Abstract

OBJECTIVE To describe the surgical technique and clinical outcome of small ruminants treated for obstructive urolithiasis using a modified tube cystostomy (MTC) technique. ANIMALS 15 goats and 2 sheep treated with an MTC between March 2018 and February 2023. CLINICAL PRESENTATION Animals were diagnosed with obstructive urolithiasis on the basis of history, physical examination, and ultrasonographic examination. An MTC was performed with sedation and a local block. Postoperative medical management was instituted to help reestablish urethral patency, and Foley catheters were removed after successful urination. RESULTS Animals were hospitalized an average of 3 nights (range, Oto 14 nights). Complications included urinespillage in the abdomen and accidental deflation of the Foley balloon. Six animals were euthanized due to poor prognosis or failure to regain urethral patency. Foley catheters were removed an average of 15.7 days postoperatively in animals that regained urethral patency. Long-term (> 1-month) follow-up was available for 8 animals, with an average postoperative survival time of 19.4 months (range, 1 to 58 months). Four animals were lost to long-term follow-up. CLINICAL RELEVANCE This MTC technique is an effective means of catheterizing the urinary bladder in small ruminants. It can be performed under field conditions and serve as a standalone procedure for providing temporary urine egress. Patient size is limited by the length of the introducer, and an intact, distended urinary bladder and plan for reestablishing urethral patency are important considerations. [ABSTRACT FROM AUTHOR]

Abstract

Las heridas crónicas representan un grave problema de salud para aquellas personas que las sufren, no solo por el número de pacientes a los que afectan, sino que, por sus costes, directos e indirectos, también empiezan a preocupar a los sistemas sanitarios. Los tratamientos convencionales empleados en este tipo de lesiones no siempre consiguen el resultado esperado, además de que conllevan largos periodos de recuperación. Los microinjertos autólogos en sello (MAS) surgen como una opción terapéutica prometedora para el tratamiento de estas heridas, reduciendo tiempos de cicatrización y complicaciones asociadas. Nuestro objetivo es comparar la efectividad de los MAS con respecto a otros métodos terapéuticos tradicionales en el tratamiento de heridas crónicas. Se ha elaborado una revisión sistemática siguiendo las recomendaciones de la declaración CASPe. Los artículos seleccionados se han limitado a los últimos 10 años y se han encontrado en las bases de datos de Pubmed, Google Académico, Scielo, Cochrane Library y Dialnet. Se ha demostrado la eficacia de los MAS frente al tratamiento tradicional en la cura de heridas crónicas. Se ha evidenciado una mejora en los tiempos de cicatrización, así como en la mejoría del dolor y prevención de infecciones. Abstract

The health training project led by the University of Las Palmas de Gran Canaria in Mozambique has two distinct impacts: on the one hand, it contributes to improving the education system and, on the other hand, also affects its health system. The evaluation of the results of this initiative will determine the fulfillment of the objectives, the impact and their sustainability., El proyecto de formación sanitaria liderado por la Universidad de Las Palmas de Gran Canaria en Mozambique tiene dos impactos diferenciados: por un lado, contribuye a mejorar el sistema educativo del país, y por otro lado, también tiene efectos sobre su sistema sanitario. La evaluación de los resultados de esta iniciativa permitirá determinar el cumplimiento de los objetivos, el impacto y la sostenibilidad del mismo., Le projet de formation en santé dirigé par l’Université de Las Palmas au Mozambique a deux impacts distincts: d’une part, contribue à l’amélioration du système d’éducation et d’autre part a également des effets sur leur système de santé. 34. Calidad de vida y función sexual en mujeres con cirrosis hepática en Colombia.

Abstract

Introduction: The female sex has been systematically identified as one of the predictors of poor quality of life in patients with cirrhosis. However, the relationship between quality of life and sexual function in women with cirrhosis has not been widely studied. Aim: To determine the relationship between quality of life and sexual function in women with cirrhosis. Materials and methods: A cross-sectional observational study. Female patients aged 18 to 69 years were included. The SF-36-V2 survey assessed health-related quality of life, and the IFSF-6 evaluated sexual function. Univariate and multivariate linear regression was used to identify the relationship between quality of life and sexual function. Statistical significance was set at a p-value < 0.05. Results: 36 patients were available for analysis. The average age was 61 years. In the SF36-V2 survey, the physical component summary mean was 58, and the mental component summary mean was 56. The IFSF-6 had an average of 10. Sexual dysfunction occurred in 80% of patients. The IFSF-6 score was positively related to health-related quality of life. Factors associated with the deterioration of sexual function were age and menopause. The Child-Pugh score and body mass index were the main determinants of health-related quality of life. Conclusions: Sexual function could be a determinant of quality of life in women with cirrhosis. [ABSTRACT FROM AUTHOR]

35. Insulinoma: reporte de un caso con resección quirúrgica exitosa.

Abstract

Insulinoma is a pancreatic neuroendocrine tumor that produces hyperinsulinism and causes a severe lowering of blood glucose levels with chronic symptoms such as fainting. It is more common in males and can occur at any age, either as solitary or multiple neoplasms, the latter known to be part of the multiple endocrine neoplasia, type 1 (MEN 1) group. Insulinomas are rare and require resective surgery, with symptoms resolution being the marker of recovery. This article describes a case of insulinoma in a 51-year-old woman with a clinically typical presentation and signs and symptoms of hyperinsulinism. Laboratory tests revealed increased insulin secretion, while magnetic resonance imaging showed features of insulinoma, where the pancreatic tumor could be seen. This case was treated by surgical resection of the pancreatic tumor during an exploratory laparotomy, using the enucleation technique, which is described in detail herein. There were no intraoperative or postoperative complications, i.e., the result was successful in terms of health recovery. This surgical technique is used at Hospital Nacional Arzobispo Loayza in Lima, Peru, when facing cases such as the one described. This type of neoplasm is very uncommon, so it should be considered highly suspicious and, in this case, the images were essential for choosing the surgical resection. Other cases of insulinoma have been treated surgically at the Hospital; moreover at least one case of nesidioblastosis could not receive an adequate treatment. [ABSTRACT FROM AUTHOR]
