Abstract Background Renal leiomyoma is an extremely rare benign tumour of the kidney. It is mostly asymptomatic. Our aim was to report the first case of symptomatic renal leiomyoma in Nigeria which was suspected to be renal cell carcinoma and our objective was to document the clinical, radiological, histological and treatment of this rare lesion. Case presentation We describe a 46-year-old woman with a two-week history of right flank pain and intermittent haematuria. The examination was remarkable for the ballotable right kidney. Abdominopelvic sonogram and computed tomography were suggestive of cystic right renal mass with suspicion of right renal cell carcinoma. She had surgical resection of the mass and histopathological analysis showed renal leiomyoma. Conclusion We have illustrated our experience on the first case of symptomatic renal leiomyoma masquerading as cystic renal cell carcinoma in Nigeria. A high index of suspicion is required for accurate diagnosis. This may go a long way in considering kidney-sparing surgery rather than radical nephrectomy.

Introduction: Renal leiomyoma is a rare renal tumor that originates from smooth muscle. Among all existing benign renal tumors, leiomyoma is one of the least common benign renal tumors., Case Presentation: We report of a case report of a 43-year-old male complaints of palpable mass on the upper left abdomen, abdominal discomfort and hematuria. Contrast-enhanced CT scan revealed a solid heterogenous mass on the left kidney, adherent to the left abdominal wall and pushed the spleen cranially. Patient underwent radical nephrectomy and histopathology results revealed leiomyoma. Two weeks after surgery, the patient was asymptomatic., Discussion: Imaging of renal leiomyoma may provide a clue with a general finding of a well-defined tumor margin and no local invasion. In our case preoperative CT findings made the initial diagnosis inconclusive since it showed the renal mass was adherent to the abdominal wall. Renal leiomyomas are rare benign mesenchymal tumors that arise from the smooth muscle cells in the renal capsule, renal pelvis, and the smooth muscles of vessels in the kidney. They are usually found by accident during autopsies or on different imaging modalities made on other occasions. The clinical presentation may include hematuria and abdominal or flank pain, although renal leiomyomas are most frequently asymptomatic. This case concerns a 45-year-old woman with an asymptomatic tumor mass in the left kidney, discovered incidentally on an outpatient CT scan conducted on an unrelated occasion. Histological examination revealed well-demarcated tumor formation on regular hematoxylin and eosin (H&E) staining consisting of intersecting fascicles of spindle cells with blunt-ended, cigar-shaped nuclei and eosinophilic cytoplasm. The neoplasm showed no mitotic activity. Immunohistochemistry (IHC) was performed to exclude different spindle cell pathologies, such as solitary fibrous tumor of the kidney, primary leiomyosarcoma, and, less probably, sarcomatoid variants of renal cell carcinoma (RCC). Tumor cell cytoplasm was positive for h-caldesmon, smooth muscle actin (SMA), and desmin. The proliferation index Kiel-67 (Ki-67) was evaluated as less than 1%. Considering the microscopic description and the IHC results, the neoplasm was interpreted as a renal leiomyoma. In light of the rarity of these tumors, pathologists should consider differential diagnosis with precision. In challenging cases, immunohistochemical examination can be beneficial to distinguish between several spindle cell renal neoplasms, such as solitary fibrous tumor, leiomyosarcoma, sarcomatoid variant of RCC, fat-poor angiomyolipoma, benign peripheral nerve sheath tumors, renomedullary interstitial cell tumor, benign fibrous histiocytoma, primary angiosarcoma, mucinous tubular and spindle cell renal cell carcinoma (MTSRCC)., Competing Interests: Human subjects: Consent was obtained or waived by all participants in this study. Abstract Background Renal leiomyomas are exceptionally rare, benign, mesenchymal tumours originating from smooth muscle in the kidney. Historically, because of their small size, most renal leiomyoma cases were discovered incidentally based on autopsy findings. However, since the advent and improved access to imaging modalities such as ultrasound and computed tomography (CT), renal leiomyomas are being discovered more frequently. Although usually incidental discoveries, clinical presenting signs and symptoms comprise abdominal or flank pain, a palpable flank mass, and haematuria in 20% of those with symptoms. Case presentation We study the case of an incidentally found, asymptomatic, left kidney mass that presented in a 60-year-old female. Initial suspicions on CT imaging of either renal cell carcinoma or oncocytoma resulted in a radical nephrectomy of the left kidney. Postoperative pathological examination of the mass revealed a renal leiomyoma; a rare, benign tumour that is mostly indistinguishable from malignant tumours on imaging. Conclusions With the current availability of ultrasonography and CT, they are often discovered incidentally, and the radiological differential diagnoses are often inadequate or challenging in such cases. The gold standard management of these suspicious cancer cases is still a radical nephrectomy with postoperative pathological and immunohistochemical analysis. Due to its benign nature, patients enjoy excellent prognoses without recurrence. We discuss and briefly review the relevant literature of the clinical, imaging and pathological features of renal leiomyomas and those of the differential diagnoses.

Renal leiomyoma is extremely rare benign lesion with low incidence (1:1000), has been reported mostly in adults and very few cases have been described in the pediatric age-group. It is often asymptomatic and can be diagnosed when reaches large sizes. Hereby, we present a case of renal leiomyoma in a smaller size, in the pediatric age group.

Only the histopathology and immunohistochemistry confirmed the diagnosis of RL. Even though partial nephrectomy may be possible, radical nephrectomy remains the recommended treatment for large leiomyoma.

Leiomyomas are rare, benign and solid tumors of the kidney. Although the developments in radiological imaging methods provide early detection of kidney tumors, it is difficult to differentiate leiomyomas radiologically from other malignant renal tumors. Moreover, the definitive diagnosis of leiomyomas can only be achieved by histopathological and also immunohistochemical evaluation after surgical intervention. Immunohistochemically, positive staining with smooth muscle actin and vimentin, whereas negative staining with cytokeratin, S100, Mart1 and HMB45 are the methods used in the differential diagnosis of leiomyomas. In this case series, after preoperative radiological evaluation, two female and two male patients between 45 and 89 years of age underwent laparoscopic treatment with the diagnosis of a malignant mass in the kidney. We aimed to illustrate the clinical, radiological and histopathological relationship of four adult patients who were managed by laparoscopic approach and diagnosed with renal leiomyoma. Leiomyoma is an uncommon tumor of the kidney. The authors report a rare case of renal leiomyoma in a 39-year-old male patient who presented with a right flank mass. Laparoscopic nephrectomy was performed. The histopathology and immunohistochemistry confirm the diagnosis of renal leiomyoma. The review of literature in the clinicoradiopathological correlation was illustrated.

Background: Primary renal leiomyoma was confirmed as a rare benign tumor formed by smooth muscle fibers and usually asymptomatic. Purpose: to describe a subcapsular cortical renal leiomyoma with probable vascular origin. Materials and methods: case report of a patient with pain in the left flank with imaging exploration (ultrasonoangiography, abdominal CT and MRI) and microscopic and immunohistochemically postoperatively evaluation. Results: a well-defined solid nodular tumor of about 40mmx50mmx45mm was observed in the upper polar renal cortex, beneath the renal capsule. The tumor mass was poorly vascularized, non-invasive and of benign aspect, hypoechogenic and isodense with renal cortex, with hypo intense heterogeneous signal in T2-weighted image, with fibrous/muscular aspect, and central hypodense area (potential necrosis). Histopathology (fusiform bundled smooth muscle cells) and immunohistochemistry of the tumor cells (positive to actin and negative to MB45) supported the diagnosis of renal leiomyoma. Postoperative evolution was normal at 3 months with no recurrence. Conclusions: Histopathology results confirmed the vascular origin for subscapular leiomyoma. Intratumoral necrosis is associated to degenerative lesions of the muscle fibers. The present tumor mass had an absolute surgical indication. [ABSTRACT FROM AUTHOR]

Abstract: INTRODUCTION: Leiomyoma of kidney is an exceptional finding in the urinary tract. Though most of these tumors are detected on autopsy, still it remains a diagnostic challenge. PRESENTATION OF CASE: We report three adult cases of renal leiomyoma in our institute who presented with abdominal mass and haematuria in last 8 years. DISCUSSION: Leiomyoma has been reported in other organs but renal leiomyoma is rare tumour, though in autopsy series they are second in order of frequency among the renal mesenchymal neoplasms, and few cases have been described in the literature. Still no Indian experience is evaluated on this rare site of occurrence. CONCLUSION: Considering the difficulty of clinical diagnosis of this tumour, strong suspicion is indicated when a patient presents with voluminous, well circumscribed renal lesions. Abstract: Renal leiomyoma is a rare benign smooth muscle tumor of the kidney. Most cases are reported in adults, with very few cases described in children. We report a case of right renal leiomyoma in a 6-year-old boy which was suspected of being a Wilms tumor. Contrast-enhanced computed tomography could not differentiate leiomyoma in the kidney, and the mass was diagnosed as a Wilms tumor. Right radical nephrectomy was performed. Diagnosis of renal leiomyoma could only be achieved after histopathologic examination and immunohistochemistry. We recommend that total nephrectomy has to be done in most cases of pediatric renal tumors to avoid the risk of malignancy. There may be a role for nephron sparing surgery, provided the tumor is small, and one has a strong suspicion for the lesion being benign. [ABSTRACT FROM AUTHOR]
