Background: Resistance to metronidazole is one of the most common reasons for Helicobacter pylori treatment failure with classic triple therapy. The clarithromycin–based regimen is not cost – effective for developing countries and also there is a lot of evidence about increase of clarithromycin resistance H-pylori. Furazolidone is a good substitute, but several side effects, limit its widespread use our previous revealed that most of side effects occurred in the beginning of second week of treatment, so decrease the period of furazolidone therapy to one week may be considered. The aim of the present study was to assess the efficacy and side effect of one week furazolidone in combination with bismuth subcitrate ,amoxycillin and omprazole Methods: In this study, a total of 177 patients were enrolled and randomized to two groups and received the following medications: Group I: omprazole 220mg + amoxycillin 21gr + bismuth subcitrate 4120 mg for two weeks and furazolidone 2200mg for 1 week; Group II: the same regimen as in group I except that one week furazolidone in group II patients followed by one week metronidazole 2500 mg/day (one week furazolidone and one week metronidazole).patients were followed after 2 weeks for side effect of drugs and compliance and control endoscopy was performed after 6 weeks and three biopsies spicimens from the antrum and three from the corpus were taken for a urease test and histology. Eradication was concluded if all the tests were negative for H-pylori Results: One hundred fifty seven patients completed the study. Mean age was 38.84+10.42 and 55.4% were male and 44.6% were female. Two patients from group I and three patients from group II did not tolerate the regimen and were excluded from the study. No serious complication was detected in two groups. The eradication rate per protocol analysis and intention to treat analysis were 89% and 79.3% in group 1 and 86.6% and 74.4% in group 2.No significant statistical differents between two groups (P=0.32 , P=0.33). Conclusion: One week furazolidone in combination with two weeks amoxicillin, omprazole and bismuth subcitrate is a good, safe and cost effective regimen for eradication of H-pylori. Addition of metronidazole in the second week of treatment period did not increase eradication rate.

Background: Eradication of helicobacter pylori is important for treatment of GU but an ideal regimen is not available. HP is resistant to metronidazole and clarithromycin. Clarithromycin is expensive and is not available in under developing countries. This study aimed to compare two regimens containing clarithromycin or azithromycin. Methods: Totally, seventy-eight patients with GU (confirmed with endoscopy) and infection of HP (Confirmed by Rapid Urease Test (RUT)) were allocated to one of the groups of study (35 participants in each group). Two weeks regimen of Clarithromycin (2×500 mg) + Amoxicillin (2×1 gr) + omeprazole (2×20 mg) was administered for group A of patients while group B got a 10 days regimen of Azithromycin (1×250 mg) + 14 days Amoxicillin (2×1 gr) + omeprazole (2×20 mg). At the end of the treatment course, the patients were evaluated according to the side effects of the drugs. In addition, two months after the end of therapy, patients underwent endoscopy and biopsy to evaluate HP eradication. Results: After two weeks, the side effects of the drug were: Nausea 8 patients in group A and 7 patients in group B, Diarrhea 2 patients in group A, 3 patients in group B and vomiting 2 patients in group A, 3 patients in group B. There were no serious side effects in any group. Eradication rate in group A was 82.9% (based on per protocol analysis (PPA)) and 84.6 % (intention to treat (ITT)). In group B, eradication rate was 77.1 % (PPA) and 79.5 % (ITT) (P=0.55). Conclusion: Based on our study results, azithromycin can be used in HP eradication regimen because of its similar efficacy to clarithromycin but also have lower cost, side effects and resistance. Introduction: Active Learning Method (ALM) is a model in which students are active in the class. This aim of this study is to compare stability of information and satisfaction of students in classic method of lecture and active learning method. Methods: This descriptive cross-sectional study was performed on 48 medical students (29 females and 19 males) selected through census sampling method and spending their physiopathology courses in Yazd Shahid Sadoughi University of Medical Sciences. Some sessions from the courses of «respiratory, digestive, and urinary diseases» were selected. Two forms for assessing students' satisfaction were used as pretest and post-test. The stability of information was assessed by 8 essay questions one month after teaching. SPSS software was used for data analysis using descriptive statistics, Mann Whitney and McNemar tests. Results: Fifty six percent of students were generally satisfied with the project implementation. The median for stability assessment exam in classes held using ALM was 5 while it was 3.42 for classes using lecture method which had a significant difference (P=0.00). Silicides with potential to form a protective silica layer have garnered considerable attention as engineering ceramic materials. This research investigates the influence of initial composition and mechanical activation on the synthesis performance and microstructure of products in the Ti-Si-Mo system. Several compositions, including Ti8Mo29Si63, Ti15Mo25Si60, Ti22Mo22Si56, Ti40Mo12Si48, Ti52Mo6Si42, Ti62.5Si37.5, and Mo33Si67, were prepared and synthesized via mechanically activated self-propagating high-temperature synthesis (MASHS). XRD, SEM, and EDS analyses, along with related investigations such as grain size calculations and morphology studies, were performed. The results indicate that at low Ti concentrations, the composite contains (Ti 0.8 ,Mo 0.2 )Si 2 and MoSi 2 , whereas moderate Ti concentrations enable the formation of the MoSi 2 -Ti 5 Si 3 composite. Moreover, a high amount of Mo can extensively dissolve into the titanium and titanium silicide structure, resulting in the synthesis of the (Ti,Mo) 5 Si 3 phase in Ti-rich samples. The dissolution of Mo in the crystal structure of the compound decreases the lattice parameters of titanium silicide. We created AA2024-AA1050 and AA2024-AA1050/0.005 vol.% Al 2 O 3 nanocomposites by six accumulative roll bonding (ARB) process cycles. We used AA2024 and AA1050 sheets with a thickness of 0.7 mm and plate-shaped alumina nanoparticles to create a composite. The two AA1050 and one AA2024 sheets (among the two AA1050 sheets) were ARB-ed up to six cycles with and without adding alumina nanoparticles. Also, a sample of the AA1050 without composite making was ARB-ed up to six cycles. We aged some composites after the ARB process in the furnace at 110, 150, and 190 °C. This project performed SEM, TEM, and EDS-MAP analyses, tensile strength, microhardness, and Pin-on-Disc tests to study the ARB-ed sheets. The results of the tensile tests showed that the tensile strength of AA2024-AA1050 created by the six cycles ARB process was two times more than primary AA1050. Also, the wear resistance of this composite was 74% more than six cycles ARB-ed the AA1050. Using 0.005 vol.% alumina nanoparticles in AA2024-AA1050 composite improved its wear resistance by 30%. This paper reports on a combined experimental and numerical modeling investigation of cracking of concrete slabs with GFRP reinforcement. At this stage of the project, attention is given to early-age cracking driven by plastic shrinkage, preceding longer term considerations of cracking resistance over the service life of field applications. Of interest is the effectiveness of GFRP reinforcement in restricting plastic shrinkage cracking. Nine small-scale slab specimens were subjected to controlled evaporation rates. Images of crack development were acquired periodically, from which crack width estimations were made. Comparisons were made between slabs reinforced with conventional steel and those reinforced with GFRP, along with control specimens lacking reinforcement. During the period of plastic shrinkage, the time of crack initiation and subsequent crack openings do not appear to be influenced by the presence of the reinforcing bars. To understand this behavior, six early-age bond tests were conducted for both types of the bars after 1, 2, and 3 h exposure to the controlled evaporation rate. In addition, concrete strength development and time of settings were measured using penetration resistance tests on a representative mortar. The numerical modeling component of this research is based on a Voronoi cell lattice model; in this approach, the relative humidity, temperature, and displacement fields are discretized in three-dimensions, allowing for a comprehensive investigation of material behavior within the controlled environment. Based on the measured bond properties, our simulations confirm that the reinforcing bars restrict crack development, though they do not prevent it entirely.

Background: Peptic ulcer is one of the most prevalent diseases. Its prevalence is estimated between 6-15% and, about 10% of the people, experience its symptom in a period of their life. Studies showed that quality of life and health status of peptic ulcer patients is affected by disease., Objectives: The aim of this study was to determine the Quality of life Related Factors in patients with peptic ulcer based on Precede Model., Context: A cross-sectional study with 120 subject patients, who were referred to Shahid Sadoghi Hospital of Yazd., Methods: The patients were selected by simple Random sampling and data were collected by researchers making questionnaire. The validity and reliability of the questionnaire was approved. The data collected were transferred directly into SPSS. For data analysis, correlations, T-test, One way - ANOVA were used., Results and Conclusion: Results showed that there was a significant difference between the Quality of Life and Self-Management (p=0.05) and Quality of Life with Health Status (p=0.01). Pearson correlation showed that there was a significant correlation between the Quality of Life and Reinforcing factors (P=0.01). Totally, it was found that the health status, Self-Management, Predisposing and Enabling factors predicted 0.64 of the quality of life variance, among which, the Health Status was the strongest predictor (β=0.575). What should be taken into account is the cultural situation of Iran and the fact that the PRECEDE model is a model for planning intervention. It can be used as a framework for planning intervention in order to promote the quality of life of ulcer peptic patients in Iran.

The environment versus the genetic influence on the size of the mastoid pneumatization has been debated for years. A concrete evidence of the influence of environmental factors (e.g. inflammation) or heredity on pneumatization is still lacking. In this study the extent to mastoid peumatization in thirty otosclerotic ears was compared with control ears. The size of mastoid pneumatization was measured by the use of Law lateral x-ray projection with the help of planimetry. It was found that the average pneumatized area in otosclerotic ears was 14.73+ -3.55 cm2 compared to 10.60+3.62 cm2 in control ears which was statistically significant. In addition the mean area of mastoid air system was found to be more in the operated ears compared to the contralateral ears, the difference being statistically significant. However there was no correlation of mastoid pneumatization with degree and duration of hearing loss and type of otosclerotic focus. Our findings therefore show a link between otosclerosis and highly pneumatized mastoids thus indicating a likelihood of heredity playing a role in determining final type ot pneumatization.
