When referring to the study of Rutherford et al. (2015) on short- and long-term effects of rTMS in Alzheimer's disease, we erroneously stated in our article (Lefaucheur et al., 2020) that a clinical benefit on cognitive performance (tested by the Montreal Cognitive Assessment) was observed at weeks 2–3, only in the real stimulation condition during an initial sham-controlled 4-week period of treatment, and that then this benefit was prolonged by “2 additional weeks” of open-label real rTMS. In fact, this extended open-label treatment was administered for up to 19 months in multiple 2-week blocks with 2–7 months intervals between blocks and was not limited to a single additional 2-week block of treatment immediately following the original 4 weeks of treatment. Therefore, the results of that open-label extended follow-up study support the value of long-term maintenance treatment using multiple rTMS sessions rather than be interpreted as short-term 2-week extension.

Decreased/Poor vascular health - e.g. increased vascular resistance, tortuosity - may lead to a delayed arrival of labelled blood to brain tissue causing vascular artefacts on arterial spin labeling (ASL) images [Figure 1]. Although these artefacts are traditionally treated as a nuisance, their presence can be quantified by the spatial coefficient of variance (CoV) parameter and used as an indication of vascular insufficiency. . Here, the goal was to investigate the ability of spatial CoV to assess the vascular health by comparing it between healthy controls and subjects with Alzheimer’s (AD) and Parkinson’s (PD) diseases. Methods: We analyzed the MRI scans of 143 APGeM study participants, including healthy controls (HC, n=56) and subjects with AD- (n=41) or PD-related (n=46) mild cognitive impairment or dementia [Table]. We calculated CBF, spatial CoV, and WMH volume using ExploreASL [ref]. Pearson’s correlations of spatial CoV with age and WMH volume were investigated, as well as a t-test for the relation between spatial CoV and sex. A linear regression model was used to evaluate whether spatial CoV was able to discriminate HC vs. AD, HC vs. PD, and AD vs. PD after correction for age and sex. Background: In elderly populations, cerebral blood perfusion (CBF) measurements with arterial spin labelling (ASL) are affected by macro-vascular artefacts as a result of prolonged arterial transit time (ATT) [Figure 1]. The spatial coefficient of variance (CoV) provides a robust indirect estimate of ATT, hence it might be used as a proxy of the vascular health of the subject2, alone or in conjunction with other measures such as white matter hyperintensities (WMH) volume. The goal of this study was to compare spatial CoV between healthy controls (HCs) and subjects with Alzheimer’s (AD) or Parkinson’s (PD) disease. Historically, magnetic resonance imaging (MRI) has contributed little to the study of Parkinson's disease (PD), but modern MRI approaches have unveiled several complementary markers that are useful for research and clinical applications. Iron- and neuromelanin-sensitive MRI detect qualitative changes in the substantia nigra. Quantitative MRI markers can be derived from diffusion weighted and iron-sensitive imaging or volumetry. Functional brain alterations at rest or during task performance have been captured with functional and arterial spin labeling perfusion MRI. These markers are useful for the diagnosis of PD and atypical parkinsonism, to track disease progression from the premotor stages of these diseases and to better understand the neurobiological basis of clinical deficits. A current research goal using MRI is to generate time-dependent models of the evolution of PD biomarkers that can help understand neurodegeneration and provide reliable markers for therapeutic trials. This article reviews recent advances in MRI biomarker research at high-field (3T) and ultra high field-imaging (7T) in PD and atypical parkinsonism.

The discovery of α-synuclein (α-syn) as a major component of Lewy bodies, neuropathological hallmark of Parkinson's disease (PD), dementia with Lewy bodies and of glial inclusions in multiple system atrophy initiated the investigation of α-syn as a biomarker in cerebrospinal fluid (CSF). Due to the involvement of the periphery in PD the quantification of α-syn in peripheral fluids such as serum, plasma and saliva has been investigated as well. We review how the development of multiple assays for the quantification of α-syn has yielded novel insights into the variety of α-syn species present in the different fluids; the optimal preanalytical conditions required for robust quantification and the potential clinical value of α-syn as biomarker. We also suggest future approaches to use of CSF α-syn in neurodegenerative diseases. © 2015 Future Medicine Ltd.

We report the case history of a 75-year-old woman with Parkinson's disease who developed severe cognitive problems after deep brain stimulation (DBS) of the bilateral subthalamic nuclei (STN). After a brief cognitive improvement, the patient gradually deteriorated until she developed full-blown dementia. We discuss the case with respect to the cognitive effects of STN DBS and the possible risk factors of dementia after STN DBS surgery.
