THE purpose of this paper is to discuss multiple peritoneal autotransplantation of splenic tissue following traumatic rupture of the spleen and to present a summary of reported cases, together with the first report of a case occurring in an adult. In the cases hitherto reported, all patients had in youth survived traumatic rupture of the spleen and later, either at reoperation or autopsy, numerous spleenlike nodules were found scattered over the various peritoneal surfaces, including the omentum, small and large intestine, diaphragm and pelvic cavity. Abstract

1. 1. The seriousness of metacarpal fractures, commonly regarded as trivial injuries but frequently resulting in crippling deformities is emphasized. These disabilities far in excess of the apparent importance of the fracture are due, to a considerable extent, to the lack of interest and the less consideration generally given to the early treatment and immobilization of these fractures than that given to fractures of the larger bones. 2. 2. The difficulties of treatment with special reference to past progress made in the treatment of these fractures are briefly reviewed and discussed. 3. 3. A new method of treatment used in twenty-two cases of fractures of the four inner metacarpals including both shaft and the so-called knuckle fractures is presented. This method is believed to be superior to the older methods of treatment. The basis of treatment is the early correction of the primary anatomic deformity and the rigid fixation of the fragments, with the hand in the position of grasp (optimum function) but with all metacarpophalangeal and interphalangeal joints free during the period of immobilization. 4. 4. The results of treatment are shown in the accompanying tables and representative roentgenograms.

Abstract

LOEWENSTEIN and Foster,1 in 1945, devised the term iridoschisis (iris splitting) for an ophthalmic disease which had first been reported by Schmitt,2 in 1922. This is a condition characterized by atrophy of the anterior layers of the iris, separation of these layers from the posterior layer and shredding of the radial fibers. Loewenstein and Foster described these fibers as floating in the aqueous in much the fashion of seaweed in a pool of water. Sander3 had already described them as looking as though they had been teased out with a crochet needle.Up to the time of Loewenstein and Foster's report, eight cases of iridoschisis had been recorded in the medical literature. THERE are certain diseases in all fields of medicine whose essential data have come to be more or less taken for granted. Sympathetic ophthalmia is one of these. Its existence, as Woods1 pointed out in an excellent review of the whole subject, has been realized for almost a thousand years. Its clinical aspects were very well described more than 100 years ago. The pathologic changes in the exciting eye were set forth in considerable detail almost 50 years ago. The potentially devastating character of the disease is thoroughly appreciated by any ophthalmologist who has ever encountered it in his practice. No really satisfactory treatment for it has ever been devised.A recent experience of our own, in which the histologic picture was at complete variance with the classic clinical picture of sympathetic ophthalmia which the patient presented, has made us wonder whether it would not be well to examine

Abstract

Global communication about HIV/AIDS requires the creation of new communities that can bridge distances and distinctions of nationality, language, class, race, gendered-identities and other forms of local identification on a disease that is associated with the realm usually understood as private (sexuality). Global AIDS, characterized as ‘the disease of our time’, is responsible for spawning an entire industry devoted to the prevention, detection, treatment, and potential cure of HIV/AIDS. In terms of scale, this industry works primarily cross-nationally, with donors from the North funding programs for AIDS prevention and care in the South. Anti-retroviral drugs (ARVs), typically produced as generics by manufacturers in India or South Africa and purchased by aid funding, are central to global AIDS programs. Yet, mobilizing for global AIDS treatment embodies the logic of marketing, in which Africans with AIDS are sold as lives to be saved. This article will draw from international relations theory, sociology and anthropology to offer an interdisciplinary perspective on mobilizing communication globally.
