Abstract

SpectroPhotometric Imaging in Astronomy with Kinetic Inductance Detectors (SPIAKID) aims at designing, building, and deploying on the sky a spectrophotometric imager based on microwave kinetic inductance detectors (MKIDs) in the optical and near-infrared bands. MKIDs show a fast response and the ability to resolve photon energy compared to the conventional Charge-coupled Devices (CCDs). In this paper, we present the design and simulation of the MKID arrays for SPIAKID. The detectors consist of four arrays with each array of 20,000 lumped-element pixels, and each array will be read with 10 readout lines. %The array is designed to have resonances between 4-8GHz with a frequency spacing of 2 MHz and a coupling quality factor (Qc) of about 50000. The meander material of the resonators is trilayer TiN/Ti/TiN to have better uniformity of the critical temperature across the array. We also present the measurement result for a test array with $30\times30$ pixels which is a subset of the designed 2000-pixel array to verify the design and fabrication. The current measured best energy resolving power $R = E/\Delta E$ is 2.4 at $\lambda = 405~$nm and the current medium R is around 1.7. Abstract

We are developing superconducting Microwave Kinetic Inductance Detectors to operate at near infrared and optical wavelengths for astronomy. In order to efficiently meet with the requirements of astronomical applications, we propose to replace the interdigitated capacitor by a metal, insulator, metal capacitor which has the advantage of presenting a larger capacitance value within a much smaller space. The pixel will occupy a space of typically 100 micrometers by 85 micrometers which is nine times less than a typical pixel size using the interdigitated capacitor operating at the same frequency, below 2 GHz.

Abstract

The digitalization of stored information in hospitals now allows for the exploitation of medical data in text format, as electronic health records (EHRs), initially gathered for other purposes than epidemiology. Manual search and analysis operations on such data become tedious. In recent years, the use of natural language processing (NLP) tools was highlighted to automatize the extraction of information contained in EHRs, structure it and perform statistical analysis on this structured information. The main difficulties with the existing approaches is the requirement of synonyms or ontology dictionaries, that are mostly available in English only and do not include local or custom notations. In this work, a team composed of oncologists as domain experts and data scientists develop a custom NLP-based system to process and structure textual clinical reports of patients suffering from breast cancer. The tool relies on the combination of standard text mining techniques and an advanced synonym detection method. It allows for a global analysis by retrieval of indicators such as medical history, tumor characteristics, therapeutic responses, recurrences and prognosis. The versatility of the method allows to obtain easily new indicators, thus opening up the way for retrospective studies with a substantial reduction of the amount of manual work. With no need for biomedical annotators or pre-defined ontologies, this language-agnostic method reached an good extraction accuracy for several concepts of interest, according to a comparison with a manually structured file, without requiring any existing corpus with local or new notations., Comment: In submission

Abstract

Elexacaftor–tezacaftor–ivacaftor has been approved in Europe for people with cystic fibrosis with at least one F508del CFTR variant. Additionally, it is approved by the US Food and Drug Administration (FDA) for people with cystic fibrosis with at least one of 177 rare variants. The aims of this study were to describe the clinical response to elexacaftor–tezacaftor–ivacaftor for people with cystic fibrosis without a F508del CFTR variant in France and to determine CFTR variant responsiveness to elexacaftor–tezacaftor–ivacaftor based on the observed clinical response. The French compassionate programme expanded access to elexacaftor–tezacaftor–ivacaftor to people with cystic fibrosis, aged 6 years and older, without a F508del variant, excluding those with two variants previously characterised as non-responsive. Participants at France's 47 cystic fibrosis centres were given a 4–6 week trial of elexacaftor–tezacaftor–ivacaftor and response was determined by a centralised committee based on evolution of clinical data, lung function, and sweat chloride concentration. Responsiveness of individual CFTR variants was derived from observed clinical responses. The first compassionnate programme was launched on May 19, 2022; by March 8, 2024, 516 people with cystic fibrosis had been identified for inclusion in this real-word study: 37 were not included due to the presence of two variants previously characterised as non-responsive to elexacaftor–tezacaftor–ivacaftor, and 479 (229 females [48%] and 250 males [52%]) received elexacaftor–tezacaftor–ivacaftor for 4–6 weeks. Among 443 participants who received no CFTR modulator before elexacaftor–tezacaftor–ivacaftor, 83 had at least one FDA-approved variant, of whom 81 (98%) were responders and continued elexacaftor–tezacaftor–ivacaftor; in responders, mean absolute change in sweat chloride was –44·5 mmol/L (95% CI –39·1 to –49·8) and percentage of predicted FEV 1 (ppFEV 1) was 11·1 percentage points (95% CI 8·4 to 13·7; both comparisons p<0·0001). Among 360 participants with no FDA-approved variant and no previous CFTR modulator, 177 (49%) were responders; in responders, mean absolute change in sweat chloride was –20·5 mmol/L (–17·2 to –23·8) and ppFEV 1 was 13·2 percentage points (11·4 to 15·0; both comparisons p<0·0001). Among 36 participants who were receiving ivacaftor before elexacaftor–tezacaftor–ivacaftor, 32 (89%) continued elexacaftor–tezacaftor–ivacaftor. Of 251 individual CFTR variants, 64 (28 FDA-approved) were classified as responsive or possibly responsive to elexacaftor–tezacaftor–ivacaftor, and 123 (two FDA-approved) as non-responsive or possibly non-responsive to elexacaftor–tezacaftor–ivacaftor. In France, over half of the population with cystic fibrosis without a F508del variant responded to elexacaftor–tezacaftor–ivacaftor, with most responders having no FDA-approved variant. The treatment period was relatively short and further research is warranted to describe the long-term safety and effectiveness of elexacaftor–tezacaftor–ivacaftor in this population. 22. Embolization of Persistent Type 2 Endoleaks After Endovascular Treatment of Abdominal Aortic Aneurysms: A Study of Long-Term Success

Abstract

Endovascular treatment of abdominal aortic aneurysms (endovascular aneurysm repair [EVAR]) has been shown to be effective, with lower perioperative mortality and morbidity than open surgical treatment in patients with compatible anatomy. This benefit is lost after 3 years of follow-up. This could be explained by the higher rate of reintervention in EVAR patients due to endoleaks. Around half of patients with a persistent type 2 endoleak (T2EL) have an increase in aneurysm size. Either endovascular or conventional surgery techniques have been described to treat this type of endoleak. The aim of this study was to measure the long-term success rate of embolization of T2EL in a pragmatic retrospective series.

Abstract

The European Space Agency's Planck satellite was launched on 14 May 2009, and has been surveying the sky stably and continuously since 13 August 2009. Its performance is well in line with expectations, and it will continue to gather scientific data until the end of its cryogenic lifetime. We give an overview of the history of Planck in its first year of operations, and describe some of the key performance aspects of the satellite. Adherence to medication is important for successful CF management.Objective: To describe medication adherence according to age, therapeutic class, and pharmaceutical form in adults and children followed in four regional CF centers in France.Methods: We conducted a cross-sectional study with non-transplanted patients followed in two adult and two pediatric centers during 2015 who were covered by the French National Health Insurance (NHI). Sociodemographic, clinical, hospitalization, and prescription data were collected from patient medical records. Medication dispensations were extracted from the regional French NHI database. Adherence was calculated over 12 months using continuous medication availability (CMA) accounting for dose adjustments and hospitalizations. Drug-specific CMA was computed in R with the AdhereR package for each medication prescribed more than 3 months, which was averaged to obtain a composite CMA score (cCMA) for all treatments and per therapeutic class as well as pharmaceutical form for each patient.Results: A total of 228 patients were included. 28. Les soins palliatifs au Liban, un essor freiné mais une résilience à toute épreuve

Abstract

L’histoire des soins palliatifs au Liban est marquée par la résilience et la persévérance. Portés par des initiatives personnelles, des combats législatifs et des donations privées, les soins palliatifs ont réussi à s’ancrer petit à petit dans le paysage médical, malgré les maigres moyens proposés par l’État. La délivrance des soins palliatifs au Liban a la particularité d’être soutenue principalement par des ONG à but non lucratif, dont le fonctionnement est à mi-chemin entre HAD et EMSP françaises et qui offrent des soins gratuits aux patients. Plus de 30 % des patients suivis par l’ONG BALSAM souffrent de maladies chroniques non cancéreuses. Les soins palliatifs pédiatriques sont quasi-inexistants, BALSAM étant la seule ONG assurant un accompagnement des enfants à leur domicile. Face aux difficultés financières majeures que connaît le pays, ces ONG se tournent aujourd’hui vers des partenariats internationaux, à la fois pour s’assurer les moyens de poursuivre leurs missions, et pour se réserver une place sur la scène mondiale. Le modèle libanais développé par BALSAM est présenté dans les pays en voie de développement comme un exemple possible de fonctionnement en contexte de crise. Une délégation du centre médicochirurgical de réadaptation des massues (CMCR) à Lyon est par ailleurs intervenue au centre hospitalier Rafik Hariri de Beyrouth pour échanger sur trois thématiques : management, rééducation et soins palliatifs. Les mêmes difficultés que nous rencontrons en France concernant la temporalité d’intervention dans une prise en charge ont pu être relevées : il est peu courant pour les confrères médecins libanais de solliciter les soins palliatifs précocement. La place des soins palliatifs au sein de l’institution hospitalière est encore nouvelle, et les moyens alloués trop faibles, autant en temps soignants qu’en locaux adaptés. La route reste longue, mais tout est mis en œuvre pour que les soins palliatifs continuent de faire parler d’eux et de s’étendre toujours plus profondément au sein du système sanitaire libanais.

Abstract

IntroductionInterstitial lung disease in children (chILD) are rare and mostly severe lung diseases. Very few epidemiological data are available in limited series of patients. The aim of this study was to assess the prevalence and incidence of chILD in France.MethodsWe performed within the RespiRare network a multicentre retrospective observational study in patients with chILD from 2000 to 2022 and a prospective evaluation of chILD’s incidence between February 2022 and 2023.ResultschILD was reported in 790 patients in 42 centres. The estimated 2022 prevalence in France was 44 /million children (95% CI 40.76 to 47.46) and the computed incidence was 4.4 /million children (95% CI 3.44 to 5.56). The median age at diagnosis was 3 months with 16.9% of familial forms. Lung biopsy and genetic analyses were performed in 23.4% and 76.9%, respectively. The most frequent chILD aetiologies in the <2 years group were surfactant metabolism disorders (16.3%) and neuroendocrine cell hyperplasia of infancy (11.8%), and in the 2–18 years group diffuse alveolar haemorrhage (12.2%), connective tissue diseases (11.4%), hypersensitivity pneumonitis (8.8%) and sarcoidosis (8.8%). The management included mainly oxygen therapy (52%), corticosteroid pulses (56%), oral corticosteroids (44%), azithromycin (27.2%), enteral nutrition (26.9%), immunosuppressants (20.3%) and hydroxychloroquine (15.9%). The 5-year survival rate was 57.3% for the patients diagnosed before 2 years and 86% between 2 and 18 years.ConclusionThis large and systematic epidemiological study confirms a higher incidence and prevalence of chILD than previously described. In order to develop international studies, efforts are still needed to optimise the case collection and to harmonise diagnostic and management practices.

45. Late-onset pulmonary complications following allogeneic hematopoietic cell transplantation in pediatric patients: a prospective multicenter study

Abstract

The primary objective of our multicenter prospective study was to describe the incidence of late-onset non-infectious pulmonary complications (LONIPCs) in children undergoing hematopoietic cell transplantation (HCT) using sensitive criteria for pulmonary function test (PFT) abnormalities including the non-specific pattern of airflow obstruction. Secondary objectives were to assess the factors associated with LONIPC occurrence and the sensitivity of the 2014 NIH-Consensus Criteria of bronchiolitis obliterans syndrome (BOS). PFT and clinical assessment were performed prior to HCT and at 6, 12, 24, and 36 months post-HCT. LONIPC diagnosis was validated by an Adjudication Committee. The study comprised 292 children from 12 centers. Thirty-two individuals (11%, 95% CI: 8–15%) experienced 35 LONIPCs: 25 BOS, 4 interstitial lung diseases, 4 organizing pneumonia and 2 pulmonary veno-occlusive diseases. PFT abnormalities were obstructive defects (FEV1/FVC z-score < −1.645; n= 12), restrictive defects (TLC < 80% predicted, FEV1and FVC z-scores < −1.645; n= 7) and non-specific pattern (FEV1and FVC z-score< −1.645, FEV1/FVC z-score > −1.645, and TLC > 80% predicted; n= 8). HCT for malignant disease was the only factor associated with LONIPC (P= 0.04). The 2014 NIH-Consensus Criteria would only diagnose 8/25 participants (32%) as having BOS.
