Abstract

Background: Despite recent advances in the treatment and prevention of cardiovascular disease, a treatment gap for secondary prevention medications still exists. Objective: To develop and implement a program ensuring appropriate prescription of aspirin, statins, β-blockers, angiotensin-converting enzyme inhibitors, and warlarin at hospital discharge. Design: A nonrandomized before-after study comparing patients hospitalized before (1996-1998) and after (1999-2002) implementation of a discharge medication program (DMP). Patients were followed for up to 1 year. Setting: The 10 largest hospitals in the Utah-based Intermountam Health Care system. Patients: In the pre-DMP and DMP time periods, 26 000 and 31 465 patients, respectively, were admitted to cardiovascular services (n = 57465). Measurements: Prescription of indicated medications at hospital discharge; post discharge death or readmission. Results: By 1 year, the rate of prescription of each medication increased significantly to more than 90% (P< 0.001); this rate was sustained. At 1 year, unadjusted absolute event rates for readmission and death, respectively, were 210 per 1000 person-years and 96 per 1000 person-years before OMP implementation and 191 per 1000 person-years and 70 per 1000 person-years afterward. Relative risk for death and readmission at 30 days decreased after OMP implementation; hazard ratios (HRs) for death and readmission were 0.81 (95% CI, 0.73 to 0.89) and 0.92 (CI, 0.87 to 0.99) (P < 0.001 and P = 0.017, respectively). At I year, risk for death continued to decrease (hazard ratio, 0.79 [CI, 0.75 to 0.84]; P < 0.001) while risk for readmission stabilized (hazard ratio, 0.94 [CI, 0.90 to 0.98]; P= 0.002), probably because survivors had more opportunities to be readmitted. Limitations: The study design was observational and nonrandomized, and the authors could not control for potential confounders or determine the extent to which secular trends accounted for the observed improvements. Conclusions: A relatively simple quality improvement program aimed at enhancing the prescription of appropriate discharge medications among cardiovascular patients is feasible and can be sustained within an integrated multihospital system. 45. Burden of Clostridioides difficile in a State's Long-term Care Facilities.

Abstract

Nearly 500,000 persons in the United States developed a Clostridioides difficile infection (CDI) in 2015 and more than 100,000 of these infections were in residents of long-term care facilities (LTCF). It is difficult to estimate the true burden of CDI in LTCF as reporting is not required. To gain a better understanding of CDI, the state health department (SHD) encourages LTCF to enroll and report CDI data into the National Healthcare Safety Network (NHSN). The SHD analyzed CDI data reported into NHSN's Long-Term Care Component from March 2017 to October 2018. Descriptive analyses were performed on CDI LabID events which were categorized as community-onset (CO), long-term care onset (LO) and acute care-transfer-long-term care onset (ACT-LO) according to NHSN definitions. CDI rates were also calculated based on the NHSN definitions. As of October 2018, 44 out of 320 LTCFs in the state were enrolled in NHSN. The median bed size was 103 (38–444). Between March 2017–October 2018, 29 out of 44 facilities reported 174 CDI LabID events with a range of 1–40. Of the total CDI events, 31 events were CO, 143 were LO and 64 LO events were categorized as ACT-LO. The overall CDI rate was 0.70 per 10,000 resident days. The CDI LO-Incidence rate was 0.52 per 10,000 resident days compared to a rate of 5.05 in acute care hospitals during the same period. 14% of long-term care facilities in the state are reporting CDI data voluntarily into NHSN. These data allow the SHD to better characterize the true burden of CDI statewide. Accurate reporting of infection data is imperative in order to establish a baseline infection rate and guide infection prevention strategies. 50. Improving SSI – COLO Surveillance: Applying Lessons Learned from External NHSN Data Validation.

Abstract

Accurate reporting to the National Healthcare Safety Network (NHSN) is critical in providing actionable surveillance data to inform infection prevention (IP) programs. The state health department (SHD) validated Surgical Site Infections following colon procedures (SSI – COLO) to assess data accuracy and identify common reporting errors. Using NHSN's External Validation Toolkit, the SHD selected 24 facilities based on data reported from January to June 2017. Facilities were targeted based on the highest number of predicted infections, and stratified by SIR; additionally, a 5% random sample of facilities was selected. The SHD selected up to 20 reported SSI – COLO events and up to 40 random colon procedures at each facility. The SHD reviewed patient medical records, including clinician documentation, radiology reports, surgical notes, laboratory records, re-admissions, and any available outpatient records. The SHD reviewed 925 colon procedures from 23 participating facilities. SHD identified 183 (20%) SSI – COLO events. 48 events were unreported. Superficial Incisional accounted for 48% of unreported events. Organ/Space and Deep Incisional constituted the remaining 35% and 17%, respectively. 75% of unreported events were due to a missed report (surgical, radiology, etc.) within the medical record. Facility specific sensitivities ranged from 0% to 100%. The sensitivities varied by depth of infection: Superficial Incisional (72%), Deep Incisional (70%), and Organ/Space (77%). The majority of unreported Organ/Space and Deep SSIs were due to missed radiology reports. Routine review of radiology reports as part of SSI surveillance was not common practice, but often the most reliable indication of an event. Most Superficial SSIs were found in the nursing/physician notes of purulence or incision opened at the bedside, therefore missed by IPs. In post-validation interviews with IPs, positive culture reports were cited as the most commonly used source to trigger an investigation.
