1. Detection of early-stage lung cancer in sputum using automated flow cytometry and machine learning

Abstract

Abstract Background Low-dose spiral computed tomography (LDCT) may not lead to a clear treatment path when small to intermediate-sized lung nodules are identified. We have combined flow cytometry and machine learning to develop a sputum-based test (CyPath Lung) that can assist physicians in decision-making in such cases. Methods Single cell suspensions prepared from induced sputum samples collected over three consecutive days were labeled with a viability dye to exclude dead cells, antibodies to distinguish cell types, and a porphyrin to label cancer-associated cells. The labeled cell suspension was run on a flow cytometer and the data collected. An analysis pipeline combining automated flow cytometry data processing with machine learning was developed to distinguish cancer from non-cancer samples from 150 patients at high risk of whom 28 had lung cancer. Flow data and patient features were evaluated to identify predictors of lung cancer. Random training and test sets were chosen to evaluate predictive variables iteratively until a robust model was identified. The final model was tested on a second, independent group of 32 samples, including six samples from patients diagnosed with lung cancer. Results Automated analysis combined with machine learning resulted in a predictive model that achieved an area under the ROC curve (AUC) of 0.89 (95% CI 0.83–0.89). The sensitivity and specificity were 82% and 88%, respectively, and the negative and positive predictive values 96% and 61%, respectively. Abstract

Low dose computed tomography (LDCT) is the standard of care for lung cancer screening in the United States (US). LDCT has a sensitivity of 93.8% but its specificity of 73.4% leads to potentially harmful follow-up procedures in patients without lung cancer. Thus, there is a need for additional assays with high accuracy that can be used as an adjunct to LDCT to diagnose lung cancer. Sputum is a biological fluid that can be obtained non-invasively and can be dissociated to release its cellular contents, providing a snapshot of the lung environment. We obtained sputum from current and former smokers with a 30+ pack-year smoking history and who were either confirmed to have lung cancer or at high risk of developing the disease. Dissociated sputum cells were counted, viability determined, and labeled with a panel of markers to separate leukocytes from non-leukocytes. After excluding debris and dead cells, including squamous epithelial cells, we identified reproducible population signatures and confirmed the samples' lung origin. In addition to leukocyte and epithelial-specific fluorescent antibodies, we used the highly fluorescent meso-tetra(4-carboxyphenyl) porphyrin (TCPP), known to preferentially stain cancer (associated) cells. We looked for differences in cell characteristics, population size and fluorescence intensity that could be useful in distinguishing cancer samples from high-risk samples. We present our data demonstrating the feasibility of a flow cytometry platform to analyze sputum in a high-throughput and standardized matter for the diagnosis of lung cancer.

Abstract

BACKGROUND: The efficacy and safety of benralizumab, an interleukin-5 receptor alpha-directed cytolytic monoclonal antibody, for the prevention of exacerbations in patients with moderate to very severe chronic obstructive pulmonary disease (COPD) are not known.METHODS: In the GALATHEA and TERRANOVA trials, we enrolled patients with COPD (at a ratio of approximately 2:1 on the basis of eosinophil count [≥220 per cubic millimeter vs. RESULTS: In GALATHEA, the estimates of the annualized exacerbation rate were 1.19 per year (95% confidence interval [CI], 1.04 to 1.36) in the 30-mg benralizumab group, 1.03 per year (95% CI, 0.90 to 1.19) in the 100-mg benralizumab group, and 1.24 per year (95% CI, 1.08 to 1.42) in the placebo group; the rate ratio as compared with placebo was 0.96 for 30 mg of benralizumab (P = 0.65) and 0.83 for 100 mg of benralizumab (P = 0.05). In TERRANOVA, the estimates of the annualized exacerbation rate for 10 mg, 30 mg, and 100 mg of benralizumab and for placebo were 0.99 per year (95% CI, 0.87 to 1.13), 1.21 per year (95% CI, 1.08 to 1.37), 1.09 per year (95% CI, 0.96 to 1.23), and 1.17 per year (95% CI, 1.04 to 1.32), respectively; the corresponding rate ratios were 0.85 (P = 0.06), 1.04 (P = 0.66), and 0.93 (P = 0.40). At 56 weeks, none of the annualized COPD exacerbation rate ratios for any dose of benralizumab as compared with placebo reached significance in either trial. Types and frequencies of adverse events were similar with benralizumab and placebo.CONCLUSIONS: Add-on benralizumab was not associated with a lower annualized rate of COPD exacerbations than placebo among patients with moderate to very severe COPD, a history of frequent moderate or severe exacerbations, and blood eosinophil counts of 220 per cubic millimeter or greater (Funded by AstraZeneca [GALATHEA and TERRANOVA] and Kyowa Hakko Kirin [GALATHEA]; GALATHEA and TERRANOVA ClinicalTrials.gov numbers, NCT02138916 and NCT02155660.).

Abstract

We assessed safety and tolerability of treatment with pirfenidone (1602–2403 mg·day −1 ) and nintedanib (200–300 mg·day −1 ) in patients with idiopathic pulmonary fibrosis (IPF). This 24-week, single-arm, open-label, phase IV study (ClinicalTrials.gov identifier NCT02598193) enrolled patients with IPF with forced vital capacity % pred ≥50% and diffusing capacity of the lung for carbon monoxide % pred ≥30%. Before initiating nintedanib, patients had received pirfenidone for ≥16 weeks and tolerated a stable dose of ≥1602 mg·day −1 for ≥28 days. The primary end-point was the proportion of patients who completed 24 weeks of combination treatment on pirfenidone (1602–2403 mg·day −1 ) and nintedanib (200–300 mg·day −1 ). Investigators recorded treatment-emergent adverse events (TEAEs), attributing them to pirfenidone, nintedanib, both or neither. 89 patients were enrolled; 73 completed 24 weeks of treatment (69 meeting the primary end-point) and 16 discontinued treatment prematurely (13 due to TEAEs). 74 patients had 418 treatment-related TEAEs, of which diarrhoea, nausea and vomiting were the most common. Two patients had serious treatment-related TEAEs. Combined pirfenidone and nintedanib use for 24 weeks was tolerated by the majority of patients with IPF and associated with a similar pattern of TEAEs expected for either treatment alone. These results encourage further study of combination treatment with pirfenidone and nintedanib in patients with IPF.

Investigators recorded treatment-emergent adverse events (TEAEs), attributing them to PFD, NIN, both or neither. Change from baseline FVC, DLco and K-BILD were assessed at 24 weeks. The study is monitored by a data monitoring committee. Results: Eighty-nine patients were enrolled. A pre-specified interim analysis was conducted once 63 patients (mean age 68.7 years, 85.7% male) completed (n=50) or discontinued (n=13) 24 weeks’ combination treatment. Fifty patients had 330 treatment-related TEAEs (Table); 11 patients discontinued due to TEAEs. Two patients had serious treatment-related TEAEs (Table) but none led to death. Final results for all 89 patients, including change from baseline FVC, DLco and K-BILD, will be presented at ERS. Conclusions: Combined PFD and NIN use for 24 weeks did not reveal a different safety profile to that expected for either treatment alone. Patients had tolerated a stable dose of PFD before initiation of NIN, which may explain why investigators attributed more TEAEs to NIN than PFD.

Abstract

Attempts to establish a biological basis for classifying human races into definable groups, arranged hierarchically from most advanced to least advanced, have a long and sordid history. From the days of the Spanish Inquisition, to the colonization of North and South America, the beginnings of the slave trade, to more recent claims about inborn racial differences in intelligence and personality, racists have tried to find biological differences that would separate the various races and provide a justification for social and economic exploitation. Biologically, races have been equated with subspecies as defined in the general biological literature. But the evidence from modern, molecular genetics, indicates that humans do not form the same kinds of distinguishable sub-groups common to other animal, especially mammalian, species. Biologically, human populations are 99% similar genetically, and the various visible characteristics that have traditionally been used to distinguish one “race” from another, do not correlate with any other characters of importance. Humans have evolved as a single lineage with many local populations that have always been in reproductive contact with each other. Thus, human sub-populations have not diverged to the extent that is found in other species, such as chimpanzees.

Abstract

In two of three phase 3 trials, pirfenidone, an oral antifibrotic therapy, reduced disease progression, as measured by the decline in forced vital capacity (FVC) or vital capacity, in patients with idiopathic pulmonary fibrosis ; in the third trial, this end point was not achieved. We sought to confirm the beneficial effect of pirfenidone on disease progression in such patients. In this phase 3 study, we randomly assigned 555 patients with idiopathic pulmonary fibrosis to receive either oral pirfenidone (2403 mg per day) or placebo for 52 weeks. The primary end point was the change in FVC or death at week 52. Secondary end points were the 6-minute walk distance, progression-free survival, dyspnea, and death from any cause or from idiopathic pulmonary fibrosis. RESULTS: In the pirfenidone group, as compared with the placebo group, there was a relative reduction of 47.9% in the proportion of patients who had an absolute decline of 10 percentage points or more in the percentage of the predicted FVC or who died ; there was also a relative increase of 132.5% in the proportion of patients with no decline in FVC (P

The next-generation C66x DSP integrated fixed and floating-point DSP implemented in TSMC 40nm process is presented in this paper. The DSP core runs at 1.25GHz at 0.9V and has a standby power consumption of 800mW. The core transistor count is 21.5 million. The DSP core features 8-way VLIW floating point Data path and a two level memory system and delivers 40 GMACS or 10 GFLOPS floating point MAC performance at 1.25GHz.

Abstract

Chronic obstructive pulmonary disease (COPD) is a common and underdiagnosed disease that is a major cause of disability and death in the US and abroad. A more aggressive approach to managing COPD in primary care is critical to improving outcomes in patients with COPD. Since asthma is usually the main condition considered in the differential diagnosis of COPD, a thorough knowledge of the differences between asthma and COPD is particularly important. A careful history, a thorough physical examination, and the early use of spirometry may help identify patients earlier in the course of the disease. Smoking cessation remains a mainstay of COPD therapy. Other nonpharmacologic therapy includes avoidance of risk factors, patient education, pulmonary rehabilitation and, when necessary, oxygen therapy. Once COPD has been diagnosed, pharmacologic treatment depends on symptoms as well as the severity of disease. Therapy for mild disease includes smoking cessation and the use of short-acting bronchodilators as needed. Pharmacotherapy for moderate and severe disease typically includes one or more long-acting bronchodilators in addition to the treatment already described for mild disease. Inhaled corticosteroids are indicated in severe and very severe patients with recurrent exacerbations. Long-term oxygen therapy is indicated for all patients with documented hypoxemia. Patients with severe and very severe disease should receive pulmonary rehabilitation, and in patients with very severe disease, lung transplantation or other surgical treatment, such as lung volume reduction surgery or bullectomy, should be considered for appropriate patients.

Analise cladistica das sequencias dos genes Epsilon-globin a e IRBP fornece informacāe complementar importante para urn esboco das principais linhas da filogenia dos macacos do Novo Mundo. As abordagens morfologicas e de genetica molecular sao razoavelmente consistentes com as evidencias disponiveis atraves do reg istro fossil , significando que as formas modernas fornecem uma boa base para o desenvolvimento de uma classificācao dos platirrineos, e que o entendimento das relacōes entre fosseis podem ser facil itados com a inclusao dos generos viventes nas analises. Os estudos moleculares e morfologicas fortalecem a ideia de tres grandes grupos modernos, possivelmente divergindo num intervalo de tempo relativamente curto . Considerando as discordâncias nos estudos da sistematica de platirrineos nas ultimas decadas - a correta localizacoo filogenetica de Cebus, Saimiri, Aotus e Callicebus - a cornbina#x00E7;oo das evidencias colocam Callicebus definitivamente como parente dos pitecineos. Elas reforcam tambem a ligacoo entre Saimiri e os calitriquineos, o elo entre Cebus e Saimiri, e sua associacoo com calitriquineos como uma linhagem monofiletica do grupo dos «cebideos». Os dados de DNA divergem, porem, com a colocacoo de Aotus como uma linhagem basal desse agrupamento, urn achado inconsistente com as evidencias morfologicas. A analise de DNA tambem aponta a necessidade de uma reconsideracoo da taxonomia do genero Callitrhix, que talvez noo seja monofiletica. Os dados confirmam parcialmente o padroo de ramificacoo do clade dos atelideos, posicionando Alouatta como a linhagem mais velha. Problemas que permanecem dentro dos calitriquineos e atelineos incluem: 1)os afinidades precisas entre os atelineos, Lagothrix, Ateles e Brachyt eles; e 2) a sequencia de ramificacoo dentre os calitriquineos, i.e., Callithrix/Cebuella, Leontopithecus , Saguinus e Callimico.
