Inter- and intraspecific perspectives are used to highlight the unique features of the human physiological adaptation to pregnancy and their significance for reproductive successes. Both these perspectives suggest that natural selection has in the past and continues to operate in the present on maternal physiological responses to pregnancy.

ABSTRACT Several studies demonstrate that human ovarian function is responsive to the energetic environment, which has led to the development of theoretical models that explain this phenomenon. Although many genes are involved in ovarian hormone production, the possibility that genetic polymorphism may affect ovarian response to energetic conditions has not been considered. Cytochrome P450c17α is an enzyme that produces androgen precursors used to make estrogens during ovarian steroidogenesis, and is encoded by the CYP17 gene. A functionally significant variant within the promoter region of CYP17 has been linked to variation in steroid production, and some evidence suggests that this polymorphism could alter transcription of CYP17 in an insulin-dependent manner. We tested the hypothesis that the CYP17 variant affected the relationship between anthropometric measurements and salivary estradiol in healthy women in the United States ( n = 28). PCR-RLFP analysis was used to genotype women for the genetic variant, and estradiol was assayed from saliva by EIA. Moderated regression analysis of these preliminary data revealed a significant interaction between waist-to-hip ratio and CYP17 genotype ( P = 0.004). Our study provides evidence that gene-environment interactions should be considered in future adaptive models for human ovarian function. Objectives: Global patterns of the incidence of cancer are often attributed to environmental and lifestyle differences between regions. Less attention has been given to global patterns of allelic variation of genes that may contribute to the risk of developing cancer. Methods: We genotyped samples from 21 populations for four variants of the progesterone receptor (PR) gene. One is an Alu insertion in intron 7 which defines the PROGINS haplotype. The others include a promoter region SNP 331+ G/A (rs10895068), a haplotype defining T/C substitution in intron 6 (rs561650), and an A/T substitution (rs608995) in the 3′ untranslated region of the gene. All variants have been investigated elsewhere in association with female reproductive cancers in western populations. Results: We found population differences in the frequency of each of these alleles across study populations ( P < 0.01, log-likelihood G statistic, computed in FSTAT) and therefore examined the correlation between the frequency of each genetic variant and the incidence of three female reproductive cancers (breast, uterine, and ovarian) obtained from the Globocan 2008 database. Breast and ovarian cancer incidence were significantly correlated with the frequency of the Alu insertion ( r = 0.86 and 0.53) and the +331 A variant ( r = 0.57 and 0.73). Conclusions: Our data expand the information on genetic variation at the PR locus in non-western populations and support an argument for more work on the genetic epidemiology of cancer among nonwestern populations. Instrumented head acceleration measurement devices are commonly used in research studies to determine head acceleration exposure in certain populations. Instrumented mouthguards pair directly to the user's teeth and offer six-degree-of-freedom measurements. Though many studies have recently used these devices, post-processing techniques vary by study. Other studies have attempted to label impact quality or coupling status, also with varying methods. This study sought to compare the effect of post-processing and labeling methods on reported exposure distribution characteristics in instrumented mouthguard data from ice hockey players. We collected data from 18 female adolescent ice hockey players on two teams for an entire season. We then post-processed the measured signals using five different techniques: (1) the instrumented mouthguard manufacturer's data output, (2) a 500 Hz linear acceleration filter and a 300 Hz angular velocity filter, (3) HEADSport, (4) a 100 Hz linear acceleration filter and a 175 Hz angular velocity filter, and (5) a salvaging process to detect and remove decoupling based on signal frequency content. The post-processing techniques affected the reported exposure distributions by changing the mean, median, and 95th percentile values of peak linear and angular kinematics. We also compared labeling techniques by measuring agreement and inter-rater reliability between three labeling techniques: the instrumented mouthguard manufacturer's label, Luke et al.'s coupling label, and our classification learner that detects and labels decoupling. We found that the labeling techniques had low agreement about which acceleration events were the best to keep. Labeling technique also influenced the reported distributions' descriptive statistics. Post-processing and event labeling are crucial components of head acceleration event exposure studies. Methods should be described by researchers, and standardization should be sought to allow for better cross-study comparison. Published and publicly available techniques can help move the field toward this ideal. In response to growing evidence that repetitive head impact exposure and concussions can lead to long-term health consequences, many research studies are attempting to quantify the frequency and severity of head impacts incurred in various sports and occupations. The most popular apparatus for doing so is the instrumented mouthguard (iMG). While these devices hold greater promise of head kinematic accuracy than their helmet-mounted predecessors, data artifacts related to iMG decoupling still plague results. We recreated iMG decoupling artifacts in a laboratory test series using an iMG fit to a dentition mounted in a NOCSAE headform. With these data, we identified time, frequency, and time-frequency features of decoupled head impacts that we used in a machine learning classification algorithm to predict decoupling in six-degree-of-freedom iMG signals. We compared our machine learning algorithm predictions on the laboratory series and 80 video-verified field head acceleration events to several other proprietary and published methods for predicting iMG decoupling. We also present a salvaging method to remove decoupling artifacts from signals and reduce peak resultant error when decoupling is detected. Future researchers should expand these methods using on-field data to further refine and enable prediction of iMG decoupling during live volunteer use. Combining the presented machine learning model and salvaging technique with other published methods, such as infrared proximity sensing, advanced triggering thresholds, and video review, may enable researchers to identify and salvage data with decoupling artifacts that previously would have had to be discarded., Competing Interests: Declarations. Many recent studies have used boil-and-bite style instrumented mouthguards to measure head kinematics during impact in sports. Instrumented mouthguards promise greater accuracy than their predecessors because of their superior ability to couple directly to the skull. These mouthguards have been validated in the lab and on the field, but little is known about the effects of decoupling during impact. Decoupling can occur for various reasons, such as poor initial fit, wear-and-tear, or excessive impact forces. To understand how decoupling influences measured kinematic error, we fit a boil-and-bite instrumented mouthguard to a 3D-printed dentition mounted to a National Operating Committee on Standards for Athletic Equipment (NOCSAE) headform. We also instrumented the headform with linear accelerometers and angular rate sensors at its center of gravity (CG). We performed a series of pendulum impact tests, varying impactor face and impact direction. We measured linear acceleration and angular velocity, and we calculated angular acceleration from the mouthguard and the headform CG. We created decoupling conditions by varying the gap between the lower jaw and the bottom face of the mouthguard. We tested three gap conditions: 0 mm (control), 1.6 mm, and 4.8 mm. Mouthguard measurements were transformed to the CG and compared to the reference measurements. We found that gap condition, impact duration, and impact direction significantly influenced mouthguard measurement error. Error was higher for larger gaps and in frontal (front and front boss) conditions. Higher errors were also found in padded conditions, but the mouthguards did not collect all rigid impacts due to inherent limitations. We present characteristic decoupling time history curves for each kinematic measurement. Exemplary frequency spectra indicating characteristic decoupling frequencies are also described. Researchers using boil-and-bite instrumented mouthguards should be aware of their limitations when interpreting results and should seek to address decoupling through advanced post-processing techniques when possible., (© 2024. In the United States, heart failure (HF) is the leading reason for hospital readmissions, with 27% of Medicare recipients with HF being readmitted within 30 days The purpose of this quality improvement project was to decrease HF readmissions during their first 30 days of care with our home health agency. The Supportive Heart Failure Care education program was based on the results of a population assessment and included establishment of agency-wide best practices, nursing education sessions, and implementation of best practices focused on evidence-based self-management. After implementation of this project, the hospital readmission rate decreased from 32% to 21%. The nurses who completed the education sessions (N = 35) showed an increase in knowledge of 4.1% from pre-test scores. However, chart audits showed varying levels of documented practice in alignment with best practices. Although results of this project showed a small improvement in nurse knowledge and varied changes to documented practice, the overall project goal of decreasing hospital readmissions was achieved. Understanding individual and systems-level barriers to translating education to practice is needed to better meet the needs of home health nurses and the HF patients they serve., (Copyright © 2023 Wolters Kluwer Health, Inc. Purification of peptides typically includes expensive reverse phase (RP) processes, which utilize high pressure and large volumes of solvent. For two conjugated peptides, chromatography process development targeted a low-pressure aqueous process that could achieve target product purities of ≥95%, comparable to purities seen with traditional RP. A high throughput screening approach of different modalities was used to identify binding and elution conditions on a cation exchange resin and small-scale columns were used in order to assess impurity removal and process yield. The parameters for load and gradient elution were optimized to increase product purity and process productivity with a wide operating window identified where high purity and productivity are achieved. Computational modeling was then used to validate experimental chromatography results and to gain insight on the effect of the chemical modifications on the surface properties of the two peptides. Both modeling and experimental data showed that with optimization, cation exchange could be utilized as a single polishing step for conjugated peptides. Purpose: To analyze serum markers of bone turnover, angiogenesis, endocrine function, and inflammation in patients with bisphosphonate-related osteonecrosis of the jaw (BRONJ) who discontinued long-term intravenous bisphosphonate (BP) therapy., Patients and Methods: Serum samples were obtained from 25 BRONJ patients who had discontinued long-term intravenous BP therapy for an average of 11.4 ± 8.7 months and 48 non-BRONJ controls who continued receiving intravenous BP therapy. Samples were analyzed for total alkaline phosphatase, bone-specific alkaline phosphatase, osteocalcin, C-telopeptide, vascular endothelial growth factor, triiodothyronine, thyroxine, thyroid-stimulating hormone, 25-hydroxyvitamin D, and C-reactive protein., Results: The mean number of BP infusions was significantly higher in BRONJ patients compared with controls (38.4 ± 26.3 infusions vs 18.8 ± 7.2 infusions, P < .0001); however, the duration of BP therapy was not significantly different between the groups (P = .23). Overall, there were no significant differences in any of the markers between BRONJ patients and controls (all P values ≥ .16). In a subgroup analysis that matched BRONJ patients and controls according to mean age and number of BP infusions (10 BRONJ patients and 48 controls), log10 vascular endothelial growth factor (2.9 ± 0.4 pg/mL vs 2.4 ± 0.4 pg/mL, P < .001) and C-reactive protein (34 ± 26 mg/L vs 13 ± 8 mg/L, P < .01) levels were significantly higher in BRONJ patients compared with controls. Within BRONJ patients, none of the serum markers were correlated with duration of BP discontinuation., Conclusions: Levels of bone turnover and endocrine markers in BRONJ patients who discontinue long-term intravenous BP therapy are similar to those in non-BRONJ controls receiving intravenous BP therapy. However, levels of angiogenesis and inflammation markers are higher in BRONJ patients who discontinue long-term intravenous BP therapy. The prolonged skeletal half-life of BPs may suppress bone turnover markers in BRONJ patients for several years after discontinuation of intravenous BP therapy, suggesting an extended effect on bone homeostasis., (Copyright © 2016 American Association of Oral and Maxillofacial Surgeons. Background: Previous case reports and animal studies suggest that periodontitis is associated with bisphosphonate-related osteonecrosis of the jaw (BRONJ). This case-control study is conducted to evaluate the association between clinical and radiographic measures of periodontal disease and BRONJ., Methods: Twenty-five patients with BRONJ were matched with 48 controls. Trained examiners measured probing depth, clinical attachment level (CAL), and bleeding on probing on all teeth except third molars and gingival and plaque indices on six index teeth. Alveolar bone height was measured from orthopantomograms. Most patients with BRONJ were using antibiotics (48%) or a chlorhexidine mouthrinse (84%) at enrollment. Adjusted comparisons of patients with BRONJ versus controls used multiple linear regression., Results: The average number of bisphosphonate (BP) infusions was significantly higher in patients with BRONJ compared with controls (38.4 versus 18.8, P = 0.0001). In unadjusted analyses, patients with BRONJ had more missing teeth (7.8 versus 3.1, P = 0.002) and higher average CAL (2.18 versus 1.56 mm, P = 0.047) and percentage of sites with CAL ≥3 mm (39.0 versus 23.3, P = 0.039) than controls. Also, patients with BRONJ had lower average bone height (as a fraction of tooth length, 0.59 versus 0.62, P = 0.004) and more teeth with bone height less than half of tooth length (20% versus 6%, P = 0.001). These differences remained significant after adjusting for age, sex, smoking, and number of BP infusions., Conclusions: BRONJ patients have fewer teeth, greater CAL, and less alveolar bone support compared with controls after adjusting for number of BP infusions. Group differences in antibiotics and chlorhexidine rinse usage may have masked differences in the other clinical measures.

Enlargement of the uterine artery (UA) during pregnancy is diminished in women residing at a high altitude. We asked whether chronic hypoxia alters the rise in DNA synthesis in uteroplacental vessels and, if so, whether the reduction is related to the intrauterine growth retardation (IUGR) observed under conditions of chronic hypoxia. We used bromodeoxyuridine (BrdU) labelling to measure DNA synthesis in all vascular layers of the UA, mesometrial arteries (MA), thoracic aorta and mesenteric artery of guinea pigs, residing throughout pregnancy at a low (1600 m) or high (3962 m) altitude. Pregnancy increased DNA synthesis throughout the UA at both altitudes, yet the maximal value was less at high than low altitude (P<0.05). Likewise, pregnancy increased DNA synthesis throughout the MA, yet at high altitude pregnancy elevated levels returned to non-pregnant values after 42 days of gestation, whereas at low altitude DNA synthesis continued to be elevated until near term. Fetal weights were lower (P=0.01) and placental/fetal weight ratios tended to be greater in high than low altitude, near term pups (P = 0.09). We conclude that a diminished growt response by the uteroplacental vasculature to pregnancy may contribute to the previously reported reduced uterine artery blood flow and resulting IUGR at high altitude., (Copyright 2000 Harcourt Publishers Ltd.)

Objective: Mercury vapor release from amalgams during setting significantly decreases when the amalgams are prepared with binary Hg-In liquid alloys. The objective of this study was to compare the cytotoxicity of amalgams made with experimental Hg-In alloys with that of amalgam without In and a commercial In-containing amalgam., Methods: Amalgam specimens were prepared by triturating a high-Cu alloy powder (Tytin, Kerr) with pure Hg or Hg-In liquid alloy containing 5, 20 or 50% In and also by triturating an In-containing high-copper alloy powder (Indiloy, Shofu) with pure Hg. After the specimens were aged for 2 wk, a cylindrical specimen of each amalgam was immersed consecutively in cell culture medium for 0-8, 8-48 and 48-72 h. The cytotoxicity of the extracts was determined by placing them in contact with Balb/c 3T3 mouse fibroblasts for 24 h, after which the succinic dehydrogenase (SDH) activity was measured and expressed as a percentage of the Teflon negative controls. The results were statistically compared using ANOVA and Tukey's test (alpha = 0.05). The concentration of elements released into the extracts was determined by atomic absorption spectrophotometry and evaluated by Kruskal-Wallis and nonparametric multiple comparisons., Results: For the 0-8 h and 8-48 h intervals, the 20% In amalgam was significantly (p < 0.05) less toxic than the other amalgams, and not different from the Teflon control. Results for the other amalgams were only slightly depressed compared to the Teflon control. For the 48-72 h interval, all amalgams were essentially no different from the control. Copper was the element dominantly released into the medium from all the amalgams tested., Significance: For amalgam tested after aging, alloying indium to mercury did not deleteriously affect the cytotoxicity of the resultant amalgam compared to the amalgam without indium.

Eighteen commercial Angus cross-bred feedlot steers of similar hip height and live weight were randomly assigned to one of three dietary treatment groups: corn-, corn/barley, or barleybased diets (n = 6 per treatment). Steers were fed for 102-103 days on their respective diets prior to slaughter. Live animal performance traits, carcass characteristics, total lipid and descriptive flavor and descriptive palatability attributes of beef strip loin steaks were determined. End live weight (P = 0.88) did not differ between dietary treatments. Beef carcasses from steers fed corn-, barley-, and corn/barley-based diets did not differ in hot carcass weight (P = 0.18), ribeye area (P = 0.21), kidney, pelvic and heart fat (KPH) (P = 0.35), and yield grade (P = 0.14). However, adjusted preliminary yield grade was higher (P = 0.03) for carcasses from steers fed corn/barley-based diets than carcasses from steers fed barley as the dietary energy source. These data suggest that carcasses from steers fed barley-based diets were lower in external fat. Quality grade characteristics were not different in beef carcasses from steers fed either corn-, barley-, or a corn/barley-based diet. Cook time (P = 0.37), cooking loss (P = 0.83), descriptive meat palatability attributes (P > 0.27), Warner-Bratzler shear force (P = 0.25), and descriptive sensory flavor attributes (P ≥ 0.17) did not differ for steaks from steers fed the three diets prior to slaughter. The Japanese have claimed that feeding cattle barley-based high energy diets result in beef with different flavor than when cattle are fed high-energy corn-based diets. These results indicated that the eating quality, tenderness and flavor attributes of beef steaks were not influenced by the dietary grain source fed to young steers in this study prior to slaughter.

This prospective study describes the epidemiology of adult sexually transmitted disease agents in 1538 children ages 1 to 12 years being evaluated for possible sexual abuse. Infections with these agents were related to the presence or absence of a history of sexual contact. Neisseria gonorrhoeae (GC) was found in 2.8% (41 of 1469); human papillomavirus presenting as condyloma acuminata, 1.8%; Chlamydia trachomatis, 1.2% (17 of 1473); Treponema pallidum (syphilis), 0.1% (1 of 1263); and herpes simplex virus, 0.1%. Overall a history of sexual contact was present in 83% of children with N. gonorrhoeae; condyloma acuminata, 43%; Chlamydia trachomatis, 94%; syphilis, 0%; and herpes simplex virus, 50%. Selected vaginal discharges were examined for Trichomonas vaginalis and bacterial vaginosis. In children comprehending questions regarding sexual contact (i.e. were "verbal"), 89% with N. gonorrhoeae, 100% with Chlamydia trachomatis and 63% with condyloma acuminata had a history of sexual contact, indicating that in "verbal" children any infection with N. gonorrhoeae or C. trachomatis was highly associated with sexual contact.

Entry of certain free amino acids (alpha aminoisobutyric acid (AIB), alanine and proline), but not of leucine into rat thymic lymphocytes increased progressively when the cells were incubated in amino acid deficient medium. Actinomycin D, cycloheximide, or a high concentration of AIB abolished the time-related increase in AIB accumulation, whereas exposure to a high concentration of leucine had no effect. This phenomenon could not be attributed to a progressive alteration in the nature of the incubation medium nor to reduced transinhibition of AIB uptake. The exodus of AIB also increased with time, but to a smaller degree than AIB entry. Initial rates of AIB entry and exodus increased with increases in the pH of the incubation medium over the range 6.5-8.0. The effects of pH on entry and exodus were time-related, increasing progressively oveb nullified the magnified time related increments in AIB transport caused by prolonged incubation at pH 8.0. The influence of a given pH on transport of AIB decreased rapidly when the cells were transferred to medium of another pH, but this tendency diminished the longer the cells were exposed to the initial pH. pH influenced the entry of alanine and proline in the same fashion as that of AIB, but did not affect leucine entry. These results indicate that thymic lymphocytes exhibit adaptive enhancement in the accumulation of free amino acids that are transported largley by the A or alanine-preferring system, and that the adaptive process involves both entry and exodus. Moreover, alterations in pH modify entry and exodus of these same amino acids, profoundly affect the magnitude of time-released increases, and may induce fundamental changes in the mechanism(s) serving amino acid transport.
