- Abstract
Models of electrical activation and recovery in cardiac cells and tissue have become valuable research tools, and are beginning to be used in safety-critical applications including guidance for clinical procedures and for drug safety assessment. As a consequence, there is an urgent need for a more detailed and quantitative understanding of the ways that uncertainty and variability influence model predictions. In this paper, we review the sources of uncertainty in these models at different spatial scales, discuss how uncertainties are communicated across scales, and begin to assess their relative importance. We conclude by highlighting important challenges that continue to face the cardiac modelling community, identifying open questions, and making recommendations for future studies. This article is part of the theme issue 'Uncertainty quantification in cardiac and cardiovascular modelling and simulation'.

- Abstract
Patient-specific cardiac models are now being used to guide therapies. The increased use of patient-specific cardiac simulations in clinical care will give rise to the development of virtual cohorts of cardiac models. These cohorts will allow cardiac simulations to capture and quantify inter-patient variability. However, the development of virtual cohorts of cardiac models will require the transformation of cardiac modelling from small numbers of bespoke models to robust and rapid workflows that can create large numbers of models. In this review, we describe the state of the art in virtual cohorts of cardiac models, the process of creating virtual cohorts of cardiac models, and how to generate the individual cohort member models, followed by a discussion of the potential and future applications of virtual cohorts of cardiac models. Introduction: Psilocybin-assisted therapy (PAT) has demonstrated significant potential in alleviating anxiety, depression, and psychological distress among individuals with terminal illnesses. However, numerous barriers prevent equitable access to this transformative treatment. Objectives: This study seeks to gather the perspectives of patients on the waitlist of PAT. Methods: Semi-structured interviews highlight the challenges faced by patients seeking PAT and their care providers and propose recommendations to enhance accessibility. Results: Through a case study of Roots to Thrive, a non-profit healthcare practice offering group-based PAT, obstacles such as complex application processes, fear of judgement, logistical and financial constraints, and systemic inequities are revealed. Moreover, Health Canada's stringent control of PAT access via clinical trials and the Special Access Program (SAP) presents challenges for primary care providers and hinders the involvement of trained practitioners. The moral distress experienced by patients and providers due to delayed or denied access further emphasizes the urgency of addressing these barriers. Conclusions: Advocates are calling for streamlined referral systems, expedited services for end-of-life patients, formal billing infrastructure, practitioner education, expanded coverage, legislative adjustments, post-therapy support, and collaboration with non-profit organizations and Indigenous Healers to promote equitable and effective PAT. By implementing these recommendations, barriers to PAT can be overcome, allowing more individuals to benefit from this therapy and find relief from the psychological distress associated with their conditions. Abstract Complete hydatidiform mole (CHM) is a rare type of pregnancy, in which 15 to 20% of the cases may develop into gestational trophoblastic neoplasia (GTN). The diagnostic of GTN must be done as early as possible through weekly surveillance of serum hCG after uterine evacuation.We report the case of 23-year-old primigravida, with CHM but without surveillance of hCG after uterine evacuation. Two months later, the patient presented to the emergency with vaginal bleeding and was referred to the Centro de Doenças Trofoblásticas do Hospital São Paulo. She was diagnosed with high risk GTN stage/score III:7 as per The International Federation of Gynecology and Obstetrics/World Health Organization (FIGO/WHO). The sonographic examination revealed enlarged uterus with a heterogeneous mass constituted of multiple large vessels invading and causing disarrangement of the myometrium. The patient evolved with progressive worsening of vaginal bleeding after chemotherapy with etoposide, methotrexate, actinomycin D, cyclophosphamide and vincristine (EMA-CO) regimen. She underwent blood transfusion and embolization of uterine arteries due to severe vaginal hemorrhage episodes, with complete control of bleeding. The hCG reached a negative value after the third cycle, and there was a complete regression of the anomalous vascularization of the uterus as well as full recovery of the uterine anatomy. Apesar dos impactos ambientais gerados pelos resíduos poluentes da indústria sucroenergética, estudos avaliando essa temática na região Nordeste do Brasil ainda são incipientes. Portanto, objetivou-se com este estudo avaliar os impactos ambientais dos resíduos poluentes produzidos pelas indústrias sucroenergéticas no estado de Alagoas. Foi realizado um levantamento, aplicando-se um questionário em três usinas e uma destilaria autônoma, localizadas no estado de Alagoas. As usinas estão identificadas neste estudo como usinas A, B, C e D. As quatro usinas conduziram a fermentação em batelada alimentada (processo descontínuo alimentado) e possuem dorna pulmão. Duas possuem dornas abertas e duas fermentadores fechados, para recuperação do etanol arrastado pelos gases desprendidos durante a fermentação etanólica. O questionário foi preenchido pelos responsáveis com dados operacionais de cada usina. Foi avaliada a moagem de cana, produção de açúcar, produção de etanol hidratado, produção de etanol anidro, kg de açúcar por tonelada de cana e litros de etanol por tonelada de cana, consumo de ácido sulfúrico e de ciclohexano; água utilizada na lavagem da cana e na lavagem dos gases liberados durante a fermentação. As usinas sucroenergéticas no estado de Alagoas avaliadas neste estudo, que produzem açúcar e etanol, além de outros subprodutos comercializáveis, utiliza métodos e processos produtivos com elevado potencial de impacto ambiental.

In early 2012, three clusters of human Salmonella infections were identified through PulseNet, a national network of public health and food regulatory agency laboratories coordinated by CDC that subtypes disease-causing organisms. Initial investigations indicated many of the ill persons in these three clusters had contact with live poultry (e.g., chicks and ducklings) from a single mail-order hatchery; therefore, the three investigations were merged. During March 1-September 24, 2012, a total of 195 persons infected with the outbreak strains of Salmonella serotypes Infantis, Newport, and Lille were reported from 27 states.

Apesar de toda tradição da agroindústria sucroenergética, é notória a preocupação da população, e principalmente, do meio científico relacionado aos impactos ambientais negativos que seus processos industriais produzem no meio ambiente. Com isto, objetivou-se com este estudo avaliar a produção de etanol, açúcar e outros derivados da cana-de-açúcar de quatro unidades industriais de Alagoas, durante três safras e pontuar os possíveis impactos ambientais provocados pelo processo de industrialização. Foi feito um levantamento com um questionário em três destilarias anexas e em uma destilaria autônoma, localizadas no estado de Alagoas. As quatro destilarias conduziram a fermentação em batelada alimentada (processo descontínuo alimentado) e possuem dorna pulmão. Duas possuem dornas abertas e duas fermentadores fechados, para recuperação do etanol arrastado pelos gases desprendidos durante a fermentação etanólica. O questionário foi preenchido com dados operacionais de cada destilaria. Foi avaliada a matéria prima por meio das variáveis cana – de - açúcar moída para açúcar, cana moída para álcool, % cana moída crua, moagem horária, tipo de mosto (caldo / misto / melaço), açúcares redutores totais do mosto. Foi quantificada a produção, resíduos, eficiências e rendimentos das usinas. As destilarias avaliadas neste trabalho utilizam seus resíduos dentro de uma amplitude aceitável do ponto de vista ambiental, buscando sempre melhores tecnologias com intuito de minimizar os impactos ambientais. O procedimento operacional industrial adotado pelas quatro unidades é, salvo pequenas variações, padrão para todo o estado de Alagoas.

Introduction: In conventional abdominoplasty, the creation of a new umbilical scar is challenging. Several surgical techniques and approaches have previously been described and applied, but not always with satisfactory results. The objective is to demonstrate the applicability and satisfaction with omphaloplasty based on an isosceles triangle with double fixation in abdominoplasty. Methods: The study included 97 female patients aged between 25 and 65 years. All underwent classic abdominal dermolipectomy with moderate abdominal liposuction of the entire anterior abdomen and flanks by the same surgeon and were evaluated at 90, 180, and 360 days postoperatively. Results: Patients were satisfied with the umbilicus in most cases (92.8%). Some umbilical scars had contracted (3.1%) and others appeared unsightly (4.1%). No necrosis was observed. Conclusion: This technique was effective and easy to perform, with satisfactory umbilical scar aesthetic outcomes in abdominal dermolipectomy.

Human salmonellosis linked to contact with live poultry is an increasing public health concern. In 2012, eight unrelated outbreaks of human salmonellosis linked to live poultry contact resulted in 517 illnesses. In July 2012, PulseNet, a national molecular surveillance network, reported a multistate cluster of a rare strain of Salmonella Braenderup infections which we investigated. We defined a case as infection with the outbreak strain, determined by pulsed-field gel electrophoresis, with illness onset from 25 July 2012–27 February 2013. Ill persons and mail-order hatchery (MOH) owners were interviewed using standardized questionnaires. Traceback and environmental investigations were conducted. We identified 48 cases in 24 states. Twenty-six (81%) of 32 ill persons reported live poultry contact in the week before illness; case-patients named 12 different MOHs from eight states. The investigation identified hatchery D as the ultimate poultry source. Sampling at hatchery D yielded the outbreak strain. Hatchery D improved sanitation procedures and pest control; subsequent sampling failed to yield Salmonella. This outbreak highlights the interconnectedness of humans, animals, and the environment and the importance of industry knowledge and involvement in solving complex outbreaks. Preventing these infections requires a ‘One Health’ approach that leverages expertise in human, animal, and environmental health. [ABSTRACT FROM AUTHOR]

Introduction: Liposuction is the second commonest procedure performed in Brazil. The scope for liposuction has broadened with advances in the technique, with better equipment, and increased safety in the surgical environment. However, there are concerns about patient safety. Safety committees have made recommendations on various aspects of the procedure including infiltration, anesthesia, patient selection, and aspirated volume. The Brazilian Society of Plastic Surgery and the Federal Council of Medicine have also determined the parameters for aspiration safety volumes, and for the aspiration of body surface. However, supporting literature for the recommendations is scarce. The objective is to evaluate changes in hemoglobin, serum iron, and total protein levels in the peri-operative days (between 7 and 10 days), in addition to its impact in patients. Methods: We performed a prospective study, with patients submitted to either liposuction alone, or in conjunction with abdominal dermolipectomy for esthetic indications. We evaluated 30 patients, collecting their demographic data, weight, body mass index, hemoglobin, serum iron, and total protein levels in both, the pre-operative period, and between 7 and 10 post-operative days. Results: The fall in hemoglobin levels were between 2 and 6 g /dL, with an average of 3.1 g /dL between 7 and 10 post-operative days . A minimum hemoglobin value of 7.8 g/dL was noted between 7 and 10 days after surgery, while the mean reduction of serum iron was found to be 44.87 g/dL. Conclusion: The reduction in hemoglobin levels was responsible for the clinical symptoms. Iron supplementation was required in the immediate post-operative period.

The article discusses the results of the analysis made by the U.S. Centers for Disease Control and Prevention of how chronic obstructive pulmonary disease (COPD) affects adults in North Carolina and what resources are used by COPD patients from 2007 and 2009. The researchers investigated the influence of age on the prevalence of self-reported COPD. They found that 5.7 percent of the 26,227 respondents stated that they have been informed by a health professional that they had COPD. Results also revealed that a majority of adult respondents with COPD reported having had a diagnostic breathing test.

INTRODUCTION: In classic abdominoplasty, an undesirable result after abdominal flap traction is the occurrence of a suprapubic retraction in the middle of the horizontal scar when it coincides with the superior point of the old umbilical scar. METHODS: Forty-five patients underwent abdominal dermolipectomy associated with liposuction of the entire abdomen and flanks, with decortication of the suprapubic area approximately 5 cm above the initial marking of the incision line, with posterior fixation at the lower border of the abdominal flap. RESULTS: No depressions or necroses were observed in the suprapubic region. CONCLUSION: We observed that the presence of a dermofat flap in the central portion of the suture line in the classic abdominoplasties associated with the complete reconstruction of the surgical planes and the fixation of this flap in the upper abdominal flap avoids the occurrence of depressions in this region, prevents occurrence of necrosis, and improves the quality of the scar by reducing the tensile stress on the tissues.

OBJETIVO: Avaliar as características da distribuição desses por um Banco de Tecidos no Brasil. MÉTODOS: Base de dados do Banco de Tecidos entre setembro de 2006 e junho de 2008. Características dos receptores foram tabuladas. Os tipos de tecidos processados foram: cabeças femorais, osso metafisio-epifisário, osso cortical, ossos curtos ou chatos e tendões. O destino dos enxertos foi analisado. Frequências das distribuições foram obtidas e analisadas. RESULTADOS: Foram distribuídas 734 unidades tecidos fresco-congelados, transplantadas em 683 receptores. Doadores de múltiplos órgãos originaram 97,9% dos tecidos e doadores vivos os demais. Foram transplantados 489 unidades de osso córtico-esponjoso, 137 de osso metafisio-epifisário, 44 de osso chato ou curto, 3 de tendão, 29 de osso particulado e 32 de cabeças femorais. A média de idade dos receptores foi 50,3 anos, sendo 59,5% do sexo feminino e 40,5% do masculino. Os tecidos foram destinados para uso ortopédico em 21,1% dos casos e buco-maxilo-facial, em 78,9%. CONCLUSÃO: O Banco de Tecidos aumentou o número de distribuições em resposta à demanda crescente de tecidos, principalmente para uso em cirurgia buco-maxilo-facial.OBJECTIVE: Is to evaluate the characteristics of the distribution of these grafts by a Tissue Bank in Brazil. METHODS: Tissue Bank database from September 2006 to June 2008. The characteristics of the recipients were drawn up in the table form. The types of tissue processed were: femoral heads, metaphyseal-epiphyseal bone, cortical bone, flat or short bones and tendons. The intended purpose of the grafts was analyzed, and distribution frequencies were also obtained and analyzed. RESULTS: Altogether, 734 units of fresh-frozen tissue were distributed and transplanted into 683 recipients. In terms of origin of the tissues, 97.9% came from multiple organ donors, and the remainder from living donors. A total of 489 units of cortical bone were transplanted, 137 of metaphyseal-epiphyseal bone, 44 of short or flat bones, 3 of tendon, 29 of particulate bone and 32 femoral heads. The mean age of the recipients was 50.3 years; 59.5% were women and 40.5% men. The tissues were used in orthopedic surgeries in 21.1% of the cases, and in oral and maxillofacial procedures in 78.9%. CONCLUSION: The Tissue Bank has increased the number of distributions in response to the growing demand for tissues, particularly for use in oral and maxillofacial procedures.

Entre as décadas de 1910 e 1930, foram fundadas várias instituições assistenciais ligadas à imigração judaica em São Paulo, que definiram, de certa forma, os contornos de uma 'comunidade'. A primeira instituição assistencial foi a Sociedade Beneficente das Damas Israelitas, fundada em 1915, e a ela se seguiram várias outras com diferentes perfis e modelos de atendimento, incluindo dois 'lares' de criança, uma policlínica e um sanatório para tuberculosos. Essas instituições ocuparam um espaço de assistência não preenchido pelo Estado, e sua atuação esteve na linha de frente das discussões e tensões em torno do modelo e da problematização do viver em 'comunidade' (entre a filantropia e o controle social) e de diferentes projetos de inserção social e cultural dos imigrantes no Novo Mundo.Between the 1910s and 1930s, a number of social assistance institutions with links to Jewish immigration were founded in São Paulo, to some extent sketching the outlines of a 'community'. The first of these was the Israelite Ladies' Beneficent Society, founded in 1915, which was followed by a number of others with different profiles and models of assistance, including two children's 'homes', a polyclinic, and a tuberculosis sanatorium. These institutions fulfilled a role ignored by the State, and their activities kept them on the front lines, both in discussions and tensions surrounding the model and issue of living in 'community' (between philanthropy and social control) and also in different projects for helping New World immigrants achieve social and cultural integration.

A partir de uma pesquisa em fontes primárias das instituições centrais associadas à imigração judaica em São Paulo e Rio de Janeiro, este artigo mostra como, durante o Estado-Novo e a Segunda Guerra Mundial, as entidades judaicas funcionaram de forma corriqueira, adaptaram-se às restrições nacionalistas do governo Getúlio Vargas e, muitas vezes, engendraram estratégias sofisticadas para enfrentar a lei e a ideologia. Esta perspectiva de história social e do cotidiano evidencia, portanto, uma leitura distinta daquela que - analisando exclusivamente a lei, a ideologia e o preconceito do regime Vargas - considera que havia um clima de medo e perseguição generalizado entre os imigrantes judeus residentes no País. Este artigo mostra, complementarmente, que 1937-1945 foram anos decisivos para a implantação de uma comunidade etnicamente ativa e para a sedimentação de uma identidade judaico-brasileira.This article shows that, during the Estado Novo and World War II, Jewish institutions adapted themselves to Vargas and nationalist restrictions, and created sophisticated strategies to undermine both law and national ideology. This social history perspective, based on primary documents from major institutions associated with Jewish immigrants in Sao Paulo and Rio de Janeiro, provides an important corrective to the historiography that focuses on law, elite ideology and prejudice during the Vargas regime, giving the erroneous impression that Jews lived in a climate of fear and generalized persecution in Brazil. This article also shows that the years 1937- 1945 were decisive in for formation of an ethnically active community that created a clear Jewish-Brazilian identity.

Background: The term gestational trophoblastic disease (GTD) includes both complete and partial moles, which are uncommon nonviable pregnancies with the potential to evolve into a malignancy known as gestational trophoblastic neoplasia. While highly curable, the potential for malignancy associated with molar pregnancies worries the patients, leading them to seek information on the internet. A Facebook page headed by Brazilian specialized physicians in GTD was created in 2013 to provide online support for GTD patients.Objective: The objective of our study was to describe the netnography of Brazilian patients with GTD on Facebook (FBGTD) and to evaluate whether their experiences differed depending on whether they received care in a Brazilian gestational trophoblastic disease reference center (BRC) or elsewhere.Methods: This was a cross-sectional study using G Suite Google Platform. The members of FBGTD were invited to participate in a survey from March 6 to October 5, 2017, and a netnographic analysis of interactions among the members was performed.Results: The survey was answered by 356 Brazilian GTD patients: 176 reference center patients (RCP) treated at a BRC and 180 nonreference center patients (NRCP) treated elsewhere. On comparing the groups, we found that RCP felt safer and more confident at the time of diagnosis of GTD (P=.001). RCP were more likely to utilize FBGTD subsequent to a referral by health assistants (P<.001), whereas NRCP more commonly discovered FBGTD through Web searches (P<.001). NRCP had higher educational levels (P=.009) and were more commonly on FBGTD for ≥ 6 months (P=.03). NRCP were more likely to report that doctors did not adequately explain GTD at diagnosis (P=.007), had more doubts about GTD treatment (P=.01), and were less likely to use hormonal contraception (P<.001). Overall, 89% (317/356) patients accessed the internet preferentially from home and using mobile phones, and 98% (349/354) patients declared that they felt safe reading the recommendations posted by FBGTD physicians.Conclusions: This netnographic analysis of GTD patients on FBGTD shows that an Web-based doctor-patient relationship can supplement the care for women with GTD.
