OBJECTIVES Pediatric patients are at higher risk of nonadherence to immunosuppressive medication after kidney transplant and the resulting adverse outcomes. Factors associated with nonadherence vary, which follow an epidemiological framework and according to health system patterns. The Brazilian public health system covers all costs of kidney transplant, including immunosuppressive medications. We aimed to assess the prevalence and correlates of nonadherence to immunosuppressive medications in a pediatric kidney transplant population who received free access to immunosuppressive medications within the health care system. MATERIALS AND METHODS In this single-center crosssectional study, we studied a convenience sample of 156 outpatients (< 18 years old) who were a minimum of 4 weeks posttransplant. Implementation nonadherence to immunosuppressive medications was measured by the 4 questions of the Basel Assessment of Adherence to Immunosuppressive Medications Scale. Multilevel correlates to non - adherence (patient, micro, and macro levels) were assessed. RESULTS In our patient population, 61% were males, mean age was 13.6 ± 3.1 years, 77% were adolescents, and 84% received organs from deceased donors. We found that 33% were nonadherent to immuno - suppressive medications, mainly in timing (25%) and taking (10.9%) dimensions. Being an adolescent (odds ratio: 2.66; CI, 1.02-6.96), religion other than Catholic or Protestant (odds ratio: 4.33; CI, 1.13-16.67), and family income higher than 4 reference wages (odds ratio: 3.50; CI, 1.14-10.75) were factors associated with nonadherence. CONCLUSIONS In our patient population of mostly adolescents, one-third displayed nonadherence to immunosuppressants. Unexpectedly, a higher economic profile, potentially representing better previous access to health care, was independently associated with nonadherence. This result highlights the need for identifying specific correlates to non - adherence before designing interventions.

Background The relevance that adherence to treatment plays in liver transplantation, and the impact this factor may have on the success of treatment, are fundamental in assessing the variables which affect patient adherence during the pretransplantation period. Objectives This study aims to determine factors that affect liver transplant candidates' adherence to treatment, and analyze the association between adherence, socioeconomic and demographic factors, clinical characteristics, and patient understanding about the disease and liver transplantation. Methods This epidemiological, observational, and prospective study included 62 patients registered in the technical database of the Universidade Federal de Sao Paulo during the period November 2012–May 2014. The dependent variable was adherence to treatment among liver transplantation candidates, and the independent variables included understanding the disease and the transplantation process, and depression and anxiety symptoms among liver transplantation candidates. Results Work situation ( P = .038), understanding about the disease ( P = .002), and use of laxatives ( P = .045) were the factors related to statistically significant adherence, and it can be concluded that implementation of an educational program may increase adherence up to 3.48 times in the pretransplantation phase, as adherence was 3.48 times lower in patients who reported little or no knowledge of the disease or the procedure.

Resumo Objetivo: Mensurar a adesão aos medicamentos imunossupressores em receptores de transplante renal, utilizando a Basel Assessment of Adherence with Immunosuppressive Medication Scale - BAASIS® e descrever as características sociodemográficas, os fatores clínicos em relação aos imunossupressores e os níveis de creatinina. Métodos: Estudo prospectivo, transversal, realizado no período de 2014 a 2015, no Ambulatório Pós-Transplante do Hospital do Rim (complexo hospitalar da Universidade Federal de São Paulo (UNIFESP)). Resultados: Dos 181 participantes, 53,6% eram homens e a idade dos receptores variou de 18 a 74 anos. A Hipertensão Arterial Sistêmica foi o diagnóstico inicial de Insuficiência Renal Crônica em 38,7% dos receptores. Aproximadamente 95% dos receptores relataram que nunca se esqueceram de tomar o medicamento; 58,6% dos pacientes relataram ter aderido totalmente aos imunossupressores e 41,4% não aderiram a uma ou mais das quatro situações avaliadas pelos itens da BAASIS®. Não foi encontrada correlação significativa pelos itens da BAASIS® entre os níveis de creatinina e o escore total. Conclusão: Foi identificado um alto índice de pacientes que não aderiram ao tratamento imunossupressor (41,4%). Não houve correlação significativa entre o nível de creatinina e a idade do receptor no momento do TxR, bem como o tempo após TxR, tempo de separação do imunossupressor e escore total, e escores dos itens individuais da BAASIS®. Esse estudo mostrou que os receptores negros, do sexo masculino, sem uma rede de apoio familiar, obesos e autônomos são mais propensos a não aderir à terapia imunossupressora. A avaliação da adesão do receptor de transplante renal deve ser considerada durante todas as fases das ações do processo de enfermagem que compõem o plano terapêutico após o transplante renal. Resultados: de los 181 participantes, el 53,6% eran hombres; la edad de los receptores varió entre los 18 y los 74 años. Abstract Objective: To measure adherence of renal transplant recipients to immunosuppressive drugs by using the Basel Assessment of Adherence with immunosuppressive medication Scale (BAASIS®) and to describe sociodemographic characteristics and clinical factors in relation to immunosuppressive drugs and creatinine levels. Methods: This retrospective, cross-sectional study was carried out from 2014 to 2015 in the post-transplantation ambulatory unit of the Kidney Hospital and Universidade Federal de São Paulo (UNIFESP). Results: Of the 181 participants, 53.6% were men; recipients ages ranged from 18 to 74 years. Systemic arterial hypertension was the initial diagnosis of chronic renal failure in 38.7% of recipients. About 95% reported that they never forgot to take several consecutive doses of the medicine, 58.6% of recipients reported total adherence to the immunosuppressive drugs, and 41.4% did not adhere to one or more of the four assessed situations. No significant correlation was found between of creatinine levels and total score and by BAASIS® items. Conclusion: A high rate of patients not adherent to the immunosuppressive treatment was identified (41.4%). There was no significant correlation between creatinine level and receptor age at time of KTP, time after KTP, immunosuppressive separation time and total score and scores of individual BAASIS® items. This study showed that black, male recipients without a family support network, obese and autonomous are more likely not to adhere to immunosuppressive therapy. The evaluation of renal transplant recipient adhesion should be considered during all phases of the nursing process actions that make up the therapeutic plan after renal transplantation.

Background Few studies have investigated whether quality initiatives in the process of organ donation yield better results of the organ donation process. Objective To analyze whether the indicators of the organ donation process in Brazilian hospitals meet the standards established by the Organ Donation European Quality System (ODEQUS). Design We evaluated the quality of the organ donation in a selected group of Brazilian hospitals served by the Nucleus of Organ Procurement (NOP) using standards of the ODEQUS. Results Structural and process indicators had 100% conformity. Indicators of results showed a family consent rate of 61% (29% lower than ODEQUS goal); a conversion rate of potential donors to effective donors of 47% (28% below the goal); and a 12% rate of sudden cardiac arrest (higher than the quality limit). Conclusions Our findings highlight the importance for the development of quality initiatives in identifying gaps and weaknesses in the process that should be corrected or even restructured, therefore maximizing the number of donors and organs transplanted. Hospitals that participate in the NOP process met 61% of the quality indicators proposed by ODEQUS. Identification of potential donors, family consent, conversion, and sudden cardiac arrest rates are areas that did not conform to ODEQUS standard and revealed a great opportunity for improvement.

OBJECTIVE To evaluate the viability of a professional specialist in intra-hospital committees of organ and tissue donation for transplantation. METHODS Epidemiological, retrospective and cross-sectional study (2003-2011 and 2008-2012), which was performed using organ donation for transplants data in the state of Sao Paulo, Southeastern Brazil. Nine hospitals were evaluated (hospitals 1 to 9). Logistic regression was used to evaluate the differences in the number of brain death referrals and actual donors (dependent variables) after the professional specialist started work (independent variable) at the intra-hospital committee of organ and tissue donation for transplantation. To evaluate the hospital invoicing, the hourly wage of the doctor and registered nurse, according to the legislation of the Consolidation of Labor Laws, were calculated, as were the investment return and the time elapsed to do so. RESULTS Following the nursing specialist commencement on the committee, brain death referrals and the number of actual donors increased at hospital 2 (4.17 and 1.52, respectively). At hospital 7, the number of actual donors also increased from 0.005 to 1.54. In addition, after the nurse started working, hospital revenues increased by 190.0% (ranging 40.0% to 1.955%). The monthly cost for the nurse working 20 hours was US$397.97 while the doctor would cost US$3,526.67. The return on investment was 275% over the short term (0.36 years). CONCLUSIONS This paper showed that including a professional specialist in intra-hospital committees for organ and tissue donation for transplantation proved to be cost-effective. Further economic research in the area could contribute to the efficient public policy implementation of this organ and tissue harvesting model. OBJETIVO Avaliar a viabilidade de profissional especialista em comissões intra-hospitalares de doação de órgãos e tecidos para transplantes. MÉTODOS Estudo epidemiológico, retrospectivo e transversal (2003 a 2011 e 2008 a 2012), realizado com dados de doação de órgãos para transplantes no estado de São Paulo. Foram avaliados nove hospitais (hospitais 1 a 9). Foram avaliadas, por regressão logística, diferenças em número de notificações de morte encefálica e doadores efetivos (variáveis dependentes) após a entrada do profissional especialista (variável independente) na comissão intra-hospitalar de doação de órgãos e tecidos para transplantes. Para avaliação do faturamento hospitalar, foram calculados o salário-hora de médico e enfermeiro, conforme legislação da Consolidação das Leis do Trabalho, o retorno e o prazo do retorno do investimento. RESULTADOS Após a entrada de enfermeiro especialista na comissão, as notificações de morte encefálica e o número de doadores efetivos aumentaram no hospital 2 (4,17 e 1,52, respectivamente). No hospital 7, o número de doadores efetivos também aumentou de 0,005 para 1,54. E, após entrada de enfermeiro especialista, o faturamento hospitalar aumentou em 190,0% (variação de 40,0% a 1.955%). O custo mensal para 20 horas semanais do enfermeiro foi R$940,00 e, do médico, R$8.330,00. O retorno do investimento foi de 275%, em curto prazo (0,36 anos). CONCLUSÕES A inserção de profissional especialista nas comissões intra-hospitalares para captação de doação de órgãos e tecidos para transplantes mostra-se custo-efetiva. Novos estudos econômicos na área podem contribuir com uma política pública eficiente de implantação desse modelo de captação de órgãos e tecidos para transplantes.

OBJETIVO: Identificar a frequência de diabetes mellitus e a presença de fatores de risco cardiovascular em pacientes pediátricos após um ano de transplante renal. MÉTODOS: Estudo retrospectivo, de cunho documental e exploratório, realizado de janeiro de 2000 a janeiro de 2006, abrangendo 111 prontuários de pacientes pediátricos (0 a 18 anos incompletos) submetidos a transplante renal no Hospital do Rim e Hipertensão e no Hospital São Paulo da Universidade Federal de São Paulo. RESULTADOS: Foram analisados 111 pacientes, 50,5% utilizavam anti-hipertensivos antes do transplante renal. Um ano após este número caiu para 28%. No pré-transplante 13,5% pacientes apresentaram sobrepeso e após um ano não houve alteração importante (12,6%). O número de pacientes obesos aumentou 50% após um ano de transplante renal. Aproximadamente 1% das crianças desenvolveram diabetes mellitus pós-tranplante renal. CONCLUSÃO: A presença de excesso de peso (sobrepeso e obesidade), hipertensão arterial e diabetes mellitus são freqüentes em pacientes pediátricos pós-tranplante renal. OBJECTIVE: To identify the prevalence of diabetes mellitus and the presence of cardiovascular risk factors in pediatric patients after one year of renal transplant. METHODS: This was an exploratory retrospective study. Data were collected from 111 medical records of pediatric patients (aged 0 to incomplete 18 year old) who underwent renal transplant at both the "Hospital do Rim and Hypertension (Renal and High Blood Pressure Hospital) and the São Paulo Hospital of the Federal University of São Paulo between January 2000 and January 2006. RESULTS: A half of patients (50.5%) used high blood pressure medications prior to undergoing renal transplant. A year after renal transplant the use of high blood pressure medications by the pediatric patients was reduced by 28%. Before transplant, 13.5% of the patients were overweighed and did not have any significant changes in their weight after the transplant (12.6%). There was an increase in the number of overweight patients after a year of renal transplant by 50.0%. Approximately 1.0% of the patients developed diabetes mellitus after the renal transplant. CONCLUSION: Obesity, high blood pressure, and diabetes mellitus were common conditions among pediatric patients after renal transplant. MÉTODOS: Estudio retrospectivo, de orden documental y exploratorio, realizado de enero de 2000 a enero de 2006, abarcando 111 fichas de pacientes pediátricos (0 a 18 años incompletos) sometidos a trasplante renal en el Hospital del Riñón e Hipertensión y en el Hospital Sao Paulo de la Universidad Federal de Sao Paulo. RESULTADOS: Fueron analizados 111 pacientes, 50,5% utilizaban anti-hipertensores antes del trasplante renal. Un año después este número cayó para 28%. En el pre-trasplante, 13,5% pacientes presentaron sobrepeso y después de un año no hubo alteración importante (12,6%). El número de pacientes obesos aumentó 50% después de un año de trasplante renal. Aproximadamente 1% de los niños desarrollaron diabetes mellitus después del trasplante renal. CONCLUSIÓN: La presencia de exceso de peso (sobrepeso y obesidad), hipertensión arterial y diabetes mellitus son frecuentes en pacientes pediátricos después del trasplante renal.
