This is an observational and cross-sectional study, carried out in May 2020, targeting adult individuals of both sexes who are members of multiprofessional teams working in Brazilian HSCT units in the current period of the pandemic by completing and analyzing a questionnaire. pre-formulated. HSCT units that cannot access the questionnaire were excluded from the study. The analysis of the operation profile of HSCT units in Brazil, through the application of a pre-structured questionnaire, is not an accurate tool, since it assumes some premises that may prove to be wrong, especially in this current scenario in Brazil. However, the data reveal the vulnerability of patients with onco-hematological diseases to infection by COVID-19, especially during HSCT procedures, in relation to the general population. Despite its limitations, it can be valuable to plan policies.

In 2021 the Brazilian society of stem cell transplantation and cell therapy published the consensus guidelines regarding hematopoietic stem cell transplantation (HSCT) for hemoglobinopathies. No changes have been added for thalassemia. HSCT with a matched sibling donor (MSD) or a related cord blood is the treatment of choice for young patients with transfusion dependent thalassemia. Matched unrelated and haploidentical HSCT, using bone marrow graft, are a clinical option. For sickle cell disease (SCD), conditioning regimen should be myeloablative for patients < 16 years old. For adults, fludarabine, busulfan and ATG is a safe and effective regimen. The chemo-free regimen with alemtuzumab and TBI, pioneered by the NIH group, was successfully reproduced by other centers and is a good option for adults and patients with established organ damage. Haploidentical HSCT with post-transplant cyclophosphamide showed high rejection rates in the beginning but improvements in the conditioning regimen were performed, including the use of pre-transplant immunosuppression, the increase in TBI dose from 2 cGy to 4 cGy and the addition of thiotepa, which significantly reduced rejection rates. We consider haploidentical transplant as a clinical option in children with significant neurological alteration and in adults with the established indications. The optimal timing for HSCT in SCD children with MSD is not well established. Previous international reports showed excellent outcomes in young children. The risk of grade 2-4 acute graft-versus-host disease (GVHD) and chronic GVHD is significantly higher in patients older than 15 years old. Despite potential complications of HSCT (GVHD, gonadal dysfunction), transplantation at an earlier age may prevent organ dysfunction, strokes, iron overload and improve patients’ quality of life. Therefore, an early referral to HSCT is strongly recommended. The medical dilemma in older SCD patients, will be the assessment of established organ damage and the risk of transplantation. The clinical course of SCD is extremely variable and no validated genetic risk score has been established so far. Most of the risk scores use phenotypic characteristics together with laboratory biomarkers and imaging parameters to define outcomes in SCD.

Telômeros são sequências de nucleotídeos que selam o final dos cromossomos, protegendo-os de danos. O reparo telomérico defeituoso nas células-tronco hematopoéticas pode desencadear doenças graves relacionadas à falência das células-tronco, como a anemia aplástica grave (AAG) congênita ou adquirida. Para os pacientes menores de 40 anos, com doador HLA (antígeno leucocitário humano - do inglês: Human leukocyte antigen) idêntico, o transplante de medula óssea (TMO) alogênico é considerado terapia de primeira linha. A resposta ao TMO depende da ocorrência ou não de complicações relacionadas ao tratamento como: falha de enxertia, doença do enxerto contra o hospedeiro, toxicidade relacionada ao TMO. O comprimento telomérico é um marcador de senescência e capacidade replicativa das células. Dessa forma, o estudo do comprimento telomérico é oportuno no ambiente de alta proliferação celular como o transplante. O presente estudo incluiu sessenta e seis pacientes, que realizaram TMO alogênico para AAG e seus doadores relacionados ou não relacionados que apresentavam amostras de DNA avaliáveis pré-transplante, entre 1992 e 2014, em dois centros de TMO. O comprimento telomérico dos leucócitos do sangue periférico de receptores e doadores pré-TMO foram avaliados pelo método real-time polymerase chain reaction (qPCR) e classificados em tertis. O objetivo deste estudo foi avaliar a associação do comprimento telomérico ajustado para idade de doadores e receptores de TMO para AAG com os desfechos clínicos após o transplante. Não houve associação entre o comprimento telomérico do receptor e do doador e enxertia de neutrófilos e plaquetas, doença do enxerto contra hospedeiro (DECH) crônica, falha de enxertia tardia, toxicidade relacionada ao TMO e sobrevida global. Em estudo prévio, o telômero curto do paciente com AAG foi associado com menor sobrevida global e maior recaída da doença durante o tratamento imunossupressor. No presente estudo, não houve associação entre menor comprimento telomérico do receptor e doador e menor sobrevida global no contexto de TMO alogênico para AAG em sua maioria com doadores relacionados. Em conclusão, o TMO alogênico relacionado permanece como melhor opção terapêutica para os pacientes jovens com diagnóstico de AAG e presença de telômero curto. Telomeres are nucleotide repeats at the ends of the chromosomes, protecting them from damage and shortening. The defective telomere repair in hematopoietic stem cells can trigger serious diseases related to the bone marrow failure, such as severe aplastic anemia (SAA) congenital or acquired. Aplastic anemia is characterized by peripheral blood cytopenias and reduction of progenitors in bone marrow. For patients younger than 40 years, with identical HLA donor, hematopoeitic stem cell transplantation (HSCT) is considered first-line therapy. The outcomes of HSCT depend on the occurrence of treatment-related complications such as graft failure, graft-versus-host disease and toxicities related HSCT. The telomere length is a marker of senescence and replicative capacity of cells. Thus, the evaluation of telomere length is appropriate in high proliferation as cell transplantation environment. This study included sixty six consecutive patients underwent HSCT for SAA and donors who had pre - HSCT blood samples from two centers between 1992 and 2014. Recipient and donor pre - HSCT blood leukocytes telomere length were measured by real - time quantitative PCR and classified into tertiles. The aim of this study was to evaluate the association between age adjusted recipient and donor telomere length with clinical outcomes after transplantation. There was no association between donor and recipient telomere length and neutrophil and platelet engraftment, chronic graft-versus-host disease (GVHD), late graft failure and overall survival. In a previous study, the short telomere length patient with AAG receiving immunosuppressive therapy was associated with risk of relapse and overall survival. In this study, there was no association between recipient and donor short telomere length and overall survival in allogeneic HSCT for AAG (mostly related donors). In conclusion, HLA-matched sibling donor transplants for young patients with severe acquired aplastic anemia and short telomere remains the standard treatment.

O transplante de células-tronco hematopoiéticas (TCTH) é uma das terapias mais importantes em hematologia. Entretanto, trata-se de um procedimento dispendioso, associado a morbimortalidade e que envolve uma farmacoterapia complexa. Neste cenário, destaca-se a possibilidade de contribuição do farmacêutico clínico. Este estudo objetivou avaliar o impacto da inserção do farmacêutico clínico na equipe de TCTH alogênico nos desfechos: mortalidade relacionada ao transplante, falha de enxertia, incidência de doença do enxerto contra o hospedeiro (DECH), tempo de internação, tempo para enxertia, número de reinternações, número de Problemas Relacionados à Farmacoterapia (PRFs), adesão e conhecimento sobre a farmacoterapia. Trata-se de um estudo de intervenção com controle histórico em que o farmacêutico atuou no grupo intervenção durante as fases: pré-TCTH, TCTH propriamente dito e pós-TCTH imediato. O acompanhamento farmacoterapêutico consistiu na monitorização diária (dias úteis) das prescrições e da condição clínica dos pacientes, identificação, monitorização e resolução de PRFs e participação nas discussões com a equipe clínica. Após a alta hospitalar, o farmacêutico realizou consultas, monitorando a adesão e o conhecimento sobre os medicamentos em uso. Ao todo, 33 indivíduos foram incluídos no grupo intervenção (GI) e 28 no grupo controle histórico (GC). Os dois grupos apresentaram características sociodemográficas e clínicas similares. A maior parte dos pacientes recebeu indicação para o TCTH em decorrência de uma doença hematológica maligna, com predomínio de medula óssea como principal fonte de células-tronco. A frequência de transplantes aparentados foi superior no GC (p = 0,0427). Foram encontrados 250 PRFs, esclarecidas 59 dúvidas da equipe e realizadas 309 intervenções no GI. Os PRFs decorreram principalmente de interações medicamentosas (36,80%), reações adversas à medicamentos (18,0%) e sobredoses (12,0%). As classes terapêuticas mais envolvidas foram os anti-infecciosos de uso sistêmico e os antineoplásicos e imunomoduladores. Obteve-se uma média de 9,36 (DP= 6,97) intervenções por paciente, abrangendo principalmente reduções de dose (19,09%), ajustes de horário de administração (18,12%), atividades educativas/qualificação da equipe (15,21%) e retirada de fármacos (10,68%). Quanto à aceitabilidade, 85,8% das intervenções foram aceitas, 3,9% foram aceitas com alterações, 7,1% não foram aceitas sem justificativa e 3,2% não foram aceitas com justificativa. As intervenções significantes ou muito significantes representaram 75,4% dos casos. Em média, cada paciente passou por 4,68 consultas farmacêuticas (DP = 1,91) durante o pós-TCTH imediato e o aumento no conhecimento e na adesão pôde ser evidenciado (p = 0,0001; p = 0,0115, respectivamente). Não foi possível demonstrar diferenças entre GI e GC na mortalidade, falha de enxertia, DECH, tempo de internação, tempo para enxertia e reinternações. Contudo, as intervenções farmacêuticas foram necessárias principalmente em pacientes com internações prolongadas (p < 0,001), que necessitaram de reinternações (p < 0,001), que utilizaram mais medicamentos (p < 0,001) e/ou que apresentaram DECH (p < 0,001) ou falha de enxertia (p = 0,010). Apesar da dificuldade na mensuração do impacto do acompanhamento farmacoterapêutico nos desfechos clínicos, é notável a contribuição em pontos relevantes do transplante. O farmacêutico realizou intervenções clinicamente relevantes e auxiliou no gerenciamento da farmacoterapia, contudo, a abordagem multidisciplinar é fundamental para maximizar os ganhos. Hematopoietic stem cell transplantation (HSCT) is one of the most important therapies in hematology. However, it is an expensive procedure, associated with substantial rates of morbidity and mortality and which involves highly complex pharmacotherapy. In this scenario, the potential contribution of the clinical pharmacist is highlighted. This study aimed to evaluate the impact of the inclusion of a clinical pharmacist in the allogeneic HSCT team on the outcomes: transplant-related mortality, graft failure, incidence of graft versus host disease (GVHD), length of hospital stay, time for grafting, number hospital readmissions, number of Drug Related Problems (DRPs), adherence and knowledge about pharmacotherapy. This is an interventional study with historical control in which the pharmacist performed tasks in the intervention group during the transplantation phases: preHSCT, HSCT itself and immediate post-HSCT. This professional daily reviewed prescriptions and patient\'s clinical condition, identified, monitored and solved DRPs and participated in discussions with the multidisciplinary team. After hospital discharge, the pharmacist also conducted consultations, monitoring adherence and knowledge about the drugs in use. 33 participants were included in the intervention group (IG) and 28 in the historical control group (CG). Both groups had similar sociodemographic and clinical characteristics. Most patients received an indication for HSCT due to malignant hematological diseases, with predominance of bone marrow as the main source of stem cells. The frequency of related transplants was higher in the CG (p = 0.0427). In the IG, a total of 250 DRPs were obtained, 59 team doubts were solved and 309 interventions were carried out. DRPs were mainly drug interactions (36.80%), adverse drug reactions (18.0%) and overdoses (12.0%). The therapeutic classes most involved were anti-infectives for systemic use, antineoplastic and immunomodulators. An average of 9.36 (SD = 6.97) interventions per patient was obtained, mainly covering dose reductions (19.09%), administration schedule adjustments (18.12%), educational activities (15.21 %) and drug withdrawal (10.68%). Regarding acceptability, 85.8% of the interventions were accepted, 3.9% were accepted with changes, 7.1% were not accepted without justification and 3.2% were not accepted with justification. Significant or very significant interventions represented 75.4% of cases. On average, each patient participated in 4.68 pharmaceutical consultations (SD = 1.91) during the immediate post-HSCT and the increase in knowledge and adherence could be proved (p = 0.0001; p = 0.0115, respectively). O objetivo deste trabalho foi estudar os pedidos de componentes sanguíneos e identificar as não-conformidades. Foram avaliados os pedidos encaminhados para dois hospitais públicos do Estado do Rio de Janeiro, de janeiro a dezembro de 2012. A metodologia foi desenvolvida de acordo com o modelo de pesquisa ex post facto, descritivo, com a evolução da técnica do estudo qualitativo e quantitativo. Nós também fizemos uma pesquisa para identificar como as escolas de medicina oferecem o curso de hemoterapia, como um curso separado, nas grades curriculares, no Brasil. Somente três instituições de ensino superior têm hemoterapia em seus currículos a nível nacional, como disciplina isolada, e no Rio de Janeiro apenas um (UFRJ). Foi aplicado um questionário entre estudantes de medicina (n = 146) e os resultados indicaram que 99,32% ≤ µ (9,3) obteve taxa de sucesso inferior ou igual à média das pontuações para os 95% (8, 4-10,2 p ≤ 0,05). Em relação à coleta de dados de não-conformidades o HOSP1 recebeu apenas 3,76% dos pedidos conformes e HOSP2 12,94%. Em relação às categorias dos diferentes profissionais, as não-conformidades foram de 100% para R1, de 88,89% para R2, de 87,09% para R3 e 94,1% para os Staff. Conclui-se que existem graves deficiências na formação de profissionais em nível acadêmico e institucional. Alterações urgentes na estrutura dos cursos de medicina são necessários para melhorar a formação em hemoterapia. Essas modificações devem incluir um conjunto de ações pedagógicas relacionadas com o planejamento institucional que visa o desenvolvimento integral dos profissionais. The objective of this work was to study the blood component requests and identify the nonconformities. We evaluated the requests send to 2 public hospitals in the state of Rio de Janeiro, from January to December 2012. The methodology was developed in accordance with the research model ex post facto, descriptive, with the evolution of qualitative and quantitative study of the technique. We also made a survey to identify if medical schools offer hemotherapy as a separate course in Brazil. Only three higher education institutions have hemotherapy in their curricula nationwide and in Rio de Janeiro, only one (UFRJ). We applied a questionnaire among medical students (n = 146) and the results indicated that 99.32% ≤ µ (9.3) achieved success rate below or equal to the average of the scores for the 95% (8, 4 to 10.2 p ≤0,05). Regarding the data collection of nonconformities HOSP1 received only 3.76% of compliant requests and HOSP2 12.94%. Regarding the different professionals the nonconformities were 100% for R1, 88,89% for R2, 87,09% for R3 and 94,1% for Staff members. We concluded that there are severe deficiencies in the training of professionals in the academic and institutional level. Urgent modifications in the structure of medical courses are needed to improve training in hemotherapy. These modifications must include a set of pedagogical actions related to institutional planning aimed at the integral development of professionals.
