Obesity in the United States and Western countries represents a major health challenge associated with an increased risk of metabolic diseases, including cardiovascular disease, hypertension, diabetes, and certain cancers. Our past work revealed a more pronounced obesity-cancer link in certain ethnic groups, motivating us to develop a tailored dietary intervention called the Healthy Diet and Lifestyle 2 (HDLS2). The study protocol is described herein for this randomized six-month trial examining the effects of intermittent energy restriction (5:2 Diet) plus the Mediterranean dietary pattern (IER + MED) on visceral adipose tissue (VAT), liver fat, and metabolic biomarkers, compared to a standard MED with daily energy restriction (DER + MED), in a diverse participant group. Using MRI and DXA scans for body composition analysis, as well as metabolic profiling, this research aims to contribute to nutritional guidelines and strategies for visceral obesity reduction. The potential benefits of IER + MED, particularly regarding VAT reduction and metabolic health improvement, could be pivotal in mitigating the obesity epidemic and its metabolic sequelae. The ongoing study will provide essential insights into the efficacy of these energy restriction approaches across varied racial/ethnic backgrounds, addressing an urgent need in nutrition and metabolic health research. Functional MRI (fMRI) is an important tool for non-invasive studies of brain function. Over the past decade, multi-echo fMRI methods that sample multiple echo times has become popular with potential to improve quantification. While these acquisitions are typically performed with Cartesian trajectories, non-Cartesian trajectories, in particular spiral acquisitions, hold promise for denser sampling of echo times. However, such acquisitions require very high acceleration rates for sufficient spatiotemporal resolutions. In this work, we propose to use a physics-driven deep learning (PD-DL) reconstruction to accelerate multi-echo spiral fMRI by 10-fold. We modify a self-supervised learning algorithm for optimized training with non-Cartesian trajectories and use it to train the PD-DL network. Results show that the proposed self-supervised PD-DL reconstruction achieves high spatio-temporal resolution with meaningful BOLD analysis.

Anatomy instructional methods varied widely during the COVID-19 pandemic and programs are assessing innovations for retention. Learning preferences were assessed among medical students dichotomized as elective dissectors (ED) or non-dissectors (ND) during the COVID-19 partial re-opening in 2020 (preclinical) and again in 2022 after clinical exposure (post-clinical) to assess the viability of elective dissection post-pandemic. A mixed-method approach was used for the assessment of test scores, learning preference surveys, learning activities rankings, and thematic analyses. No significant differences occurred in anatomy examination scores. Dissection was considered useful by both preclinical groups but significantly more so by ED, while the presence of an instructor was significantly preferred by ED although a majority of ND agreed. Elective dissection was significantly preferred by ND but also by a large minority of ED students. Pre- and post-clinical ND believed that elective dissection offered more academic flexibility, did not hinder clinical learning, and did not negatively impact medical education. The corresponding ED stated that confidence improved, clinical experiences were enhanced, and dissection was irreplaceable. Preclinical ND preferred self-learning, while ED students preferred online learning, but these differences largely disappeared post-clinically. Learning activity rankings were not significantly different among all groups (ND, ED, preclinical, and post-clinical). A hybrid laboratory with a virtual learning environment ranked highest across groups and preferences increased over time suggesting that students benefited from this instructional method during clinical exposure. Purpose: Extended reality (XR) technology enhances learning in medical education. The purpose of this study was to develop and apply a case-based approach for teaching radiological anatomy utilizing XR technology for improved student exploration and engagement., Methods: The workflow consisted of MRI scanning cadavers followed by radiological, pathological, and anatomical assessment, and finally case presentation based on XR visualizations and student interaction. Case information (Subject, History, and Physical Exam) was presented to student groups who generated and recorded hypotheses using Google Forms., Results: Use of all components of the system was voluntary and a total of 74 students responded to the survey request (response rate = 95%). Assessment of the experience was conducted through a qualitative survey comprising four Likert scale questions (1-5, 1 lowest), three binary questions, and open-ended comments. Mean, standard deviation, and overall agreement (mean ± SD, OA) showed that students found MRI scans of cadavers to be helpful for dissections (4.14 ± 1.1, 74.3%) and provided an understanding of relevant anatomy (4.32 ± 0.9, 79.7%), while 78.4% of students used the DICOM viewer to visualize scans of cadavers. The difficulty of use was found to be average (2.90 ± 1.0, 23%). zSpace visualizations were used by 40.5% of students, generally agreeing that an understanding of spatial relationships improved as a result (3.60 ± 1.0, 43.2%). More case-based sessions were favored by 97.3% of students., Conclusions: Results suggest that cadaveric MRI radiological visualization and XR technology enhance understanding of case-based anatomical dissections and encourage student exploration and engagement., (Copyright © 2021 Elsevier B.V. Background: Hepatocellular carcinoma (HCC) pathogenesis involves the alteration of multiple liver-specific metabolic pathways. We systematically profiled cancer- and liver-related classes of metabolites in HCC and adjacent liver tissues and applied supervised machine learning to compare their potential yield for HCC biomarkers., Methods: Tumor and corresponding liver tissue samples were profiled as follows: Bile acids by ultra-performance liquid chromatography (LC) coupled to tandem mass spectrometry (MS), phospholipids by LC-MS/MS, and other small molecules including free fatty acids by gas chromatography-time of flight MS. The overall classification performance of metabolomic signatures derived by support vector machine (SVM) and random forests machine learning algorithms was then compared across classes of metabolite., Results: For each metabolite class, there was a plateau in classification performance with signatures of 10 metabolites. Phospholipid signatures consistently showed the highest discrimination for HCC followed by signatures derived from small molecules, free fatty acids, and bile acids with area under the receiver operating characteristic curve (AUC) values of 0.963, 0.934, 0.895, 0.695, respectively, for SVM-generated signatures comprised of 10 metabolites. Similar classification performance patterns were observed with signatures derived by random forests., Conclusion: Membrane phospholipids are a promising source of tissue biomarkers for discriminating between HCC tumor and liver tissue.

Purpose: To describe a simultaneous multislice (SMS) ultrashort echo time (UTE) imaging method using radiofrequency phase encoded half-pulses in combination with power independent of number of slices (PINS) inversion recovery (IR) pulses to generate multiple-slice images with short T 2 * contrasts in less than 3 min with close to an eightfold acceleration compared with a standard 2D approach., Theory and Methods: Radiofrequency phase encoding is applied in an SMS (N SMS = 4) excitation scheme using "sinc" half-pulses. With the use of coil sensitivity encoding (SENSE) and controlled aliasing in parallel imaging (CAIPI) in combination with a gradient echo 2D spiral readout trajectory and IR PINS pulses for contrast enhancement a fast UTE sequence is developed. Images are obtained using a model-based reconstruction method. Sequence details and performance tests on phantoms as well as the heads of healthy volunteers at 3T are presented., Results: An SMS UTE sequence with an undersampling factor of 4 is developed using radiofrequency phase encoded half-pulses and PINS IR pulses which enables the acquisition of 8 slices at 0.86 2 mm 2 resolution in less than 3-min scan time. UTE images of the head are obtained showing highlighted signal of cortical bone. Image quality and T 2 contrast are comparable to the one obtained by corresponding single slice acquisitions with only minor deviations., Conclusions: The method combining radiofrequency phase encoded SMS half-pulses and PINS IR pulses presents a suitable approach to SMS UTE imaging. The usage of coil sensitivity information and increased sampling density by means of interleaved slice group acquisitions allows to reduce the total scan time by a factor close to 8., (© 2019 International Society for Magnetic Resonance in Medicine.)

Purpose: To demonstrate a circular EPI (CEPI) sequence as well as a generalized EPI reconstruction for fast fMRI with parallel imaging acceleration., Methods: The CEPI acquisition was constructed using variable readout lengths and maximum ramp sampling as well as blipped-CAIPI z-gradient encoding for simultaneous multislice (SMS) and 3D volumetric imaging. A signal equation model with constant and linear phase terms was used to iteratively reconstruct images with low ghosting. Simulation, phantom, and human imaging experiments including audio/visual fMRI were performed at 3T using a 52-channel coil., Results: Application of CEPI gradients with duration of 27 ms covering a 22-cm FOV at a 64 × 64 pixel resolution in SMS and 3D acquisitions resulted in images with comparable quality to those of standard Cartesian EPI. With parallel imaging techniques robust detection of BOLD fMRI activation with temporal sampling down to 275 ms was possible. The high temporal resolution enabled higher activation statistics at a penalty in increased noise and residual aliasing. The un-accelerated 3D acquisition showed large temporal instability compared with a standard 2D acquisition., Conclusion: Nonuniform sampling and generalized image reconstructions can be applied to EPI acquisitions including those with blipped-CAIPI z gradients. We herein report the catalytic enantioselective hydrodehalogenation based on the interplay of a chiral molecular nickel(I)/nickel(II)hydride system. Prochiral geminal dihalogenides are dehalogenated via a secondary configurationally unstable, potentially metal-stabilized radical intermediate. In a subsequent step, the liberated radical is then trapped by the nickel(II) hydrido complex, present in a large excess under the catalytic conditions, which in turn induces the enantioselectivity during the hydrogen atom transfer onto the radical intermediate. These new chiral nickel(I) complexes were found to catalyze the asymmetric hydrodehalogenation of geminal dihalogenides with moderate to good enantiomeric excess values using LiEt3 BH as reductant. The main side product generally observed is the dehalogenated alkene, whereas the hydrodehalogenation of the chiral monohalogen compound occurred much more slowly despite the large excess of reductant., (© 2014 WILEY-VCH Verlag GmbH & Co. The enantioselective trifluoromethylthiolation of β-ketoesters using chiral copper-boxmi complexes as catalysts is reported.
