BackgroundWe previously reported that impaired type I IFN activity, due to inborn errors of TLR3- and TLR7-dependent type I interferon (IFN) immunity or to autoantibodies against type I IFN, account for 15-20% of cases of life-threatening COVID-19 in unvaccinated patients. Therefore, the determinants of life-threatening COVID-19 remain to be identified in ~ 80% of cases.MethodsWe report here a genome-wide rare variant burden association analysis in 3269 unvaccinated patients with life-threatening COVID-19, and 1373 unvaccinated SARS-CoV-2-infected individuals without pneumonia. Among the 928 patients tested for autoantibodies against type I IFN, a quarter (234) were positive and were excluded.ResultsNo gene reached genome-wide significance. Under a recessive model, the most significant gene with at-risk variants was TLR7, with an OR of 27.68 (95%CI 1.5-528.7, P = 1.1 × 10-4) for biochemically loss-of-function (bLOF) variants. We replicated the enrichment in rare predicted LOF (pLOF) variants at 13 influenza susceptibility loci involved in TLR3-dependent type I IFN immunity (OR = 3.70[95%CI 1.3-8.2], P = 2.1 × 10-4). This enrichment was further strengthened by (1) adding the recently reported TYK2 and TLR7 COVID-19 loci, particularly under a recessive model (OR = 19.65[95%CI 2.1-2635.4], P = 3.4 × 10-3), and (2) considering as pLOF branchpoint variants with potentially strong impacts on splicing among the 15 loci (OR = 4.40[9%CI 2.3-8.4], P = 7.7 × 10-8). Finally, the patients with pLOF/bLOF variants at these 15 loci were significantly younger (mean age [SD] = 43.3 [20.3] years) than the other patients (56.0 [17.3] years; P = 1.68 × 10-5).ConclusionsRare variants of TLR3- and TLR7-dependent type I IFN immunity genes can underlie life-threatening COVID-19, particularly with recessive inheritance, in patients under 60 years old.

Circulating autoantibodies (auto-Abs) neutralizing high concentrations (10 ng/mL, in plasma diluted 1 to 10) of IFN-α and/or -ω are found in about 10% of patients with critical COVID-19 pneumonia, but not in subjects with asymptomatic infections. We detect auto-Abs neutralizing 100-fold lower, more physiological, concentrations of IFN-α and/or -ω (100 pg/mL, in 1/10 dilutions of plasma) in 13.6% of 3,595 patients with critical COVID-19, including 21% of 374 patients > 80 years, and 6.5% of 522 patients with severe COVID-19. These antibodies are also detected in 18% of the 1,124 deceased patients (aged 20 days-99 years; mean: 70 years). Moreover, another 1.3% of patients with critical COVID-19 and 0.9% of the deceased patients have auto-Abs neutralizing high concentrations of IFN-β. We also show, in a sample of 34,159 uninfected subjects from the general population, that auto-Abs neutralizing high concentrations of IFN-α and/or -ω are present in 0.18% of individuals between 18 and 69 years, 1.1% between 70 and 79 years, and 3.4% >80 years. Moreover, the proportion of subjects carrying auto-Abs neutralizing lower concentrations is greater in a subsample of 10,778 uninfected individuals: 1% of individuals 80 years. By contrast, auto-Abs neutralizing IFN-β do not become more frequent with age. Auto-Abs neutralizing type I IFNs predate SARS-CoV-2 infection and sharply increase in prevalence after the age of 70 years. They account for about 20% of both critical COVID-19 cases in the over-80s, and total fatal COVID-19 cases.

Over the last two decades, strain and GPS measurements have shown that slow slip on earthquake faults is a widespread phenomenon. Slow slip is also inferred from correlated small amplitude seismic signals known as nonvolcanic tremor and low frequency earthquakes (LFEs). Slow slip has been reproduced in laboratory and simulation studies, however the fundamental physics of these phenomena and their relationship to dynamic earthquake rupture remains poorly understood. Here we show that, in a laboratory setting, continuous seismic waves are imprinted with fundamental signatures of the fault's physical state. Using machine learning on continuous seismic waves, we can infer several bulk characteristics of the fault (friction, shear displacement, gouge thickness), at any time during the slow slip cycle. This analysis also allows us to infer many properties of the future behavior of the fault, including the time remaining before the next slow slip event. Our work suggests that by applying machine learning approaches to continuous seismic data, new insight into the physics of slow slip could be obtained in Earth.

Nearly all aspects of earthquake rupture are controlled by the friction along the fault that progressively increases with tectonic forcing, but in general cannot be directly measured. We show that fault friction can be determined at any time, from the continuous seismic signal. In a classic laboratory experiment of repeating earthquakes, we find that the seismic signal follows a specific pattern with respect to fault friction, allowing us to determine the fault's position within its failure cycle. Using machine learning, we show that instantaneous statistical characteristics of the seismic signal are a fingerprint of the fault zone shear stress and frictional state. Further analysis of this fingerprint leads to a simple equation of state quantitatively relating the seismic signal power and the friction on the fault. These results show that fault zone frictional characteristics and the state of stress in the surroundings of the fault can be inferred from seismic waves, at least in the laboratory., Comment: 12 pages, 4 figures. Early diagnosis of inborn errors of immunity (IEIs) has been shown to reduce mortality, morbidity, and healthcare costs. The need for early diagnosis has led to the development of computational tools that trigger earlier clinical suspicion by physicians. Primary care professionals serve as the first line for improving early diagnosis. To this end, a computer-based tool (based on extended Jeffrey Modell Foundation (JMF) Warning Signs) was developed to assist physicians with diagnosis decisions for IEIs in the primary care setting. Two expert-guided scoring systems (one pediatric, one adult) were developed. IEI warning signs were identified and a panel of 36 experts reached a consensus on which signs to include and how they should be weighted. The resulting scoring system was tested against a retrospective registry of patients with confirmed IEI using primary care EHRs. A pilot study to assess the feasibility of implementation in primary care was conducted. The scoring system includes 27 warning signs for pediatric patients and 24 for adults, adding additional clinically relevant criteria established by expert consensus to the JMF Warning Signs. Cytopenias, ≥ 2 systemic infections, recurrent fever and bronchiectasis were the leading warning signs in children, as bronchiectasis, autoimmune diseases, cytopenias, and > 3 pneumonias were in adults. The PIDCAP (Primary Immune Deficiency "Centre d'Atenció Primària" that stands for Primary Care Center in Catalan) tool was implemented in the primary care workstation in a pilot area. The expert-based approach has the potential to lessen under-reporting and minimize diagnostic delays of IEIs. It can be seamlessly integrated into clinical primary care workstations. We exploit nonlinear elastodynamic properties of fractured rock to probe the micro‐scale mechanics of fractures and understand the relation between fluid transport and fracture aperture under dynamic stressing. Experiments were conducted on rough, tensile‐fractured Westerly granite subject to triaxial stresses. We measure fracture permeability for steady‐state fluid flow with deionized water. Pore pressure oscillations are applied at amplitudes ranging from 0.2 to 1 MPa at 1 Hz frequency. During dynamic stressing we transmit ultrasonic signals through the fracture using an array of piezoelectric transducers (PZTs) to monitor evolution of interface properties. We examine the influence of fracture aperture and contact area by conducting measurements at effective normal stresses of 10–20 MPa. Additionally, the evolution of contact area with stress is characterized using pressure sensitive film. These experiments are conducted separately with the same fracture and map contact area at stresses from 9 to 21 MPa. The measurements are a proxy for "true" contact area for the fracture surface and we relate them to elastic properties using the calculated PZT sensor footprints via numerical modeling of Fresnel zones. We compare the elastodynamic response of the fracture using the stress‐induced changes in ultrasonic wave velocities for transmitter‐receiver pairs to image spatial variations in contact properties. We show that nonlinear elasticity and permeability enhancement decrease with increasing normal stress. Additionally, post‐oscillation wave velocity and permeability exhibit quick recoveries toward pre‐oscillation values. Estimates of fracture contact area (global and local) demonstrate that the elastodynamic and permeability responses are dominated by fracture topology. Plain Language Summary: We perform laboratory experiments with fractured rock to understand the relation between fluid flow, fracture openness, and elastic properties under oscillating stresses. These experiments are conducted on rough, pre‐fractured granite specimens under normal stress conditions similar to those found in the shallow earth, a few kilometers in depth. Fluid pressure in the fracture is oscillated at various amplitudes at a fixed frequency. During this dynamic stressing, we use an ultrasonic device to monitor the evolution of the fracture interface. We examine the influence of fracture aperture and contact area by conducting measurements at increasing normal stress state. Additionally, the evolution of contact area with stress is characterized using pressure sensitive film, showing an image of where the two halves of the fracture are in contact. The ultrasonic monitoring reveals spatial variations in contact properties, which is informed by fracture contact area measurements. These measurements are also related to the fluid‐flow in response to dynamic stressing and similar comparisons are made for how the fracture interface evolves, recovers, following the stress perturbations. We study the influence of relative humidity (RH) on the nonlinear elastic response of granular media. Previous work has shown that the nonlinear elastic response of consolidated granular media like rocks likely arises from two distinct mechanisms; however, we do not have a clear understanding of their physical origins at the microscopic scale. Here, we conduct dynamic acousto-elastic testing (DAET) on samples of glass beads under dry (∼10%), ambient (∼60%), and humid (∼100%) conditions at room temperature and a constant static stress of 4 MPa. DAET allows us to retrieve the full nonlinear elastic response, including transient softening and hysteretic effects. We find that the elastic nonlinearity of humid samples is an order of magnitude larger than dry samples. Moreover, we find that all extracted nonlinear parameters increase with RH. This overall increase in nonlinearity is consistent with findings from previous studies and with the hypothesis that water adsorption on the grains makes the contact junctions weaker and prone to greater disturbances when subjected to dynamic stressing. Our results also suggest that, if indeed both mechanisms coexist, they are affected in a similar fashion in these glass bead samples and cannot be distinguished by varying RH. [ABSTRACT FROM AUTHOR]

Ce livre explore les œuvres et les travaux de Freud et Proust en étudiant ce qu’ils apportent de nouveau en psychologie, et les progrès qu’ils peuvent nous faire accomplir dans la connaissance de ce qu’on appelait, à l’âge classique, le cœur humain. Ces progrès peuvent être de deux ordres : Ils peuvent consister dans la conquête de nouveaux sentiments, de nouvelles sensations, de nouvelles couches de la conscience, ou dans l’invention de nouvelles méthodes pour explorer celle-ci, dans l’invention d’une nouvelle manière d’attaquer les sentiments et les sensations. Nous distinguerons ces deux ordres de progrès, mais sans trop de rigueur pour ne pas nous interdire les points de vue qui se présenteraient et qui excluraient cette différence.Une remarque encore. Notre étude ne sera pas un simple exposé. Il ne faudra pas vous attendre à sortir de ces causeries avec la connaissance du système de Freud et du système de Proust, comme on peut sortir d’un cours de la Sorbonne avec la connaissance du système de Platon. Je ne chercherai qu’à extraire du système de nos deux auteurs — si tant est qu’ils aient tous les deux un système — ce qui pourra être mis en rapport avec notre expérience intime et ce qui recevra de celle-ci une lumière.

- Abstract
Multisystem inflammatory syndrome in children (MIS-C) is a rare but severe disease temporarily related to SARS-CoV-2. We aimed to describe the epidemiological, clinical, and laboratory findings of all MIS-C cases diagnosed in children < 18 years old in Catalonia (Spain) to study their trend throughout the pandemic. This was a multicenter ambispective observational cohort study (April 2020– April 2022). Data were obtained from the COVID-19 Catalan surveillance system and from all hospitals in Catalonia. We analyzed MIS-C cases regarding SARS-CoV-2 variants for demographics, symptoms, severity, monthly MIS-C incidence, ratio between MIS-C and accumulated COVID-19 cases, and associated rate ratios (RR). Among 555,848 SARS-CoV-2 infections, 152 children were diagnosed with MIS-C. The monthly MIS-C incidence was 4.1 (95% CI: 3.4–4.8) per 1,000,000 people, and 273 (95% CI: 230–316) per 1,000,000 SARS-CoV-2 infections (i.e., one case per 3,700 SARS-CoV-2 infections). During the Omicron period, the MIS-C RR was 8.2 (95% CI: 5.7–11.7) per 1,000,000 SARS-CoV-2 infections, which was significantly lower (p < 0.001) than that for previous variant periods in all age groups. The median [IQR] age of MIS-C was 8 [4–11] years, 62.5% male, and 80.2% without comorbidities. Common symptoms were gastrointestinal findings (88.2%) and fever > 39 °C (81.6%); nearly 40% had an abnormal echocardiography, and 7% had coronary aneurysm. Clinical manifestations and laboratory data were not different throughout the variant periods (p > 0.05). Conclusion: The RR between MIS-C cases and SARS-CoV-2 infections was significantly lower in the Omicron period for all age groups, including those not vaccinated, suggesting that the variant could be the main factor for this shift in the MISC trend. 62 páginas, 5 figuras, 2 tablas, Multisystem inflammatory syndrome in children (MIS-C) is a rare and severe condition that follows benign COVID-19. We report autosomal recessive deficiencies of OAS1, OAS2, or RNASEL in five unrelated children with MIS-C. The cytosolic double-stranded RNA (dsRNA)-sensing OAS1 and OAS2 generate 2'-5'-linked oligoadenylates (2-5A) that activate the single-stranded RNA-degrading ribonuclease L (RNase L). Monocytic cell lines and primary myeloid cells with OAS1, OAS2, or RNase L deficiencies produce excessive amounts of inflammatory cytokines upon dsRNA or severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) stimulation. Exogenous 2-5A suppresses cytokine production in OAS1-deficient but not RNase L-deficient cells. Cytokine production in RNase L-deficient cells is impaired by MDA5 or RIG-I deficiency and abolished by mitochondrial antiviral-signaling protein (MAVS) deficiency.
