1. Analysis of the accuracy and reliability of vertical jump evaluation using a low-cost acquisition system

Abstract Background The vertical jump can be analyzed based on the flight time achieved by the individual. This measurement can be obtained using a force platform or a three-dimensional infrared camera system, but such equipment is expensive and requires training for data collection and processing. Thus, this study aimed to evaluate the accuracy and reliability of using a smartphone and the Kinovea software compared with a force platform as a method of vertical jump analysis. Methods For this purpose, two independent evaluators analyzed videos of bipodal and unipodal vertical jumps by counting the variables among participants. The participants performed three consecutive jumps in bipodal and unipodal conditions with the dominant and non-dominant legs. Results The intra-rater analysis for bipodal jumps was found to have excellent reproducibility (ICC = 0.903 to 0.934), whereas for unipodal jumps, the reproducibility was moderate to excellent (ICC = 0.713 to 0.902). The inter-rater analysis showed that for bipodal jumps, the reproducibility is substantial to excellent (ICC = 0.823 to 0.926), while for unipodal jumps, it is moderate (ICC = 0.554 to 0.702). Conclusions Therefore, it can be concluded that the vertical jump evaluation can be performed using the smartphone-Kinovea system. However, the same evaluator should carry out the evaluation to maintain reliable indices.

Abstract Aims: Muscular strength (MS) and jump power (JP) tests are used to assess athletic ability and measure the effectiveness of training programs. However, their use in various sport modalities needs to be investigated further. This study aimed to explore the changes in MS and JP during three different moments of a macrocycle training session and verify the validity of the tests used to predict the effectiveness of basketball training programs. Methods: During macrocycle training (three different moments), sixteen basketball players were evaluated for MS (measured using isokinetic dynamometry at the speed of 60°/s) during concentric contraction of knee flexor and extensor muscles and JP, using countermovement vertical jump (CMVJ) on a force platform. Results: Peak torque and maximal work values for knee extension and flexion showed no differences, during the three moments analyzed. Additionally, no changes were observed for CMVJ. Conclusions: Our results suggest that the effectiveness of basketball training programs does not seem to be related to the performance achieved by athletes on the tests used. Moreover, the lack of changes in MS and JP values during the macrocycle could be related to the training structure used; volume, intensity, density and workload specificity.

5. Frame-Based Multiple-Description Video Coding with Extended Orthogonal Filter Banks

Abstract

We propose a frame-based multiple-description video coder. The analysis filter bank is the extension of an orthogonal filter bank which computes the spatial polyphase components of the original video frames. The output of the filter bank is a set of video sequences which can be compressed with a standard coder. The filter bank design is carried out by taking into account two important requirements for video coding, namely, the fact that the dual synthesis filter bank is FIR, and that loss recovery does not enhance the quantization error. We give explicit results about the required properties of the redundant channel filter and the reconstruction error bounds in case of packet errors. We show that the proposed scheme has good error robustness to losses and good performance, both in terms of objective and visual quality, when compared to single description and other multiple description video coders based on spatial subsampling. PSNR gains of 5 dB or more are typical for packet loss probability as low as 5%.

Abstract

To evaluate the impact of the additional use of early neuromuscular electrical stimulation (NMES) on an early mobilization (EM) protocol.Randomized controlled trial.ICU of the Clinical Hospital of Ribeirão Preto, University of São Paulo, Brazil.One hundred and thirty-nine consecutive mechanically ventilated patients were included in the first 48 hours of ICU admission.The patients were divided into two groups: EM and EM+NMES. Both groups received EM daily. In the EM+NMES group, patients additionally received NMES 5 days a week, for 60 minutes, starting in the first 48 hours of ICU admission until ICU discharge.Functional status, muscle strength, ICU and hospital length of stay (LOS), frequency of delirium, days on mechanical ventilation, mortality, and quality of life were assessed. Patients in the EM+NMES group presented a significant higher score of functional status measured by the Functional Status Score for the ICU scale when compared with the EM group in the first day awake: 22 (15-26) versus 12 (8-22) (p = 0.019); at ICU discharge: 28 (21-33) versus 18 (11-26) (p = 0.004); and hospital discharge: 33 (27-35) versus 25 (17-33) (p = 0.014), respectively. They also had better functional status measured by the Physical Function Test in the ICU scale, took less days to stand up during the ICU stay, and had a significant shorter hospital LOS, lower frequency of ICU-acquired weakness, and better global muscle strength.The additional application of early NMES promoted better functional status outcomes on the first day awake and at ICU and hospital discharge. The patients in the EM+NMES group also took fewer days to stand up and had shorter hospital LOS, lower frequency of ICU-acquired weakness, and better muscle strength. Future studies are still necessary to clarify the effects of therapies associated with EM, especially to assess long-term outcomes.

Abstract

Introduction: There is, so far, no universal definition of severe asthma. This definition usually relies on: number of exacerbations, inhaled therapy, need for oral corticosteroids, and respiratory function. The use of such parameters varies in the different definitions used. Thus, according to the parameters chosen, each patient may result in having severe asthma or not. The aim of this study was to evaluate how the choice of a specific definition of severe asthma can change the allocation of patients. Methods: Data collected from the Severe Asthma Network Italy (SANI) registry were analyzed. All the patients included were then reclassified according to the definitions of U-BIOPRED, NICE, WHO, ATS/ERS, GINA, ENFUMOSA, and TENOR. Results: 540 patients, were extracted from the SANI database. We observed that 462 (86%) met the ATS/ERS criteria as well as the GINA criteria, 259 (48%) the U-Biopred, 222 (41%) the NICE, 125 (23%) the WHO, 313 (58%) the Enfumosa, and 251 (46%) the TENOR criteria. The mean eosinophil value were similar in the ATS/ERS, U-Biopred, and Enfumosa (528, 532 and 516 cells/mcl), higher in WHO and Tenor (567 and 570 cells/mcl) and much higher in the NICE classification (624 cells/mcl). Lung function tests resulted similarly in all groups, with WHO (67%) and ATS/ERS-GINA (73%), respectively, showing the lower and upper mean FEV1 values. Conclusions: The present observations clearly evidence the heterogeneity in the distribution of patients when different definitions of severe asthma are used. However, the recent definition of severe asthma, provided by the GINA document, is similar to that indicated in 2014 by ATS/ERS, allowing mirror reclassification of the patients examined. This lack of homogeneity could complicate the access to biological therapies. The definition provided by the GINA document, which reflects what suggested by ATS/ERS, could partially overcome the problem.

Abstract

Background and aims: Severe asthma is burdened by frequent exacerbations and use of oral corticosteroids (OCS) which worsen patients’ health and increase healthcare spending. Aim of this study was to assess the clinical and economic effect of adding mepolizumab (MEP) for the treatment of these patients. Methods: Patients >18 years old, referred to 8 asthma clinics, starting MEP between May 2017 and December 2018, were enrolled and followed-up for 12 months. Information in the 12 months before mepolizumab were collected retrospectively. The evaluation parameters included: OCS use, number of exacerbations/hospitalizations, concomitant therapies, comorbidity, and annual number of working days lost due to the disease. The primary objective was to compare the annual total cost per patient pre- and post-MEP. Secondary outcomes included rates of exacerbations and number of OCS-dependent patients. Results: 106 patients were enrolled in the study: 46 male, median age 58 years. Mean annual cost pre- and post-MEP (cost of biologic excluded) was €3996 and €1,527, respectively. Total savings due to MEP resulted in €2469 (95%CI 1945–2993), 62% due to exacerbations reduction and 33% due to productivity increase. Such savings could fund about 22% of the total cost of MEP for one year. The introduction of MEP induced a clinical benefit by reducing both OCS-dependent patients (OR = 0.12, 95%CI 0.06–0.23) and exacerbation rate (RR = 0.19, 95%CI 0.15–0.24). Conclusions: Patients with severe eosinophilic asthma experienced a clinical benefit in asthma control adding MEP to standard therapy. Biologic therapy can be, partially, funded by the savings produced by patients’ improvement.

37. O uso do ultrassom terapêutico na regeneração de nervos periféricos após lesão por esmagamento

Abstract

O ultrassom terapêutico (UST) é um recurso não invasivo que pode auxiliar na regeneração nervosa periférica. Objetivo: Revisar os efeitos da aplicação do UST na regeneração de nervos periféricos. Material e métodos: A revisão de literatura não sistemática teve como base de dados Lilacs, Medline, Cochrane e Pubmed, além das consultas em referências de livros e periódicos do acervo da biblioteca da Faculdade de Medicina de Ribeirão Preto. Foram analisadas 66 publicações entre artigos e teses, sendo selecionados 28 trabalhos publicados entre 1994 a 2009. Incluí­ram-se estudos constando as seguintes dimensões: caracterí­sticas da regeneração nervosa, lesões nervosas periféricas por esmagamento, caracterí­sticas e aplicação do UST no tratamento de lesões nervosas periféricas. Primeiramente, serão descritas as lesões nervosas periféricas e suas etapas de regeneração, seguido por um breve comentário sobre lesões por esmagamento. A seguir será abordada a aplicação do UST como forma de tratamento em lesões nervosas periféricas. Resultados: A literatura disponí­vel demonstra que o uso do UST no modo pulsado com baixa intensidade foi efi caz no tratamento, acelerando o processo de regeneração. Conclusão: O uso do UST apresentou respostas satisfatórias no processo de regeneração do nervo periférico, sendo necessários maiores estudos para padronizar os parâmetros fí­sicos do UST, da pressão exercida durante a lesão por esmagamento, do perí­odo de tratamento e acompanhamento, bem como das análises realizadas. Abstract

The quality of grapes in the production of wine is highly influenced by vine water status, where optimal water deficit or selective harvesting can improve berry quality. It is in this context that the rapid advancement in small unmanned aerial system (sUAS) technology and the potential application of real-time, high-spatial resolution hyperspectral imagery for vineyard moisture assessment, have become tractable. This study sought to further sUAS hyperspectral imagery as a tool to model water status in a commercial vineyard in Upstate New York. High-spatial resolution (2.5 cm ground sample distance) hyperspectral data were collected in the visible/near-infrared (VNIR; 400-1000nm) regime on three flight days. A Scholander pressure chamber was used to directly measure the midday stem water potential (Ψstem) within imaged vines at the time of flight. High spatial resolution pixels enabled the targeting of pure (sunlit) vine canopy with vertically trained shoots and significant shadowing. We used the partial least squares-regression (PLS-R) modeling method to correlate our hyperspectral imagery with measured field water status and applied a wavelength band selection scheme to detect important wavelengths. We evaluated spectral smoothing and band reduction approaches, given signal-to-noise ratio (SNR) concerns. Our regression results indicated that unsmoothed curves, with the range of wavelength bands from 450- 1000 nm, provided the highest model performance with R2 = 0.68 for cross-validation. Future work will include hyperspectral flight data in the short-wave infrared (SWIR; 1000-2500 nm) regime that were also collected. Ultimately, models will need validation in different vineyards with a full range of plant stress.

Abstract

In this work we develop an adaptive scheme for quantization of subband or transform coded frames in a typical video sequence coder. Using a generalized Gaussian model for the subband or transform coefficients, we present a procedure to determine the optimum dead-zone quantizer for a given entropy of the quantizer output symbols. We find that, at low bit rates, the dead-zone quantizer offers better performance than the uniform quantizer. The model is used to develop an adaptive procedure to update the quantizer parameters on the basis of the state of a channel buffer with constant output rate and variable input rate. We compare the accuracy of the generalized Gaussian model in predicting the actual bit rate to that achievable using the simpler and more common Laplacian model. Experimental results show that the generalized Gaussian model has superior performance than the Laplacian model, and that it can be effectively used in a practical scheme for buffer control. Abstract

Nowadays, disruptive events pose significant threats to organizations, making resilience a critical focus. Evaluating supply chain resilience is essential to avoid escalating disruptions. However, the literature on this topic is fragmented, and the assessment of resilience remains an open gap due to the lack of a clear definition of the dimensions and elements for its evaluation. To fill this gap, this study integrates a systematic literature review (SLR) and a systematic literature network analysis (SLNA) to propose a MAturity MOdel for REsilient Supply Chains (MaMoReS). Through content analysis and stringent selection of 15 resilience maturity models (ResMMs) via SLR, along with an SLNA on 6,474 sources, this method defines dimensions and sub-dimensions. The proposed MaMoReS is framed around five levels and five dimensions: risk management, agility, flexibility and adaptability, redundancy and robustness, transparency and visibility, and collaboration and relationships. The MaMoReS is applied to two case studies, pinpointing the practicality of the MaMoReS for measuring the resilience maturity level of the two companies along with their dimensions and sub-dimensions scoring. Thus, the MaMoReS can be used as an assessment tool by supply chain managers and researchers to evaluate the resilience of a supply chain.

Several approaches for decreasing the estimation time of autocorrelation functions from finite data are presented. The frequency-domain techniques, which were derived for application to spectral estimation and finite impulse response Wiener filtering, are applied in the fast Fourier transform (FFT) solution of two-dimensional signals. They are also applicable to the FFT computation of multidimensional signals.

Abstract

Hyperglycemia increases glycosylation with O-linked N-acetyl-glucosamine (O-GlcNAc) contributing to placental dysfunction and fetal growth impairment. Our aim was to determine how O-GlcNAc levels are affected by hyperglycemia and the O-GlcNAc distribution in different placental regions.Female Wistar rats were divided into the following groups: severe hyperglycemia (300 mg/dL; n = 5); mild hyperglycemia (140 mg/dL, at least than two time points during oral glucose tolerance test; n = 7) or normoglycemia (120 mg/dL; n = 6). At 21 days of pregnancy, placental tissue was collected and processed for morphometry and immunohistochemistry analyses, or properly stored at -80 °C for protein quantification by western blot.Placental index was increased only in severe hyperglycemic rats. Morphometric analysis showed increased junctional zone and decreased labyrinth region in placentas exclusively from the severe hyperglycemic group. Proteins targeted by O-GlcNAc were detected in all regions, with increased O-GlcNAc levels in the hyperglycemic group compared to control and mild hyperglycemic rats. Proteins in endothelial and trophoblast cells were the main target for O-GlcNAc. Whereas no changes in O-GlcNAc transferase (OGT) expression were detected, O-GlcNAcase (OGA) expression was reduced in placentas from the severe hyperglycemic group and augmented in placentas from the mild hyperglycemic group, compared with their respective control groups.Placental O-GlcNAc overexpression may contribute to placental dysfunction, as indicated by the placental index. Additionally, morphometric alterations, occurring simultaneously with increased O-GlcNAc accumulation in the placental tissue may contribute to placental dysfunction during hyperglycemia.

Abstract

Abstract Objective To analyze the bioelectrical impedance parameters of the lower limbs of individuals with hip osteoarthritis and healthy individuals. Design Cross-sectional study. Setting The study was carried out at the Hip Surgery Outpatient Clinic. Participants The volunteers had to be between 45 and 70 years of age, of both sexes, with a clinical and radiological diagnosis of hip osteoarthritis for at least three years, unilateral involvement, or a significant complaint in one hip. Methods This was a cross-sectional study. Fifty-four individuals were recruited for the study, 31 individuals with hip osteoarthritis (OA group) and 29 healthy individuals for the control group (C group). Demographic and anthropometric data were collected and then the Numerical Pain Rating Scale, WOMAC, Harris Hip Score, and bioimpedance assessment were applied. Main outcome measure(s) Electrical bioimpedance parameters. Phase angle (PhA), impedance, reactance, and muscle mass. Results There was a significant difference in phase angle (PhA), impedance, and muscle mass at 50 kHz frequency on the side affected by OA when compared to the contralateral side. In the OA group, there was a significant decrease in phase angle (PhA) -0.54 (-0.85 to -0.23) and muscle mass − 0.29 (-0.40 to -0,19), as well as an increase in impedance at the 50 kHz frequency on the side affected by OA when compared to contralateral side 21.71 (13.69 to 29.74). In the C group, there was no difference between the dominant and non-dominant sides (P > 0.05). Conclusion The segmental electrical bioimpedance equipment can detect differences between limbs affected and unaffected by hip osteoarthritis.
