mehr für die kombinierten PET-CT Scanner gilt, die die hohe Sensitivität der PET mit der Ortsauflösung der CT vereinen.Bei der Abklärung fokaler Leberläsionen müssen neben der Leistungsfähigkeit der einzelnen Untersuchungsmethoden das Spektrum möglicher Läsionen im Kontext der jeweiligen Grunderkrankung des Patienten berücksichtigt werden.

9. Different Imaging Strategies in Patients With Possible Basilar Artery Occlusion Cost-Effectiveness Analysis

Abstract

Background and Purpose This study evaluated the cost-effectiveness of different noninvasive imaging strategies in patients with possible basilar artery occlusion. Methods A Markov decision analytic model was used to evaluate long-term outcomes resulting from strategies using computed tomographic angiography (CTA), magnetic resonance imaging, nonenhanced CT, or duplex ultrasound with intravenous (IV) thrombolysis being administered after positive findings. The analysis was performed from the societal perspective based on US recommendations. Input parameters were derived from the literature. Costs were obtained from United States costing sources and published literature. Outcomes were lifetime costs, quality-adjusted life-years (QALYs), incremental cost-effectiveness ratios, and net monetary benefits, with a willingness-to-pay threshold of $80 000 per QALY. The strategy with the highest net monetary benefit was considered the most cost-effective. Extensive deterministic and probabilistic sensitivity analyses were performed to explore the effect of varying parameter values. Results In the reference case analysis, CTA dominated all other imaging strategies. CTA yielded 0.02 QALYs more than magnetic resonance imaging and 0.04 QALYs more than duplex ultrasound followed by CTA. At a willingness-to-pay threshold of $80 000 per QALY, CTA yielded the highest net monetary benefits. The probability that CTA is cost-effective was 96% at a willingness-to-pay threshold of $80 000/QALY. Sensitivity analyses showed that duplex ultrasound was cost-effective only for a prior probability of 0.02 and that these results were only minimally influenced by duplex ultrasound sensitivity and specificity. Nonenhanced CT and magnetic resonance imaging never became the most cost-effective strategy. Conclusions Our results suggest that CTA in patients with possible basilar artery occlusion is cost-effective.

Abstract

A multicenter study has been employed to evaluate the diagnostic efficacy of magnetic resonance imaging (MRI) using the new liver-specific contrast agent gadoxetic acid (Gd-EOB-DTPA, Primovist), as opposed to contrast-enhanced biphasic spiral computed tomography (CT), in the diagnosis of focal liver lesions, compared with a standard of reference (SOR). One hundred and sixty-nine patients with hepatic lesions eligible for surgery underwent Gd-EOB-DTPA-enhanced MRI as well as CT within 6 weeks. Pathologic evaluation of the liver specimen combined with intraoperative ultrasound established the SOR. Data sets were evaluated on-site (14 investigators) and off-site (three independent blinded readers). Gd-EOB-DTPA was well tolerated. Three hundred and two lesions were detected in 131 patients valid for analysis by SOR. The frequency of correctly detected lesions was significantly higher on Gd-EOB-DTPA-enhanced MRI compared with CT in the clinical evaluation [10.44%; 95% confidence interval (CI): 4.88, 16.0]. In the blinded reading there was a trend towards Gd-EOB-DTPA-enhanced MRI, not reaching statistical significance (2.14%; 95% CI: -4.32, 8.6). However, the highest rate of correctly detected lesions with a diameter below 1 cm was achieved by Gd-EOB-DTPA-enhanced MRI. Differential diagnosis was superior for Gd-EOB-DTPA-enhanced MRI (82.1%) versus CT (71.0%). A change in surgical therapy was documented in 19 of 131 patients (14.5%) post Gd-EOB-DTPA-enhanced MRI. Gd-EOB-DTPA-enhanced MRI was superior in the diagnosis and therapeutic management of focal liver lesions compared with CT.

Abstract

The International Society for Strategic Studies in Radiology ((ISR)-R-3) brings together thought leaders from academia and industry from around the world to share ideas, points of view and new knowledge. This article summarizes the main concepts presented at the 2007 (ISR)-R-3 symposium, providing a window onto trends shaping the future of radiology. Topics addressed include new opportunities and challenges in the field of interventional radiology; emerging techniques for evaluating and improving quality and safety in radiology; and factors impeding progress in molecular imaging and nanotechnology and possible ways to overcome them. Regulatory hurdles to technical innovation and drug development are also discussed more broadly, along with proposals for addressing regulators' concerns and streamlining the regulatory process.
