Neuromotor dysfunction after a concussion is common, but balance tests used to assess neuromotor dysfunction are typically subjective. Current objective balance tests are either cost- or space-prohibitive, or utilize a static balance protocol, which may mask neuromotor dysfunction due to the simplicity of the task. To address this gap, our team developed an Android-based smartphone app (portable and cost-effective) that uses the sensors in the device (objective) to record movement profiles during a stepping-in-place task (dynamic movement). The purpose of this study was to examine the extent to which our custom smartphone app and protocol could discriminate neuromotor behavior between concussed and non-concussed participants. Data were collected at two university laboratories and two military sites. Participants included civilians and Service Members (N = 216) with and without a clinically diagnosed concussion. Kinematic and variability metrics were derived from a thigh angle time series while the participants completed a series of stepping-in-place tasks in three conditions: eyes open, eyes closed, and head shake. We observed that the standard deviation of the mean maximum angular velocity of the thigh was higher in the participants with a concussion history in the eyes closed and head shake conditions of the stepping-in-place task. Consistent with the optimal movement variability hypothesis, we showed that increased movement variability occurs in participants with a concussion history, for which our smartphone app and protocol were sensitive enough to capture. The objective of this study was to examine cardio hypothalamic-pituitary coupling and to better understand how the temporal relations between these systems are altered during rest and exercise conditions. An intensive within subjects study design was used. Seven adult males completed two visits, each consisting of either a 24 h period of complete rest or a 24 h period containing a high-intensity exercise bout. An intravenous catheter was used to collect serum samples every 10 min throughout the 24 h period (i.e., 145 samples/person/condition) to assess growth hormone (GH) dynamics throughout the 24 h period. Cardiac dynamics were also collected throughout the 24 h period and epoched into 3 min windows every 10 min, providing serial short-time measurements of heart rate variability (HRV) concurrent to the GH sampling. The standard deviation of the normal RR interval (SDNN), the root mean square of successive differences (rMSSD), and sample entropy (SampEn) was calculated for each epoch and used to create new profiles. The dynamics of these profiles were individually quantified using SampEn and recurrence quantification analysis (RQA). To address our central question, the coupling between these profiles with GH was assessed using cross-SampEn and cross-RQA (cRQA). A comparison between the epoched HRV profiles indicated a main effect between profiles for sample entropy (p < 0.001) and several measures from RQA. An interaction between profile and condition was observed for cross-SampEn (p = 0.04) and several measures from cRQA. These findings highlight the potential application of epoched HRV to assess changes in cardiac dynamics, with specific applications to assessing cardio hypothalamic-pituitary coupling. Individuals with chronic ankle instability (CAI) present somatosensory dysfunction following an initial ankle sprain. However, little is known about how individuals with CAI adapt to a sudden sensory perturbation of instability with increasing task and environmental constraints to maintain postural stability. Forty-four individuals with and without unilateral CAI performed the Adaptation Test to a sudden somatosensory inversion and plantarflexion perturbations (environment) in double-, injured-, and uninjured- limbs. Mean sway energy scores were analyzed using 2 (group) × 2 (somatosensory perturbations) × 3 (task) repeated measures analysis of variance. There were significant interactions between the group, environment, and task (P =.025). The CAI group adapted faster than healthy controls to a sudden somatosensory inversion perturbation in the uninjured- (P =.002) and injured- (P <.001) limbs, as well as a sudden somatosensory plantarflexion perturbation in the double- (P =.033) and uninjured- (P =.035) limbs. The CAI and healthy groups presented slower postural adaptation to a sudden inversion perturbation than a sudden somatosensory plantarflexion perturbation in double-limb (P <.001). Whereas both groups demonstrated faster postural adaptation to a sudden somatosensory inversion perturbation compared to somatosensory plantarflexion perturbation while maintaining posture in the injured- (P <.001) and uninjured- (P <.001) limbs. The CAI and healthy groups adapted faster to a sudden somatosensory inversion perturbation in the injured- (P <.001) and uninjured- (P <.001) limbs than in double-limb, respectively. Postural adaptation in individuals with and without CAI depended on environmental (somatosensory perturbations) and task constraints. This study examined the training effect of attentional focus (external focus, internal focus, or no focus instructions) on a dynamic balance task. Participants completed baseline balance testing, seven consecutive days of dynamic balance board training, and retention testing 24 hours after the last session. The novel finding of this study was the presence of a training effect on balance control when adopting an external focus relative to an internal focus or no focus instructions. Further, we report the unique observation that more patterned behavior was adopted regardless of the focus instructions. Context: Movement screening has become increasingly popular among tactical professionals. This popularity has motivated the design of interventions that cater to improving outcomes on the screens themselves, which are often scored in reference to an objective norm. In contrast to the assumptions underlying this approach, dynamical systems theory suggests that movements arise as a function of continuously evolving constraints and that optimal movement strategies may not exist. To date, few data address behavioral complexity in the fundamental movement tasks commonly used in clinical screenings. Objective: To provide evidence of complex variability during movement screens and test the role of modifiable--that is, trainable--constraints in mediating loss of complexity during experimental-task manipulations. Design: Crossover study. Setting: Research laboratory. Patients or Other Participants: Twenty-five male (age = 23.96 ± 3.74 years, height = 178.82 ± 7.51 cm, mass = 79.66 ± 12.66 kg) and 25 female (age = 22.00 ± 2.02 years, height = 165.40 ± 10.24 cm, mass = 63.98 ± 11.07 kg) recreationally active adults. Intervention(s): Participants performed tests of balance, range of motion, and strength. Additionally, they performed cyclical movement tasks under a control (C) condition and while wearing an 18.10-kg weight vest (W). Main Outcome Measure(s): Ground reaction forces were sampled at 1000 Hz and used to calculate center of pressure during cyclical movement tests. Multivariate multiscale entropy (MMSE) for the center-of-pressure signal was then calculated. Condition effects (C versus W) were analyzed using paired t tests, and penalized varying-coefficients regression was used to identify models predicting entropy outcomes from balance, range of motion, and strength. Results: The MMSE decreased during the W condition (MMSEC > MMSEW; t49 range = 3.17-5.21; all Pvalues < .01). Conclusions: Moderate evidence supported an association between modifiable constraints and behavioral complexity, but a role in mediating load-related loss of complexity was not demonstrated. We examined sagittal-plane thigh angular kinematics in individuals with and without recurrent ankle sprains using a clinical smartphone app called AccWalker. Sagittal-plane ankle kinematics were also compared to ascertain that altered ankle dorsiflexion, which is typically displayed with chronic ankle instability, is also present in individuals with recurrent ankle sprains. Participants with (n = 22) and without (n = 22) recurrent ankle sprains were evaluated on average sagittal-plane ankle kinematics during walking and average sagittal-plane thigh angular kinematics during stepping-in-place with AccWalker. Significant group-by-limb interactions were found for sagittal-plane ankle kinematics (F (1,42) = 63.786, P <.010) during walking and sagittal-plane average thigh angular range-of-motion (F (1,42) = 6.166, P =.017) with AccWalker. Individuals with recurrent ankle sprains displayed more ankle dorsiflexion in affected (P <.001) and unaffected (P =.001) limbs during walking than healthy controls and exhibited more ankle dorsiflexion in their affected-limb compared to their unaffected-limb (P <.001). The average sagittal-plane thigh angular range-of-motion was lower in the unaffected-limb for recurrent ankle sprains compared to their affected-limb (P =.038) and the assigned unaffected-limb of healthy controls (P =.035). Increased dorsiflexion was present in both limbs of the recurrent ankle sprain group with walking. AccWalker does not assess ankle movement, but uniquely identified thigh motion impairments associated with recurrent ankle sprains in their unaffected-limb, potentially identifying central deficits associated with recurrent ankle sprains. The assessment of gait variability using stochastic signal processing techniques such as detrended fluctuation analysis (DFA) has been shown to be a sensitive tool for evaluation of gait alterations due to aging and neuromuscular disease. However, previous studies have suggested that the application of DFA requires relatively long recordings (600 strides), which is difficult when working with clinical populations or older adults. In this paper we propose a model for predicting DFA variance in experimental data and conduct a Monte Carlo simulation to estimate the sample size and number of trials required to detect a change in DFA scaling exponent. We illustrate the model in a simulation to detect a difference of 0.1 (medium effect) between two groups of subjects when using short gait time series (100 to 200 strides) in the context of between- and within-subject designs. We assumed that the variance of DFA scaling exponent arises due to individual differences, time series length, and experimental error. Results showed that sample sizes required to achieve acceptable power of 80% are practically feasible, especially when using within-subject designs. For example, to detect a group difference in the DFA scaling exponent of 0.1, it would require either 25 subjects and 2 trials per subject or 12 subjects and 4 trials per subject using a within-subject design. We then compared plausibility of such power predictions to the empirically observed power from a study that required subjects to synchronize with a persistent fractal metronome. The results showed that the model adequately predicted the empirical pattern of results. Blast exposure is a prevalent cause of mild traumatic brain injury (mTBI) in military personnel in combat. However, it is more common for a service member to be exposed to a low-level blast (LLB) that does not result in a clinically diagnosable mTBI. Recent research suggests that repetitive LLB exposure can result in symptomology similar to symptoms observed after mTBI. This manuscript reports on the use of an Android-based smartphone application (AccWalker app) to capture changes in neuromotor functioning after blast exposure. Active duty U.S. Navy personnel (N = 59) performed a stepping-in-place task before repetitive LLB exposure (heavy weapons training), and again immediately after, 24 hours after, and 72 to 96 hours after the completion of the training. The AccWalker app revealed that there are changes in neuromotor functioning after LLB exposure (slower self-selected movement pace and increased stride time variability) in participants who experienced neurocognitive decline. Gait control challenges commonly coincide with vestibular dysfunction and there is a long history in using balance and gait activities to enhance functional mobility in this population. While much has been learned using traditional rehabilitation exercises, there is a new line of research emerging that is using visual stimuli in a very specific way to enhance gait control. For example, avatars can be created in an individualized manner to incorporate specific gait characteristics. The avatar could then be used as a visual stimulus to which the patient can synchronize their own gait cycle. This line of research builds upon the rich history of sensorimotor control research in which augmented sensory information (visual, haptic, or auditory) is used to probe, and even enhance, human motor control. This review paper focuses on gait control challenges in patients with vestibular dysfunction, provides a brief historical perspective on how various visual displays have been used to probe sensorimotor and gait control, and offers some recommendations for future research. Postural control is commonly assessed by quantifying center of pressure (CoP) variability during quiet stance. CoP data is traditionally filtered prior to analysis. However, some researchers suggest filtering may lead to undesirable consequences. Further, sampling frequency may also affect CoP analysis, as filtering CoP signals of different sampling frequencies may influence variability metrics. This study examined the influence of sampling frequency and filtering on metrics that index the magnitude and structure of variability in CoP displacement and velocity. Healthy adults ( N = 8, 27.4 ± 2.6 years) balanced on their right foot for 60 s on a force plate. CoP data recorded at 100 Hz was then downsampled and/or filtered (2nd order dual-pass 10 Hz low-pass Butterworth) to create six different CoP time series for each participant: (1) original, (2) filtered, (3) downsampled to 50 Hz, (4) downsampled to 25 Hz, (5) downsampled to 50 Hz and filtered, and (6) down-sampled to 25 Hz and filtered. Data were then analyzed using four common variability metrics (standard deviation [SD], root mean square [RMS], detrended fluctuation analysis α [DFA α ], and sample entropy [SampEn]). Data processing techniques did not influence the magnitude of variability (SD and RMS), but did influence the structure of variability (DFA α and SampEn) in CoP displacement. All metrics were influenced by data processing techniques in CoP velocity. Thus, when interpreting changes in CoP variability, one must be careful to identify how much change is driven by the neuromotor system and how much is a function of data processing technique. The maintenance of upright stance requires the simultaneous control of posture in both the anterior–posterior (AP) and medial–lateral (ML) dimensions. Postural sway is typically measured by quantifying the movement of the center of pressure (CoP) in the AP and ML dimensions independently. Metrics such as path length and 95% ellipse area have been developed to take into account movement in both the AP and ML directions, but these metrics only quantify the magnitude of the CoP movement. The movement of the CoP is technically a vector quantity with both magnitude and direction characteristics. The direction of displacement, or heading, of the CoP may provide further insight into the control of posture. Accordingly, we present a novel variable that describes the rate of change in direction of CoP displacement in two dimensions, the heading change (Δ ϕ ), which is derived from the CoP heading ( ϕ ). We then compared the standard deviation (SD) and the dynamic structure characterized by sample entropy (SampEn) of the heading change time series to previously examined metrics presented in the literature (SD and SampEn of the AP and ML time series, path length, SD and SampEn of the CoP resultant magnitude time series) during a 60 s single-leg stance performed by healthy participants and patients with a ruptured anterior cruciate ligament (ACL) prior to surgical intervention. Patients with an ACL rupture exhibited a different dynamic structure in Δ ϕ compared to healthy controls, t (14) = 2.44, p = 0.029, whereas none of the other metrics differed between groups (all p > 0.05). The novelty and utility of Δ ϕ is that it characterizes directional changes of the CoP, whereas previously documented postural control analyses describe only changes in magnitude. Previous work has shown that fractal patterns in gait can be altered by entraining to a fractal stimulus. However, little is understood about how long those patterns are retained or which factors may influence stronger entrainment or retention. In experiment one, participants walked on a treadmill for 45 continuous minutes, which was separated into three phases. The first 15 minutes (pre-synchronization phase) consisted of walking without a fractal stimulus, the second 15 minutes consisted of walking while entraining to a fractal visual stimulus (synchronization phase), and the last 15 minutes (post-synchronization phase) consisted of walking without the stimulus to determine if the patterns adopted from the stimulus were retained. Fractal gait patterns were strengthened during the synchronization phase and were retained in the post-synchronization phase. In experiment two, similar methods were used to compare a continuous fractal stimulus to a discrete fractal stimulus to determine which stimulus type led to more persistent fractal gait patterns in the synchronization and post-synchronization (i.e., retention) phases. Both stimulus types led to equally persistent patterns in the synchronization phase, but only the discrete fractal stimulus led to retention of the patterns. The results add to the growing body of literature showing that fractal gait patterns can be manipulated in a predictable manner. Further, our results add to the literature by showing that the newly adopted gait patterns are retained for up to 15 minutes after entrainment and showed that a discrete visual stimulus is a better method to influence retention. Fractal patterns characterize healthy biological systems and are considered to reflect the ability of the system to adapt to varying environmental conditions. Previous research has shown that fractal patterns in gait are altered following natural aging or disease, and this has potential negative consequences for gait adaptability that can lead to increased risk of injury. However, the flexibility of a healthy neurological system to exhibit different fractal patterns in gait has yet to be explored, and this is a necessary step toward understanding human locomotor control. Fifteen participants walked for 15 min on a treadmill, either in the absence of a visual stimulus or while they attempted to couple the timing of their gait with a visual metronome that exhibited a persistent fractal pattern (contained long-range correlations) or a random pattern (contained no long-range correlations). The stride-to-stride intervals of the participants were recorded via analog foot pressure switches and submitted to detrended fluctuation analysis (DFA) to determine if the fractal patterns during the visual metronome conditions differed from the baseline (no metronome) condition. DFA α in the baseline condition was 0.77 ± 0.09. The fractal patterns in the stride-to-stride intervals were significantly altered when walking to the fractal metronome (DFA α = 0.87 ± 0.06) and to the random metronome (DFA α = 0.61 ± 0.10) (both p < .05 when compared to the baseline condition), indicating that a global change in gait dynamics was observed. A variety of strategies were identified at the local level with a cross-correlation analysis, indicating that local behavior did not account for the consistent global changes. Context: Previous work suggests that balance behavior is a sex-dependent, complex process that can be characterized by linear and nonlinear metrics. Although a certain degree of center of pressure variability may be expected based on sexual dimorphism, there is evidence to suggest that these effects are obscured by potential interactions between sex and anthropometric factors. To date, no study has accounted for such interactive effects using both linear and nonlinear measures. Objective: This investigation sought to analyze interactive models featuring sex, height, and weight as predictors of linear and nonlinear aspects of postural control. Design: Cross-sectional study. Setting: Controlled laboratory. Participants: A total of 26 males (23.80 [3.44] y, 177.87 [6.44] cm, 81.70 [10.80] kg) and 28 females (21.14 [2.03] y, 169.57 [8.80] cm, 64.48 [8.86] kg) were sampled from a healthy university population. MainOutcomeMeasures: Linear (range [RNG], velocity [VEL], and SD) and nonlinear (detrended fluctuation analysis scaling exponent, multivariate multiscale sample entropy [MMSECI]) summary metrics of center of pressure time series. Procedure: Participants stood on a force plate for 20 seconds in 3 conditions: double (D), single (S), and tandem (T) stance. Data for each stance condition were analyzed using regression models with interaction terms for sex × height and sex × weight. In D, weight had a positive, significant main effect on VELy, MMSECId, and MMSECIv. In men, height was observed to have a positive effect on SDy (S), RNGy (S), and RNGx (T) and a negative effect on MMSECIv (T). In women, weight was observed to have a positive effect on SDy and VELx (both T). Conclusions: Our findings suggest that men and women differ with respect to certain linear and nonlinear aspects of balance behavior, and that these differences may reflect sex-specific behavioral patterns in addition to effects related to sexual dimorphism. Purpose: Injuries from falls are a serious health issue. Approaches to preventing falls should consider increasing relevant visual information of an obstacle. Obstacle parameters, such as position and height, may be specified by the visible structure of an obstacle. The present study examined the relationship between visible structure of an obstacle and locomotor behaviour. This relationship may be modified as a function of experience with navigating obstacles. Since workers at construction sites must navigate through cluttered environments with varied obstacles, these workers may have superior skills at avoiding obstacles. Therefore, the effect of work experience was also examined. Methods: Nine construction workers and 10 age- and gender-matched control subjects participated. Subjects stepped over obstacles in an 8 m walkway. Three different obstacles were examined, arranged according to a hierarchy ranging from most to least visible structure: a solid obstacle, a three-edge outline obstacle and a top-edge obstacle. The obstacles were 10, 20 or 30 cm high. In addition, visual information was decreased with goggles which obstructed the lower visual field, removing information of the obstacle and foot-relative-to-obstacle in the two steps before the obstacle. All conditions were presented randomly. Results: Higher risk of contact and higher lead and trail toe clearance variability were observed for the top-edge obstacle. Higher risk of contact was observed when the lower visual field was obstructed and for the 30 cm obstacle. Work experience did not influence risk of contact. Construction workers had lower trail toe clearances and lower trail toe clearance variability for the 10 cm obstacle, but were not different from controls for the 30 cm obstacle. Conclusions: Decreased visible structure of an obstacle resulted in increased gait variability and increased risk of contact. The changes are consistent with decreased accuracy of the sensory-to-motor transformation used to control the lead and trail limb during obstacle crossing when only the top-edge was visible. There is some evidence that construction workers were better able to transform the visual information to motor actions, as reflected by decreased gait variability, but these findings were not supported by decreased risk of obstacle contact. Background: Over the last two decades, various measures of entropy have been used to examine the complexity of human postural control. In general, entropy measures provide information regarding the health, stability and adaptability of the postural system that is not captured when using more traditional analytical techniques. The purpose of this study was to examine how noise, sampling frequency and time series length influence various measures of entropy when applied to human center of pressure (CoP) data, as well as in synthetic signals with known properties. Such a comparison is necessary to interpret data between and within studies that use different entropy measures, equipment, sampling frequencies or data collection durations. Methods and Findings: The complexity of synthetic signals with known properties and standing CoP data was calculated using Approximate Entropy (ApEn), Sample Entropy (SampEn) and Recurrence Quantification Analysis Entropy (RQAEn). All signals were examined at varying sampling frequencies and with varying amounts of added noise. Additionally, an increment time series of the original CoP data was examined to remove long-range correlations. Of the three measures examined, ApEn was the least robust to sampling frequency and noise manipulations. Additionally, increased noise led to an increase in SampEn, but a decrease in RQAEn. Thus, noise can yield inconsistent results between the various entropy measures. Finally, the differences between the entropy measures were minimized in the increment CoP data, suggesting that long-range correlations should be removed from CoP data prior to calculating entropy. Conclusions: The various algorithms typically used to quantify the complexity (entropy) of CoP may yield very different results, particularly when sampling frequency and noise are different. Background: During locomotion, vision is used to perceive environmental obstacles that could potentially threaten stability; locomotor action is then modified to avoid these obstacles. Various factors such as lighting and texture can make these environmental obstacles appear larger or smaller than their actual size. It is unclear if gait is adapted based on the actual or perceived height of these environmental obstacles. The purposes of this study were to determine if visually guided action is scaled to visual perception, and to determine if task experience influenced how action is scaled to perception. Methodology/Principal Findings: Participants judged the height of two obstacles before and after stepping over each of them 50 times. An illusion made obstacle one appear larger than obstacle two, even though they were identical in size. The influence of task experience was examined by comparing the perception-action relationship during the first five obstacle crossings (1-5) with the last five obstacle crossings (46-50). In the first set of trials, obstacle one was perceived to be 2.0 cm larger than obstacle two and subjects stepped 2.7 cm higher over obstacle one. After walking over the obstacle 50 times, the toe elevation was not different between obstacles, but obstacle one was still perceived as 2.4 cm larger. Conclusions/Significance: There was evidence of locomotor adaptation, but no evidence of perceptual adaptation with experience. External focus (attention to the movement effect) has been found effective in motor performance and learning. However, while some investigators have suggested that the effect of attentional focus varies with task difficulty, others reported external focus benefits regardless of difficulty. We hypothesized that attentional focus effects would vary with practice, due to changes in the individual's processing efficiency. We had three 20-person participant groups (external focus instructions, internal focus instructions, control) practice three difficulty levels of a Fitts reciprocal tapping task over two days. Participants in the external/internal focus groups were instructed to "mentally focus on moving the pen/your hand as fast and accurately as possible," while control participants were instructed to "mentally focus only on doing your best to achieve the task goal." We then analyzed the effect of attentional focus by task difficulty at the initial performance (the beginning of the practice) and after learning (the retention/transfer phase), using movement time (MT) and number of error taps (Err) as performance measures. The internal focus group made more errors than the control group only at the retention/transfer phase. We found no error differences between the external and internal focus groups, and there were no MT differences between any groups. Our primary hypothesis about the differential effect of attentional focus by practice was supported. The attentional focus effect on Err differed in the retention/transfer phase from the immediate phase, suggesting that practice mediated the attentional focus effect. Gait asymmetries are a common problem in clinical populations, such as those with a history of stroke or Parkinson's disease. The use of a split-belt treadmill is one way to enhance gait symmetry but relies on specialty (and typically expensive) equipment. Alternatively, visual cues have been shown as a method to alter gait mechanics, but their utility in altering gait symmetry has been relatively understudied. Before deploying this method to clinical populations, a proof-of-concept study is needed to explore using visual cues to alter gait symmetry in healthy adults. Therefore, the purpose of this study was to examine the extent to which healthy adults could synchronize to an asymmetric visual cue with a small or large gait asymmetry using wearable sensors to measure gait asymmetries. Seventy-two healthy adults (ages: 23.89 ± 6.08 years) walked on the treadmill for two conditions: with and without the visual cue. Each walking condition lasted 10 min at the participant's preferred walking speed. Inertial sensors were used to measure gait asymmetries. Some participants did not respond to the visual cue, and groups were separated into responders and non-responders. Participants in the small and large asymmetry-responder groups exhibited statistically significant increased asymmetries in single limb support % (p < 0.01) and step duration (s) (p < 0.05, p < 0.01, respectively). Only the large asymmetry-responder group showed statistically significant (p < 0.01) increased asymmetries in stride length. Assessing postural control is important for the assessment of motor function after concussion. Data used for postural control assessment typically do not take the sport played, age, or sex of the athlete into consideration. It is plausible these variables may be significant when making return-to-play decisions. This study used the BTrackS database to examine differences in postural control in athletes playing different types of sports and across sex and age. BTrackS data from 9093 high school to college-aged athletes (aged 14–22 years) were examined employing a One-way ANOVA with a post-hoc test to compare CoP path length between sport types. A moderation analysis was used to test interaction effects of sex and age on a CoP/BMI ratio. Significant differences were observed between sport types, F(3,9089) = 42.4, p <.001, η2 = 0.014. Post hoc tests indicated that collision (M = 25.0, SD = 7.6) sport athletes exhibited significantly higher CoP measures compared to the contact (M = 23.4, SD = 7.4), limited contact (M = 22.9, SD = 6.9), and non-contact (M = 23.0, SD = 7.4) athletes. There was no difference between other sport types (p >.20). A significant mean sex difference (Mmale = 0.924, Mfemale = 0.898, p <.001) and a quadratic association with age, (β = −0.042, p <.001) was observed. Further, magnitude of those age differences decreased with age (β = 0.011, p <.001). An interaction of age and sex was significant for linear (β = 0.020, p <.001) and quadratic terms (β = −0.006, p <.001). Athletes exhibited different postural control when the type of sport, age, and sex was taken into consideration. Abstract: Variable practice may be beneficial for learning novel motor patterns. The purpose of this study was to determine the effect of a variable practice intervention during treadmill walking on the dynamic properties of gait. Using a counterbalanced design, 16 subjects with hemiparesis following chronic stroke performed 20min of treadmill walking in constant speed training (CST) and variable speed training (VST) conditions. The dynamic properties of the hip and knee sagittal plane angles for two minutes before (pre-data) and two minutes after (post-data) CST and VST were examined using detrended fluctuation analysis and sample entropy. A main effect for time was observed for sample entropy of the knee angle; no other differences were observed between the pre/post data for the CST or VST conditions. While variable practice conditions are intended to promote movement errors for improved learning, we were unable to detect immediate changes in movement variability as a function of practice condition following a single session of gait training for individuals post-stroke. Abstract: During locomotion over uneven terrain, gait must be adapted to avoid a trip. In the event of a foot-obstacle contact, the body reactively responds to the perturbation. However, it is unknown if any proactive adjustments are made in subsequent strides to reduce the likelihood of another contact, and how long any proactive adaptations persist. This study examined gait behavior while stepping over a 10cm obstacle placed in the middle of an 8m walkway. The four obstacle crossings that preceded a spontaneous obstacle contact were compared to the eight obstacle crossings subsequent to the contact. Foot position before the obstacle was not modified following the obstacle contact. However, toe clearance and peak toe elevation increased in the limb that was tripped; the unperturbed limb showed no differences. These findings demonstrate that the sensory information of the perturbed limb proactively influenced the ipsilateral but not the contralateral limb, supporting the idea that the lead and trail limb are controlled independently during obstacle crossing. The proactive adaptation lasted for at least eight trials, suggesting that an unexpected perturbation influences the control of adafptive gait well after obstacle contact. [Copyright &y& Elsevier]

Context: Predicting and promoting physical performance are important goals within the tactical professional community. Movement screens are frequently used in this capacity but are poor predictors of performance outcomes. It has recently been shown that prediction improved when movement quality was evaluated under load, but the mechanisms underlying this improvement remain unclear. Because balance, range of motion, and strength are mutually relevant to physical performance and movement quality, these attributes may mediate load-related decreases in movement quality and account for the resulting increase in performance prediction. Objective: To quantify the roles of balance, range of motion, and strength in mediating load-related decreases in clinical movement-screen scores. Design: Crossover study. Setting: Research laboratory. Patients or Other Participants: Twenty-five male (age = 23.96 ± 3.74 years, height = 178.82 ± 7.51 cm, mass = 79.66 ± 12.66 kg) and 25 female (age = 22.00 ± 2.02 years, height = 165.40 ± 10.24 cm, mass = 63.98 ± 11.07 kg) recreationally active adults. Intervention(s): Participants completed a clinical movement screen under a control condition and while wearing an 18.10-kg weighted vest as well as tests of balance, range of motion, and strength. Main Outcome Measure(s): Item score differences were assessed using Wilcoxon signed rank tests for matched pairs. Interactions between (1) balance, range of motion, and strength and (2) load condition were modeled using penalized varyingcoefficients regression with item scores as the dependent measure. Results: Except for the hurdle step, item scores were lower in the weighted-vest than in the control condition for all tests (P < .05). Except for rotary stability, F statistics were significant for all models (P values < .05, R² values = 0.22-0.77). Main effects of balance, range of motion, and strength on Functional Movement Screen scores were observed (P < .05); however, little evidence was found to suggest that these attributes mediated load-related decreases in Functional Movement Screen item scores. Brain–body interactions (BBIs) have been the focus of intense scrutiny since the inception of the scientific method, playing a foundational role in the earliest debates over the philosophy of science. Contemporary investigations of BBIs to elucidate the neural principles of motor control have benefited from advances in neuroimaging, device engineering, and signal processing. However, these studies generally suffer from two major limitations. First, they rely on interpretations of 'brain' activity that are behavioral in nature, rather than neuroanatomical or biophysical. Second, they employ methodological approaches that are inconsistent with a dynamical systems approach to neuromotor control. These limitations represent a fundamental challenge to the use of BBIs for answering basic and applied research questions in neuroimaging and neurorehabilitation. Thus, this review is written as a tutorial to address both limitations for those interested in studying BBIs through a dynamical systems lens. First, we outline current best practices for acquiring, interpreting, and cleaning scalp-measured electroencephalography (EEG) acquired during whole-body movement. Second, we discuss historical and current theories for modeling EEG and kinematic data as dynamical systems. Gait asymmetries are commonly observed in neurological populations and linked to decreased gait velocity, balance decrements, increased fall risk, and heightened metabolic cost. Interventions designed to improve gait asymmetries have varying methods and results. The purpose of this systematic review was to investigate non-pharmacological interventions to improve gait asymmetries in neurological populations. Keyword searches were conducted using PubMed, CINAHL, and Academic Search Complete. The search yielded 14 studies for inclusion. Gait was assessed using 3D motion capture systems (n = 7), pressure-sensitive mats (e.g., GAITRite; n = 5), and positional sensors (n = 2). The gait variables most commonly analyzed for asymmetry were step length (n = 11), stance time (n = 9), and swing time (n = 5). Interventions to improve gait asymmetries predominantly used gait training techniques via a split-belt treadmill (n = 6), followed by insoles/orthoses (n = 3). The literature suggests that a wide range of methods can be used to improve spatiotemporal asymmetries. However, future research should further examine kinematic and kinetic gait asymmetries. Additionally, researchers should explore the necessary frequency and duration of various intervention strategies to achieve the greatest improvement in gait asymmetries, and to determine the best symmetry equation for quantifying gait asymmetries. Contact and collision sports are believed to accelerate brain aging. Postmortem studies of the human brain have implicated tau deposition in and around the perivascular space as a biomarker of an as yet poorly understood neurodegenerative process. Relatively little is known about the effects that collision sport participation has on the age-related trajectories of macroscale brain structure and function, particularly in female athletes. Diffusion MRI and resting-state functional MRI were obtained from female collision sport athletes (n = 19 roller derby (RD) players; 23–45 years old) and female control participants (n = 14; 20–49 years old) to quantify structural coupling (SC) and decoupling (SD). The novel and interesting finding is that RD athletes, but not controls, exhibited increasing SC with age in two association networks: the frontoparietal network, important for cognitive control, and default-mode network, a task-negative network (permuted p = 0.0006). Age-related increases in SC were also observed in sensorimotor networks (RD, controls) and age-related increases in SD were observed in association networks (controls) (permuted p ≤ 0.0001). These distinct patterns suggest that competing in RD results in compressed neuronal timescales in critical networks as a function of age and encourages the broader study of female athlete brains across the lifespan. Adults (N = 54, 80.78 ± 6.08 years) who reported falling during the previous 12 months participated in a 12-week wobble board training program with internal focus or external focus (EF) instructions. Verbal manipulation checks were performed after training sessions as a self-report of the attentional foci used. The percentage of sessions in which participants reported using an EF (EFSR) was subsequently calculated. Mean velocity and mean power frequency in the anterior–posterior (MVELOAP and MPFAP) and medial–lateral (MVELOML and MPFML) direction were assessed during a 35-s wobble board task at Weeks 0, 6, 12, 13, 16, and 20, with the latter three as retention tests. Piecewise linear growth models estimated treatment effects on individual growth trajectories of MVELOAP and ML and MPFAP and ML during intervention and retention periods. Regardless of condition, MVELOML significantly decreased (π = −.0019, p =.005) and MPFML increased (π =.025, p <.02) during the intervention period. In analyses including interaction terms, participants in the EF group who reported greater EFSR had superior progression of MPFAP during the intervention (π =.0013, p =.025). Introduction Special Weapons and Tactics (SWAT) personnel who practice breaching with blast exposure are at risk for blast-related head trauma. We aimed to investigate the impact of low-level blast exposure on underlying white matter (WM) microstructure based on diffusion tensor imaging (DTI) and neurite orientation and density imaging (NODDI) in SWAT personnel before and after breacher training. Diffusion tensor imaging is an advanced MRI technique sensitive to underlying WM alterations. NODDI is a novel MRI technique emerged recently that acquires diffusion weighted data from multiple shells modeling for different compartments in the microstructural environment in the brain. We also aimed to evaluate the effect of a jugular vein compression collar device in mitigating the alteration of the diffusion properties in the WM as well as its role as a moderator on the association between the diffusion property changes and the blast exposure. Materials and Methods Twenty-one SWAT personnel (10 non-collar and 11 collar) completed the breacher training and underwent MRI at both baseline and after blast exposure. Diffusion weighted data were acquired with two shells (b  = 1,000, 2,000 s/mm2) on 3T Phillips scanners. Diffusion tensor imaging metrices, including fractional anisotropy, mean, axial, and radial diffusivity, and NODDI metrics, including neurite density index (NDI), isotropic volume fraction (fiso), and orientation dispersion index, were calculated. Tract-based spatial statistics was used in the voxel-wise statistical analysis. Post hoc analyses were performed for the quantification of the pre- to post-blast exposure diffusion percentage change in the WM regions with significant group difference and for the assessment of the interaction of the relationship between blast exposure and diffusion alteration. Results The non-collar group exhibited significant pre- to post-blast increase in NDI (corrected P  < .05) in the WM involving the right internal capsule, the right posterior corona radiation, the right posterior thalamic radiation, and the right sagittal stratum. A subset of these regions showed significantly greater alteration in NDI and fiso in the non-collar group when compared with those in the collar group (corrected P  < .05). In addition, collar wearing exhibited a significant moderating effect for the alteration of fiso for its association with average peak pulse pressure. Conclusions Our data provided initial evidence of the impact of blast exposure on WM diffusion alteration based on both DTI and NODDI. Sport participation can have immense health benefits. However, mechanical loading of the head that is common in contact and collision sports could change brain structure and function in a way that accelerates the onset of cognitive decline later in life. The generalizability of most research on athlete brain health is limited by a primary focus on collegiate male athletes. PURPOSE: We sought to compare brain connectivity between Women's Flat Track Roller Derby (RD) athletes and female non-athletes (NA). METHODS: RD athletes (n=19, 24-41 years old) and NA (n=14, 20-49 years old) underwent structural, functional, and diffusion-weighted MR imaging. After preprocessing of the functional data, a surface-based cortical parcellation was used to group dense timeseries (32k 'greyordinates' per hemisphere) into 500 areas corresponding to 7 widely studied intrinsic brain networks. Functional connectivity (FC) was estimated by elastic-net regularization of the areal BOLD signals. After diffeomorphic reconstruction of pre-processed diffusion data, fiber tracking was performed until 1 million streamlines had been constructed for each individual. Structural connectivity (SC) was defined as the number of streamlines connecting each pair of 500 cortical areas. Group differences in structural and functional connectivity were tested using the network based statistic (5000 permutations) with a primary threshold t>2.04. In whole-brain networks that were observed to be larger ('intensity') than 95% of the permuted networks, 'edges' that passed the initial threshold were averaged within and between 7 intrinsic brain networks. The resulting graphs (7 x 7 networks) were inspected to characterize group differences. RESULTS: RD athletes exhibited a pattern of lower FC (p=.032) and lower SC (p = .046) than NA. The greatest differences in FC were observed within the limbic, ventral attention, and dorsal attention networks. The greatest differences in SC were observed within the visual and frontoparietal networks and between the frontoparietal and attention networks. CONCLUSIONS: Compared to non-athletes, brain structural and functional connectivity was lower in women competing in Roller Derby. Military service members are frequently subjected to subconcussive blast events during training and deployment. Emerging evidence suggests blast exposures of these magnitudes may have long-term consequences for dimensions of cognitive function. Less is known about cognitive sequelae acutely following deployment-related subconcussive blast events. The current study addressed this knowledge gap by assessing the extent to which subconcussive blast exposure affected performance on the Automated Neuropsychological Assessment Metrics 4 TBI-MIL (ANAM). Baseline-referenced and normative comparisons of archival ANAM data were analyzed for a cohort of personnel who were exposed to blast (blast group; n = 27) and personnel who were not exposed to blast (no-blast group; n = 36) that were otherwise asymptomatic for a concussion. The blast group exhibited statistically significant lower scores compared to the no-blast group (between-subjects), baseline assessments (within-subjects), and an age-matched normative population. Normative comparisons revealed that the scores for the reaction time subtests (i.e., procedural and both simple reaction time tasks) were outside the range of normal functioning (1 SD) and reliable change indices revealed clinically meaningful change only for simple reaction time. Background: The development of stable postural control is characterized by changes in sway variability and periods of rapid reorganization of motor system components.Research Question: The current study examined whether changing biomechanical and perceptual demands influences the postural control behavior during development.Method: The center of pressure (COP) was assessed via a cross-sectional design. Data were collected from 48 females in three age groups (late childhood, mid-adolescence, and young adulthood) during four quiet stance conditions: (1) eyes open with feet apart, (2) eyes open with feet together, (3) eyes closed with feet apart, and (4) eyes closed with feet together. Linear measures included total path length of the COP and the mean/standard deviation of the overall COP position and speed. To characterize the sway patterns via nonlinear analyses, the speed and two-dimensional positional time series were submitted to sample entropy and Renyi entropy, respectively.Results: The linear results indicated that the late childhood group displayed longer COP trajectories (p < .001) and faster and more variable COP speed (p's < .001). These results held for both the feet apart and feet together conditions, independent of vision. The nonlinear results indicated that the late childhood group exhibited less regularity, overall, in their COP sway position (i.e., Renyi entropy) compared to the two older groups in the feet apart condition (p's ≤ .041), and to the young adults in the feet together condition, independent of vision (p <  .001). However, the mid-adolescent group demonstrated greater regularity in their COP speed (i.e., sample entropy) when their eyes were closed compared to the other two groups, independent of stance (p's < .05).Significance: The linear results support previous findings, while the nonlinear measures indicate sway characteristics that may provide a window into the development of underlying control processes that regulate quiet standing. The use of virtual reality as a training mechanism continues to gain popularity as equipment becomes more readily available. It is important to not only understand the relationship between virtual reality training and motor learning, but to understand the extent to which practice manipulations enhance performance in virtual reality. One common practice manipulation is adopting an external focus of attention, which has been shown to facilitate motor learning in a variety tasks. The purposes of the present study were to investigate the effectiveness of an external focus of attention and the effects of target occlusion times in virtual reality. Fifty-six participants performed a single-leg long jump during baseline, training, and retention and were randomly assigned to either an external or control group. During baseline and retention, all participants performed the task in both a virtual reality (VR) and real world (RW) environments. Training was all done in VR where participants were provided an external focus cue or no cue. Results revealed that individuals jumped significantly further in RW than VR in both baseline and retention (p < .001). During training, the external group jumped significantly further than control (p < .05). These results suggest that the adoption of an external focus improves performance during training. However, we did not see a benefit of an external focus in retention. These findings should be taken into consideration when using virtual reality as a training tool when performance must be transferred to a real world environment. [ABSTRACT FROM AUTHOR]

Objective: Examine the acceptability of an evidence-based, contextually tailored, virtual avatar coaching approach for nutrition education among adult-child dyads with low income. Design: Structured observations and semistructured interviews. Setting: Low-income communities in 2 midsized urban areas. Participants: Fifteen African American adult and child (aged 5−10 years old) dyads recruited from community agencies. Phenomenon of Interest: Structured observation during the use of the virtual avatar coaching program and qualitative feedback on program experience after using the program. Analysis: Transcripts were independently coded by 2 trained coders. Content analysis was conducted to determine trends and patterns in responses, and salient quotes were extracted from the manuscripts. Results: The program generated dialogue between the children and adults around preferences and dietary habits. Adults and children described the program as being helpful, informative, motivational, and fun. Adults and children liked that the avatar acknowledged them at a personal level, provided accountability, and was tailored to them and their environment. Children wanted the ability to customize the avatar, more gamification, and avatar demonstration of healthy behaviors. Children felt the program would improve their health behaviors and were interested in using it again. Conclusions and Implications: This study highlights information that can be used to improve avatarbased programs for nutrition education with children and their adult caregivers who have low income. Future work should assess the impact of user customization experience, program impact on dietary behaviors, and resources required to scale and maintain the program with community agencies. Obstacle crossing, such as stepping over a curb, becomes more challenging with natural aging and could lead to obstacle-related trips and falls. To reduce fall-risk, obstacle training programs using physical obstacles have been developed, but come with space and human resource constraints. These barriers could be removed by using a virtual obstacle crossing training program, but only if the learned gait characteristics transfer to a real environment. We examined whether virtual environment obstacle crossing behavior is transferred to crossing real environment obstacles. Forty participants (n = 20 younger adults and n = 20 older adults) completed two sessions of virtual environment obstacle crossing, which was preceded and followed by one session of real environment obstacle crossing. Participants learned to cross the virtual obstacle more safely and that change in behavior was transferred to the real environment via increased foot clearance and alterations in foot placement before and after the real environment obstacle. Further, while both age groups showed transfer to the real environment task, they differed on the limb in which their transfer effects applied. This suggests it is plausible to use virtual reality training to enhance gait characteristics in the context of obstacle avoidance, potentially leading to a novel way to reduce fall-risk. Special Weapons and Tactics (SWAT) personnel who conduct breacher exercises are at risk for blast-related head trauma. We aimed to investigate the potential impact of low-level blast exposure during breacher training on the neural functioning of working memory and auditory network connectivity. We also aimed to evaluate the effects of a jugular vein compression collar, designed to internally mitigate slosh energy absorption, preserving neural functioning and connectivity, following blast exposure. A total of 23 SWAT personnel were recruited and randomly assigned to a non-collar (n = 11) and collar group (n = 12). All participants completed a 1-day breacher training with multiple blast exposure. Prior to and following training, 18 participants (non-collar, n = 8; collar, n = 10) completed functional magnetic resonance imaging (fMRI) of working memory using N-Back task; 20 participants (non-collar, n = 10; collar, n = 12) completed resting-state fMRI. Key findings from the working memory analysis include significantly increased fMRI brain activation in the right insular, right superior temporal pole, right inferior frontal gyrus, and pars orbitalis post-training for the non-collar group (p < 0.05, threshold-free cluster enhancement corrected), but no changes were noted for the collar group. The elevation in fMRI activation in the non-collar group was found to correlate significantly (n = 7, r = 0.943, p = 0.001) with average peak impulse amplitude experienced during the training. In the resting-state fMRI analysis, significant pre- to post-training increase in connectivity between the auditory network and two discrete regions (left middle frontal gyrus and left superior lateral occipital/angular gyri) was found in the non-collar group, while no change was observed in the collar group. These data provided initial evidence of the impact of low-level blast on working memory and auditory network connectivity as well as the protective effect of collar on brain function following blast exposure, and is congruent with previous collar findings in sport-related traumatic brain injury. Obstacle crossing, such as stepping over a curb, becomes more challenging with natural aging and could lead to obstacle-related trips and falls. To reduce fall-risk, obstacle training programs using physical obstacles have been developed, but come with space and human resource constraints. These barriers could be removed by using a virtual obstacle crossing training program, but only if the learned gait characteristics transfer to a real environment. We examined whether virtual environment obstacle crossing behavior is transferred to crossing real environment obstacles. Forty participants (n = 20 younger adults and n = 20 older adults) completed two sessions of virtual environment obstacle crossing, which was preceded and followed by one session of real environment obstacle crossing. Participants learned to cross the virtual obstacle more safely and that change in behavior was transferred to the real environment via increased foot clearance and alterations in foot placement before and after the real environment obstacle. Further, while both age groups showed transfer to the real environment task, they differed on the limb in which their transfer effects applied. This suggests it is plausible to use virtual reality training to enhance gait characteristics in the context of obstacle avoidance, potentially leading to a novel way to reduce fall-risk. Context: Greater knee-joint laxity may lead to a higher risk of knee injury, yet it is unknown whether results of self-reported outcome measures are associated with distinct knee-laxity profiles. Objective: To identify the extent to which multiplanar knee laxity is associated with patient-reported outcomes of knee function in healthy individuals during activities of daily living and sport. Design: Descriptive laboratory study. Setting: University research laboratory. Patients or Other Participants: Forty healthy individuals (20 men, 20 women; age = 18-31 years). Main Outcome Measure(s): All participants were given the Knee Outcome Survey Activities of Daily Living Scale (KOSADL) and Sports Activities Scale (KOS-SAS) and subsequently measured for knee laxity in the sagittal, frontal, and transverse planes. Separate backward stepwise regression analyses were performed to determine the extent to which multiplanar kneelaxity values predicted KOS-ADL and KOS-SAS scores within each sex. Results: Women had higher magnitudes of anterior, posterior (POSTLAX), varus (VARLAX), valgus (VALLAX), and internal-rotation laxity than men and trended toward greater external rotation (ERLAX) laxity. Greater POSTLAX, less VALLAX, and greater VARLAX was associated with lower KOS-ADL scores (KOS-ADL = -4.8 [POSTLAX], + 3.3 [VALLAX] - 2.2 [VARLAX] + 100.4, R² = 0.74, P < .001) and greater POSTLAX and less VALLAX was associated with lower KOS-SAS scores (KOS-SAS = -8.2 [POSTLAX],+3.6 [VALLAX]+96.4, R²=0.67, P < .001) in women. In men, greater POSTLAX and less ERLAX was associated with lower KOS-SAS scores (KOS-ADL = -4.7 [POSTLAX], + 0.9 [ERLAX] + 96.4, R² = 0.49, P < .001). Conclusions: The combination of POSTLAX with less relative VALLAX (women) or less relative ERLAX (men) was a strong predictor of KOS scores, suggesting that a self-reported outcome measure may be beneficial as part of a preparticipation screening battery to identify those with perceived functional deficits associated with their knee laxity. Focus of attention during dual-tasks and practice schedules are important components of motor skill performance and learning; often studied in isolation. The current study required participants to complete a simple key-pressing task under a blocked or random practice schedule. To manipulate attention, participants reported their finger position (i.e., skill-focused attention) or the pitch of an auditory tone (i.e., extraneous attention) while performing two variations of a dual-task key-pressing task. Analyses were conducted at baseline, 10 min and 24h after acquisition. The results revealed that participants in a blocked schedule, extraneous focus condition had significantly faster movement times during retention compared to a blocked schedule, skill focus condition. Furthermore, greatest improvements from baseline to immediate and delayed retention were evident for an extraneous attention compared to the skill-focused attention, regardless of practice schedule. Nonlinear dynamics quantifies gait variability patterns, which can be useful in evaluating functional ability. A commonly used nonlinear technique is detrended fluctuation analysis (DFA). Safety support structures have previously been shown to alter DFA during gait. However, the effect of a nonweight-supporting treadmill harness on DFA during gait has yet to be determined. The purpose of this study was to determine whether a nonweight-supporting harness influenced the DFA alpha metric (DFA α) of variables typically used to examine gait function. Twenty participants (10 young adults and 10 older adults) were recruited for this study. Each participant completed one testing session on a treadmill consisting of three conditions: (1) no harness, (2) harnessed, but not attached to the support frame, and (3) harnessed and attached to the support frame. Participants walked for 15 min at the same self-selected speed for each condition. The gait variables of stride time, stride length, and step width for each condition were analyzed using DFA α to examine gait function. There were no significant interactions between age group and condition for DFA α of each variable. Additionally, there were no main effects for DFA α for age group or condition. These data indicate that a nonweight-supporting harness can be used for safety without impeding the emergence of natural gait dynamics when stride time, stride length, and step width are the primary variables of interest. Discrete wavelet analysis is used to resolve the center of pressure time series data into several timescale components, providing new insights into postural control. Healthy young and elderly participants stood quietly with their eyes open or closed and either performed a secondary task or stood quietly. Without vision, both younger and older participants had reduced energy in the long timescales, supporting the concept that vision is used to control low frequency postural sway. Furthermore, energy was increased at timescales corresponding to closed-loop (somatosensory and vestibular) and open-loop mechanisms, consistent with the idea of a shift from visual control to other control mechanisms. However, a relatively greater increase was observed for older adults. With a secondary task a similar pattern was observed—increased energy at the short and moderate timescales, decreased energy at long timescales.
