Introduction: diaphragmatic injury is a challenge for surgeons. It is an injury that can be isolated. It is frequent in penetrating thoracoabdominal trauma. It represents a diagnostic challenge and the ideal approach is not yet well established. The occurrence of spontaneous healing of these injuries is still much discussed and even more, if it does, what is the healing mechanism? Objective: to macroscopically and histologically evaluate the natural evolution of perforation and cutting wounds equivalent to 30% of the left diaphragm. Method: 50 specimens of rats underwent a surgical procedure and, after 30 days, were euthanized and those that presented scar tissue in the diaphragm, the samples were submitted to histopathological study, using the hematoxylin and eosin stains, Massons trichrome and Picrosirius to assess the presence of collagen or muscle fibers (hyperplasia) in the scar. Results: it was found that healing occurred in diaphragmatic injuries in 90% of rats. We also observed the presence of fibrosis in all analyzed samples. Conclusion: Spontaneous healing occurred in most diaphragmatic injuries and the inflammatory reaction represented by the presence of fibrosis and collagen deposition was observed in all our samples. Muscle fiber hyperplasia did not occur. RESUMO Introdução: o ferimento diafragmático é um desafio para os cirurgiões. É uma lesão que pode ser isolada. É frequente nos traumas penetrantes toracoabdominais. Representa um desafio diagnóstico e a conduta ideal ainda não está bem estabelecida. A ocorrência da cicatrização espontânea dessas lesões é ainda muito discutida e mais ainda, se ocorre, qual o mecanismo de cicatrização? Objetivo: avaliar macroscopicamente e histologicamente a evolução natural das feridas perfuro cortantes equivalentes a 30% do diafragma esquerdo. Método: 50 espécimes de ratos, foram submetidos a procedimento cirúrgico e, após 30 dias, foram submetidos à eutanásia e aqueles que apresentaram tecido cicatricial no diafragma, as amostras foram submetidas a estudo histopatológico, usando as colorações de hematoxilina e eosina, tricrômico de Masson e Picrossirius para avaliar a presença de colágeno ou de fibras musculares (hiperplasia) na cicatriz. Resultados: verificou-se que ocorreu nas lesões diafragmáticas, a cicatrização em 90% dos ratos. Observamos também, a presença de fibrose em todas as amostras analisadas. Conclusão: ocorreu cicatrização espontânea na maioria das lesões diafragmáticas e a reação inflamatória representada pela presença de fibrose e deposição de colágeno foi observada em todas as nossas amostras. Não ocorreu hiperplasia de fibras musculares.

SUMMARY BACKGROUND: Serum tumor markers are molecules that are secreted by tumor cells and may be present in small amounts in the serum of healthy individuals. Their role as prognostic factors in lung cancer remains controversial. OBJECTIVE: To assess the prognostic role of CEA, CA 19-9, CA 15-3, and CA 125 in non-squamous non-small cell lung cancer. PATIENTS AND METHODS: A total of 112 patients with non-squamous non-small cell lung cancer from two Oncology Centers were retrospectively analyzed. Tumor marker levels were measured prior to treatment. Data regarding clinical characteristics and overall survival were collected. RESULTS: Median overall survival of all patients was 15.97 months. Pre-treatment elevations of CA 125 and CA 15-3 were associated with shorter overall survival (p=0.004 and p=0.014, respectively). Single CEA and CA 19-9 elevations were not associated with a worse prognosis. Patients with two or more elevated markers had a statistically significant decrease in overall survival (p=0.008). In the multivariate analysis, smoking status and number of positive tumor markers at diagnosis were independently associated with a worse prognosis. CONCLUSION: High pre-treatment levels of tumor markers were correlated with decreased survival in patients with non-squamous non-small cell lung cancer.

RESUMO Objetivo: avaliar uma nova técnica operatória para o tratamento do enfisema pulmonar avançado. Métodos: análise prospectiva de nove pacientes portadores de enfisema pulmonar grave, submetidos à pneumostomia. O procedimento foi realizado sob anestesia local, na parede torácica anterior, linha hemiclavicular, no segundo espaço intercostal, através de toracotomia anterior de 5cm para acesso ao lobo superior, cujo segmento anterior foi pinçado e fixado à pleura parietal. Realizada pneumotomia com eletrocautério e inserção romba de dispositivo (dreno) intrapulmonar. Para avaliação do procedimento, foram realizados os seguintes exames: testes de função pulmonar, exames de imagens, teste da caminhada de seis minutos e questionários de qualidade de vida, medidos todos no pré-operatório e 30 dias após o procedimento. Resultados: não houve mortes relacionadas ao procedimento. Exames de imagens mostraram diminuição do volume pulmonar. A função pulmonar mostrou significante redução do volume residual. O teste de caminhada de seis minutos mostrou um aumento na distância percorrida no pós-operatório. Houve melhora significante da qualidade de vida, demonstrada por meio dos seguintes questionários: Medical Outcomes Study 36 Item Short - Form Health Survey (SF-36), Saint-George Respiratory Questionnaire (SGRQ), Medical Research Council scale (MRC) e Eastern Cooperative Oncology Group Performance status (ECOG). Conclusão: a técnica proposta é viável, segura, de fácil realização e manutenção. ABSTRACT Objective: to evaluate a new operative technique for the treatment of advanced pulmonary emphysema. Methods: we conducted a prospective analysis of nine patients with severe pulmonary emphysema submitted to pneumostomy. The procedure was performed under local anesthesia, in the anterior thoracic wall, hemiclavicular line, in the second intercostal space, through an anterior thoracotomy of 5cm for access to the upper lobe, whose anterior segment was pinched and fixed to the parietal pleura. We carried out the pneumostomy with electrocautery and blunt insertion of an intrapulmonary drain. To assess the procedure, we performed pulmonary function tests, imaging tests, six-minute walk test, and applied quality of life questionnaires, all measured preoperatively and 30 days after the procedure. Results: no deaths occurred related to the procedure. Imaging studies showed a decrease in lung volume. The pulmonary function showed a significant reduction in the residual volume. The six-minute walk test showed an increase in the distance covered in the postoperative period. There was significant improvement of the quality of life as demonstrated through questionnaires Medical Outcomes Study 36 Item Short-Form Health Survey (SF-36), Saint-George Respiratory Questionnaire (SGRQ), Medical Research Council scale (MRC), and Eastern Cooperative Oncology Group Performance status (ECOG). Conclusion: the proposed technique is feasible, safe, easy to perform and to maintain.

A hidatidose é uma infecção parasitária causada por um grupo de helmintos da classe Eucestoda, podendo levar à formação de cistos em seus hospedeiros intermediários, principalmente em fígado e pulmões. A doença apresenta grande relevância na América do Sul, tanto por endêmica em países da região quanto por sua grande morbimortalidade. Neste artigo, é descrito o caso de uma mulher de 36 anos de idade, natural de área rural do Peru e residente em São Paulo - Brasil, que apresentou quadro de tosse, dispneia e hemoptise progressiva ao longo de um ano e meio, além de dor em quadrante superior direito de abdome. Por meio de sorologia e também de tomografias, nas quais foram evidenciados cistos pulmonares de conteúdo líquido e cisto hepático semelhante, foi diagnosticada hidatidose.Após diagnóstico, paciente foi abordada pela equipe da cirurgia torácica do Hospital Central da Santa Casa de Misericórdia de São Paulo para retirada de cistos pulmonares. Em segundo momento, foi realizada segmentectomia hepática esquerda parcial e dada continuidade ao tratamento clínico com Albendazol. Low-dose computed tomography (LDCT) screening for lung cancer has been demonstrated to be effective in reducing cancer mortality. However, these studies have not been undertaken in countries where the incidence of granulomatous disease is high. The First Brazilian Lung Cancer Screening Trial (BRELT1) has completed initial accrual and is now in the follow-up phase. We present results from the initial prevalence round of screening.The inclusion criteria were the same as those for the National Lung Cancer Screening Trial (NLST). Pulmonary nodules larger than 4 mm were considered positive and required evaluation by a multidisciplinary team. Indeterminate nodules were evaluated with fluorodeoxyglucose positron emission tomography/computed tomography (PET/CT) or biopsy when indicated. Statistical analysis was performed with Fisher's exact test to compare our positive findings with those of the NLST.From January 2013 to July 2014, 790 participants were enrolled. Positive LDCT scans were reported in 312 (39.4%) participants, with a total of 552 nodules larger than 4 mm. The comparison between positive findings in the NLST (7,191 of 26,722 cases) and those in the BRELT1 (312 of 790 cases) showed a significant difference (p0.001). The positive predictive value was lower in BRELT1 than in the NLST (3.2% versus 3.8%, respectively). Follow-up imaging was indicated in 278 of 312 (89.1%) participants; 35 procedures were performed in 25 participants. In 15 cases, benign lesions were diagnosed. Non-small-cell lung cancer (NSCLC) was diagnosed in 10 patients (prevalence of 1.3%). In 8 patients (stage IA/IB disease), treatment was by resection only, in 1 patient neoadjuvant chemotherapy was used (stage IIIA), and in 1 patient advanced disease was diagnosed (stage IV).Using NSLT criteria, a larger number of patients had positive scans (nodules), compared with previous lung cancer screening studies. However, the number of participants requiring surgical biopsy procedures and who were ultimately identified as having cancer was similar to other reports. This supports the role of screening in patient populations with a high incidence of granulomatous inflammation.

O adenocarcinoma colorretal é uma neoplasia muito frequente, originando metástases para diversos órgãos do corpo. Seus sintomas envolvem o sistema digestivo e podem variar. A doença tem uma evolução rápida, tornando o prognóstico mais reservado para casos com diagnóstico tardio. Uma disseminação metastática rara desse tipo de câncer é a região endotraqueal, apresentamos o relato de uma paciente de 46 anos, com história de adenocarcinoma colorretal T 3 N 1 M1, submetida a retossigmoidectomia, quimioterapia e radioterapia neoadjuvantes. Com evolução, em 3 anos, de metástase pulmonar tratada com segmentectomia pulmonar e lobectomia inferior direita. Após um ano da operação, ainda em quimioterapia, desenvolveu metástase endotraqueal confirmada por estudo anatomopatológico e imunohistoquímica.Descritores: Neoplasias da traqueia, Adenocarcinoma, Neoplasias colorretais, Metástase neoplásica

PURPOSE: To evaluate macro and microscopically, changes following the use of the aqueous extract of babassu (Orbignya phalerata) in the lung parenchyma and pleura of rats. METHODS: Sixty adult male rats with average weight of 350 g, were randomized into two groups of 30 animals (experimental and control) further divided into sub-groups of 10 to be sacrificed at 48 h, 72 h and 21 days. The substance was injected into the right pleura of the animals. RESULTS: There was intense pleuropulmonary macroscopic reaction with statistically significant differences between groups respectively (p0.05). CONCLUSION: The aqueous extract of babassu (Orbignya phalerata) was found to be highly irritating to the pleura and lung of rats, evidenced macroscopically by numerous adhesions and inflammation while no major changes were evident microscopically.

Objective: To analyze thoracic trauma assisted by the Emergency Service of Hospital da Irmandade da Santa Casa de Misericórdia de São Paulo. Methods: One hundred patients with thoracic trauma were assisted throughout six months in 2006. Data from their records were collected and a protocol of thoracic trauma was fulfilled. The Revised Trauma Score was used to evaluate gravity of injury and to calculate the survival index. Results: Prevalence of trauma injury in male from 20 to 29 years old was observed. Out of all patients, 44 had blunt trauma and 56 penetrating trauma (78.6% presented stab wounds and 21.4% gun shots). Up to the settings of injuries, 23% were in the thoracoabdominal transition, 7% in the precordium and 70% in the remainder thoracic area. In those with the thoracoabdominal transition injury, 22.7% were hemodynamically unstable and 77.3% stable. Thoracoabdominal injury patients presented 40.9% of diaphragm wound and all were stable. Of those with precordium wound, 37.5% presented cardiac injury. In cardiac onset, 66.7% presented stable and 33.3% unstable. Thoracic drainage was the most accomplished surgical procedure (71%). Conclusions: The thoracic trauma patient is most prevalently young male with stab wound penetrating injury, without associated injuries, hemodynamically stable, presenting hemothorax, with high probability of survival. RESUMO Objetivo: Analisar os traumas torácicos atendidos pelo Serviço de Emergência do Pronto-Socorro Geral do Hospital da Irmandade da Santa Casa de Misericórdia de São Paulo. Métodos: Foi preenchido um Protocolo de Trauma Torácico com dados dos prontuários de cem pacientes atendidos em 2006 por seis meses. Utilizamos o Escore Revisado de Trauma para avaliar a gravidade da lesão e calcular a taxa de sobrevida. Resultados: Houve prevalência de homens entre 20 e 29 anos. Sobre as lesões, 44% eram traumas fechados e 56% traumas penetrantes, sendo desses 78,6% por arma branca e 21,4% por arma de fogo. Quanto à localização das lesões, 23% encontravam-se na transição toracoabdominal, 7% na região precordial e 70% no restante da área torácica. Dos pacientes com ferimento toracoabdominal, 22,7% eram instáveis e 77,3% estáveis. Nas lesões toracoabdominais, 40,9% apresentavam lesões diafragmáticas, e todos os pacientes eram estáveis. Entre os pacientes com ferimentos precordiais, 37,5% apresentavam lesões cardíacas, sendo estas 66,7% estáveis e 33,3% instáveis. O procedimento cirúrgico mais realizado foi a drenagem de tórax (71%). Conclusões: O paciente de trauma torácico mais frequente é o homem jovem, com ferimento penetrante por arma branca, estável, apresentando hemotórax sem lesões associadas, e com alta probabilidade de sobrevida.

Objective: to evaluate natural evolution of right diaphragmatic injury after the surgical removal of a portion from hemi diaphragm. Methods: the animals were submitted to a surgical removal of portion from right hemi diaphragm by median laparotomy. The sample consists of 42 animals being 2 animals from pilot project and 40 operated animals. And the variables of the study were herniation, liver protection, healing, persistent diaphragm injury, evaluation of 16 channels tomography and the variables "heart rate" and "weight". Results: we analyzed 40 mice, we had two post-operative deaths; we had 17 animals in this group suffered from herniation (42.5%) and 23 animals didn't suffer from herniation (57.5%). Analyzing the tomography as image method in the evaluation of diaphragmatic hernia, we had as a method with good sensitivity (78.6%), good specificity (90.9%), and good accuracy (86.1%) when compared to necropsy. Conclusion: there was a predominance of healing of right hemi diaphragm, the size of initial injury didn't have influence on occurrence of the liver protection or hernia in mice. Objetivo: avaliar a evolução natural do ferimento diafragmático à direita após a retirada cirúrgica de uma porção do hemidiafragma. Métodos: os animais foram submetidos à ressecção de uma porção do hemidiafragma à direita através da laparotomia mediana. Foram operados 40 ratos. As variáveis analisadas foram: herniação, proteção hepática, cicatrização, lesão persistente do diafragma, avaliação da tomografia computadorizada, frequência cardíaca e peso. Resultados: foram analisados 40 ratos. Houve duas mortes no pós-operatório. Dezessete animais tiveram herniação (42,5%) e 23 (57,5%), não. Analisando emprego da tomografia computadorizada na avaliação da hérnia diafragmática, tivemos um método com boa sensibilidade (78,6%), boa especificidade (90,9%) e boa acurácia (86,1%) quando comparados com a necrópsia. Conclusão: houve predomínio de cicatrização do hemidiafragma à direita e o tamanho da lesão inicial não influenciou na ocorrência de proteção hepática ou hérnia em ratos.

Summary Background and objectives The change from the sitting position to supine position, general anesthesia, and surgical procedure reduce lung volumes and this effect can be greater in obese patients. The objective of the present study was to evaluate the influence of the sitting position, 30 dorsal inclination, and horizontal dorsal decubitus on spirometry of grade III obese patients. Methods Twenty-six adult patients in the preoperatory period were selected according to the following criteria: BMI > 40 kg.m −2 , age between18 years and 60 years, and female gender. Variables analyzed included: age, weight, height, BMI, percentage of predictive values of FVC, FEV 1 , and VEF 1 /FVC in the sitting position (90°), 30° dorsal elevation, and horizontal dorsal decubitus (0°). ANOVA, followed or not by Tukey test were used to compare mean predicted values on the different positions, considering significant a p value lower than 0.05. Results Percentage values of FVC, FEV 1 , and FEV 1 /FVC ratio regarding predicted values in the sitting position (90°), 30° dorsal elevation, and horizontal dorsal decubitus (0°), and p value of the corresponding statistical analysis were, respectively: FVC = 92.8%, 88.2%, and 86.5%, p = 0.301 (ANOVA); FEV 1 : 93.1%, 83.8%, and 83.3%, p = 0.023 (ANOVA), p = 0.038 (Tukey test – 90° × 0°); FEV 1 /FVC: 100,8%, 95.5%, and 96.8%, p = 0.035 (ANOVA), p = 0.035 (Tukey test – 90° × 30°). Conclusions Changes in position produced changes in spirometry results of patients with grade III obesity.

No presente artigo, relatamos o caso de um paciente portador de teratoma de mediastino anterior, de evolução lenta, o qual foi submetido à ressecção cirúrgica. O exame anatomopatológico da peça revelou degeneração maligna para tumor carcinóide. Tal evolução é extremamente rara, sendo encontrados na literatura apenas três artigos correlatos. Apresentamos uma descrição clínico-patológica do tumor e, por fim, discutimos a conduta terapêutica. Houve evolução satisfatória, e o paciente foi submetido a tratamento oncológico.

Relato de caso de um homem de 49 anos com quadro de dor torácica e escarros hemópticos de início há seis meses. Pela radiografia e tomografia de tórax visualizou-se opacificação em lobo superior esquerdo. A broncoscopia mostrou restos hemáticos em segmento brônquico lingular. Frente à necessidade diagnóstica e terapêutica, optou-se por submetê-lo a uma toracotomia inframamilar à esquerda. O estudo anatomopatológico da peça cirúrgica revelou teratoma intrapulmonar. O paciente evoluiu satisfatoriamente e segue em acompanhamento ambulatorial.

OBJETIVO: Avaliar a função respiratória em indivíduos submetidos à abdominoplastia, através de parâmetros espirométricos. MÉTODO: Estudo prospectivo de 33 mulheres hígidas e não tabagistas, submetidas à abdominoplastia, e com resultados normais para as radiografias de tórax realizadas antes e quatro dias após a operação. Foram realizadas provas espirométricas no pré-operatório, no quarto, 15o e no 30o dias de pós-operatório. RESULTADOS: Provas espirométricas evidenciaram que, exceto pela relação VFE1/CVF e FEF 25-75%/CVF, todos os demais parâmetros avaliados (CVF, VEF1, FEF 25-75% e PFE) se apresentaram significativamente diminuídos no 4o PO em relação aos valores pré-operatórios. Os valores de CVF e PFE se mostraram normalizados à avaliação realizada no 30o PO. Já os valores de VEF1 ainda se revelaram significativamente inferiores aos valores pré-operatórios mesmo na última avaliação. CONCLUSÕES: Ocorreu diminuição da função respiratória no 4º dia de pós-operatório com normalização até o 30º dia de pós-operatório.

Objective: To analyze the late results of advanced Chagasic megaesophagus treatment by esophagectomy associated with the use of proton pump inhibitor (omeprazole) as for the incidence of esophagitis and Barrett's esophagus in the remaining stump. Methods : We studied patients with advanced megaesophagus undergoing esophagectomy and transmediastinal esophagogastroplasty. Patients were divided into three groups: A (20) with esophageal replacement by full stomach, without the use of omeprazole; B (20) with esophageal replacement by full stomach, with omeprazole 40 mg/day introduced after the first postoperative endoscopy and maintained for six years; and C (30) with esophageal replacement by gastric tube with use of omeprazole. Dysphagia, weight loss and BMI were clinical parameters we analyzed. Upper gastrointestinal endoscopy was performed in all patients, and determined the height of the anastomosis, the aspect of the mucosa, with special attention to possible injuries arising from gastroesophageal reflux, and the patency of the esophagogastric anastomosis. Results : We studied 50 patients, 28 males (56%) and 22 (44%) females. All underwent endoscopy every year. In the first endoscopy, erosive esophagitis was present in nine patients (18%) and Barrett's esophagus, in four (8%); in the last endoscopy, erosive esophagitis was present in five patients (8%) and Barrett's esophagus in one (2%). When comparing groups B and C, there was no evidence that the manufacturing of a gastric tube reduced esophagitis and Barrett's esophagus. OBJETIVOS: Com o intuito de definir a função da videotoracoscopia no diagnóstico e tratamento no trauma torácico, foram estudados 51 traumatizados por traumas penetrantes ocasionados por arma branca, ferimentos por projétil de arma de fogo ou traumas fechados do tórax, com suspeita diagnóstica por exame clínico e/ou radiológico de lesões torácicas. MÉTODOS: Foram selecionados doentes estáveis vítimas de trauma torácico (pressão arterial sistólica igual ou superior a 90mmHg) com diagnóstico de: hemotórax em 20 (cinco hemotórax estacionários, quatro hemotórax progressivos e 11 hemotórax coagulados), contusões e ferimentos precordiais (três), ferimentos da zona de transição tóraco-abdominal (24), corpo estranho no tórax (dois) e ferimentos transfixantes do mediastino (dois). Todos foram submetidos à videotoracoscopia. RESULTADOS: A videotoracoscopia se mostrou eficiente na investigação diagnóstica nos casos de hemotórax progressivo (quatro casos), hemotórax coagulado (11 casos), contusões e ferimentos precordiais (três casos), ferimentos da zona da transição tóraco-abdominal (confirmação de nove lesões diafragmáticas em 24 traumatizados examinados, 37,5%) e corpos estranhos no tórax, retirada do corpo estranho com sucesso (dois casos). O procedimento também foi eficiente, além do diagnóstico, no tratamento de hemotórax progressivo (ligadura de artéria mamária um caso, cauterização de vasos intercostais um caso), hemotórax coagulado (remoção de coágulos e decorticação, 11 casos) tendo evitado a realização de toracotomia em 33,3% dos traumatizados examinados. CONCLUSÕES: A videotoracoscopia é método eficiente para diagnóstico e tratamento no traumatismo do tórax e ainda pode evitar a toracotomia em expressivo número de pacientes submetidos ao procedimento. BACKGROUND: The videothoracoscopy in diagnosis and management of thoracic trauma was evaluated, 51 individuals attended in the Emergency Service of the Surgery Department of Santa Casa de São Paulo with the intention of defining the function of the procedure in cases of penetrating trauma caused by stab, fire-arms projectile or blunt, in diagnosis suspected clinical or radiologic of thoracic lesions. METHODS: Were selected patients victims of thoracic trauma with diagnosis of hemothorax, precordial contusions and wounds, wounds of thoracic-abdominal transition, embedded knife in the chest and transfixing wounds of mediastinum. The procedures were accomplished in having traumatized stable (blood pressure same or superior 90mmHg). All of them were submitted to videothoracoscopy. RESULTS: videoassisted thoracic surgery is an efficient procedure in the diagnostic investigation, in cases of progressive hemothorax (4 cases) and coagulated hemothorax (11 cases), precordial contusions and wounds (3 cases), wounds of thoracic-abdominal transition (24 cases, diaphragmatic injury was confirmed in 9, 37.5%) and embedded knife from the chest (2 cases). The procedure was efficient as well in the management of progressive and coagulated hemothorax and to remove embedded knife from the chest, having avoided the thoracotomy in 33.3% of the examined individuals. CONCLUSIONS: the videothoracoscopy is efficient procedure for diagnosis and treatment in thoracic trauma and it can still avoid the thoracotomy in expressive number of patients submitted to the procedure.

Os tumores do mediastino anterior incluem várias entidades com diferentes manifestações radiológicas e clínicas, consistindo em um grupo heterogêneo de condições congênitas, inflamatórias ou neoplásicas. Entre essas lesões, o tumor primário mais comum do mediastino é o timoma, seguido de perto por tumores de células germinativas e linfomas. A tuberculose do timo é extremamente rara, embora o envolvimento dos linfonodos mediastinais por essa entidade seja comum. Apresentamos aqui achados patológicos, radiológicos e clínicos de um caso de tuberculose tímica em um paciente de 18 anos de idade, que apresentou dor torácica, dispnéia a pequenos esforços e piora contínua dos sintomas em uma semana. A radiografia torácica mostrou uma grande massa no mediastino e a tomografia computadorizada mostrou que a localização era anterior. O paciente foi operado, sendo feita a exérese da massa, com aspecto microscópico de uma reação inflamatória maciça e presença de granulomas no tecido tímico. A pesquisa de bacilos álcool ácido resistentes pelo método de Ziehl-Neelsen foi positiva e o diagnóstico de tuberculose foi fechado. Portanto, o cirurgião e o patologista devem estar alertas quanto à essa entidade, bem como incluí-la na lista de diagnósticos diferenciais de massas mediastinais.

OBJECTIVE: To evaluate the natural healing of the rat diaphragm that suffered an extensive right penetrating injury.METHODS: Animals were submitted to an extensive penetrating injury in right diaphragm. The sample consisted of 40 animals. The variables studied were initial weight, weight 21 days after surgery; healing of the diaphragm, non-healing of the diaphragm, and herniated abdominal contents into the chest.RESULTS: Ten animals were used as controls for weight and 30 animals were operated. Two animals died during the experiment, so 28 animals formed the operated group; healing of the diaphragm occurred in 15 animals (54%), 11 other animals showed diaphragmatic hernia (39%) and in two we observed only diaphragmatic injury without hernia (7%). Among the herniated organs, the liver was found in 100% of animals, followed by the omentum in 77%, small bowel in 62%, colon in 46%, stomach in 31% and spleen in 15%. Os ferimentos da região tóracoabdominal podem cursar com ferimentos diafragmáticos em até 48% dos casos. Lesões associadas ao trauma diafragmático, estão presentes na maioria das vezes, o que facilita o diagnóstico da lesão diafragmática e em consequência impõe o tratamento operatório. Porém em 8 a 10% dos casos, as lesões diafragmáticas são isoladas e por apresentarem pouco ou nenhum sintoma, estas lesões podem passar despercebidas. Nestas situações, o diafragma poderia ser tratado de modo conservador, sem a sutura do mesmo? Após a realização de trabalhos experimentais com animais e de analisar a literatura, concluímos que ainda não podemos afirmar com certeza de que é possível não suturar um ferimento diafragmático. Wounds to the abdominal-thoracic region are associated with diaphragm wounds in up to 48% of cases. Lesions secondary to diaphragm trauma are present in the majority of cases, facilitating the diagnosis of diaphragm lesion and subsequent surgical management. However, diaphragm lesions are isolated in 8 to 10% of cases and because they present few or no symptoms may be overlooked. In such situations, can the diaphragm be treated conservatively without suturing? Based on experiments in animals and a review of the literature, we concluded there is currently insufficient evidence to affirm that diaphragm injuries require no suturing.

JUSTIFICATIVA E OBJETIVOS: A mudança de posição sentada para a posição supina, a anestesia geral e o procedimento cirúrgico reduzem os volumes pulmonares, e esse efeito pode ser maior nos obesos. O objetivo deste estudo foi avaliar a influência das posições sentada, inclinação dorsal de 30º e decúbito dorsal horizontal na espirometria de doentes portadores de obesidade grau III. MÉTODO: Foram selecionados 26 pacientes adultos no período pré-operatório, obedecendo aos seguintes critérios de inclusão: portadores de IMC > 40 kg.m-2, maiores de 18 e menores de 60 anos e gênero feminino. As variáveis analisadas foram: idade, peso, altura, IMC, porcentagens dos valores preditos da CVF, VEF1 e VEF1/CVF nas posições sentada (90º), com elevação dorsal de 30º e decúbito dorsal horizontal (0º). A comparação entre as médias dos valores previstos nas diversas posições foi realizada por meio do teste de ANOVA, seguido ou não do teste de Tukey, sendo considerado significativo valor de p inferior a 0,05. RESULTADOS: Os valores das porcentagens da CVF, do VEF1 e da relação VEF1/CVF em relação aos valores previstos nas posições sentada (90º), com elevação dorsal de 30º e decúbito dorsal horizontal (0º) e o valor de p da análise estatística correspondente, foram, respectivamente: CVF: 92,8% / 88,2% / 86,5%, p = 0,301 (ANOVA); VEF1: 93,1% / 83,8% / 83,3%, p = 0,023 (ANOVA), p = 0,038 (teste de Tukey - 90º x 0º); VEF1/CVF: 100,8% / 95,5% / 96,8%, p = 0,035 (ANOVA), p = 0,035 (teste de Tukey - 90º x 30º). CONCLUSÕES: As mudanças de posição produzem alteração nos resultados da espirometria de pacientes portadores de obesidade grau III. BACKGROUND AND OBJECTIVES: The change from the sitting position to supine position, general anesthesia, and surgical procedure reduce lung volumes and this effect can be greater in obese patients. The objective of the present study was to evaluate the influence of the sitting position, 30º dorsal inclination, and horizontal dorsal decubitus on spirometry of grade III obese patients. METHODS: Twenty-six adult patients in the preoperatory period were selected according to the following criteria: BMI > 40 kg.m-2, age between18 years and 60 years, and female gender. Variables analyzed included: age, weight, height, BMI, percentage of predictive values of FVC, FEV1, and VEF1/FVC in the sitting position (90º), 30º dorsal elevation, and horizontal dorsal decubitus (0º). ANOVA, followed or not by Tukey test were used to compare mean predicted values on the different positions, considering significant a p value lower than 0.05. RESULTS: Percentage values of FVC, FEV1, and FEV1/FVC ratio regarding predicted values in the sitting position (90º), 30º dorsal elevation, and horizontal dorsal decubitus (0º), and p value of the corresponding statistical analysis were, respectively: FVC = 92.8%, 88.2%, and 86.5%, p = 0.301 (ANOVA); FEV1: 93.1%, 83.8%, and 83.3%, p = 0.023 (ANOVA), p = 0.038 (Tukey test - 90º x 0º); FEV1/FVC: 100,8%, 95.5%, and 96.8%, p = 0.035 (ANOVA), p = 0.035 (Tukey test - 90º x 30º). CONCLUSIONS: Changes in position produced changes in spirometry results of patients with grade III obesity. El objetivo de este estudio fue evaluar la influencia de las posiciones sentada, inclinación dorsal de 30º y decúbito dorsal horizontal en la espirometría de enfermos portadores de obesidad grado III. MÉTODO: Se seleccionaron 26 pacientes adultos en el período preoperatorio, obedeciendo a los siguientes criterios de inclusión: portadores de IMC > 40 kg.m-2, mayores de 18 y menores de 60 años y sexo femenino. Las variables analizadas fueron: edad, peso, altura, IMC, porcentajes de los valores previstos de la CVF, VEF1 y VEF1/ CVF en las posiciones sentada (90º), con elevación dorsal de 30º y decúbito dorsal horizontal (0º). OBJETIVO: Avaliar a incidência de fístula e estenose da anastomose esofagogástrica cervical com invaginação do coto esofágico no interior do estômago na esofagectomia para tratamento do carcinoma do esôfago. MÉTODOS: Foram estudados dois grupos de doentes com carcinoma do esôfago torácico ou abdominal submetidos à esofagectomia subtotal e esofagogastroplastia. O grupo I (estudo) foi constituído por 29 doentes operados no período de 1998 a 2007, no qual foi realizada a anastomose esofagogástrica cervical com invaginação de segmento do coto esofágico no interior do estômago. O grupo II (controle) foi constituído por 36 doentes operados no período de 1989 a 1997 submetidos à anastomose esfagogástrica cervical término-terminal sem invaginação. RESULTADOS: No grupo I, 3 (10,3%) doentes apresentaram fístula da anastomose esofagogástrica com repercussão clínica mínima. No grupo II observou-se fístula com franca saída de saliva em 11 (30,5%) doentes. A freqüência de fístula nos doentes do grupo I foi significantemente menor (p=0,04) do que nos do grupo II. No grupo I, estenose fibrótica da anastomose ocorreu em 7 (24,1%) enfermos, ao passo que no grupo II 10 (27,7%) evoluíram com estenose, não se constatando diferença significante (p=0,72) entre esses grupos. CONCLUSÃO: No tratamento do carcinoma do esôfago, a esofagectomia com anastomose esofagogástrica cervical com invaginação do coto esofágico no interior do estômago determina menor ocorrência de fístula esofagogástrica quando comparado à anastomose sem invaginação. A incidência de estenose da anastomose esofagogástrica não diferiu em ambos os grupos. OBJECTIVE: To assess the incidence of fistula and stenosis of cervical esophagogastric anastomosis with invagination of the esophageal stump into the gastric tube in esophagectomy for esophagus cancer. METHODS: Two groups of patients with thoracic and abdominal esophagus cancer undergoing esophagectomy and esophagogastroplasty were studied. Group I comprised 29 patients who underwent cervical esophagogastric anastomosis with invagination of the proximal esophageal stump segment within the stomach, in the period of 1998 to 2007 while Group II was composed of 36 patients submitted to end-to-end cervical esophago-gastric anastomosis without invagination during the period of 1989 to 1997. RESULTS: In Group I, esophagogastric anastomosis by invagination presented fistula with mild clinical implications in 3 (10.3%) patients, whereas in Group II, fistulas with heavy saliva leaks were observed in 11 (30.5%) patients. The frequency of fistulas was significantly lower in Group I patients (p=0.04) than in Group II. In Group I, fibrotic stenosis of anastomoses occurred in 7 (24.1%) subjects, and 10 patients (27.7%) in Group II evolved with stenosis, while no significant difference (p=0.72) was found between the two groups. CONCLUSION: In esophagectomy for esophagus cancer, cervical esophagogastric anastomosis with invagination presented a lower rate of esophagogastric fistula versus anastomosis without invagination. Stenosis rates in esophagogastric anastomosis proved similar in both approach with or without invagination.

OBJETIVO: Relatar os resultados obtidos no pré- e pós-operatório de três pacientes portadores de enfisema pulmonar difuso grave, empregando uma nova técnica: ventilação colateral com drenagem do parênquima pulmonar. MÉTODOS: Para a avaliação da drenagem pulmonar, foram selecionados pacientes que já haviam sido submetidos à terapêutica clínica máxima, incluindo a reabilitação pulmonar, e que ainda assim sofriam de falência respiratória com dispnéia incapacitante. Os pacientes foram submetidos, no pré- e no pós operatório, à pletismografia e ao teste da caminhada de seis minutos, assim como responderam aos seguintes questionários de qualidade de vida: Medical Outcomes Study 36-item Short-Form Health Survey, Saint George's Respiratory Questionnaire, Eastern Cooperative Oncology Group Performance Status e Medical Research Council Scale. Todos os pacientes tiveram seguimento de no mínimo 300 dias de pós-operatório. Os testes foram realizados no pré-operatório, entre 30 e 40 dias de pós-operatório e após 300 dias de pós-operatório. Os dados foram analisados pelo método de gráficos de perfis de médias. RESULTADOS: Quando comparados os resultados do pré-operatório com os do pós-operatório nos dois momentos, verificou-se que houve melhora em todos os parâmetros estudados. CONCLUSÕES: Os resultados sugerem que a técnica operatória proposta para o tratamento de doentes portadores de enfisema pulmonar difuso grave foi capaz de diminuir os sintomas debilitantes destes pacientes, tornando sua qualidade de vida muito melhor. OBJECTIVE: To report the results obtained in three patients with diffuse pulmonary emphysema during the pre- and post-operative periods following a new surgical technique: collateral ventilation with lung parenchyma drainage. METHODS: Patients suffering from pulmonary failure and disabling dyspnea, despite having received the gold standard treatment, including pulmonary rehabilitation, were selected for the evaluation of pulmonary drainage. During the pre- and post-operative periods, patients were submitted to plethysmography and six-minute walk tests, as well as completing the following quality of life questionnaires: Medical Outcomes Study 36-item Short-Form Health Survey, Saint George's Respiratory Questionnaire, Eastern Cooperative Oncology Group Performance Status and Medical Research Council Scale. In all three cases, the postoperative follow-up period was at least 300 days. The tests were performed at the following time points: during the pre-operative period; between post-operative days 30 and 40; and on post-operative day 300. Data were analyzed using profile plots of the means. RESULTS: When the results obtained in the pre-operative period were compared to those obtained at the two post-operative time points evaluated, improvements were observed in all parameters studied. CONCLUSIONS: The results suggest that the surgical technique proposed for the treatment of patients suffering from severe diffuse emphysema successfully reduces the debilitating symptoms of these patients, improving their quality of life considerably.
