2. Deep neural network improves the estimation of polygenic risk scores for breast cancer

Abstract

Polygenic risk scores (PRS) estimate the genetic risk of an individual for a complex disease based on many genetic variants across the whole genome. In this study, we compared a series of computational models for estimation of breast cancer PRS. A deep neural network (DNN) was found to outperform alternative machine learning techniques and established statistical algorithms, including BLUP, BayesA and LDpred. In the test cohort with 50% prevalence, the Area Under the receiver operating characteristic Curve (AUC) were 67.4% for DNN, 64.2% for BLUP, 64.5% for BayesA, and 62.4% for LDpred. BLUP, BayesA, and LPpred all generated PRS that followed a normal distribution in the case population. However, the PRS generated by DNN in the case population followed a bi-modal distribution composed of two normal distributions with distinctly different means. This suggests that DNN was able to separate the case population into a high-genetic-risk case sub-population with an average PRS significantly higher than the control population and a normal-genetic-risk case sub-population with an average PRS similar to the control population. This allowed DNN to achieve 18.8% recall at 90% precision in the test cohort with 50% prevalence, which can be extrapolated to 65.4% recall at 20% precision in a general population with 12% prevalence. Interpretation of the DNN model identified salient variants that were assigned insignificant p-values by association studies, but were important for DNN prediction. Abstract

Drinking practices across Britain are not uniform and in each locality reflect the historical, socio-economic and cultural context unique to each specific part of the country. In South Yorkshire, the drinking cultures that exist in the area reflect the industrial heritage of the area, particularly the steel, rail and coal industries. The decline of traditional industries across Britain has had a significant impact on these drinking practices as Britain has transitioned from having an economy dominated by heavy industry to a post-industrial economy defined by Neoliberalism. The decline of the coal industry in Doncaster in particular has had a significant impact on the general everyday lives of people in communities that once had economies centred around the coal industry. This impact is reflected in the changes to drinking cultures. This study focuses specifically on two post-mining communities in the Doncaster Borough (Rossington and Denaby Main and Mexborough), which have faced significant change in the socioeconomic landscape due to the decline and eventual loss of the mining industry. Post-mining communities have been subject to significant study and analysis from an economic regeneration aspect, but there was an identified gap in literature regarding research that specifically focuses on drinking practices in these areas. From a symbolic interactionist perspective, the research utilised a constructivist grounded theory methodological approach, as gaining intimate familiarity with the research topic was of high importance, due to criticisms of past research having the preconceived notions or specific interests being imposed on them by the researcher. The grounded theory methodology was initially chosen due to the advantages it has when conducting research on under-researched topics such as attitudes to alcohol in post-mining communities. Twenty-four participants (five from Denaby Main and Mexborough and twenty-one from Rossington) were questioned about their attitudes to alcohol in semi-structured interviews in the case study areas. This study made significant use of reflexivity as a methodological tool to put to the forefront the complexities of conducting qualitative research. This became particularly important in documenting the impact of the political context at the time and how this may have impacted on how the participants engaged with the researcher (the Brexit campaign, Brexit Referendum and the post-referendum period). The research revealed that in both case study areas economic and cultural identities of the past have had a significant influence in shaping drinking practices. Drinking practices in the post-mining period have formed as a result to adapting to endogenous influences that have drastically changed the economy in the two areas. Whilst endogenous traditions such as maintaining closely-knit ties and respectability and sociability were emphasised by participants to have been maintained, the function of drinking associated with the mining industry (such as recovery) and institutions with links to the industry had declined. This decline in both the function of drinking and of institutions such as pubs and clubs had faced decline due to changing local socioeconomic context. Exogenous influences such as large supermarkets, an increasing presence in both areas from national off-licence franchises and increasing opportunities to engage in privatised leisure significantly influenced the strategies that participants used to adapt to post-industrial Doncaster. The study contributes to knowledge in this area by contributing knowledge of localised drinking practices within a post-mining context in South Yorkshire although a gap may still remain for future research regarding research into drinking practices in post-mining communities. The research emphasises the need for researchers and policy makers that engage in future research in communities similar to the two case study areas to engage in reflexive practice, specifically in the current political context.

Abstract

Giant ichthyosaurs with body length estimates exceeding 20 m were present in the latest Triassic of the UK. Here we report on the discovery of a second surangular from the lower jaw of a giant ichthyosaur from Somerset, UK. The new find is comparable in size and morphology to a specimen from Lilstock, Somerset, described in 2018, but it is more complete and better preserved. Both finds are from the uppermost Triassic Westbury Mudstone Formation (Rhaetian), but the new specimen comes from Blue Anchor, approximately 10 km west along the coast from Lilstock. The more complete surangular would have been >2 m long, from an individual with a body length estimated at ~25 m. The identification of two specimens with the same unique morphology and from the same geologic age and geographic location warrants the erection of a new genus and species, Ichthyotitan severnensis gen. et sp. nov. Thin sections of the new specimen revealed the same histological features already observed in similar giant ichthyosaurian specimens. Our data also supports the previous suggestion of an atypical osteogenesis in the lower jaws of giant ichthyosaurs. The geological age and giant size of the specimens suggest shastasaurid affinities, but the material is too incomplete for a definitive referral. Ichthyotitan severnensis gen. et sp. nov., is the first-named giant ichthyosaur from the Rhaetian and probably represents the largest marine reptile formally described. [ABSTRACT FROM AUTHOR]

Abstract

Cancer patients show the signs and symptoms of a failing circadian organization. Melatonin, secreted only during nightly darkness, promotes nightly sleep and synchronizes daily molecular and physiologic clocks. Using a placebo-controlled randomized double-blind clinical trial, we determined whether the hypothesized proper physiologic daily timing, the evening, is essential for cancer patients to benefit from melatonin. Eighty-four advanced non-small cell lung cancer (NSCLC) patients from two centers (42 each) receiving standard etoposide/cisplatin therapy, were randomly assigned to one of three arms (I, 8AM & 8PM placebo; II, 8AM 20 mg melatonin and 8PM placebo; III, 8AM placebo and 8PM 20 mg melatonin. Each patient was followed until death. After adjusting for covariates, the Cox proportional hazards regression analysis found that the overall survival was enhanced only among those patients randomized to receive evening melatonin (p = 0.031 HR = 0.39). Survival benefit in the PM melatonin arm was optimized in patients who reported normal sleep quality. After adjusting for significant covariates at baseline, we found an overall survival advantage for PM melatonin when compared against placebo. Abstract

The Facilities Engineering and Architectural Branch is responsible for the design and maintenance of buildings, laboratories, and civil structures. In order to improve efficiency and quality, the FEAB has dedicated itself to establishing a data infrastructure based on Geographic Information Systems, GIS. The value of GIS was explained in an article dating back to 1980 entitled "Need for a Multipurpose Cadastre" which stated, "There is a critical need for a better land-information system in the United States to improve land-conveyance procedures, furnish a basis for equitable taxation, and provide much-needed information for resource management and environmental planning." Scientists and engineers both point to GIS as the solution. What is GIS? According to most text books, Geographic Information Systems is a class of software that stores, manages, and analyzes mapable features on, above, or below the surface of the earth. GIS software is basically database management software to the management of spatial data and information. Simply put, Geographic Information Systems manage, analyze, chart, graph, and map spatial information. GIS can be broken down into two main categories, urban GIS and natural resource GIS. Further still, natural resource GIS can be broken down into six sub-categories, agriculture, forestry, wildlife, catchment management, archaeology, and geology/mining. Agriculture GIS has several applications, such as agricultural capability analysis, land conservation, market analysis, or whole farming planning. Forestry GIs can be used for timber assessment and management, harvest scheduling and planning, environmental impact assessment, and pest management. GIS when used in wildlife applications enables the user to assess and manage habitats, identify and track endangered and rare species, and monitor impact assessment.

Abstract

Abstract Background Many cancer patients report poor sleep quality, despite having adequate time and opportunity for sleep. Satisfying sleep is dependent on a healthy circadian time structure and the circadian patterns among cancer patients are quite abnormal. Wrist actigraphy has been validated with concurrent polysomnography as a reliable tool to objectively measure many standard sleep parameters, as well as daily activity. Actigraphic and subjective sleep data are in agreement when determining activity-sleep patterns and sleep quality/quantity, each of which are severely affected in cancer patients. We investigated the relationship between actigraphic measurement of circadian organization and self-reported subjective sleep quality among patients with advanced lung cancer. Methods This cross-sectional and case control study was conducted in 84 patients with advanced non-small cell lung cancer in a hospital setting for the patients at Midwestern Regional Medical Center (MRMC), Zion, IL, USA and home setting for the patients at WJB Dorn Veterans Affairs Medical Center (VAMC), Columbia, SC, USA. Prior to chemotherapy treatment, each patient's sleep-activity cycle was measured by actigraphy over a 4-7 day period and sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI) questionnaire. Results The mean age of our patients was 62 years. 65 patients were males while 19 were females. 31 patients had failed prior treatment while 52 were newly diagnosed. Actigraphy and PSQI scores showed significantly disturbed daily sleep-activity cycles and poorer sleep quality in lung cancer patients compared to healthy controls. Nearly all actigraphic parameters strongly correlated with PSQI self-reported sleep quality of inpatients and outpatients. Conclusions The correlation of daily activity/sleep time with PSQI-documented sleep indicates that actigraphy can be used as an objective tool and/or to complement subjective assessments of sleep quality in patients with advanced lung cancer. These results suggest that improvements to circadian function may also improve sleep quality.
