Abstract

Background: During the COVID-19 pandemic many patients were switched from warfarin to direct)acting oral anticoagulants (DOACs), which require the creatinine clearance (CrCl) calculated to ensure the correct dose is prescribed to avoid bleeding or reduced efficacy. Aim: To identify the study population proportion prescribed a DOAC. Of these, the proportion with recorded: weight, estimated glomerular filtration rate (eGFR), creatinine, CrCl and atrial fibrillation (AF). To analyse the proportion of patients with recorded AF and CrCl prescribed a recommended DOAC dose. Design & setting: A retrospective cohort study of 20.5 million adult NHS patients’ electronic health records (EHRs) in England in the OpenSAFELY-TPP platform (January 2018–February 2023). Method: Patients on DOACs were analysed for age, sex, recorded weight, eGFR, creatinine, CrCl and AF. Prescribed DOAC doses in patients with recorded AF were compared with recommended doses for recorded CrCl and determined as either recommended, higher than recommended (overdose), or lower than recommended (underdose). Results: In February 2023, weight, eGFR, creatinine, CrCl, and AF were recorded in 72.8%, 92.4%, 94.3%, 73.5%, and 73.9% of study population, respectively. Both AF and CrCl were recorded for 56.7% of patients. Of these, 86.2% received the recommended, and 13.8% non-recommended, DOAC doses. Conclusion: CrCl is not recorded for a substantial number of patients on DOACs. We recommend that national organisations tasked with safety, collectively update guidance on the appropriate weight to use in the Cockcroft–Gault equation, clarify that CrCl is not equivalent to eGFR, and work with GP clinical system suppliers to standardise the calculation of CrCl in the EHR. Abstract

Programs within the Cooperative Extension Service often develop partnerships with other agencies and organizations to better meet their common goals. While there are many benefits of partnerships, they can be challenging when the partners are unequal or have dissimilar needs. Using a survey and interviews with faculty and administrators at two land-grant institutions we explore the similarities and differences that pull and push at their partnership. Abstract

The initial lesions observed in Crohn's disease typically occur over lymphoid aggregates in tne large bowel and Heyers patches in the small bowel where M-cells are present. An increased presence of Adherent and Invasive Escherichia coli (AlEC) has been reported in Crohn's disease mucosa. Crohn's disease is common in westernised countries where low levels of soluble fibre are eaten and where the use of emulsifiers in processed food is common. It is possible that dietary factors may be implicated in the pathogenesis of the disease. The aims of this work were to establish if AIEC interact with intestinal M-cells, and to establish the effect that dietary factors have on these interactions.

Abstract

This report has discussed the Space Exploration Initiative (SEI) and the U.S. Antarctic Program (USAP) in the context of assessing the potential rationale and strategy for conducting a cooperative NASA/NSF (National Science Foundation) effort. Specifically, such an effort would address shared research and data on living and conducting scientific research in isolated, confined, hostile, and remote environments. A review of the respective goals and requirements of NASA and the NSF indicates that numerous opportunities exist to mutually benefit from sharing relevant technologies, data, and systems. Two major conclusions can be drawn: (1) The technologies, experience, and capabilities existing and developing in the aerospace community would enhance scientific research capabilities and the efficiency and effectiveness of operations in Antarctica. The transfer and application of critical technologies (e.g., power, waste management, life support) and collaboration on crew research needs (e.g., human behavior and medical support needs) would streamline the USAP operations and provide the scientific community with advancements in facilities and tools for Antarctic research. (2) Antarctica is the most appropriate earth analog for the environments of the the Moon and Mars. Using Antarctica in this way would contribute substantially to near- and long-term needs and plans for the SEI. Antarctica is one of the few ground-based analogs that would permit comprehensive and integrated studies of three areas deemed critical to productive and safe operations on the Moon and Mars: human health and productivity; innovative scientific research techniques; and reliable, efficient technologies and facilities.

Abstract

Background: Primary angiitis of the central nervous system is a rare condition, which is often difficult to diagnose and is associated with significant morbidity and mortality. There is no standardised treatment protocol or randomised clinical trial evidence to guide management. Aim: To describe the clinical features, diagnostic modalities, treatment and outcomes in an Australian hospital population‐based series of primary cerebral angiitis. Methods: Data were collected via retrospective medical record review of patients with primary angiitis of the central nervous system for the period 1 July 1998 to 30 June 2009, using previously published diagnostic criteria. Eligible patient records were identified in two ways; from routinely collected hospital episode data, coded using the ICD‐10‐AM coding standard and by review of cerebral biopsy data. Results: Ten of 12 included patients had a positive cerebral biopsy, with two patients diagnosed by angiography. Mean age at diagnosis was 47.2 years (range 18–73 years), with a female predominance of 5:1. Headache was the most common symptom experienced. Seventy per cent of the biopsy specimens showed a lymphocytic vasculitis. All patients received treatment with either steroids alone or a combination of steroids and cyclophosphamide, the latter treatment being initiated for those with a higher modified Rankin score of disability. Nine (75%) responded to treatment. There was one in‐hospital death, and two patients had no documented response to treatment. Conclusion: This study presents the first Australian case series data of primary cerebral angiitis. Better understanding of management and outcomes of this rare condition would be gained through multicentre studies.

For example, does clinical need override cost, or should clinicians be guided purely by what is listed on the hospital formulary? The aim was to use an expert panel, a little like the popular TV show, to find the ‘X Factor’ of dressing choice.
