Abstract

Federated learning (FL), as an emerging artificial intelligence (AI) approach, enables decentralized model training across multiple devices without exposing their local training data. FL has been increasingly gaining popularity in both academia and industry. While research works have been proposed to improve the fault tolerance of FL, the real impact of unreliable devices (e.g., dropping out, misconfiguration, poor data quality) in real-world applications is not fully investigated. We carefully chose two representative, real-world classification problems with a limited numbers of clients to better analyze FL fault tolerance. Contrary to the intuition, simple FL algorithms can perform surprisingly well in the presence of unreliable clients.

4. Multi-modality imaging assessment of cerebral small vessel disease biomarkers after stroke due to spontaneous intracerebral haemorrhage

Abstract

In most cases, spontaneous intracerebral haemorrhage (ICH) is thought to result from cerebral small vessel diseases (SVDs). Cerebral amyloid angiopathy (CAA) and arteriolosclerosis (non-CAA SVD) are the two main types of SVD associated with ICH. The risk of recurrent ICH and post-stroke dementia may be higher with CAA-associated ICH compared with non-CAA SVD-associated ICH. It is, therefore, important to identify the type of SVD associated with ICH. Commonly, ICH in the cerebral lobes (lobar ICH) is thought to be associated with CAA, whereas non-lobar ICH is associated with non-CAA SVD. However, this generalisation is not accurate. The aims of my thesis were to (a) histopathologically assess the types and severity of SVDs associated with lobar and non-lobar ICH using brain autopsy tissue, (b) explore whether brain imaging (magnetic resonance imaging (MRI), positron emission tomography (PET) and computed tomography (CT)) biomarkers can be used in living patients to identify the type of SVDs associated with ICH and (c) determine whether CT biomarkers can predict outcome after ICH. Throughout my thesis, I use data from two overlapping studies of ICH. The Lothian Audit of the Treatment of Cerebral Haemorrhage (LATCH) is a prospective community-based audit of all residents in the Lothian Health board region of Scotland who were aged 16 years or above and had an incident ICH between 1st June 2010 and 31st May 2013 inclusive. During LATCH and until 31st May 2016, consecutive adults with SVD-associated ICH were able to consent to have apolipoprotein E genotyping, brain MRI and brain autopsy as part of the prospective Lothian INtraCerebral Haemorrhage Pathology, Imaging and Neurological outcome (LINCHPIN) study. In an autopsy study of 126 LINCHPIN participants with first-ever SVDassociated ICH, I found that 98% of participants with a non-lobar ICH had moderate or severe non-CAA SVD. In contrast, 60% of participants with a lobar ICH had moderate or severe CAA, while many had moderate or severe non-CAA SVD, either with or without CAA. The most common approach used in clinical practice to identify CAAassociated ICH is the MRI-based modified Boston criteria for CAA. However, I found that only about one-third of ICH patients during LATCH were able to undergo MRI. Also, in 16 LINCHPIN participants with research MRI, the modified Boston criteria showed limited accuracy against the histopathological assessment of CAA on subsequent research autopsy. Molecular imaging using tracers designed to detect parenchymal β-amyloid in Alzheimer’s disease, such as flutemetamol, may also identify the perivascular β-amyloid found in CAA. I found that in LINCHPIN brain tissue samples, the amyloid tracer flutemetamol labelled both parenchymal and perivascular β amyloid. Among 20 participants with first-ever ICH, flutemetamol PET scans were 86% sensitive and 77% specific for CAA-associated ICH based on the modified Boston criteria. Brain CT is the test that usually diagnoses ICH. I found in 62 LINCHPIN participants with first-ever lobar ICH that subarachnoid haemorrhage and finger-like projections on CT, and APOE ε4 allele possession were independently associated with CAA-associated lobar ICH defined on subsequent autopsy. I developed diagnostic models and criteria (“Edinburgh criteria”) based on these predictors that could accurately rule in or exclude CAA-associated lobar ICH. I performed a multicentre external validation study of the Edinburgh criteria. In a preliminary analysis, the CT-only diagnostic criteria (subarachnoid haemorrhage and finger-like projections) had high sensitivity (88%) and good specificity (84%) for CAA-associated lobar ICH. The risk of recurrent ICH is a key outcome for ICH survivors. I found that the risk of recurrent ICH was significantly higher in participants with first-ever lobar compared with non-lobar ICH. Among lobar ICH participants, the risk of recurrent ICH was significantly higher in those classified as high risk on the CT-only Edinburgh criteria for CAA-associated lobar ICH compared with those classified as low risk. Predicting death or disability is also important in ICH. I found that the severity of SVD on the diagnostic brain CT was an independent predictor of death or disability at one year after the ICH, after adjusting for known prognostic factors. Brain imaging biomarkers, particularly on brain CT, are potentially useful for identifying the type of SVD associated with ICH. Having developed the CTbased Edinburgh criteria for CAA-associated lobar ICH, I performed preliminary multicentre studies assessing their diagnostic accuracy and prognostic value. In the future, I aim to perform the full analyses of these diagnostic test accuracy and prognostic studies to determine the clinical utility of the Edinburgh criteria. Other research should assess the clinical and economic effect of the Edinburgh criteria on the prognosis of ICH. More work is needed to investigate the prognostic relevance of coexistence of CAA and non-CAA SVDs in lobar ICH, and whether these groups can be differentiated using imaging biomarkers.

Abstract

[eng] Introduction and objective: Obesity is a metabolic disorder characterized by excessive accumulation of adipose tissue in the body and is associated with the development of abnormalities in blood glucose metabolism, either pre-diabetes or type 2 diabetes. There are different scales that assess the risk of diabetes, the most widely used being the Findrisc scale. The aim of this study is to determine the relationship of various scales of overweight and obesity with the values of the Findrisc scale. Methods: A descriptive, cross-sectional study of 48,366 Spanish workers in which the risk of type 2 diabetes was determined using the Findrisc scale. Different scales of overweight and obesity such as BMI, waist/height, waist/hip, body adiposity index and abdominal volume index were also assessed. Results: The prevalence of high values of all the scales analyzed in this study are much higher in those people who are at high risk of type 2 diabetes. This prevalence is higher in men. Conclusion: There is a clear relationship between the values of the Findrisc scale and the values of all the overweight and obesity scales analyzed. El objetivo de este estudio es conocer la relación de varias escalas de sobrepeso y obesidad con los valores de la escala de Findrisc. Material y métodos: Estudio descriptivo y transversal en 48.366 trabajadores españoles en los que se determina el riesgo de padecer diabetes tipo 2 mediante la escala Findrisc. También se valoran diferentes escalas de sobrepeso y obesidad como IMC, cintura/altura, cintura/cadera, índice de adiposidad corporal e índice de volumen abdominal. Resultados: La prevalencia de valores elevados de todas las escalas analizadas en este estudio son mucho mayores en aquellas personas que presentan un alto riesgo de padecer diabetes tipo 2. Esta prevalencia es mayor en los hombres. Conclusión: Existe una clara relación entre los valores de la escala de Findrisc y los valores de todas las escalas de sobrepeso y obesidad analizadas.

36. Cerebral microbleeds: from depiction to interpretation

Abstract

Cerebral microbleeds (CMBs) are defined as hypointense foci visible on T2*-weighted and susceptible-weighted MRI sequences. CMBs are increasingly recognised with the widespread use of MRI in healthy individuals as well as in the context of cerebrovascular disease or dementia. They can also be encountered in major critical medical conditions such as in patients requiring extracorporeal mechanical oxygenation. The advent of MRI-guided postmortem neuropathological examinations confirmed that, in the context of cerebrovascular disease, the vast majority of CMBs correspond to recent or old microhaemorrhages. Detection of CMBs is highly influenced by MRI parameters, in particular field strength, postprocessing methods used to enhance T2* contrast and three dimensional sequences. Despite recent progress, harmonising imaging parameters across research studies remains necessary to improve cross-study comparisons. CMBs are helpful markers to identify the nature and the severity of the underlying chronic small vessel disease. In daily clinical practice, presence and numbers of CMBs often trigger uncertainty for clinicians especially when antithrombotic treatments and acute reperfusion therapies are discussed. In the present review, we discuss those clinical dilemmas and address the value of CMBs as diagnostic and prognostic markers for future vascular events.

These arrangements may lead to sub-optimal educational outcomes to the extent that prospective students reject a VET education on the basis of short-term financial constraints. In this paper we analyse some of the important issues related to the adoption of FEE-HELP (a 2005 federal government financial instrument based on the Higher Education Contribution Scheme (HECS)). It is argued that income contingent loans of this kind are associated with the advantages of both default-protection and consumption smoothing. Using data from the first three waves of the Household Income and Labour Dynamics in Australia (HILDA) survey, we examine various empirical issues associated with the adoption of FEE-HELP in VET, including the extent of private salary returns to VET qualifications. As well, we explore issues related to the public subsidies inherent in the adoption of FEE-HELP in VET, and illustrate the time periods involved in loan repayments for various assumptions concerning the size of the charge and the future income of VET graduates. Administrative issues are considered, as are the implications for the Commonwealth government with respect to potential subsidies associated with the design parameters. In the 2007-08 Federal Budget, the former government announced a small extension of the FEE-HELP system into Australian VET, a reform consistent with improved tertiary funding arrangements. Abstract

Background: Hospital-based studies have reported variable associations between outcome after spontaneous intracerebral hemorrhage and peri-hematomal edema volume. Aims: In a community-based study, we aimed to investigate the existence, strength, direction, and independence of associations between intracerebral hemorrhage and peri-hematomal edema volumes on diagnostic brain CT and one-year functional outcome and long-term survival. Methods: We identified all adults, resident in Lothian, diagnosed with first-ever, symptomatic spontaneous intracerebral hemorrhage between June 2010 and May 2013 in a community-based, prospective inception cohort study. We defined regions of interest manually and used a semi-automated approach to measure intracerebral hemorrhage volume, peri-hematomal edema volume, and the sum of these measurements (total lesion volume) on first diagnostic brain CT performed at ≤3 days after symptom onset. The primary outcome was death or dependence (scores 3–6 on the modified Rankin Scale) at one-year after intracerebral hemorrhage. Results: Two hundred ninety-two (85%) of 342 patients (median age 77.5 y, IQR 68–83, 186 (54%) female, median time from onset to CT 6.5 h (IQR 2.9–21.7)) were dead or dependent one year after intracerebral hemorrhage. Peri-hematomal edema and intracerebral hemorrhage volumes were colinear (R 2 = 0.77). In models using both intracerebral hemorrhage and peri-hematomal edema, 10 mL increments in intracerebral hemorrhage (adjusted odds ratio (aOR) 1.72 (95% CI 1.08–2.87); p = 0.029) but not peri-hematomal edema volume (aOR 0.92 (0.63–1.45); p = 0.69) were independently associated with one-year death or dependence. 10 mL increments in total lesion volume were independently associated with one-year death or dependence (aOR 1.24 (1.11–1.42); p = 0.0004). Conclusion: Total volume of intracerebral hemorrhage and peri-hematomal edema, and intracerebral hemorrhage volume alone on diagnostic brain CT, undertaken at three days or sooner, are independently associated with death or dependence one-year after intracerebral hemorrhage, but peri-hematomal edema volume is not. Abstract

BACKGROUND: Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage. We did subgroup analyses of the RESTART trial to explore whether these brain imaging features modify the effects of antiplatelet therapy. METHODS: RESTART was a prospective, randomised, open-label, blinded-endpoint, parallel-group trial at 122 hospitals in the UK that assessed whether starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. For this prespecified subgroup analysis, consultant neuroradiologists masked to treatment allocation reviewed brain CT or MRI scans performed before randomisation to confirm participant eligibility and rate features of the intracerebral haemorrhage and surrounding brain. We followed participants for primary (recurrent symptomatic intracerebral haemorrhage) and secondary (ischaemic stroke) outcomes for up to 5 years (reported elsewhere). For this report, we analysed eligible participants with intracerebral haemorrhage according to their treatment allocation in primary subgroup analyses of cerebral microbleeds on MRI and in exploratory subgroup analyses of other features on CT or MRI. The trial is registered with the ISRCTN registry, number ISRCTN71907627. FINDINGS: Between May 22, 2013, and May 31, 2018, 537 participants were enrolled, of whom 525 (98%) had intracerebral haemorrhage: 507 (97%) were diagnosed on CT (252 assigned to start antiplatelet therapy and 255 assigned to avoid antiplatelet therapy, of whom one withdrew and was not analysed) and 254 (48%) underwent the required brain MRI protocol (122 in the start antiplatelet therapy group and 132 in the avoid antiplatelet therapy group). There were no clinically or statistically significant hazards of antiplatelet therapy on recurrent intracerebral haemorrhage in primary subgroup analyses of cerebral microbleed presence (2 or more) versus absence (0 or 1) (adjusted hazard ratio [HR] 0·30 [95% CI 0·08-1·13] vs 0·77 [0·13-4·61]; pinteraction=0·41), cerebral microbleed number 0-1 versus 2-4 versus 5 or more (HR 0·77 [0·13-4·62] vs 0·32 [0·03-3·66] vs 0·33 [0·07-1·60]; pinteraction=0·75), or cerebral microbleed strictly lobar versus other location (HR 0·52 [0·004-6·79] vs 0·37 [0·09-1·28]; pinteraction=0·85). There was no evidence of heterogeneity in the effects of antiplatelet therapy in any exploratory subgroup analyses (all pinteraction>0·05). INTERPRETATION: Our findings exclude all but a very modest harmful effect of antiplatelet therapy on recurrent intracerebral haemorrhage in the presence of cerebral microbleeds. Further randomised trials are needed to replicate these findings and investigate them with greater precision. FUNDING: British Heart Foundation.
