Palliative care and medical-legal partnership are complementary disciplines dedicated to integrating care to treat the whole patient and intervening before a legal or medical issue is at a crisis point. In this paper, we discuss the founding and operations of the Yale Palliative Medical Legal Partnership, give examples of typical cases, explain special considerations in this area of law, and propose areas for further research. 2. Duration and extent of solar X-ray flares and shortwave fadeouts likely to impact high frequency radio wave propagation based on an evaluation of absorption at 30 MHz

Abstract

High frequency (HF; 3–30 MHz) radio wave propagation can be impacted by absorption that results from enhanced photoionization in the dayside D-region following a solar X-ray flare. A database of >25,000 solar X-ray flares was evaluated to characterize the relationship between flare duration and the peak of the 0.1–0.8 nm solar X-ray flux. Expressions describing the mean and 90th percentile duration were developed. Based on these models, mean durations of 13, 18, 27, and 39 min and 90th percentile durations of 30, 48, 77, and 123 min are expected for C1, M1, X1 and X10 solar X-ray flares, respectively. A probability distribution of flare durations was developed to describe the probability of flare duration lasting 0–15, 15–30, 30–45, 45–60, 60–90, >90 min. In addition to flare duration, the duration of the expected impact to HF radio waves was evaluated. By considering examples where HF radio wave propagation in the 5–15 MHz range was impacted by space weather, a 0.5 dB threshold at 30 MHz was observed in samples of riometer data. Absorption modelled at 1-min increments from 1986 to 2017 was evaluated to create a probability distribution of impact duration, defined as the length of time the modelled 30 MHz absorption exceeded 0.5 dB during a single event. Modelled absorption was further evaluated to demonstrate the geographic extent of enhanced absorption, and to determine the minimum solar X-ray flux required to exceed the 0.5 dB impact threshold at a given latitude as a function of solar zenith angle and time of year. The results of this paper provide a better understanding of the impact of solar X-ray flares on high frequency radio wave propagation and aid in the development of tools and services for mitigating space weather impacts to systems that rely on HF radio wave propagation.

The article discusses a series of strikes by Outagamie County, Wisconsin, dairy farmers in 1933. The article describes violence in surrounding counties involving striking dairy farmers, increases in the cost of milk production and decreases in the market price for milk leading up to the strikes, and the impact of the Great Depression upon the dairy industry. It also discusses the organization known as the Wisconsin Cooperative Milk Pool and its involvement in the strikes, blockades of milk deliveries, and a milk embargo by then-Wisconsin governor Albert G. Schmedeman.

18. PROFESSIONALISM AND THE INDEPENDENT PIANO TEACHER: A COLLECTIVE CASE STUDY

Abstract

The independent piano studio owner combines entrepreneurial and pedagogical skill sets when creating a business to house their chosen occupation. These skill sets help them create a reputation as pedagogues and business owners and affects their status as professionals. In this paper, the evolution of independent piano teaching in America will be traced in order to evaluate the impact of negative stereotypes on the independent piano teachers’ pursuit of professional standing. This collective case study will interview four piano studio owners. Using Structural-Functional and Process models from sociological literature, subjects will be evaluated on their degree of professionalism. Results will be separated by categories of topics related to pedagogical and business development. The discussion outlines the differences and commonalities between the four subjects in responses to interview questions. Results show that subjects in this study meet a majority of traits required of obtaining professional status. More importantly, subjects have freedoms to choose how to become professional and how to gain community recognition as being professionals. Conclusions find how these subjects created professionalism and offers suggestions for further research into the topic.

Abstract

Objective: Awareness of cancer family history is dependent upon communication between family members. Communication of this information and related decision-making could be important factors influencing breast cancer risk reduction and early detection behaviors. Using survey data from 2328 women (mean age 62.5 years) from 372 families enrolled in the Minnesota breast cancer family study, we explored adult daughter's reports of breast cancer risk reduction advice received from their mothers., Methods and Results: Approximately 212 (9%) of respondents reported receiving breast cancer risk reduction advice from their mothers and 130 (89%) reported acting upon such advice. Having a mother or first degree relative (FDR) with a history of breast cancer was significantly correlated with following advice to a higher degree as compared with those not having such family history (p=0.003). Most frequently reported types of advice were to have mammograms (36%) and to have clinical breast exams (35%). Using multivariable logistic regression and after accounting for non-independence of the sample, significant independent correlates of receiving advice included younger age, having an affected mother, and having a higher perceived breast cancer risk. Receiving advice was also correlated with engaging in a higher number of health promoting behaviors and ever having received a mammogram., Conclusions: Our preliminary findings are consistent with social influence theory and suggest that mother-daughter communication about reducing risk, especially among those having a FDR with breast cancer, could be a potential pathway through which breast cancer family history is associated with the adoption of breast cancer screening and risk reduction behaviors., (Copyright (c) 2008 John Wiley & Sons, Ltd.)
