Disorders of mineral metabolism, including secondary hyperparathyroidism, are thought to contribute to extraskeletal (including vascular) calcification among patients with chronic kidney disease. It has been hypothesized that treatment with the calcimimetic agent cinacalcet might reduce the risk of death or nonfatal cardiovascular events in such patients.In this clinical trial, we randomly assigned 3883 patients with moderate-to-severe secondary hyperparathyroidism (median level of intact parathyroid hormone, 693 pg per milliliter [10th to 90th percentile, 363 to 1694]) who were undergoing hemodialysis to receive either cinacalcet or placebo. All patients were eligible to receive conventional therapy, including phosphate binders, vitamin D sterols, or both. The patients were followed for up to 64 months. The primary composite end point was the time until death, myocardial infarction, hospitalization for unstable angina, heart failure, or a peripheral vascular event. The primary analysis was performed on the basis of the intention-to-treat principle.The median duration of study-drug exposure was 21.2 months in the cinacalcet group, versus 17.5 months in the placebo group. The primary composite end point was reached in 938 of 1948 patients (48.2%) in the cinacalcet group and 952 of 1935 patients (49.2%) in the placebo group (relative hazard in the cinacalcet group vs. the placebo group, 0.93; 95% confidence interval, 0.85 to 1.02; P=0.11). Hypocalcemia and gastrointestinal adverse events were significantly more frequent in patients receiving cinacalcet.In an unadjusted intention-to-treat analysis, cinacalcet did not significantly reduce the risk of death or major cardiovascular events in patients with moderate-to-severe secondary hyperparathyroidism who were undergoing dialysis. Background— Patients with kidney disease have disordered bone and mineral metabolism, including elevated serum concentrations of fibroblast growth factor-23 (FGF23). These elevated concentrations are associated with cardiovascular and all-cause mortality. The objective was to determine the effects of the calcimimetic cinacalcet (versus placebo) on reducing serum FGF23 and whether changes in FGF23 are associated with death and cardiovascular events. Methods and Results— This was a secondary analysis of a randomized clinical trial comparing cinacalcet to placebo in addition to conventional therapy (phosphate binders/vitamin D) in patients receiving hemodialysis with secondary hyperparathyroidism (intact parathyroid hormone ≥300 pg/mL). The primary study end point was time to death or a first nonfatal cardiovascular event (myocardial infarction, hospitalization for angina, heart failure, or a peripheral vascular event). This analysis included 2985 patients (77% of randomized) with serum samples at baseline and 2602 patients (67%) with samples at both baseline and week 20. The results demonstrated that a significantly larger proportion of patients randomized to cinacalcet had ≥30% (68% versus 28%) reductions in FGF23. Among patients randomized to cinacalcet, a ≥30% reduction in FGF23 between baseline and week 20 was associated with a nominally significant reduction in the primary composite end point (relative hazard, 0.82; 95% confidence interval, 0.69–0.98), cardiovascular mortality (relative hazard, 0.66; 95% confidence interval, 0.50–0.87), sudden cardiac death (relative hazard, 0.57; 95% confidence interval, 0.37–0.86), and heart failure (relative hazard, 0.69; 95% confidence interval, 0.48–0.99). Conclusions— Treatment with cinacalcet significantly lowers serum FGF23. Treatment-induced reductions in serum FGF23 are associated with lower rates of cardiovascular death and major cardiovascular events. Clinical Trial Registration— URL: http://www.clinicaltrials.gov . Στην παρούσα εργασία μελετήθηκε σε μία πρώτη φάση το λιγνιτικό κοίτασμα Αχλάδας Ν. Φλωρίνης από ανθρακοπετρογραφικής σκοπιάς, με στόχο την ταξινόμηση του λιγνίτη σε λιθότυπους και την εξαγωγή πρώτων συμπερασμάτων για το παλαιοπεριβάλλον σχηματισμού. Κατά την υπόψη μελέτη, που είχε στόχο μία πρόδρομη ανθρακοπετρογραφική διερεύνηση του κοιτάσματος, ελήφθησαν 12 δείγματα λιγνίτη από δύο αντιπροσωπευτικές γεωτρήσεις. Με βάση τα αποτελέσματα της ανθρακοπετρογραφικής μελέτης και την ποσοτική αξιολόγηση αυτών διαμορφώθηκαν οι δείκτες συνθηκών υγρασίας (Gl), βλάστησης (VI), στάθμης υδροφόρου ορίζοντα (GWI) και διατήρησης ιστών (ΤΡΙ). Με το συσχετισμό δε των δεικτών αυτών και τη χρήση διαγραμμάτων, εξάγονται συμπεράσματα για το παλαιοπεριβάλλον και τις συνθήκες σχηματισμού., The present paper is a first stage study of the Achlada lignite deposit (Fiorina Domain) from the coal petrography aspect, aiming at the lignite classification in lithotypes and the extraction of initial conclusions concerning the paleoenvironment of the lignite formation. The Achlada deposit spreads at the NE end of the east borderlands of the Fiorina basin of a SW-NE axis, occupies an area of approximately 7,2 Km2 and is a part of the broader tectonic trench of Monastirion-Florina- Ptolemais-Kozani-Servia, which is ΝW-SE directed and of Miocene age. Due to erosion processes, the lignite deposit with its host rocks is separated into two areas, the Northeastern, where greater thicknesses occur, and the Southwestern (KOTIS TH. et al., 1995). The lignite deposit consists of alternations of mostly thin lignite layers (of approximate thickness up to 23 m) with sediments. The sediments of the lignitiferous suite are mainly sands; mostly fine grained and every now and then clayey, rarely medium grained and including psephit. Besides the lignite layers, layers of humic clays often occur with fragments of xylite and flora residues every now and then. During the present study, which aimed at a forerunning coal pétrographie examination of the deposit, 12 lignite samples have been taken from two representative boreholes. This initial stage of the research shows that the primal macérais of the Achlada deposit basically belong to the huminite group and secondarily to the liptinite group, while the third corresponding category of the inertinite group almost vanishes. From the first category greater percentages are occupied by the macerai densinite (=31%) followed by attrinite (=25%), textoulminite (=22%) and eu-ulminite (=3%), while the second corresponding category gives cutinite (=8%), liptodetrinite (=3%), sporinite (=2,5%) and relatively low percentages of resinite and alginite. The participation of inorganic components is mainly characterized by the relatively low presence of pyrite and clay. According to the results of the coal pétrographie study and their quantative evaluation the following indexes have been calculated; gelification index (GI), vegetation index (VI), groundwater influence index (GWI) and tissue preservation index (TPI). With the correlation between these indexes and the use of diagrams, conclusions are derived concerning the paleoenvironment and the conditions of the lignite formation. From the VI/GWI relationship conclusions are derived concerning the hydrologie status and the nutritient components supply and from the TPI/GI relationship the paleoenvironment in relation to the groundwater level. The evaluation of the data shows that the host environment of the maternal flora was telmatic with mainly mesotrophic and partly rheotrophic conditions (swamp), while for a time period -at least- just before the end of the mire, the environment has changed into a wet area with more intensely telmatic conditions. The conditions of the peat formation were, of course, repeating -frequent alternations with sediments periodically- but always within the scope of a similar environment

- Abstract
Three groups of seams can be distinguished in the thick lignite-bearing sequence of the Pleistocene Khoremi Formation (Marathousa member) near Megalopolis. In the Thoknia open mine the basal 14 m of the lower group of seams (Elias group) have been studied with regard to sedimentology, palynology and organic petrology in order to reconstruct the ecological conditions existing during the initial phase of coal formation. The sedimentary sequence has been established by setting up a macropetrographic seam section. Palynology and organic petrology were studied from identical samples. Macropetrographically the section can be divided into four zones mainly based on the relative frequency of matrix-dominated lithotypes. Xylitic and tissue-rich lithotypes occur in only a few thin bands. The five palynological zones do not coincide with the macropetrographic zones. They reflect an increasing aridity and reduction of the forest vegetation outside the swamp as well as large scale fluctuations of the water table of the lake in the basin centre. On a smaller scale alternations of open lake conditions and encroaching vegetation fringes can be observed. © 1996 E. Schweizerbart'sche Verlagsbuchhandlung.

Objective: To evaluate the effects of prophylactic veno-venous hemofiltration (CVVH) in the absence of renal failure on multiple organ dysfunction syndrome after severe multiple trauma.Design: Prospective, randomized study.Setting: Intensive care unit (ICU) in a university hospital.Patients: Twenty-four patients with severe multiple trauma (injury severity score > or = 27), no renal failure on admission and no contraindication for moderate heparinization.Interventions: Twelve patients received conventional treatment while 12 patients were treated additionally with isovolemic CVVH for 5 days starting within 24 h following trauma. Signs of organ dysfunction were assessed daily including monitoring of systemic hemodynamic by means of pulmonary artery catheterization during the first 5 days after trauma.Measurements and Main Results: Prophylactic CVVH did not affect the overall severity of organ dysfunction as assessed by MOF or APACHE II scores. However, the pattern of impaired organ systems was influenced by CVVH: while the post-traumatic decrease in platelet count in patients subjected to CVVH was more pronounced than in controls (e.g. day 4: control: 115,080 +/- 15,087, CVVH: 57,383 +/- 4,201 microliters-1; p < 0.05) the development of hyperdynamic circulatory failure was simultaneously attenuated, as reflected by a limited increased in cardiac output and an attenuated decrease in systemic vascular resistance and oxygen extraction ratio (e.g. systemic vascular resistance on day 4: control: 624.3 +/- 46.17, CVVH: 842.7 +/- 79.24 dyn.s.cm-5; p < 0.005).Conclusion: CVVH blunts the cardiovascular response to multiple trauma and increases tissue oxygen extraction. However, the concomitant decrease in platelet counts represents a limitation for the use of prophylactic CVVH in surgical patients. [ABSTRACT FROM AUTHOR]- Published
