Abstract

The authors previously identified plasma plasminogen activator inhibitor-1 (PAI-1) level as a quantitative lung injury biomarker in primary graft dysfunction (PGD). They hypothesized that plasma levels of PAI-1 used as a quantitative trait could facilitate discovery of genetic loci important in PGD pathogenesis. A two-stage cohort study was performed. In stage 1, they tested associations of loci with PAI-1 plasma level using linear modeling. Genotyping was performed using the Illumina CVD Bead Chip v2. Loci meeting a p < 5 × 10(-4) cutoff were carried forward and tested in stage 2 for association with PGD. Two hundred ninety-seven enrollees were evaluated in stage 1. Six loci, associated with PAI-1, were carried forward to stage 2 and evaluated in 728 patients. rs3168046 (Toll interacting protein [TOLLIP]) was significantly associated with PGD (p = 0.006). The increased risk of PGD for carrying at least one copy of this variant was 11.7% (95% confidence interval 4.9-18.5%). The false-positive rate for individuals with this genotype who did not have PGD was 6.1%. Variants in the TOLLIP gene are associated with higher circulating PAI-1 plasma levels and validate for association with clinical PGD. A protein quantitative trait analysis for PGD risk prioritizes genetic variations in TOLLIP and supports a role for Toll-like receptors in PGD pathogenesis.

Abstract

Primary graft dysfunction (PGD) is a major cause of early mortality after lung transplant. We aimed to define objective estimates of PGD risk based on readily available clinical variables, using a prospective study of 11 centers in the Lung Transplant Outcomes Group (LTOG). Derivation included 1255 subjects from 2002 to 2010; with separate validation in 382 subjects accrued from 2011 to 2012. We used logistic regression to identify predictors of grade 3 PGD at 48/72 h, and decision curve methods to assess impact on clinical decisions. 211/1255 subjects in the derivation and 56/382 subjects in the validation developed PGD. Abstract

The UK government is considering legislation to prohibit the importation of hunting trophies. We examine documented social, ecological, and political outcomes of two previous such bans. We find that the UK government's proposal shares the shortcomings of existing bans that have (1) failed to address, or have even amplified, key threats to hunted species, (2) imposed costs on citizens of other countries, and (3) delegitimized the Convention on International Trade in Endangered Species (CITES). Trophy import bans are blunt policy instruments that can cause more problems than they solve.

Abstract

Community-based conservation can support livelihoods and biodiversity, while reinforcing local and Indigenous values, cultures, and institutions. Its delivery can help address cross-cutting global challenges, such as climate change, conservation, poverty, and food security. Therefore, understanding trends in community-based conservation is pertinent to setting and implementing global goals. We undertook a horizon scan to prioritize 15 emerging threats and opportunities expected to impact the future effectiveness of community-based conservation. Topics relate to global biodiversity policy; human rights; shifting human geography; inclusion, diversity, equity, and access; conservation finance and income; and economic reforms. Our findings offer guidance on strengthening community-based conservation to achieve global environmental and development goals.

Abstract

Background: Multiple major health organisations recommend the use of extracorporeal membrane oxygenation (ECMO) support for COVID-19-related acute hypoxaemic respiratory failure. However, initial reports of ECMO use in patients with COVID-19 described very high mortality and there have been no large, international cohort studies of ECMO for COVID-19 reported to date. Methods: We used data from the Extracorporeal Life Support Organization (ELSO) Registry to characterise the epidemiology, hospital course, and outcomes of patients aged 16 years or older with confirmed COVID-19 who had ECMO support initiated between Jan 16 and May 1, 2020, at 213 hospitals in 36 countries. The primary outcome was in-hospital death in a time-to-event analysis assessed at 90 days after ECMO initiation. We applied a multivariable Cox model to examine whether patient and hospital factors were associated with in-hospital mortality. Findings: Data for 1035 patients with COVID-19 who received ECMO support were included in this study. Of these, 67 (6%) remained hospitalised, 311 (30%) were discharged home or to an acute rehabilitation centre, 101 (10%) were discharged to a long-term acute care centre or unspecified location, 176 (17%) were discharged to another hospital, and 380 (37%) died. The estimated cumulative incidence of in-hospital mortality 90 days after the initiation of ECMO was 37·4% (95% CI 34·4–40·4). Mortality was 39% (380 of 968) in patients with a final disposition of death or hospital discharge. The use of ECMO for circulatory support was independently associated with higher in-hospital mortality (hazard ratio 1·89, 95% CI 1·20–2·97). In the subset of patients with COVID-19 receiving respiratory (venovenous) ECMO and characterised as having acute respiratory distress syndrome, the estimated cumulative incidence of in-hospital mortality 90 days after the initiation of ECMO was 38·0% (95% CI 34·6–41·5). 26. Anticipated impacts of achieving SDG targets on forests - a review

Abstract

Sustainable development requires knowledge of trade-offs and synergies between environmental and non-environmental goals and targets. Understanding the ways in which positive progress in matters of development not directly concerned with the environment can affect the natural environment, whether for better or for worse, can allow policymakers and development agencies to avoid the negative impacts of their actions, while capitalising on mutually beneficial opportunities. Through a systematic review of the literature, we consider the impacts of UN Sustainable Development Goal (SDG) targets on forest ecosystems, and identify 63 targets associated with potentially beneficial, damaging or mixed (i.e. damaging and/or beneficial depending on context or location) impacts. Types of impact are not uniform within SDGs, nor necessarily within individual targets. Targets relating to energy and infrastructure are among the most damaging and best studied, while targets expected to potentially result in beneficial outcomes, typically associated with social progress and well-being, have been investigated to a much lesser degree, especially in the context of external interventions. Thirty-eight targets have some variation in the direction of their impacts (i.e. at least one record with mixed impacts, or two or more records with different directions), suggesting the potential to achieve beneficial over damaging impacts in many cases. We provide illustrative examples of a range of impacts and use our findings to provide recommendations for researchers, development agencies and policymakers.

Abstract

The identification and analysis of reaction paths leading to palladium catalyst deactivation during cyanation of haloarenes are described. The studies have shown that in this case C-N activation has occurred via an [S.sub.N]2-type displacement of amine and has ruled out alternative 3-center C-N oxidative addition or Hofmann elimination processes.

Abstract

Purpose: The validity, responsiveness, and clinical utility of the Canadian Occupational Performance Measure was investigated when used in a sub-acute setting.Materials and Methods: Fifty older adults (mean age 78.2 years, Standard Deviation=7.2; 64% female) completed the Canadian Occupational Performance Measure, Functional Independence Measure and the Short-Form 36 Health Questionnaire. The Canadian Occupational Performance Measure was correlated with the Functional Independence Measure and Short-Form 36 Health Questionnaire and the admission and discharge Canadian Occupational Performance Measure Performance and Satisfaction scores were compared for significant differences. Qualitative interviews with were completed with six older adults and six occupational therapists to explore the utility of the Canadian Occupational Performance Measure. Result(s): The Canadian Occupational Performance Measure Satisfaction scale was significantly associated with the Functional Independence Measure and Short-Form 36 Health Questionnaire. Significant differences were found between the admission and discharge Canadian Occupational Performance Measure Performance and Satisfaction scale scores. From both the clients' and therapists' perspectives, four key qualitative themes emerged: (1) "Likes" about the Canadian Occupational Performance Measure; (2) Effects on practice; (3) Utility; and (4) Future use. Conclusion(s): This study provides evidence of the Canadian Occupational Performance Measure's validity, responsiveness and clinical utility when used in a sub-acute setting.Implications for rehabilitationThe Canadian Occupational Performance Measure appears able to effectively evaluate the outcomes of occupational therapy interventions, including in sub-acute rehabilitation settings. Abstract

The fluoride congener of Wilkinson's catalyst, [(Ph3P)3RhF] is synthesized and fully characterized. It is found that in sharp contrast with Wilkinson's catalyst, [(Ph3P)3RhF] easily cleaves the inert C-Cl bond of nonactivated chloroarenes under mild conditions to produce trans-[(Ph3P)2Rh(Ph2PF)(Cl)] as a result of C-Cl, Rh-F, and P-C bond cleavage and C-C, Rh-Cl, and P-F bond formation.

Abstract

The kinetics of the thermal decomposition of ammonium perfluorooctanoate (APFO) has been studied by high-temperature gas-phase nuclear magnetic resonance spectroscopy over the temperature range 196-234 [degrees]C. We find that APFO cleanly decomposes by first-order kinetics to give the hydrofluorocarbon 1-H-perfluoroheptane and is completely decomposed (>99%) in a matter of minutes at the upper limit of this temperature range. Based on the temperature dependence of the measured rate constants, we find that the enthalpy and entropy of activation are [DELTA]H = 150 [+ or -] 11 kJ [mol.sup.-1] and [DELTA]S = 3 [+ or -] 23 J [mol.sup.-1] [deg.sup.-1]. These activation parameters may be used to calculate the rate of APFO decomposition at the elevated temperatures (350-400 [degrees]C) at which fluoropolymers are processed; for example, at 350 [degrees]C the half-life for APFO is estimated to be less than 0.2 s. Our studies provide the fundamental parameters involved in the decomposition of the ammonium salt of perfluorooctanoic acid and indicate the utility of gas-phase NMR for thermolysis studies of a variety of materials that release compounds that are volatile at the temperature of decomposition and that contain an NMR-active nucleus.

Abstract

The unexpected thermal racemization of the cis-olefin of (E)-1,1'2,2',3,3'4,4'-octahydro-4,4'-bipehanthrylidine and its (Z)-isomer (2), at room temperature, was discovered. To confirm the easy racemization, the kinetics of enantiomeric interconversion between the two enantiomers were studied, by HPLC, CD and NMR methods, and X-ray crystallography.
