Empathetic response from the therapist is key to the success of clinical psychotherapy, especially motivational interviewing. Previous work on computational modelling of empathy in motivational interviewing has focused on offline, session-level assessment of therapist empathy, where empathy captures all efforts that the therapist makes to understand the client's perspective and convey that understanding to the client. In this position paper, we propose a novel task of turn-level detection of client need for empathy. Concretely, we propose to leverage pre-trained language models and empathy-related general conversation corpora in a unique labeller-detector framework, where the labeller automatically annotates a motivational interviewing conversation corpus with empathy labels to train the detector that determines the need for therapist empathy. Gauging therapist empathy in counselling is an important component of understanding counselling quality. While session-level empathy assessment based on machine learning has been investigated extensively, it relies on relatively large amounts of well-annotated dialogue data, and real-time evaluation has been overlooked in the past. In this paper, we focus on the task of low-resource utterance-level binary empathy assessment. We train deep learning models on heuristically constructed empathy vs. non-empathy contrast in general conversations, and apply the models directly to therapeutic dialogues, assuming correlation between empathy manifested in those two domains. We show that such training yields poor performance in general, probe its causes, and examine the actual effect of learning from empathy contrast in general conversation.

The past decade has seen an explosion of the amount of digital information generated within the healthcare domain. Digital data exist in the form of images, video, speech, transcripts, electronic health records, clinical records, and free-text. Analysis and interpretation of healthcare data is a daunting task, and it demands a great deal of time, resources, and human effort. In this paper, we focus on the problem of co-morbidity recognition from patient’s clinical records. To this aim, we employ both classical machine learning and deep learning approaches. We use word embeddings and bag-of-words representations, coupled with feature selection techniques. The goal of our work is to develop a classification system to identify whether a certain health condition occurs for a patient by studying his/her past clinical records. In more detail, we have used pre-trained word2vec, domain-trained, GloVe, fastText, and universal sentence encoder embeddings to tackle the classification of sixteen morbidity conditions within clinical records. We have compared the outcomes of classical machine learning and deep learning approaches with the employed feature representation methods and feature selection methods. We present a comprehensive discussion of the performances and behaviour of the employed classical machine learning and deep learning approaches. Finally, we have also used ensemble learning techniques over a large number of combinations of classifiers to improve the single model performance. For our experiments, we used the n2c2 natural language processing research dataset, released by Harvard Medical School. The dataset is in the form of clinical notes that contain patient discharge summaries. Given the unbalancedness of the data and their small size, the experimental results indicate the advantage of the ensemble learning technique with respect to single classifier models. In particular, the ensemble learning technique has slightly improved the performances of single classification models but has greatly reduced the variance of predictions stabilizing the accuracies (i.e., the lower standard deviation in comparison with single classifiers). In real-life scenarios, our work can be employed to identify with high accuracy morbidity conditions of patients by feeding our tool with their current clinical notes. Moreover, other domains where classification is a common problem might benefit from our approach as well.

The full comprehension of how topics change within psychotherapeutic conversation is key for assessment and therapeutic strategies to adopt by the counselor to the patients. That might enable artificial intelligence (AI) approaches to recommend the most suitable strategy for a new patient. Basically, understanding the topics dynamics of previous cases allows choosing the best therapy to perform for new patients depending on their current conversations.

Objective In an ageing world population more citizens are at risk of cognitive impairment, with negative consequences on their ability of independent living, quality of life and sustainability of healthcare systems. Cognitive neuroscience researchers have identified behavioral anomalies that are significant indicators of cognitive decline. A general goal is the design of innovative methods and tools for continuously monitoring the functional abilities of the seniors at risk and reporting the behavioral anomalies to the clinicians. SmartFABER is a pervasive system targeting this objective. Methods A non-intrusive sensor network continuously acquires data about the interaction of the senior with the home environment during daily activities. A novel hybrid statistical and knowledge-based technique is used to analyses this data and detect the behavioral anomalies, whose history is presented through a dashboard to the clinicians. Differently from related works, SmartFABER can detect abnormal behaviors at a fine-grained level. Results We have fully implemented the system and evaluated it using real datasets, partly generated by performing activities in a smart home laboratory, and partly acquired during several months of monitoring of the instrumented home of a senior diagnosed with MCI. Experimental results, including comparisons with other activity recognition techniques, show the effectiveness of SmartFABER in terms of recognition rates.

According to the World Health Organization, the rate of people aged 60 or more is growing faster than any other age group in almost every country, and this trend is not going to change in a near future. Since senior citizens are at high risk of non communicable diseases requiring long-term care, this trend will challenge the sustainability of the entire health system. Pervasive computing can provide innovative methods and tools for early detecting the onset of health issues. In this paper we propose a novel method relying on medical models, provided by cognitive neuroscience researchers, describing abnormal activity routines that may indicate the onset of early symptoms of mild cognitive impairment. A non-intrusive sensor-based infrastructure acquires low-level data about the interaction of the individual with home appliances and furniture, as well as data from environmental sensors. Based on those data, a novel hybrid statistical-symbolical technique is used to detect first the activities being performed and then the abnormal aspects in carrying out those activities, which are communicated to the medical center. Differently from related works, our method can detect abnormal behaviors at a fine-grained level, thus providing an important tool to support the medical diagnosis. In order to evaluate our method we have developed a prototype of the system and acquired a large dataset of abnormal behaviors carried out in an instrumented smart home. Experimental results show that our technique has a high precision while generating a small number of false positives.

The “big data” explicitly produced by people through social applications, or implicitly gathered through sensors and transaction records, enables a new generation of mining and analysis tools to understand the trends and dynamics of today’s interconnected society. While important steps have been made towards personal, urban, and social awareness, several research challenges still need to be addressed to fully realize the pervasive computing vision. On the one hand, the lack of standard languages and common semantic frameworks strongly limit the possibility to opportunistically acquire available context data, reason with it, and provide proactive services. On the other hand, existing techniques for identifying complex contextual situations are mainly restricted to the recognition of simple actions and activities. Most importantly, due to the unprecedented quantity of digital traces that people leave as they go about their everyday lives, formal privacy methods and trust models must be enforced to avoid the “big data” vision turning into a “big brother” nightmare. In this chapter, the authors discuss the above-mentioned research issues and highlight promising research directions.

A major challenge of ubiquitous computing resides in the acquisition and modelling of rich and heterogeneous context data, among which, ongoing human activities at different degrees of granularity. In a previous work, we advocated the use of probabilistic description logics (DLs) in a multilevel activity recognition framework. In this paper, we present an in-depth study of activity modeling and reasoning within that framework, as well as an experimental evaluation with a large real-world dataset. Our solution allows us to cope with the uncertain nature of ontological descriptions of activities, while exploiting the expressive power and inference tools of the OWL 2 language. Targeting a large dataset of real human activities, we developed a probabilistic ontology modeling nearly 150 activities and actions of daily living. Experiments with a prototype implementation of our framework confirm the viability of our solution.

Smart environments with ubiquitous sensing technologies are a promising perspective for reliable and continuous healthcare systems with reduced costs. A primary challenge for such assisted living systems is the automated recognition of everyday activities carried out by humans in their own home. In this work, we investigate the use of Markov Logic Networks as a framework for activity recognition within intelligent home-like environments equippedwith pervasive light-weight sensor technologies. In particular, we explore the ability of MLNs to capture temporal relations and background knowledge for improving the recognition performance.
