Background: The objective of this study was to evaluate wear and surface roughness of in vivo irradiated human enamel and dentin after abrasive challenge. Methods: Using teeth donated by healthy patients or with head and neck cancer who received radiotherapy (RT) or chemoradiotherapy (CRT), 42 enamel and dentin specimens were made, divided into three groups: control, RT and CRT (n = 14 in each group, both for enamel and dentin). The specimens were submitted to an in vitroexperimental abrasive using a brushing machine and wear and surface roughness were evaluated. Results: In enamel specimens, wear profile values of CRT and RT groups were not different from control (p-value >0.05). The RT group presents lower step values than CRT and control groups (p-value p-value >0.05). In dentin specimens, no significant difference in wear profile and step was observed in all groups (p-value >0.05). However, CRT and RT groups present higher values in final surface roughness (p-value Conclusion: The exposure to ionizing radiation (associated or not to chemotherapy) influenced the surface roughness of dentin and the wear (step) of enamel after the in vitro abrasive challenge. Trial registration: Ethical procedures were approved by the FORP/USP Research Ethics Committee (CAAE: 61308416.4.0000.5419) and Hospital do Câncer de Barretos/Fundação Pio XII (CAAE: 61308416.4.3001.5437). Clinical relevance: Considering probable effects of ionizing radiation on teeth structure, understanding the behaviour of enamel and dentin under physiological processes is important to prevent and treat adverse oral side effects related to radiotherapy.

Introduction: Internet users frequently search for information on health issues. The behavior of Internet users may be related to health contexts experienced in their communities, reflecting unmet demands. Objective: To analyze the interest of Internet users on breast cancer in Brazil between 2017 and 2021. Method: An infodemiological study was carried out using the relative search volume (ranging from 0 to 100) for the term “câncer de mama” (breast cancer) in Brazil, retrieved from the Google Trends tool. The annual relative search volume was compared with a non-parametric approach and significance value (p) of 5%. Results: Except between 2019 and 2020 (p-value>0.05), there was a significant increase in Internet searches for the term “câncer de mama” in Brazil (all p-values

Introduction: Breast cancer may affect different profiles of women worldwide. In addition, chemotherapy to treat breast neoplasms directly affects health-related quality of life. Objective: To describe the clinical-epidemiological profile and to compare the general and specific health-related quality of life of women with breast cancer during chemotherapy. Method: In an observational and prospective study, 140 women with breast cancer in northeastern Brazil were evaluated in the intermediate cycle and at the end of chemotherapy. Quality of life was assessed using a general instrument (EORTC-QLQ-C30) and a specific module (EORTC-QLQ-BR23). Data were compared with a paired non-parametric test, with a significance level of 5%. Results: Regarding the clinical-epidemiological profile, the median age was 50 years, 82.9% were black or mixed-race, and 95% lived in Sergipe, Brazil. In addition, 50.7% had no occupation, the median family income was one minimum wage, as well as the median of education was eight years of study and only 40.7% have completed high school. Considering the data from the C30 and BR23 questionnaires, it was observed that most items and scales worsened at the end of chemotherapy when compared to the intermediate cycle. Among the items and scales with significant differences, most had moderate or high effect sizes. Conclusion: It is possible to conclude that the clinical-epidemiological profile was unfavorable and chemotherapy reduced several aspects of the health-related quality of life of women with breast cancer. Introducción: El cáncer de mama puede afectar a diferentes perfiles de mujeres en todo el mundo. Además, la quimioterapia para tratar las neoplasias de mama afecta directamente la calidad de vida relacionada con la salud. Objetivo: Describir el perfil clínico-epidemiológico y comparar la calidad de vida relacionada con la salud general y específica de mujeres con cáncer de mama durante quimioterapia. Método: Estudio observacional y prospectivo con 140 mujeres con cáncer de mama en el Noreste de Brasil evaluadas en la mitad del ciclo y al final de la quimioterapia. La calidad de vida se evaluó mediante un instrumento general (EORTC-QLQ-C30) y un módulo específico (EORTC-QLQ-BR23). Los datos se compararon con una prueba pareada no paramétrica, con un nivel de significación del 5%. Resultados: En cuanto al perfil clínico-epidemiológico, la mediana de edad fue de 50 años, el 82,9% eran negros o mestizos y el 95% vivían en Sergipe, Brasil. Además, el 50,7 % no tenía ocupación, la renta familiar mediana era de un salario mínimo, así como el nivel educativo medio era de ocho años de estudio y solo el 40,7 % tenían la secundaria completa. Considerando los datos de los cuestionarios C30 y BR23, se observó que la mayoría de los ítems y escalas empeoró al final de la quimioterapia en comparación con el ciclo intermedio. Entre los ítems y escalas con diferencias significativas, la mayoría tuvo tamaños del efecto moderados o altos. Conclusión: Es posible concluir que el perfil clínico-epidemiológico fue desfavorable y la quimioterapia redujo varios aspectos de la calidad de vida relacionada con la salud de las mujeres con cáncer de mama. Além disso, a quimioterapia para tratar neoplasias mamárias afeta diretamente a qualidade de vida relacionada à saúde. Objetivo: Descrever o perfil clínico-epidemiológico e comparar a qualidade de vida relacionada à saúde geral e específica de mulheres com câncer de mama durante a quimioterapia. Método: Estudo observacional e prospectivo com 140 mulheres com câncer de mama no Nordeste do Brasil avaliadas no ciclo intermediário e ao final da quimioterapia. A qualidade de vida foi avaliada por um instrumento geral (EORTC-QLQ-C30) e um módulo específico (EORTC-QLQ-BR23). Os dados foram comparados com teste não paramétrico pareado, com nível de significância de 5%. Resultados: Em relação ao perfil clínico-epidemiológico, a mediana da idade foi de 50 anos, 82,9% eram pretas ou pardas e 95% moravam em Sergipe, Brasil. Além disso, 50,7% não possuíam ocupação, a mediana da renda familiar foi de um salário-mínimo, bem como a mediana da escolaridade foi de oito anos de estudo, e somente 40,7% tinham ensino médio completo. Considerando os dados dos questionários C30 e BR23, observou-se que a maior parte dos itens e escalas piorou ao final da quimioterapia quando comparados ao ciclo intermediário. Entre os itens e escalas com diferenças significativas, a maioria apresentou tamanho de efeito moderado ou alto. Conclusão: É possível concluir que o perfil clínico-epidemiológico foi desfavorável, e a quimioterapia reduziu diversos aspectos da qualidade de vida relacionada à saúde de mulheres com câncer de mama.

Introduction: The use of elastomeric separators during orthodontic treatment can cause pain and low-level laser therapy has been used to manage this outcome. Objective: To present the evidence on the effect of low-level laser therapy in the management of pain caused by the use of elastomeric separators. Methods: An integrative review of clinical studies was carried out in the PubMed, Web of Science, Scopus, BVS, SciELO and clinicaltrials.gov databases using a PICo strategy developed with descriptors, including studies published in the last ten years, without language restriction. 68 articles were tracked and 12 were reviewed. Literature review: Considering laser therapy, most studies used gallium and aluminum arsenide (Ga-Al-As), with a wavelength between 808nm and 940nm, as well as power between 40.6mW and 200mW. The visual analog scale (VAS) was used by most studies to measure the pain. Two studies did not find significant differences in low-level laser therapy in pain caused by elastomeric separators, eight studies found significant differences between the intervention, control and placebo groups (when used) and two studies were not fully conclusive and pointed out partial statistical differences between the groups in question. The application techniques were too heterogeneous in the studies included in this review and do not allow a quantitative synthesis. Conclusion: The low-level laser therapy can demonstrate beneficial effects in reducing pain caused by elastomeric separators. However, it is not yet possible to determine the best application technique.

Patients with molar-incisor hypomineralization (MIH) may manifest tooth hfypersensitivity and difficulties in undergoing dental treatment, including fear, anxiety, and behavior problems in the dental office. Then, the objective of this study was to report a clinical case series using inhalation sedation with nitrous oxide and oxygen as a clinical strategy in the management of MIH pediatric patients during dental treatment. Pediatric patients and their legal guardians were properly informed and data were extracted from clinical charts. Six clinical cases were included, whose patients were between 8 and 12 years old. Of these, 4 were female. The indication of moderate sedation took into account the clinical difficulties related to patients with MIH, such as obtaining anesthesia and cooperation, in addition to odontophobia and dental anxiety. All patients were carefully examined to undergo the sedation technique, including medical history and previous experiences with nitrous oxide inhalation sedation, as well as the vital signs were properly evaluated before, during and after the procedures. The nitrous oxide concentration ranged between 30% and 60%. The procedures were restorative, endodontic or surgical (extractions). In general, the use of sedation contributed to the performance of dental procedures. The use of inhaled sedation helped in behavioral management and during the treatment, increasing the effectiveness of local anesthesia. Patients were cooperative for treatment, while no adverse effects or complications related to sedation were observed. Thus, for the patients in this case series, the use of inhalation sedation with nitrous oxide and oxygen contributed to the clinical management of patients with MIH.

Objective: To describe the occurrence of oral mucositis (OM) in women undergoing cancer treatment for breast cancer (BC). Methods: This is a retrospective, quantitative, and analytical study in medical records of women diagnosed with BC in an oncology service. Clinical data were collected regarding the occurrence and severity of OM according to the World Health Organization criteria and the cancer treatment experienced by the participants. Results: 196 women were included. Of these, 97 (49.5%) developed OM, 43.4% of which were grade 1 or 2 (low or moderate). The occurrence was higher in white women (OR 1.93; 95% CI 1.04 - 3.57; p = 0.035), with metastatic breast cancer (OR 5.46; 95% CI 1.79 - 16.64; p = 0.002) and who experienced taxane agents at some point during chemotherapy (OR 2.26; 95% CI 1.12 - 4.56; p = 0.02). The mean severity of OM in the entire sample was 0.8 ± 1.0, and in the affected women was 1.7 ± 0.7. The difference in the severity of OM by the variables was observed only among women with grade 2 and grade 3 fatigue (p = 0.03).Conclusions: OM is a common mucocutaneous toxicity in women with BC. Despite the low severity observed, care for women with BC undergoing cancer treatment must consider the possible risks and complications associated with OM, adopting strategies to prevent, monitor, and treat them. Objetivo: Descrever a ocorrência de mucosite oral (MO) em mulheres em tratamento oncológico para câncer de mama (CM).Métodos: Trata-se de um estudo retrospectivo, quantitativo e analítico em prontuários de mulheres com diagnóstico de CM em um serviço de oncologia. Dados clínicos foram coletados quanto à ocorrência e gravidade da MO de acordo com os critérios da Organização Mundial da Saúde e o tratamento oncológico vivenciado pelas participantes.Resultados: Foram incluídas 196 mulheres. Destas, 97 (49,5%) desenvolveram MO, sendo 43,4% grau 1 ou 2 (baixo ou moderado). A ocorrência foi maior em mulheres brancas (OR 1,93; IC95% 1,04 – 3,57; p = 0,035), com câncer de mama metastático (OR 5,46; IC95% 1,79 – 16,64; p = 0,002) e que experimentaram agentes taxanos em algum momento da quimioterapia (OR 2,26; IC95% 1,12 – 4,56; p = 0,02). A gravidade média da MO em toda a amostra foi de 0,8 ± 1,0 e nas mulheres afetadas foi de 1,7 ± 0,7. Diferença na gravidade da MO pelas variáveis foi observada apenas entre as mulheres com fadiga grau 2 e grau 3 (p = 0,03).Conclusões: MO é uma toxicidade mucocutânea comum em mulheres com CM. Apesar da baixa gravidade observada, o cuidado à mulher com CM em tratamento oncológico deve considerar os possíveis riscos e complicações associadas à MO, adotando estratégias para prevenir, monitorar e tratá-las.

OBJETIVO: Identificar o perfil clínico de pacientes oncológicos e reações de hipersensibilidade aos agentes antineoplásicos sistêmicos. MÉTODO: Trata-se de um estudo documental e retrospectivo, com dados obtidos de prontuários clínicos de pacientes oncológicos em tratamento quimioterápico. Foram analisados 249 prontuários clínicos entre janeiro de 2013 e janeiro de 2014 para identificar reações de hipersensibilidade e extrair dados demográficos e clínicos. RESULTADOS: Foram identificados seis prontuários de pacientes com episódios de hipersensibilidade aos quimioterápicos. Na amostra 66,7% foram pacientes do sexo feminino, com idade média de 58,4 anos (DP: ± 14,9) e câncer em estágio III (66,7%), sendo os tumores de cólon e ovário os tipos mais prevalentes (33,3%). A incidência de reações de hipersensibilidade foi 2,4%. Nos 12 episódios estudados, o desconforto respiratório foi o sintoma mais frequente (58,3%) e hiperemia foi o sinal mais frequente (50%). O rituximabe foi o agente antineoplásico mais associado a tais reações (33,3%), seguido pela combinação de FOLFOX e bevacizumabe (25%). A maioria dos episódios ocorreram no segundo ciclo quimioterápico (25%). CONCLUSÃO: As reações de hipersensibilidade à quimioterápicos sistêmicos dependem dos fármacos selecionados e das respostas desenvolvidas pelos pacientes, com ampla variação de sinais e sintomas.

Introducao: A sindrome trico-dento-ossea e uma displasia ectodermica ultra-rara relacionada a alteracoes geneticas no gene DLX3 de interesse para as ciencias da saude devido a suas manifestacoes clinicas. Objetivo: Sintetizar as evidencias cientificas sobre a sindrome trico-dento-ossea, especialmente para odontologia. Materiais e metodos: Uma pesquisa bibliografica foi realizada nas bases de dados MEDLINE/PubMed, Web of Science e Scopus. Uma estrategia de busca com descritores foi aplicada em todas as bases de dados mencionadas para alcancar os artigos cientificos. Os artigos alcancados em todas as bases de dados tiveram seus titulos e resumos lidos em busca da adequacao ao objetivo proposto. Nao houve restricao quanto ao ano de publicacao, pais ou idioma. Revisao de literatura: Os pacientes diagnosticados com essa sindrome apresentam defeitos no esmalte dentario e taurodontismo severo, principalmente nos primeiros molares permanentemente inferiores. Alem desses sinais, outras manifestacoes podem estar assoc adas, como cabelos crespos, aumento da densidade ossea e alteracoes nos ossos craniofaciais. O diagnostico da sindrome tricodento-ossea pode ser desafiador devido a heterogeneidade e ampla variacao fenotipica apresentada pelos pacientes com mutacoes em DLX3, uma vez que esse gene esta associado a varias funcoes, principalmente relacionadas a diferenciacao e biomineralizacao celular. Alem disso, e necessario considerar que outras anomalias dentarias podem ser confundidas com a sindrome trico-dento-ossea, principalmente nos casos de amelogenese imperfeita associada ao taurodontismo. Conclusao: Para a odontologia, as manifestacoes orais causadas por essa sindrome tornam-se relevantes para fins diagnosticos e terapeuticos, embora nao existam protocolos clinicos para o tratamento odontologico especifico destes pacientes.

Modelo do Estudo: Relato de caso Importância do problema: No mundo, mais de três milhões de pessoas estão vivendo com deficiência física devido à hanseníase. O Brasil é o segundo país com o maior número de casos novos registrados. A magnitude e o alto risco de incapacidade mantêm a doença como problema de saúde pública. O diagnóstico de hanseníase em geral é simples. Porém, quadros com ausência de lesões cutâneas características, somente com alterações neurais, representam um desafio para o diagnóstico diferencial com outras doenças neurológicas. Comentários: Relatamos o caso de um paciente encaminhado ao serviço de neurologia com história clínica e eletroneuromiografia compatíveis com polineuropatia desmielinizante, sem qualquer lesão cutânea ao exame de admissão. O raciocínio clínico inicial foi direcionado para o diagnóstico das polineuropatias desmielinizantes inflamatórias adquiridas como Polineuropatia Desmielinizante Inflamatória Crônica (CIDP) e suas variantes. No entanto, após anamnese e exame físico detalhados, chamou a atenção a ausência do componente atáxico e a presença predominante de alterações sensitivas de fibra fina, espessamento de nervo e importante fator epidemiológico para hanseníase, motivando a suspeita e a investigação desta enfermidade por meio da biópsia de nervo que foi sugestiva de hanseníase. Após três meses, em novo exame do paciente para biopsiar áreas de anestesia para reforçar o diagnóstico, observou-se o surgimento de extensas lesões levemente hipocrômicas no tronco e membros inferiores, cuja biópsia definiu o diagnóstico de hanseníase.

Introduction: Dermatophytosis are very common fungal infections caused by the fungal species Microsporum, Epidermophyton or Trichophyton, which mostly affect the skin, the interdigital region, groin and scalp. Although they do not cause serious diseases, in patients with the human immunodeficiency virus the infection manifests itself and evolves exuberantly, usually with extensive and disseminated lesions. Objective: To review the literature on dermatophytosis in people living with human immunodeficiency virus and to present the experience in clinical care in a patient living with human immunodeficiency virus with extensive and disseminated dermatophytosis. Methods: A literature review on the topic was carried out in the PubMed/National Library of Medicine – USA databases, using the keywords dermatophytosis, or dermatophytosis associated with the words AIDS, human immunodeficiency virus or immunodeficiency, from 1988–2022. The clinical experience showed a patient living with human immunodeficiency virus developing AIDS and presenting with disseminated skin lesions. Samples of the lesion were collected by scraping, which were submitted to culture and there was growth of fungi of the Trichophyton sp genus. A biopsy of the lesion was also performed using the Grocott-Gomori's Methenamine Silver stain. Results: We found 1,014 articles, of which only 34 presented a direct correlation with our paper, and were used to discuss the main themes narrated in this article. We present clinical experience in the management of a patient with human immunodeficiency virus/AIDS and low adherence to antiretroviral treatment, showing extensive and disseminated erythematous-squamous lesions with a clinical diagnosis of tinea corporis, manifesting with a clinical picture usually not found in immunocompetent patients. The diagnosis was confirmed by laboratory tests with isolation of the Trichophyton sp fungus. The patient was treated with oral fluconazole, with complete remission of the clinical picture after two months. She was also thoroughly encouraged to use the prescribed antiretroviral medication correctly. Conclusion: Dermatophytosis in patients living with human immunodeficiency virus can present extensive and disseminated forms. The antifungal treatment is quite effective, with remission of the condition. Antiretroviral therapy is an important adjuvant for better recovery of the sickness.

Esta tese apresenta três ensaios sobre economia política. Cada ensaio aborda um tema diferente neste contexto. O primeiro ensaio constrói um modelo de votação sequencial na qual os votantes possuem ideologia (opinião a priori sobre o assunto a ser votado) e desejam votar na opção vencedora. Nossa estrutura pode ser aplicada para avaliar os desafios de um organizador (agenda setter, como o presidente da Câmara dos Deputados) ao decidir a ordem em que os legisladores votarão em uma agenda de grande comoção social (impeachment, por exemplo). Contribuímos para a literatura de votações sequenciais ao incluir, simultaneamente, ideologia (informação privada) e preferência pelo lado vencedor, cujo benefício é incerto. Os resultados analíticos e de simulação mostram que, em geral, o arranjo de maior probabilidade de vitória “Sim” é aquele em que o votante mais propenso ao voto “Sim” ocupa a primeira posição na votação, seguido do menos propenso e finalizado pelo jogador com propensão mediana. Nosso modelo ajuda a lançar luz no debate sobre a ordem de votação do impeachment de Dilma Roussef na Câmara dos Deputados, dirigido pelo então presidente da casa, Eduardo Cunha. O segundo ensaio desenvolve uma teoria sobre alianças eleitorais, assumindo que os partidos maiores (cabeça de chapa) são office seekers (ou pragmáticos, desejando a vitória na eleição). A ideologia (plataforma políticas anunciadas durante a campanha e implementadas em caso de vitória), por sua vez, afeta seu payoff indiretamente, através da perda de base eleitoral. Contribuímos com a escassa literatura teórica sobre o assunto ao permitir que, ao disputar o apoio de um partido menor, os maiores ofereçam tanto transferências (cargos, recursos monetários, prestígio, etc) quanto aproximação ideológica via plataforma política. Os equilíbrios encontrados em nosso modelo nos mostram que partidos cujos eleitores são “mais ideológicos”, em geral, ofertam relativamente mais transferências ao partido menor, como um resultado da sua menor aproximção política. Os resultados ajudam a entender o padrão de alianças políticas de alguns dos maiores partidos políticos brasileiros, bem como lança luz em uma das potenciais razões de um dos maiores escândalos de corrupção recentes. O terceiro ensaio apresenta um modelo microfundamentado de disciplina partidária, no qual estudamos sob quais condições partidos políticos têm maior sucesso em fazer com que seus membros sigam a orientação de voto do líder. Por meio de um jogo dinâmico de informação completa e imperfeita, no qual os políticos buscam maximizar (exclusivamente) sua chance de reeleição e partidos desejam maximizar o número de deputados reeleitos, com uma preferência para aqueles disciplinados, mostramos que, em geral, existe maior chance de equilíbrios com disciplina quando: (i) disciplina partidária tiver um peso grande no payoff do partido; (ii) a popularidade do projeto a ser votado for baixa; e (iii) a importância do cargo a ser alocado a um dos deputados é pequena. Além disso, o carisma dos deputados possui um papel fundamental no nível de controle que grupos políticos possuem sobre seus membros. Nossos resultados fornecem microfundamentos para muitos das evidências empíricas encontradas na literatura. This thesis presents three essays on political economy. Each essay addresses a different theme in this context. The first essay builds a sequential voting model in which voters have ideology (a priori opinion on the subject to be voted on) and wish to vote for the winning option. Our framework can be applied to assess an organizer’s challenges (agenda-setter, such as the president of the Congress) when deciding the order in which legislators will vote on an agenda of great social commotion (impeachment, for example). We contribute to the literature on sequential voting by simultaneously including ideology (private information) and preference for the winning side, whose benefit is uncertain. The analytical and simulation results shows that, in general, the arrangement with the highest probability of winning “Yes” is the one in which the voter most likely to vote “Yes” occupies the first position in the vote, followed by the least likely and finalized by the player with a median propensity. Our model helps to elucidate the debate on the voting order for the impeachment of Dilma Roussef in the Chamber of Deputies, directed by then president of the house, Eduardo Cunha. The second essay develops a theory about electoral alliances, assuming that the larger parties (head of the ticket) are office-seekers (or pragmatists, desiring victory in the election). Ideology (policy platform announced during the campaign and implemented in case of victory), in turn, affects its payoff indirectly, through the loss of electoral base. Due to lack of literature our contribution is on the subject by allowing that, when disputing the support of a smaller party, the larger ones offer both transfers (positions, monetary resources, prestige, etc.) The balances found in our model show us that parties whose voters are “more ideological’, in general, offer relatively more transfers. to the smaller party, as a result of its lesser political approach. The results help to understand the pattern of political alliances of some of the largest Brazilian political parties, as well as elucidate one of the potential reasons for one of the biggest recent corruption scandals. The third essay presents a micro-grounded model of party discipline, in which we study under what conditions political parties are most successful in getting their members to follow the leader’s voting guidance. Through a dynamic game of complete and perfect information, in which politicians seek to maximize (exclusively) their chance of reelection and parties wish to maximize the number of deputies re-elected, with a preference for disciplined ones, shows that, in general, there is a greater chance of balance with discipline when: (i) party discipline has a great weight in the party payoff; (ii) the popularity of the project to be voted on is low; and (iii) the importance of the position to be allocated to one of the deputies is small. Furthermore, the charisma of deputies plays a fundamental role in the level of control that political groups have over their members. Our results provide micro-foundations for much of the empirical evidence found in the literature.

The COVID-19 pandemic led to changes in academic teaching and dental education, but the impact on dental professors’ mental health and quality of life remains poorly understood. Thus, the objective of this study was to evaluate the impacts of social distancing and online teaching related to COVID-19 on the quality of life and anxiety of Brazilian dental professors. This was a cross-sectional study conducted from August 2020 to October 2020. Three instruments were used in an online version: a questionnaire about personal data, academic information and online teaching activities, the Generalized Anxiety Disorders 7 (GAD7) scale, and the Abbreviated World Health Organization Quality of Life (WHOQOL-bref) scale. All instruments were sent by e-mail, social media, and messaging apps to private and public universities and professors. Of the 318 responses, 187 (58.8%) were from female professors. Moreover, lack of good internet access and adequate place for online teaching, difficulties in producing teaching materials, and housework roles had a significant impact on the quality of life and anxiety scores (all p-values < 0.05). Also, Brazilian dental professors who declared that they would make greater efforts if the activities were face-to-face had significantly worse quality of life and anxiety scores, and female professors had significantly higher anxiety scores (all p-values < 0.05). These results provide evidence of a negative effect of social distancing and online teaching activities related to COVID-19 outbreak on the health-related quality of life and mental health of Brazilian dental professors.

Modelo do estudo: Estudo observacional, longitudinal, retrospectivo e descritivo de uma coorte de pacientes. Objetivo: Descrever o perfil clínico e epidemiológico dos pacientes com diagnóstico de hanseníase, atendidos em um centro de referência no estado do Rio de Janeiro. Metodologia: Foram incluídos todos os pacientes com diagnóstico de hanseníase atendidos no Hospital Universitário Gaffrée e Guinle entre os anos de 2008 a 2017. Os dados foram coletados através da revisão de prontuários e analisados através de estatística descritiva, utilizando-se o software SPSS 22.0 database . Resultados: Um total de 112 pacientes foi incluído no estudo, sendo que 54,5% eram do sexo masculino. A média de idade foi de 49,9 anos, 62,4% eram moradores do município do Rio de Janeiro e 69,9% apresentaram formas clínicas multibacilares da hanseníase. Foi possível avaliar o grau de incapacidade inicial de 36 (32,1%) dos pacientes incluídos. Desses, 25% apresentaram grau de incapacidade inicial II. Durante o tratamento poliquimioterápico, 21 (18,8%) pacientes apresentaram algum efeito colateral, sendo o mais comum a anemia relacionada à dapsona. Cinquenta e nove (52,7%) pacientes apresentaram algum tipo de estado reacional da hanseníase e 10 (8,9%) apresentaram complicações sistêmicas graves durante o acompanhamento. Conclusões: O presente estudo evidenciou um perfil clínico-epidemiológico diferente do descrito na literatura, com uma grande porcentagem de pacientes com grau de incapacidade inicial II, uma alta frequência de estados reacionais e de complicações advindas do curso clínico da doença ou dos tratamentos implementados.

OBJETIVO: Descrever como o Programa 5S foi implementado em um serviço de farmácia hospitalar no Brasil. MATERIAIS E MÉTODOS: Consiste em um relato de experiência do aprimoramento da gestão da qualidade total em um serviço de farmácia hospitalar. Após aprofundamento teórico e treinamento, um grupo de colaboradores realizou a análise situacional do serviço e elaborou um plano de tarefas, com prazos e metas, de acordo com os fundamentos do Programa 5S. RELATO DE EXPERIÊNCIA: Em relação ao espaço, foram proporcionados ambientes mais limpos e organizados, com identificação e otimização dos espaços de trabalho. Houve redução do desperdício e melhor controle dos estoques, desde a identificação até o armazenamento em condições adequadas. Entre os colaboradores, a implementação do Programa 5S desencadeou motivação profissional, melhoria da relação hierárquica e desenvolvimento do senso de liderança, colaboração e trabalho em equipe, além de fortalecer o vínculo com o serviço e promover uma gestão colaborativa. Além disso, o programa 5S favoreceu a identificação e o armazenamento de medicamentos e insumos, contribuindo com a uniformização das atividades de manipulação farmacêutica, dispensação e segurança dos pacientes. CONSIDERAÇÕES FINAIS: A implementação do Programa 5S modificou positivamente o processo de trabalho no serviço de farmácia hospitalar.

Objective: To compare the web search trends related to oral health issues during the COVID-19 outbreak with the previous year, focusing on dental trauma (DT), bruxism (BX) and toothache (TA). Methods: It was an infodemiological study carried out using Google Trends tool. Popular terms related to oral health issues investigated were used in Portuguese to retrieve relative search volumes (RSV) normalized by the algorithm. The case period consisted of data relating to the interval from December 12, 2019 to December 12, 2020, while the control period referred to the interval from December 11, 2018 to December 11, 2019. It has been shown that a class of drugs for diabetes control, the thiazolidinediones, leads to increased apoptosis in osteocytes. Considering the correlations between osteocytes and cementocytes, the aim of this study was to demonstrate the apoptosis on cementocytes of wild type mice that had received rosiglitazone. Twenty-four male C57BL/6 mice were divided into 3 groups: 1 control, which received only the vehicle administration via oral for 1 week (PBS+DMSO 10%) and other two groups, which received 10 mg/kg of RGZ+PBS+DMSO 10% for 1 or 2 weeks, respectively. Upon completion of the time courses, mice were killed by CO2 and the mandibles were dissected and subjected to routine histotechnical processing. The sections were analyzed through transferase-mediated dUTP nick-end labeling (TUNEL) and 4’,6- diamidino-2-phenylindole (DAPI) staining of nuclear morphology (α=0.05). Control group showed significantly lower apoptotic cells/total cells ratio when compared to the experimental groups with TUNEL and DAPI methods (p=0.010 and 0.004, respectively). TUNEL method showed approximately 20% TUNEL-positive cementocytes in control and 26% in both experimental groups, while the DAPI technique showed approximately 32% of DAPI-positive cementocytes in control and 38% to 40% in experimental groups. The rosiglitazone systemic administration can lead to cementocytes apoptosis in mice. Despite the differences between the experimental and control groups, the death of cementocytes occurred as a physiological phenomenon, important in understanding the role of these cells in periodontal tissue.

Introdução: O uso de separadores elastoméricos durante o tratamento ortodôntico pode provocar dor e a laserterapia de baixa intensidade tem sido empregada no manejo deste desfecho. Objetivo: Apresentar as evidências sobre o efeito da laserterapia de baixa intensidade no manejo da dor provocada pelo uso de separadores elastoméricos. Métodos: Foi desenvolvida uma revisão integrativa de estudos clínicos nas bases de dados PubMed, Web of Science, Scopus, BVS, SciELO e clinicaltrials.gov utilizando uma estratégia PICo elaborada com descritores, incluindo estudos publicados nos últimos dez anos, sem restrição de idioma. Foram rastreados 68 artigos e 12 foram revisados. Revisão de literatura: Considerando a laserterapia, a maioria dos estudos utilizou arsenieto de gálio e alumínio (Ga-Al-As), com comprimento de onda entre 808nm e 940nm, bem como a potência entre 40.6mW e 200mW. A escala visual analógica (EVA) foi utilizada pela maioria dos estudos para mensurar a dor. Dois estudos não verificaram diferenças significativas da laserterapia de baixa intensidade na dor provocada pelos separadores elastoméricos, oito estudos encontraram diferenças significantes entre os grupos de intervenção, de controle e o placebo (quando utilizado) e dois estudos não foram totalmente conclusivos e apontaram diferenças estatística parciais entre os grupos em questão. As técnicas de aplicação foram demasiadamente heterogêneas nos estudos incluídos nesta revisão e não permitem uma síntese quantitativa. Conclusão: O laser de baixa intensidade pode demonstrar efeitos benéficos na redução da dor provocada por separadores elastoméricos. Entretanto, ainda não é possível determinar qual a melhor técnica de aplicação., Introduction: The use of elastomeric separators during orthodontic treatment can cause pain and low-level laser therapy has been used to manage this outcome. Objective: To present the evidence on the effect of low-level laser therapy in the management of pain caused by the use of elastomeric separators. Methods: An integrative review of clinical studies was carried out in the PubMed, Web of Science, Scopus, BVS, SciELO and clinicaltrials.gov databases using a PICo strategy developed with descriptors, including studies published in the last ten years, without language restriction. 68 articles were tracked and 12 were reviewed. Literature review: Considering laser therapy, most studies used gallium and aluminum arsenide (Ga-Al-As), with a wavelength between 808nm and 940nm, as well as power between 40.6mW and 200mW. The visual analog scale (VAS) was used by most studies to measure the pain. Two studies did not find significant differences in low-level laser therapy in pain caused by elastomeric separators, eight studies found significant differences between the intervention, control and placebo groups (when used) and two studies were not fully conclusive and pointed out partial statistical differences between the groups in question. The application techniques were too heterogeneous in the studies included in this review and do not allow a quantitative synthesis. Conclusion: The low-level laser therapy can demonstrate beneficial effects in reducing pain caused by elastomeric separators. However, it is not yet possible to determine the best application technique.

Non-carious cervical lesions affect permanent teeth and cause the loss of healthy dental structure through a non-bacterial process. The occurrence of these lesions is multifactorial, although components of the occlusion are frequently associated. The objective of this article is to present a biomechanics of non-carious cervical lesions in finite element models. It is an integrative review of in silico studies carried out in PubMed, Scopus and Web of Science databases, using a PICo search strategy with descriptors. Studies in three dimensions carried out using finite element softwares that evaluated the biomechanical behavior of non-carious cervical lesions, without language restriction, published between 2000 and 2019 were included. Nine studies were eligible, analyzed and synthesized. The evidence indicates that when oblique occlusal forces are applied, the cervical zone of the dental elements receives greater stress, especially in the subsurface enamel. Furthermore, enamel can be more susceptible to stress than dentin, while V-shaped cervical lesions tend to receive more stress than U-shaped lesions, as well as the depth of these lesions potentially increases stress. Bone support and root morphology can modify the distribution of occlusal forces in dental elements. Thus, the biomechanical behavior of non-carious cervical lesions in finite element models is directly associated with oblique occlusal forces capable of generating tension in the cervical of dental elements. Introdução: Os agentes quimioterápicos para o tratamento do câncer de mama frequentemente provocam toxicidades sistêmicas nas pacientes, incluindo alterações gastrointestinais. Objetivo: Identificar toxicidades gastrointestinais em mulheres durante o tratamento quimioterápico do câncer de mama. Método: Estudo descritivo-exploratório, quantitativo e transversal, utilizando prontuários médicos de pacientes em quimioterapia entre fevereiro de 2014 e fevereiro de 2015 em um serviço de oncologia. Após rastreamento e seleção, foram incluídas 194 pacientes. Resultados: De modo geral, foram identificadas 457 manifestações clínicas gastrointestinais, das quais 50,5% das participantes apresentaram até duas e 49,5% apresentaram entre três e cinco. Além disso, 74,2% apresentaram náusea, 43,3% dor abdominal, 40,7% diarreia, 39,2% vomito e 37,6% constipação. A ocorrência de três a cinco toxicidades esteve relacionada com a ocorrência de fadiga (p=0,002) e perda de peso (p=0,003), bem como a quantidade de ciclos quimioterápicos influenciou positivamente na severidade de náusea (p=0,041) e vomito (p=0,023). Introdução: Diversos efeitos colaterais podem acometer a pele e seus anexos durante o tratamento oncológico de mulheres com câncer de mama, comprometendo a terapia. Objetivo: Identificar a ocorrência de alterações dermatológicas durante o tratamento oncológico de mulheres com câncer de mama. Método: Estudo documental e retrospectivo, de cunho quantitativo, com uso de dados secundários obtidos por meio de 190 prontuários clínicos (n=190) de um serviço privado de oncologia. Resultados: As participantes apresentaram média de idade de 53 anos (±11,2), com diagnóstico histopatológico de carcinoma ductal invasivo (85,8%). Todas foram submetidas à quimioterapia, 65,3% à mastectomia radical e 34,2% à radioterapia. As alterações dermatológicas identificadas e as ocorrências verificadas na amostra foram alopecia (94,2%), hiperpigmentação (48,4%), prurido (36,3%), eritema (6,8%), descamação (25,8%) e alterações ungueais (77,9%). Objectives: to describe and examine oral hygiene habits and self-reported gingival bleeding in women with breast cancer undergoing chemotherapy. Methods: in an observational and prospective study, 140 women were evaluated during chemotherapy between 2017 and 2019. Results and Conclusion: more than 40% of participants reported gingival bleeding at some point during chemotherapy. Flossing was unsatisfactory at baseline and in the intermediate cycle as well as did not affect self-reported gingival bleeding. The amount of tooth brushing per day was a predictor of self-reported gingival bleeding at the end of chemotherapy.

Introduction: Molluscum contagiosum is a dermatosis caused by a DNA virus of the family Poxvirus and genus Molluscipoxvirus, affecting mainly children, sexually active adults, atopic individuals and immunocompromised patients, especially those with human immunodeficiency virus (HIV) infection. Objective: To describe our experience in caring for patients living with HIV who presented with extensive and severe Molluscum contagiosum, and to conduct a literature review on the subject as well. Methods: An electronic search was carried out in the MEDLINE/PubMed and SciELO databases and in the books: ATLAIDS and AZULAY limited to the period of January 2017 to June 2021. Results: Four clinical cases are reported in people living with HIV with extensive lesions normally not found in immunocompetent patients. The treatment performed in the cases reported in this article was the punctual application of 90% trichloroacetic acid (TCA) to each lesion, with complete remission of the clinical presentation in two patients over a period of three and six months. The other two patients did not receive treatment for molluscum contagiosum as they died because of pulmonary complications. Conclusion: Infection with Molluscum contagiosum in people living with HIV has disseminated forms with large-volume lesions, with substantial stigmatizing aesthetic impairment, and treatment with 100% TCA is quite effective.

Kabuki syndrome consists in a series of congenital anomalies involving intellectual disability, hypotonia, and facial and body dysmorphism. In addition to these cardinal signs, craniofacial and dental changes are frequently observed, although they are not fully understood. Thus, the aim of this article is to present the dental findings in patients with Kabuki syndrome. This is a systematic review developed according to the PRISMA protocol. A search strategy with descriptors ("kabuki syndrome" AND "dentistry") was applied to six databases. As inclusion criteria, we selected clinical case reports that addressed dental findings in one or more patients with Kabuki syndrome between 1999 and 2019, without restrictions regarding gender, age or language. Data collection in all databases was performed by a calibrated evaluator and titles, abstracts and full-text studies were systematically analyzed. Eleven case reports that fit the proposed inclusion criteria were found. After the qualitative synthesis, teeth absence or agenesis were the most reported findings, being observed by nine authors. Less frequent dental findings in patients with this syndrome are taurodontism, microdontia, fusion, gemination and conical or screwdriver-shaped teeth. In fact, when any syndrome or genetic condition is related to manifestations of dental interest, the dental surgeon should be able to investigate them, especially teeth agenesis or absence. For this reason, the dentist should be aware of the manifestations of Kabuki syndrome, contributing to patients' understanding and management. Resumen: El síndrome de Kabuki consiste en una serie de anomalías congénitas que involucran discapacidad intelectual, hipotonía y dismorfismo facial y corporal. Además de estos signos cardinales, con frecuencia se observan cambios craneofaciales y dentales, aunque no se entienden completamente. Por lo tanto, el objetivo de este artículo es presentar los hallazgos dentales en pacientes con síndrome de Kabuki. Esta es una revisión sistemática desarrollada de acuerdo con el protocolo PRISMA. Se aplicó una estrategia de búsqueda con descriptores ("síndrome de kabuki" AND "odontología") a seis bases de datos. Como criterios de inclusión, seleccionamos informes de casos clínicos que abordaban los hallazgos dentales en uno o más pacientes con síndrome de Kabuki entre 1999 y 2019, sin restricciones de género, edad o idioma. La recolección de datos en todas las bases de datos fue realizada por un evaluador calibrado y los títulos, resúmenes y estudios de texto completo fueron analizados sistemáticamente. Se encontraron once informes de casos que se ajustan a los criterios de inclusión propuestos. Después de la síntesis cualitativa, la ausencia de dientes o la agenesia fueron los hallazgos más reportados, siendo observados por nueve autores. Los hallazgos dentales menos frecuentes en pacientes con este síndrome son taurodoncia, microdoncia, fusión, geminación y dientes cónicos o con forma de destornillador. De hecho, cuando cualquier síndrome o condición genética se relaciona con manifestaciones de interés dental, el cirujano dental debe poder investigarlas, especialmente la agenesia o ausencia de los dientes. Por esta razón, el dentista debe ser consciente de las manifestaciones del síndrome de Kabuki, lo que contribuye a la comprensión y el manejo de los pacientes.

RESUMO A doença cárie afeta indivíduos de várias faixas etárias, incluindo adultos maiores. Diversas abordagens individuais e comunitárias foram desenvolvidas e investigadas para o tratamento de lesões cariosas, como o tratamento restaurador atraumático (ART). O objetivo deste artigo é apresentar as evidências sobre o ART no tratamento da cárie em adultos maiores. Foi realizada uma revisão da literatura nas bases de dados PubMed, BVS e SciELO por meio de uma combinação de descritores, selecionando estudos publicados nos últimos dez anos. O ART é uma técnica minimamente invasiva que preserva a estrutura dentária saudável, removendo tecidos cariados usando apenas instrumentos manuais cortantes. No que se refere ao desempenho clínico das restaurações por ART em relação às restaurações convencionais, as taxas de sobrevida são comparáveis, embora o ART esteja associado à uma melhor relação custo-benefício pois requer menos recursos materiais. No entanto, é necessário ressaltar que existem limitações na técnica, especialmente em áreas interproximais e de raiz. Sendo assim, o tratamento restaurador atraumático pode ser considerado uma alternativa viável para o tratamento de algumas lesões de cárie em adultos maiores, especialmente aqueles que têm limitações no acesso aos serviços de saúde bucal e precisam de abordagens mais acessíveis em instituições de cuidados. As evidências atuais suportam o uso do ART em adultos maiores, permitindo ações de prevenção e reabilitação que preservam a estrutura dental saudável e são acessíveis em vários cenários, reduzindo a perda dentária nesta faixa etária. Several individual and community approaches have been developed and investigated for the treatment of carious lesions, such as atraumatic restorative treatment (ART). The objective of this article is to present evidence about ART in the treatment of caries in older adults. A literature review was carried out in the PubMed, BVS and SciELO databases using a combination of descriptors, selecting studies published in the last ten years. ART is a minimally invasive technique that preserves healthy tooth structure, removing decayed tissues using only sharp hand instruments. In relation to the clinical performance of ART restorations compared to conventional restorations, survival rates are comparable, although ART is associated with a better cost-benefit ratio, as it requires less material resources. However, it is necessary to emphasize that there are limitations in the technique, especially in interproximal and root areas. Thus, ART can be considered a viable alternative for the treatment of some caries lesions in older adults, especially those who have limited access to oral health services and need more accessible approaches in long-term care institutions. Current evidence supports the use of ART in older adults, allowing prevention and rehabilitation actions that preserve healthy dental structure and are accessible in various settings, reducing tooth loss in this age group. Objetivo: Descrever reações adversas após tratamento radioterápico em pessoas com câncer atendidas em um ambulatório de radioterapia. Metodologia: Trata-se de um estudo documental por meio da coleta de dados em prontuários clínicos. Foram avaliados 436 pacientes que realizaram tratamento radioterápico em busca de episódios de reações adversas registrados nos prontuários. Foram elegíveis os pacientes maiores de 18 anos com diagnóstico de neoplasia (confirmado por laudos citopatológicos e/ou histopatológico) que realizaram cinco ou mais sessões de radioterapia entre 2015 e 2016. Resultados: 353 prontuários clínicos foram incluídos (83,6%) e 1.390 reações associadas ao tratamento radioterápico foram descritas nestes pacientes, sendo 53,2% mulheres. As reações adversas mais frequentes relacionadas à radioterapia foram dor (14%), radiodermite (9%) e falta de apetite (8%). 69% dos pacientes não apresentavam sinais ou sintomas antes do diagnóstico, sendo 25% diagnosticados com adenocarcinoma. A técnica de radioterapia mais utilizada foi a 3D (67%), com cerca de 30 frações (14%) e 5000 Gy como dose total planejada (17%). 70% dos pacientes que apresentaram reações adversas possuíam ao menos uma comorbidade. Conclusão: A radioterapia pode estar associada à diversas reações adversas, variando os sinais e sintomas nos pacientes acometidos. A prevenção e o manejo destas condições podem potencializar o tratamento, reduzindo a morbidade e a mortalidade, além de proporcionar qualidade de vida aos pacientes com câncer.
