Diagnosis is challenging due to the asynchronous onset of renal and ocular symptoms, and it is necessary to exclude other causes of nephritis and uveitis. It affects both sexes, with an overall predominance of females and a median age of onset of 15 years. TINU syndrome is characterized by bilateral, non-granulomatous anterior uveitis and tubulointerstitial nephritis, which can cause acute renal failure in severe cases. Several triggers have been identified including concurrent infections, medications, or endocrine diseases. The diagnosis of TINU is mainly based on clinical and analytical findings, and although urinary β 2-microglobulin is a useful parameter no specific diagnostic test is available. Current perspectives intend to facilitate its diagnosis identifying susceptibility HLA genotypes, serologic markers and imaging tools to avoid renal biopsy. Treatment options for TINU syndrome include corticosteroids, immunosuppressive agents, and intravenous immunoglobulins, but relapses are frequent, and management can be challenging. The purpose of this review is to provide an updated summary of the diagnostic and treatment strategies of TINU syndrome, helping clinicians recognize and manage this rare autoimmune disorder.Keywords: TINU, uveitis, nephritis, inflammation

The classical stepwise approach to pharmacological therapy in adult asthma recommends that treatment is progressively stepped up by increasing the inhaled corticosteroid (ICS) dose or by adding another controller medication- to achieve symptom control and reduce the risk of exacerbations, and stepped down after a period of control. In general, asthma guideline recommendations do not reflect that there are significant differences between ICS in terms of potency. Moreover, they do not consider efficacy and safety separately, incorrectly assuming that “low” and “high” dose categories inevitably correspond with low and high risk of systemic effects. Another point of criticism is the fact that guidelines do not take into account the inflammatory profile of the patient, although substantial groups of patients with mild and moderate asthma have little evidence of “T2-high” inflammation, and by extension are likely to show a poor response to ICS treatment. On the other hand, the latest version of the Global Initiative for Asthma (GINA) equally recommends regular ICS and ICS/formoterol as needed to prevent exacerbations in step 2 patients, without taking into consideration that the therapeutic objectives (exacerbations, symptoms) may differ between individual patients and that different goals may warrant distinct treatment strategies. In this review, we bring to the table several controversial issues concerning asthma treatment and suggest an alternative proposal that takes into consideration the potential side effects of high ICS doses, the patient’s inflammatory profile and the therapeutic goals to be achieved.Keywords: asthma, asthma treatment, asthma phenotype, asthma endotype, personalized medicine

Apeles Mestres i Joan Maragall mantingueren una relació de simpatia recíproca la qual és documentalment provada per l’existència de diferents cartes escrites i rebudes entre ells. Ni l’antagonisme ideològic entre el republicanisme de Mestres i el catolicisme de Maragall ni la seva diferent concepció de la literatura van interferir a l’hora de poder establir una relació de caràcter personal. La seva amistat es basà, a banda de la necessària connexió o afinitat que anava més enllà de l’aspecte públic pel qual són coneguts, en una afició compartida, la de les plantes, i en la unió moral que els va adjuntar davant de certs paral·lelismes de tractament per part de la crítica alineada amb el Noucentisme, just al voltant de l’època en què es van a conèixer i van començar el seu intercanvi epistolar.

The primary objective of this study was to assess whether the mortality rates were different between four predefined clinical phenotypes. Patients and methods: This is a retrospective, observational study carried out at the COPD clinic of a University Hospital. A total of 891 COPD patients were classified, according to the Spanish COPD guidelines, into the following four phenotypes: asthma–COPD overlap (ACO; 75 subjects), nonexacerbator (NONEX; 531 subjects), exacerbator with chronic bronchitis (EXCB; 194 subjects), and exacerbator with emphysema (EXEMPH; 91 subjects). We compared the mortality outcomes between the phenotypes. Results: After a follow-up of 48.4±25.2 months, there were 194 deaths (21.8%). There were significant differences in all-cause mortality between phenotypes. The ACO phenotype had the best long-term prognosis, whereas EXEMPH had the highest risk of death. NONEX and EXCB mortality figures were in between the other two groups. We also found some differences in the causes of death, and patients with EXEMPH were at a higher risk of dying because of COPD itself. The differences in mortality did not seem related to the classification into phenotypes in itself but to disparities in COPD severity and comorbidity load between groups. Conclusion: Classifying COPD patients according to several predefined clinical phenotypes can identify clusters of subjects with different mortality outcomes. Some phenotypes are associated with a specific cause of death. The mechanisms that underlie these differences seem to be related to COPD severity and comorbidities. In addition, the possible (if any) differences in inflammation between both types of the disease are still not well known. The aim of this study was to assess the inflammatory profile in B-COPD and T-COPD.Methods: A total of 20 subjects (15 men and five women) with T-COPD were matched one to one for sex, age and forced expiratory volume in 1 s (FEV1) to 20 B-COPD patients. In all, 20 sex-matched healthy subjects with normal lung function without smoking history or biomass exposure were included as controls. The following biomarkers were measured: exhaled nitric oxide, serum IL-6, IL-8, IL-5, IL-13, periostin, surfactant protein-P, TNF-α, IgE, erythrocyte sedimentation rate, C-reactive protein and fibrinogen. Complete blood count was also obtained.Results: The age of the subjects was 70.2±7.9 years and FEV1% was 56.2%±14.6%. Most inflammatory biomarkers were higher in both types of COPD than in healthy controls. IL-6, IL-8 and IL-5 were significantly higher in T-COPD than in B-COPD, without other significant differences.Conclusion: Both types of COPD are associated with high levels of systemic inflammation biomarkers. T-COPD patients have a higher systemic inflammatory status than the patients with B-COPD. Keywords: biomass smoke, COPD, cytokines, inflammation, smoking

Este trabajo revisa la noción de la reformulación mediante la observación de su comportamiento en los textos escritos. El análisis del uso de un grupo de marcadores de reformulación explicativos en un corpus ha permitido delimitar sus funciones, que no solo son de naturaleza metadiscursiva sino también demarcativa y organizadora de la información. Para ello, se ha recurrido al modelo de segmentación de unidades discursivas de Basilea y su división del enunciado en bloques informativos que contribuyen de distinta manera al avance del texto. Se constata la existencia de correspondencias entre la capacidad de los marcadores de ocupar ciertas posiciones dentro del enunciado y su tendencia a realizar determinadas funciones y el diferente alcance de las instrucciones que expresan.

Aquest article es proposa indagar les principals aportacions dels romanistes alemanys, austríacs i suïssos al desenvolupament dels estudis catalans en àmbits de recerca com la Lingüística aplicada i l’ensenyament i l’aprenentatge del català, així com en els anomenats nous corrents d’investigació en lingüística general. Una revisió dels treballs més destacats, dins de camps com la Pragmàtica, l’Anàlisi Discursiva, la Lingüística Textual, la Lingüística de Corpus i la Fraseologia mostraran, d’una banda, el paper rellevant que han jugat els estudis de catalanística d’autors germanoparlants com a motor de transformacions i innovacions en l’anàlisi descriptiva de la llengua des d’una multiplicitat de noves perspectives metodològiques i, de l’altra, l’interès continuat dels acadèmics alemanys per la llengua i la cultura dels països de parla alemanya, el qual es prolonga des del propi començament de la Filologia Romànica fins els nostres dies.

Este traballo representa unha primeira aproximación cualitativa a un grupo de locucións marcadoras de reformulación (LMRs) con compoñente metalingüístico do español, catalán e alemán, que se caracterizan por incluíren entre os seus membros léxicos o verbo de lingua primario decir, dir, sagen. As LMRs comparten, como denominador común, a capacidade de actuar fóra do marco predicativo da oración e para sinalizar un tipo de relación entre as partes do texto que consiste nunha volta atrás pola que o emisor reinterpreta unha formulación anterior mediante unha nova que a explica, modifica, substitúe ou resume. A través dunha análise paralela destas unidades e das súas funcións máis características, pretendemos demostrar a existencia de características comúns que afectan tanto á selección dos seus compoñentes léxicos coma á clase de significado que achegan –que é tanto conceptual coma procedimental–, á súa capacidade distributiva e ao tipo de instrución discursiva que expresan sobre como debe ser interpretada a conexión entre os segmentos que ligan. Observaremos que os vestixios do significado léxico orixinal das LMRs estudadas aínda están presentes nas súas funcións textuais máis características; por iso consideramos que o valor metadiscursivo (plano da comunicación) que hoxe teñen, non é máis ca un desenvolvemento lóxico do seu valor metalingüístico inicial (plano léxico). Ademais, a existencia de patróns repetidos nas tres linguas parece apuntar a unha conceptualización moi similar por parte dos falantes da actividade lingüística e dos mecanismos de construción do texto. // This paper presents a first qualitative approach to a group of marker phrases of reformulation (MPRs) in Spanish, Catalan and German that contain a related metalinguistic component, namely the primary verb for speech: decir, dir, sagen. MPRs share as a common denominator the capacity to be used outside of the predicative frame of a sentence, in order to indicate a semantic relation between the parts of a text; they are used by a speaker to reinterpret a previous formulation by way of a new formulation that explains, modifies, substitutes or summarizes the first one. Through parallel analysis of these units and their characteristic functions, we will demonstrate the existence of shared features, in terms of the selection of their lexical components as well as in terms of the kind of meaning that they convey (simultaneously conceptual and procedural). As well, we will explore the distribution of these units, and the type of discursive instructions they provide to the recipient of a text regarding interpretation of the relation between the elements linked by an MPR. We will observe that some traces of the original lexical meaning of MPRs are still present in their most characteristic textual functions, and for this reason we consider that their current metadiscursive value is a logic extension of their original metalinguistic meaning. Furthermore, the existence of common patterns in all three languages seems to hint at a similar conceptualization on the part of speakers about the use of language and the mechanisms of text construction.

Este artículo analiza el papel discursivo de los marcadores de reformulación recapitulativos a partir de una definición del texto como un proceso dinámico de construcción orientado a la satisfacción de un propósito comunicativo concreto. La consideración de las dinámicas que rigen los procesos de producción textual nos permitirán analizar estos marcadores no como meros nexos textuales paratácticos, sino como los mecanismos mediante los que el emisor distribuye y presenta las distintas unidades comunicativas con las que compone su discurso. Mostraremos que kurz gesagt y mit einem Wort cumplen una doble función textual, organizativa y cognitiva, y contribuyen al intento de todo emisor de aproximar sus producciones a las expectativas del receptor a fin de facilitar la consecución de la meta de su actuación lingüística.

La reformulación es una operación de reinterpretación textual por la que el emisor vuelve sobre un segmento de habla anterior para presentarlo de distinta manera. Los elementos que señalan este movimiento discursivo son los reformuladores: palabras y sintagmas formal y semánticamente heterogéneos cuyo rasgo común es la realización de funciones conectivas y argumentativas al nivel del texto. Este artículo tiene como principal objetivo la definición y caracterización de los reformuladores alemanes. Además, propone una clasificación preliminar de estas unidades basada en su doble función como elementos de cohesión textual con valor deíctico y como indicadores de estrategias argumentativas. La adopción de una perspectiva basada en la gramática cognitiva ayudará a superar los límites de un modelo de análisis basado en la presencia/ausencia de rasgos constitutivos discretos y permitirá fundamentar la definición de esta categoría pragmática en criterios funcionales relacionados con el peculiar comportamiento de estas unidades a nivel textual.

El propósito de este artículo es ofrecer una perspectiva general de la teoría del ejemplo desarrollada por la lexicografía didáctica alemana reciente. Una revisión cronológica de las contribuciones más relevantes de los lingüistas germanoparlantes de los últimos treinta años nos permitirá realizar una descripción bastante aproximada de este componente fundamental del artículo lexicográfico e identificar sus rasgos constitutivos desde el punto de vista de su composición, estructura y funciones. La principal conclusión a la que han llegado los autores alemanes es que la forma del ejemplo de uso (así como el resto de elementos del diccionario) debe adaptarse a las necesidades del alumno de alemán como primera o segunda lengua. Esto condicionará la selección del vocabulario, la sintaxis y el estilo de los ejemplos, así como la información de tipo cultural o pragmático transmitida por el ejemplo sobre los contextos comunicativos en los que el uso de una palabra o expresión es más habitual.

Hasta hace bien poco, la fraseología no ha ocupado el lugar que les correspondía en la lingüística dada la alta frecuencia de aparición de las unidades de que se ocupa y su importancia en la lengua cotidiana. A partir de los años 90 ha crecido el interés por los estudios entorno a las unidades fraseológicas (UFs) y se ha multiplicado el número de publicaciones dedicadas a su descripción, contraste y análisis. A pesar de ello quedan todavía ciertas parcelas de la fraseología de las que la lingüística apenas se ha ocupado. Uno de estos ámbitos corresponde al tratamiento de las UFs en los diccionarios monolingües y bilingües. Nuestro propósito es ofrecer una visión de la manera en que describe, analiza y presenta las UFs uno de los diccionarios más usados por los estudiantes hispanos de lengua alemana, el Pons Wörterbuch für Schule und Studium (2003) de E. Böhle/J. Ràfols/E. Schellert. Para ello tomaremos 24 expresiones idiomáticas, que analizaremos teniendo en cuenta los siguientes factores: (a) la selección y la extensión de las UFs recogidas en el diccionario, (b) su organización macrostructural, (c) su organización microstructural, (d) otras observaciones sobre el tratamiento de las UFs.
