The increasing number of people who are colonized or infected with multidrug-resistant bacteria imposes a high economic burden on society which includes the negative impact on health status as an intangible cost. An economic analysis leads to the conclusion that currently too little is done to prevent or control infections. The reasons include insufficient incentives for health care providers, a lack of reliable data on both the prevalence and the effects of infection, and a lack of coordination among the different branches of the health care sector. A regional management of multidrugresistant bacteria which does not focus on a single branch but rather on the health care sector overall can achieve a substantial reduction in the number of infected people and of the associated economic cost for society., GMS Krankenhaushygiene Interdisziplinär; 6(1):Doc23; ISSN 1863-5245

This paper investigates the impact of population aging, driven by medical progress, upon agespecific expenditure on health care. In a model set up in discrete time, individuals at each age may catch a lethal disease which, upon receiving appropriate medical treatment, nevertheless involves a mortality risk such that length of life is stochastic. The incidence of lethal diseases, the associated survival probability conditional upon treatment, and health care expenditure conditional upon health status may all depend on an individual´s history. For a given age, the history of an individual contains information on her health status in the past. Medical progress is taken to involve an increase in the survival probability of a specified lethal disease. On the one hand, this produces a direct effect on age-specific health care expenditure to the extent that progress affects the cost of treatment of the disease. On the other hand, indirect effects may also arise. These effects are caused by individuals who, having survived the disease at some prior age due to progress, change the structure of individuals alive at current age. Specifically, the "new survivors" may have an influence on age-specific expenditure either through changes in the incidence of lethal diseases or in the associated treatment cost. The sign of an indirect effect crucially depends on health care expenditure for "new survivors" relative to their peers. The analysis yields a number of general results which are important for the discussion of the impact of medical progress on the age profile of health care expenditure. Compression of morbidity, to the extent that it involves a reduction in age-specific expenditure, is neither necessary nor sufficient for medical progress to produce a downward shift of the profile. A similar observation applies to an expansion of morbidity. Both concepts relate to "new survivors" and, thus, take into account only indirect effects of progress.

Der Beitrag untersucht die Auswirkungen des demographischen Wandels, der durch eine Erhöhung der Lebenserwartung infolge des medizinischen Fortschritts ausgelöst wird, auf die altersspezifischen Ausgaben für Gesundheitsleistungen. Dazu werden stationäre Zustände in einem Modell mit überlappenden Generationen betrachtet, das die Mortalität im Lebenszyklus explizit erfasst. Darüber hinaus wird zugelassen, dass sowohl die altersspezifische Mortalität als auch die Gesundheitsausgaben davon abhängen können, ob bereits früher eine letale Erkrankung vorgelegen hat oder nicht. Die Analyse unterscheidet zwischen einem direkten Ausgabeneffekt des medizinischen Fortschritts, der sich auf die Periode bezieht, in welcher die Verringerung der Sterblichkeit aufgetreten ist, und später anfallenden indirekten Effekten. Dabei zeigt sich, dass insbesondere das Vorzeichen eines indirekten Effekts nicht allgemein bestimmt werden kann. Der indirekte Effekt lässt sich in einen Inzidenz- und zwei Behandlungsausgabeneffekte zerlegen, deren Vorzeichen jeweils von dem Einfluss einer früheren letalen Erkrankung abhängt. Bei permanentem medizinischem Fortschritt können Bedingungen identifiziert werden, die jeweils zu einer bestimmten Veränderung der altersspezifischen Gesundheitsausgaben führen.

Die Unterscheidung zwischen den Gesundheitsausgaben für Überlebende und Verstorbene spielt eine wichtige Rolle in der aktuellen Diskussion über die Folgen des medizinischen Fortschritts für die künftige Inanspruchnahme von Ressourcen des Gesundheitswesens. Den Anlass bildet die in Querschnittsanalysen empirisch gestützte Beobachtung, der zufolge in den einzelnen Altersklassen die Gesundheitsausgaben für Verstorbene typischerweise deutlich höher ausfallen als für Überlebende. Lässt sich daraus folgern, dass eine Verringerung der altersspezifischen Sterblichkeit eine Senkung der altersbezogenen Gesundheitsausgaben pro Kopf bewirkt? Das vorliegende Papier geht dieser Frage in einem Modellrahmen nach, in dem für die Individuen in jeder Lebensperiode eine positive Inzidenz einer letalen Erkrankung gilt, die sie bei adäquater medizinischer Behandlung in der Regel mit einer positiven Wahrscheinlichkeit überleben. Während in einer einfachen Modellvariante frühere letale Erkrankungen keine Rolle spielen, lässt das allgemeine Modell einen Einfluss auf die zentralen Größen Inzidenz, Überlebenswahrscheinlichkeit im Falle einer letalen Erkrankung und Gesundheitsausgaben in späteren Lebensperioden zu. In beiden Fällen bleibt die Analyse auf stationäre Zustände beschränkt, um langfristige Effekte zu identifizieren. Die Analyse ermittelt verschiedene Ausgabeneffekte einer höheren altersspezifischen Überlebenswahrscheinlichkeit aufgrund des medizinischen Fortschritts bei Verstorbenen und Überlebenden. Während im einfachen Modell die Ausgabeneffekte auf dieselbe Lebensperiode beschränkt sind, können im allgemeinen Modell auch indirekte Effekte entstehen, die in späteren Lebensperioden auftreten. In beiden Modellvarianten tritt eine Ausgabensenkung infolge einer verringerten Sterblichkeit nur unter restriktiven Bedingungen ein.

The last two decades have seen cost-utility analysis of health care programmes becoming increasingly popular, both at the theoretical level and in empirical applications. This certainly explains why utility measurement in health care has attained such a prominent position in the field of health economics. On the benefit side of changes in resource allocation, the twin aspects of morbidity and mortality effects are to be combined to yield an assessment in line with individual preferences. Thus, at least in theory, the utility of the people involved provides the yardstick with which to gauge health effects. In order to obtain economically meaningful descriptions of the utility impact due to changes in individual health, two approaches currently coexist on somewhat less than peaceful terms. The older one, the quality-adjusted life years (henceforth: QALY) approach, lends itself to empirical application fairly easily since several methods have been devised in order to capture the QALY impact due to health care programmes. More recently, its position has been challenged by the healthy-years equivalent (henceforth: HYE) approach, mainly on the grounds that the QALY approach be likely to misrepresent individual preferences for health effects.

The paper develops and applies a Grossman-style health production model set up in discrete time to explain the impact of environmental pollution on the demand for both health and health care. In order to introduce the environment, our analysis takes changes in environmental conditions to influence the rate at which an individual's stock of health depreciates. While the theoretical part of our paper also contains a discussion of the full model, we restrict our analysis to a submodel which is known as the pure investment model. lbis is because the other submodel, the pure consumption model, implies a rather implausible case of satiation with respect to the individual's preferences. Our empirical findings' are based on data taken from the German Socio-economic Panel (SOEP). The stock of health capital and environmental pollution are treated as latent variables and estimated using a LINCS model. The quality of the environment turns out to be an important determinant of health capital. From the point of view of health economics, improvements in environmental conditions can be interpreted as preventive measures. In terms of prevention, public policies designed to protect the environment also yield significant health effects. As regards health care demand the influence is not dearcut, i.e. one cannot necessarily expect a reduction in resource use.

Im Mittelpunkt der vorliegenden Arbeit steht die theoretische Analyse des Wohlfahrtseffekts, den der Einsatz der medizinischen Diagnostik beim Patienten auslösen kann. Die Abbildung dieses Nutzeneffekts erfolgt in einem entscheidungstheoretischen Ansatz, der es zugleich gestattet, die Bedeutung wichtiger Einflußgrößen wie z.B. der medizinischen Ausgangslage oder der diagnostischen Information zu diskutieren. Darüber hinaus möchte die Arbeit einen Beitrag zur Kosten-Nutzen-Analyse diagnostischer Maßnahmen leisten, indem der Nutzen für den Patienten mit Hilfe eines speziellen Zahlungsbereitschaftskonzepts dargestellt und eingehend untersucht wird.
