Physical activity is a first-line treatment for polycystic ovary syndrome (PCOS). Resistance or aerobic exercise improves metabolic complications, reproductive outcomes, and quality of life in PCOS. DNA methylation reprogramming during exercise may be the major modifier behind these changes. We sought to evaluate genome-wide DNA methylation changes after supervised resistance and aerobic exercise in women with PCOS. Exercises were performed in 56 women with PCOS (resistance, n = 30; aerobic, n = 26), for 16 weeks (wks), three times per week, in 50-minute to one-hour sessions. Anthropometric indices and hormonal and metabolic parameters were measured before and after training. Genome-wide leukocyte DNA methylation was analysed by Infinium Human MethylationEPIC 850K BeadChip microarrays (Illumina). Both resistance and aerobic exercise improved anthropometric indices, metabolic dysfunction, and hyperandrogenism in PCOS after the training programme, but no differences were observed between the two exercises. Resistance and aerobic exercise increased genome-wide DNA methylation, although resistance changed every category in the CpG island context (islands, shores, shelve, and open sea), whereas aerobic exercise altered CpG shores and the open sea. Using a stringent FDR (>40), 6 significantly differentially methylated regions (DMRs) were observed in the resistance exercise cohort and 14 DRMs in the aerobic cohort, all of which were hypermethylated. The increase in genome-wide DNA methylation may be related to the metabolic and hormonal changes observed in PCOS after resistance and aerobic exercise. Since the mammalian genome is hypermethylated globally to prevent genomic instability and ageing, resistance and aerobic exercise may promote health and longevity through environmentally induced epigenetic changes.

ABSTRACT Objective: This study aimed to determine the differences in body fat distribution and central obesity indicators using dual-energy X-ray absorptiometry (DXA), adiposity indices, and anthropometric indices between women with and without polycystic ovary syndrome (PCOS). Materials and methods: Clinical and laboratory examination history, including transvaginal ultrasound, fasting blood samples, anthropometric measurements, and DXA scans were conducted in 179 women with PCOS (PCOS group) and 100 without PCOS (non-PCOS group). The volunteers were grouped by body mass index (BMI): normal (18-24.9 kg/m2), overweight (25-29.9 kg/m2), or obese (>30 kg/m2). The visceral adiposity index (VAI) and lipid accumulation product (LAP) were calculated, regions of interest (ROIs) were determined, and the fat mass index (FMI) was calculated using DXA. Results: VAI, LAP, ROIs, FMI, and adiposity indices by DXA were higher in women with PCOS and normal BMI. In both PCOS and non-PCOS groups, the ROIs progressively increased from normal BMI to overweight and obese, and from overweight to obese. Obese women with PCOS showed high trunk fat mass. However, obesity was not able to modify these trunk/periphery fat ratios in PCOS from overweight to higher BMI. These variables were associated with the incidence of PCOS. Conclusion: In women with PCOS and normal BMI, both DXA and the adiposity indices, VAI and LAP, are more sensitive methods to evaluate total body fat and fat accumulation in the central abdominal region. It was also observed that as BMI increased, the differences in measurements between women with and without PCOS decreased.

BackgroundModerate-intensity continuous training (MICT) is strongly recommended for polycystic ovarian syndrome (PCOS) treatment. However, recent studies have suggested that high-intensity interval training (HIIT) would promote great benefits for cardiac autonomic control. Therefore, we investigated whether the benefits of HIIT related to cardiovascular autonomic control were greater than those of MICT in women with PCOS.MethodsWomen with PCOS were randomly allocated through a blind draw into three groups: control, MICT, and HIIT. The control group did not undergo exercise, whereas those in the MICT and HIIT groups underwent 16 weeks of aerobic physical training. All groups were evaluated before and after the 16 weeks of intervention in the following aspects: quantification of serum lipids, testosterone, fasting insulin and blood glucose; physical fitness through cardiopulmonary testing; analysis of heart rate variability (HRV) by linear (time domain and frequency domain) and non-linear (symbolic analysis) methods, analysis of blood pressure variability (BPV) and spontaneous baroreflex sensitivity (BRS).ResultsThe final analysis, each group comprised 25 individuals. All groups had similar baseline parameters. After 16 weeks, intragroup comparison showed that the MICT and HIIT groups had a reduction in baseline heart rate (P < 0.001; P < 0.001, respectively) and testosterone levels P < 0.037; P < 0.012, respectively) associated with an increase in VO2peak (MICT, P < 0.001; HIIT, P < 0.001). The MICT (P < 0.36) and HIIT (P < 0.17) groups also showed an increase in cardiac vagal modulation, however only observed in the non-linear analysis. The intergroup comparison showed no differences between the MICT and HIIT groups in any of the hormonal, metabolic and autonomic parameters evaluated, including testosterone, peak oxygen uptake (VO2peak), HRV, BPV and BRS.ConclusionHIIT and MICT showed similar results for the different parameters evaluated. Abstract Objective The present study aimed to investigate the physical performance of handgrip strength (HGS) in women with polycystic ovary syndrome (PCOS). Methods A case-control study that included 70 women with PCOS and 93 agematched healthy women aged between 18 and 47 years with body mass index (BMI) between 18 Kg/m2-39.9 Kg/m2. The serum levels of total testosterone, androstenedione, insulin, estradiol, thyroid-stimulating hormone (TSH), prolactin, sex hormonebinding globulin (SHBG), and 17-hydroxyprogesterone (17-OHP) were measured. The free androgen index (FAI) and the homeostatic model assessment of insulin resistance (HOMA-IR) were calculated. The body composition regions of interest (ROIs) were assessed by dual-energy X-ray absorptiometry (DXA), and the handgrip strength (HGS) was evaluated for both the dominant and the non-dominant hands with a manual Sammons Preston (Bolingbrook, IL, US) bulb dynamometer. Results Women with PCOS had high serum levels of total testosterone (p < 0.01), androstenedione (p = 0.03), and insulin (p < 0.01), as well as high FAI (p < 0.01) and HOMA-IR (p = 0.01) scores. Compared with the non-PCOS group, the PCOS group had greater total lean mass in the dominant hand (p < 0.03) and greater HGS in both the dominant and the non-dominant hands (p < 0.01). The HGS was correlated with lean mass (p < 0.01). Conclusion Women with PCOS have greater HGS. This may be associated with age and BMI, and it may be related to lean mass. In addition, the dominance effect on muscle mass may influence the physical performance regarding HGS in women with PCOS.

Abstract Adolescence is characterized by significant biological and psychological changes. During this time, the increased production of androgens leads to increased sexual behavior, and this may contribute to early initiation of sexual activity. The objectives of the present cross-sectional study of adolescents enrolled in state schools in the city of Ribeirão Preto, state of São Paulo, Brazil, were to determine the average age at the first sexual intercourse (sexarche), the average number of sexual partners, and the frequency of contraceptive and condom use. Information on the age at sexarche, number of sexual partners, use of different contraceptive methods, and use of condoms were obtained using a semistructured questionnaire. Quantitative variables are expressed as means and standard deviations (SDs), and qualitative variables as absolute and relative frequencies. The chi-squared test was used for comparisons of qualitative variables, and the Student t-test for comparisons of continuous variables. All statistical analyses were performed using SAS (version 9.4, North Carolina State University, USA). We evaluated 202 students who answered the questionnaire, 69 males (36.36%) and 133 females (63.64%). The age at sexarche for men ranged from 7 to 18 years old, and for women from 7 to 17 years old. Forty-eight girls (36.01%) and 21 boys (30.43%) were in the first year of high school, 66.94% of adolescents reported sexual intercourse, and 56.25% used a condom during the first sexual intercourse. A total of 36.72% of students said they had safe sex most of the time, and 83.59% said that the first sexual intercourse happened because they “had a crush on” the other person.

ABSTRACT Objective To assess the utility of bioimpedance (BIA) and skinfolds thickness (SF) in body fat percentage measuring (%BF) compared to the reference method dual-energy x-ray absorptiometry (DXA) in Brazilian reproductive age women, as well as to estimate of inter- and intra-observer precision for SF. Subjects and methods 170 women aged 18-37 years with BMI between 18 and 39.9 kg/m2 were selected for this cross-sectional study. Body density was evaluated through equations proposed by Jackson, Pollock and Ward (1980) (EqJPW) and Petroski (1995) (EqPET), and %BF was estimated by BIA, DXA and Siri’s formula (1961). The SF were measured by two separate observers: A and B (to determine inter-observer variability), who measured the folds at three times with 10-minute interval between them (to determine intra-observer variability – we used only observer A). Abstract Objective The present study aimed to analyze cardiac autonomic modulation via spectral and symbolic analysis of heart rate variability (HRV) in women with polycystic ovary syndrome (PCOS) who were subjected to two consecutive tilt tests. Methods A total of 64 women were selected and divided into 2 groups: control (without PCOS), and PCOS. Concentrations of follicle-stimulating hormone, luteinizing hormone, prolactin, estradiol, homocysteine, sex hormone-binding globulin, thyroid stimulating hormone, fasting insulin, testosterone, androstenedione, and 17-hydroxyprogesterone levels, triglycerides, free androgen index (FAI), and homeostasis assessment model (HOMA-IR) were assessed. Cardiac autonomic modulation was evaluated by spectral and symbolic analyses during two consecutive tilt tests (two moments) and supine moments before, between and after (three moments) the tilt tests. Results Women with PCOS had higher fasting insulin, HOMA-IR indexes, testosterone and FAI. Additionally, we observed that the PCOS group had greater sympathetic autonomic cardiac modulation in supine 2, tilt 1, and supine 3 moments compared with controls. Conclusion Women with PCOS had higher autonomic sympathetic cardiac modulation even after a second tilt test. No adaptation to this provocative test was observed. Spectral analysis was more sensitive for identifying differences between groups than the symbolic analysis.

Abstract Objective To evaluate the effects of nutritional counseling on the dietary habits and anthropometric parameters of overweight and obese adolescentswith polycystic ovary syndrome (PCOS). Methods This was a prospective, longitudinal and auto-controlled study. Thirty adolescents aged 13-19 years-old, diagnosed with PCOS received nutritional counseling and were followed-up for 6 months. After the follow-up period, the results were evaluated through body weight, body mass index (BMI) and waist circumference (WC). Results Sixty-percent of the adolescents adhered to the nutritional counseling and, of these, 50% lost weight. Adolescents who lost weight changed their dietary habits by adopting hypocaloric diets and eating more meals per day, as per nutritional counseling. The waist circumference (WC) decreased significantly, although the body weight decreased non-significantly after adoption of a hypocaloric diet. Conclusion Although there was no significant weight loss, there was a considerable reduction in theWCassociated with hypocaloric diets and with eating a greater number of meals per day.

Introduction Brazil is a large country, with a population of mixed ethnic background and broad variation in dietary and physical activity traits across its five main regions. Because data on Brazilian women with polycystic ovary syndrome (PCOS) are still scarce, a nation-wide collaborative study was designed to determine the prevalence of metabolic and reproductive abnormalities and the presence of anxiety and depression in Brazilian women with PCOS. In addition, the study aims at describing how these characteristics are distributed across PCOS phenotypes and at detecting associations with regional demographic and lifestyle aspects, genetic variants, and epigenetic markers.Methods and analysis The Brazilian PCOS study is being conducted in the outpatient clinics of eight university hospitals within the public healthcare network (Unified Health System) across the country. Additional centres will be included following completion of the research ethics approval process. The sample includes women with PCOS according to Rotterdam criteria at inclusion in the study and a control group of healthy women matched by age, socioeconomic status and geographical region. Data will be collected in each centre and incorporated into a unified cloud database. Clinical, demographic, socioeconomic, psychological, metabolic, epigenetic and genotypic variables will be evaluated. The data resulting from this study will be useful to guide specific public strategies for primary and secondary prevention of metabolic and reproductive comorbidities in the PCOS population of Brazil.Ethics and dissemination The study protocol was approved by each local Research Ethics Committee. Written informed consent will be obtained from each participant. During data collection, analysis and publication, care will be taken to ensure confidentiality of participant information. Study results will be published in peer-reviewed journals and disseminated at international conferences. Abstract Purpose Aerobic exercises may improve quality of life (QoL) in women with polycystic ovary syndrome (PCOS). However, there is no data on the effect of resistance exercise training (RET) programs on the QoL of women with PCOS. Thus, this study aimed to assess the effect of a 16-week RET program on QoL in PCOS women. Methods This 16-week case-control study enrolled 43 women with PCOS (PCOS group, PCOSG) and 51 healthy pre-menopausal controls aged 18 to 37 years (control group, CG). All women underwent a supervised RET program for 16 weeks, and were evaluated in two different occasions: week-0 (baseline), and week-16 (after RET). Quality of life was assessed using the 36-Item Short Form Health Survey (SF-36). Results Testosterone reduced significantly in both groups after RET (p < 0.01). The PCOSG had improvements in functional capacity at week-16 relative to week-0 (p = 0.02). The CG had significant improvements in vitality, social aspects, and mental health at week-16 relative to week-0 (p ≤ 0.01). There was a weak correlation between social aspects of the SF-36 domain and testosterone levels in PCOS women. Conclusion A 16-week RET program modestly improved QoL in women with PCOS.

Abstract Background: Polycystic ovarian syndrome (PCOS) women have a high prevalence of obesity and alterations in cardiovascular autonomic control, mainly modifications in heart rate variability (HRV) autonomic modulation. However, there are few studies about other autonomic control parameters, such as blood pressure variability (BPV) and baroreflex sensitivity (BRS). In addition, there are still doubts about the obesity real contribution in altering autonomic control in these women. Objective: To investigate BPV and BRS autonomic modulation alterations in PCOS women, as well as, to evaluate whether these alterations are due PCOS or increased body fat. Methods: We studied 30 eutrophic volunteers [body mass index (BMI) < 25 kg/m2] without PCOS (control group) and 60 volunteers with PCOS divided into: eutrophic (BMI < 25 kg/m2, N = 30) and obese women (BMI > 30 kg/m2, N = 30). All volunteers were submitted to anthropometric evaluation, hemodynamic and cardiorespiratory parameters record at rest and during physical exercise, analysis of HRV, BPV and spontaneous BRS. The differences in p less than 5% (p < 0.05) were considered statistically significant. Results: Related to eutrophics groups, there were no differences in autonomic parameters evaluated. The comparison between the PCOS groups showed that both PCOS groups did not differ in the BPV analysis. Although, the obese PCOS group presented lower values of spontaneous BRS and HRV, in low frequency and high frequency oscillations in absolute units. Conclusion: Our results suggest that obesity did little to alter HRV in women with PCOS, but it may influence the spontaneous BRS.

OBJETIVOS: Comparar a frequência de síndrome metabólica (SMet) e dos fatores de risco para esta síndrome em mulheres adultas e adolescentes do sudeste brasileiro com síndrome dos ovários policísticos (SOP). MÉTODOS: Estudo transversal, realizado com 147 pacientes que apresentavam diagnóstico de SOP e que foram divididas em dois grupos: Adolescência, constituído por 42 adolescentes com 13 a 19 anos e Adultas, composto por 105 mulheres com idade entre 20 e 40 anos. Foram avaliadas características clínicas (índice de massa corporal - IMC, índice de Ferriman, circunferência abdominal - CA e pressão arterial sistêmica), o volume ovariano médio, variáveis laboratoriais (perfil androgênico sérico, lipidograma, glicemia e insulina de jejum) e frequência da SMet. Os resultados foram expressos em média±desvio padrão. Utilizou-se regressão logística múltipla tendo como variável resposta a presença de SMet e como variáveis preditoras para SMet os níveis de testosterona total, insulina e IMC. RESULTADOS: A frequência de SMet foi aproximadamente duas vezes maior no grupo de mulheres adultas em relação às adolescentes com SOP (Adolescência: 23,8 versus Adultas: 42,9%, p=0,04). Entre os critérios definidores da SMet, apenas a variável qualitativa da pressão arterial sistêmica ≥130/85 mmHg foi mais frequente nas adultas (p=0,01). O IMC foi preditor independente para SMet em mulheres adolescentes (p=0,03) e adultas (pPURPOSE: To compare the frequency of metabolic syndrome (MetS) and the risk factors associated with this syndrome in women from the Brazilian Southeast with polycystic ovary syndrome (POS) evaluated during adolescence and adult age. METHODS: This was a cross-sectional study conducted on 147 patients with a diagnosis of POS who were divided into two groups: Adolescents, 42 adolescents aged 13 to 19 years, and Adults, 105 women aged 20 to 40 years. The following factors were evaluated: clinical characteristics (body mass index - BMI, Ferriman index, abdominal circumference - AC, and systemic arterial pressure), mean ovarian volume, laboratory variables (serum androgen profile, lipid profile, glycemia, and fasting insulin), and frequency of MetS. The results were expressed as mean±standard deviation. We used multiple logistic regression with the response variable being the presence of MetS and the predictor variables the levels of total testosterone, insulin and BMI. RESULTS: The frequency of MetS was approximately twice higher in the group of adult women compared to the adolescents with POS (Adolescents: 23.8 vs. Adults: 42.9%, p=0.04). OBJETIVOS: Comparar os parâmetros metabólicos, a composição corporal e a força muscular de mulheres com Síndrome dos Ovários Policísticos (SOP) em relação a mulheres com ciclos menstruais ovulatórios. MÉTODOS: Estudo caso-controle com 27 mulheres com SOP e 28 mulheres controles com ciclos ovulatórios, com idade entre 18 e 37 anos, índice de massa corpórea entre 18 e 39,9 kg/m², que não praticassem atividade física regular. Níveis séricos de testosterona, androstenediona, prolactina, globulina carreadora dos hormônios sexuais (SHBG), insulina e glicemia foram avaliados. Índice de andrógeno livre (FAI) e resistência insulina (por HOMA) foram calculados. As voluntárias submetidas avaliação de composição corporal por dobras cutâneas e absorciometria de raio X de dupla energia (DEXA) e testes de força muscular máxima de 1-RM em três exercícios após procedimento de familiarização e de força isométrica de preensão manual. RESULTADOS: Os níveis de testosterona foram mais elevados no grupo SOP em relação ao CO (68,0±20,2 versus 58,2±12,8 ng/dL; p=0,02), assim como o FAI (282,5±223,8 versus 127,0±77,2; p=0,01), a insulina (8,4±7,0 versus 4,0±2,7 uIU/mL; p=0,01), e o HOMA (2,3±2,3 versus1,0±0,8; p=0,01). O SBHG foi inferior no grupo SOP comparado ao controle (52,5±43,3 versus 65,1±27,4 nmol/L; p=0,04). Não foram observadas diferenças significativas na composição corporal com os métodos propostos entre os grupos. O grupo SOP apresentou maior força muscular no teste de 1-RM nos exercícios supino reto (31,2±4,75 versus 27,8±3,6 kg; p=0,04) e cadeira extensora (27,9±6,2 versus 23,4±4,2 kg; p=0,01), assim como nos testes de força isométrica de preensão manual (5079,6±1035,7 versus 4477,3±69,6 kgf/m²; p=0,04). Ser portadora de SOP foi um preditor independente de aumento de força muscular nos exercícios supino reto (estimativa (E)=2,7) (p=0,04) e cadeira extensora (E=3,5) (p=0,04). Assim como o IMC no exercício de força isométrica de preensão manual do membro dominante (E=72,2) (pPURPOSE: To compare the metabolic parameters, body composition and muscle strength of women with Polycystic Ovary Syndrome (PCOS) to those of women with ovulatory menstrual cycles. METHODS: A case-control study was conducted on 27 women with PCOS and 28 control women with ovulatory cycles, aged 18 to 27 years with a body mass index of 18 to 39.9 kg/m², who did not practice regular physical activity. Serum testosterone, androstenedione, prolactin, sex hormone-binding globulin (SHBG), insulin and glycemia levels were determined. Free androgen index (FAI) and resistance to insulin (by HOMA) were calculated. The volunteers were submitted to evaluation of body composition based on skin folds and DEXA and to 1-RM maximum muscle strength tests in three exercises after familiarization procedures and handgrip isometric force was determined. RESULTS: Testosterone levels were higher in the PCOS group than in the Control Group (68.07±20.18 versus 58.20±12.82 ng/dL; p=0.02), as also were the FAI (282.51±223.86 versus 127.08±77.19; p=0.01), insulin (8.41±7.06 versus 4.05±2.73 µIU/mL; p=0.01), and HOMA (2.3±2.32 versus 1.06±0.79; p=0.01), and SBHG levels were lower (52.51±43.27 versus 65.45±27.43 nmol/L; p=0.04). No significant differences in body composition were observed between groups using the proposed methods. The PCOS group showed greater muscle strength in the 1-RM test in the bench press (31.2±4.75 versus 27.79±3.63 kg; p=0.02), and leg extension exercises (27.9±6.23 versus 23.47±4.21 kg; p=0.02) as well as handgrip isometric force (5079.61±1035.77 versus 4477.38±69.66 kgf/m², p=0.04). OBJETIVO: Avaliar os resultados do tratamento da agenesia vaginal pela técnica cirúrgica de McIndoe-Bannister modificada e pela técnica de Frank. MÉTODOS: Este estudo retrospectivo foi conduzido com uma amostra de conveniência de 25 mulheres portadoras de agenesia vaginal em seguimento no Ambulatório de Ginecologia Infanto Puberal. Quinze mulheres foram submetidas à cirúrgica modificada de McIndoe-Bannister Grupo Cirúrgico e 10 fora tratadas com a técnica de Frank Grupo Frank. Para a análise comparativa entre essas duas amostras, foram considerados os seguintes parâmentros: vaginometria final, efeitos adversos e satisfação sexual após o tratamento. Esses dados foram obtidos por meio dos registros nos prontuários médicos. A satisfação sexual foi aferida por questão simples: como está sua vida sexual? RESULTADOS: Houve diferença em relação ao comprimento da vagina tanto naquelas submetidas à técnica de Frank (comprimento inicial 2,4±2,0 cm, após o tratamento 6,9±1,1 cm, pPURPOSE: This study aimed to evaluate the results of neovaginoplasty by a modified McIndoe-Bannister technique and by the non-surgical Frank technique. METHODS: This retrospective study was conducted on a convenience sample of 25 women with vaginal agenesis undergoing surgical or conservative treatment at an Infant-Pubertal Gynecology Outpatient Clinic. Data were obtained from the medical records. Fifteen women underwent the surgical McIndoe-Bannister modified technique Surgical Group, and 10 women underwent the non-surgical Frank technique Frank Group. OBJETIVO: avaliar a concentração dos hormônios esteroides no fluido folicular (FF) de folículos pequenos (10-14 mm) e grandes (> 18 mm) de mulheres com síndrome dos ovários policísticos (SOP) submetidas à hiperestimulação ovariana controlada (HOC) e ciclos de fertilização in vitro (FIV). MÉTODOS: estudo caso-controle foi conduzido em 13 mulheres inf��rteis com SOP (17 ciclos) e 31 mulheres inférteis por fator masculino - Grupo Controle (31 ciclos). Os FF foram aspirados individualmente e dividos em 4 grupos: G1 (FF pequeno do Grupo Controle), G2 (FF pequeno do grupo SOP), G3 (FF grande do Grupo Controle) e G4 (FF grande do grupo SOP). A metodologia utilizada para as dosagens de estradiol, progesterona e β-hCG foi a quimioluminescência, e de testosterona e androstenediona o radioimunoensaio. Para a análise das dosagens hormonais no FF entre os grupos SOP e Controle utilizou-se o teste t não-pareado, e para a comparação entre os quatro grupos, o ANOVA. Para a taxa de gravidez, foi utilizado o teste exato de Fisher. RESULTADOS: os folículos pequenos dos dois grupos tiveram valores menores de progesterona (8.435±3.305 ng/mL) comparados aos grandes (10.280±3.475 ng/mL), com valor de pPURPOSE: to evaluate the concentration of steroid hormones in follicular fluid (FF) of small (10-14 mm) and large (> 18 mm) follicles of women with polycystic ovary syndrome (PCOS) submitted to controlled ovarian hyperstimulation (COH) and in vitro fertilization (IVF) cycles. METHODS: a case-control study was conducted on 13 infertile women with PCOS (17 cycles) and 31 infertile women due to male factor - Control Group (31 cycles). FF was aspirated individually and divided into four groups: G1 (FF of small follicles of the Control Group), G2 (FF of small follicles of the PCOS group), G3 (FF of large follicles of the Control Group) and G4 (FF of large follicles of the PCOS group). Estrogen, progesterone and β-hCG were determined by chemiluminescence, and testosterone and androstenedione by radioimmunoassay. The unpaired t-test was used to compare the hormone determinations in the FF of the PCOS and Control Groups, and the four groups were compared by ANOVA. OBJETIVO: determinar a relação entre a morfologia do primeiro corpúsculo polar (CP) de oócitos humanos e as taxas de fertilização e clivagem e a qualidade embrionária em procedimentos de Injeção Intracitoplasmática de Espermatozóide (ICSI). MÉTODOS: estudo retrospectivo de 582 ciclos consecutivos de ICSI no período de julho de 2003 a julho de 2005. A morfologia do primeiro CP foi avaliada com revisão de 3.177 oócitos em metáfase II, imediatamente antes da realização da ICSI, sempre pelo mesmo observador. O CP foi classificado nas seguintes categorias: CP intacto e de tamanho normal, CP fragmentado ou CP de tamanho aumentado. Avaliamos as taxas de fertilização e de clivagem, o número e a proporção de embriões de boa qualidade em cada um dos três grupos avaliados 48 horas após a ICSI (D2). Foram considerados de boa qualidade os embriões com quatro células, sem fragmentação e com blastômeros simétricos em D2. RESULTADOS: as taxas de fertilização, clivagem e de formação de embriões de boa qualidade resultantes da inseminação de oócitos com o CP aumentado (20,7, 18,7 e 5%, respectivamente) foram significativamente menores que as de oócitos com o CP intacto e de tamanho normal (70,8, 62,5 e 19%, respectivamente) ou CP fragmentado (69,7, 60,5 e 17,1%, respectivamente). CONCLUSÕES: observamos que a presença do primeiro CP aumentado relaciona-se com piores taxas de fertilização, clivagem e de formação de embriões de má qualidade. Entretanto, a fragmentação no primeiro CP parece não interferir nos resultados da ICSI.PURPOSE: to determine the relationship between the morphology of the first spindle pole of human oocytes and rates of fertilization, fragmentation and embryo quality in procedures of Intracytoplasmic Sperm Injection (ICSI). The morphology of the first spindle pole (SP) was assessed through the analysis of 3,177 oocytes in metaphase II, immediately before the ICSI procedure, always by the same observer. SP has been classified in the following categories: normal size intact, fragmented or augmented SP. Fertilization rate and fragmentation, and the number and rate of good quality embryos in each one of the three groups studied have been evaluated, 48 hours after ICSI (D2). Embryos with four cells, without fragmentation and with symmetric blastomeres in D2 were considered as of good quality. RESULTS: rates of fertilization, fragmentation and of good quality embryo formation, resulting from oocyte insemination, with augmented SP (20.7, 16.7 and 5% respectively) were significantly lower than the ones from intact and normal size SP (70.8, 62.5 and 19%, respectively) or from fragmented SP oocytes (69.7, 60.5 and 17.1%, respectively). CONCLUSIONS: it has been observed that the presence of augmented first spindle pole is related to worse rates of fertilization, fragmentation and bad quality embryo formation. Nevertheless, fragmentation in the first spindle pole of the oocyte does not seem to affect ICSI results.

OBJETIVO: avaliar o fuso meiótico e a distribuição cromossômica de oócitos maturados in vitro, obtidos de ciclos estimulados de mulheres inférteis com síndrome dos ovários policísticos (SOP) e fatores masculino e/ou tubário de infertilidade (Grupo Controle) e comparar as taxas de maturação in vitro (MIV) entre os dois grupos avaliados. MÉTODOS: cinco pacientes inférteis com SOP e oito pacientes controles, submetidas à estimulação ovariana para injeção intracitoplasmática de espermatozóide, foram selecionadas prospectiva e consecutivamente, e constituíram os grupos de estudo e Controle, respectivamente. Oócitos imaturos captados após estimulação ovariana para a realização de injeção intracitoplasmática de espermatozóide (21 e 29, respectivamente, nos Grupos SOP e Controle) foram submetidos à MIV. Apenas os oócitos que apresentaram a extrusão do primeiro corpúsculo polar após a MIV foram fixados e submetidos à imunocoloração e análise por microscopia de fluorescência para avaliação morfológica do fuso e da distribuição cromossômica. A síndrome dos ovários policísticos (SOP) é a principal endocrinopatia ginecológica na idade reprodutiva, com incidência de 6 a 10% das mulheres no menacme. A resistência insulínica e a hiperinsulinemia compensatória permanecem como os elementos mais importantes na etiopatogenia da SOP. Esta revisão teve como objetivo discutir as controvérsias no tratamento de mulheres com SOP nos diferentes contextos da infertilidade feminina e gestação, à luz das evidências atuais, com ênfase no consenso de 2008 proposto pelas sociedades européia (European Society of Human Reproduction and Embryology, ESHRE) e americana (American Society for Reproductive Medicine, ASRM) de reprodução.Polycystic ovary syndrome (PCOS) occurs in 6 to 10% of women during the reproductive age. Insulin resistance and compensatory hyperinsulinemia are currently two of the main factors involved in the etiopathogenesis of PCOS. The objective of the present review was to discuss the controversies related to the treatment of infertile women with PCOS and during their pregnancy, focusing on the European Society of Human Reproduction and Embryology (ESHRE) and the American Society for Reproductive Medicine (ASRM) current consensus.

OBJETIVO: avaliar a utilização do molde de látex natural (Hevea brasiliensis) como modificação à neovaginoplastia de McIndoe e Bannister em pacientes portadoras da síndrome de Mayer-Rokitansky-Küster-Hauser (MRKH). MÉTODOS: análise retrospectiva de nove pacientes com o diagnóstico de síndrome de MRKH, submetidas à neovaginoplastia pela técnica de McIndoe e Bannister com molde de látex natural. Foram avaliadas epitelização, amplitude e profundidade das neovaginas, ocorrência de coitos bem como satisfação e complicações cirúrgicas. RESULTADOS: após cinco semanas do procedimento, oito pacientes apresentavam neovaginas epitelizadas, com profundidade de 7 a 12 cm. Houve um caso de estenose neovaginal completa em decorrência do uso incorreto do molde pela paciente no pós-operatório. Após seguimento mínimo de um ano, todas as pacientes mantinham neovaginas com profundidade de 4 a 8 cm e capacidade para o coito, com 66,7% de satisfação. Uma paciente apresentou fístula retovaginal precoce e episódios tardios de fistulização uretrovaginal. Duas pacientes apresentaram estenose distal das neovaginas a longo prazo. Uma delas e a paciente com fístulas foram submetidas a novo procedimento. CONCLUSÕES: o uso do molde de látex natural como modificação à técnica clássica de neovaginoplastia permitiu a criação de neovaginas morfológica e funcionalmente similares ao órgão normal em pacientes com estenose vaginal.PURPOSE: to evaluate the use of natural latex mold (Hevea brasiliensis) as a modification of McIndoe and Bannister neovaginoplasty in patients presenting Mayer-Rokitansky-Küster-Hauser (MKRH) syndrome. METHODS: we retrospectively included nine patients presenting MKRH syndrome, who had been submitted to McIndoe and Bannister neovaginoplasty modified by the use of natural latex mold. Neovaginal epithelization and depth, coitus occurrence and satisfaction, and surgical complications were evaluated. RESULTS: five weeks after the procedure, eight patients presented an epithelized 7 to 12 cm deep neovagina. There was one case of complete neovaginal stenosis, because of incorrect use of the mold. After at least one year, the others maintained 4 to 8 cm deep neovaginas and capacity for intercourse, with 66.7% satisfaction. One woman presented precocious rectovaginal fistula and late episodes of uretrovaginal fistulae. Two patients presented distal neovaginal stenosis in long-term follow-up. One of these and the patient with fistulae were submitted to a new procedure. CONCLUSIONS: the use of natural latex mold as a modification of classic neovaginoplasty technique allows the creation of neovaginas morphologically and functionally similar to the normal vagina in patients with vaginal agenesis.

OBJETIVO: avaliar resultados da neovaginoplastia com utilização de enxerto amniótico humano em pacientes portadoras da síndrome de Mayer-Rokitansky-Küster-Hauser (MRKH). MÉTODOS: o estudo foi uma análise retrospectiva de uma série de 28 casos de pacientes com síndrome de MRKH, tratadas entre 1990 e 2003. As pacientes foram atendidas no Ambulatório de Ginecologia Infanto-Puberal (AGIP) do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo (FMRP-USP), sendo submetidas a neovaginoplastia pela técnica de McIndoe e Bannister modificada pela utilização de enxerto de membrana amniótica humana. Foram avaliadas epitelização, amplitude e profundidade das neovaginas com sete e 40 dias do procedimento e, no pós-operatório tardio, a satisfação das pacientes, a presença de desconforto e dispareunia às relações sexuais. RESULTADOS: no pós-operatório, sete pacientes (25%) apresentaram estenose vaginal. Destas, seis foram submetidas a nova intervenção cirúrgica, uma teve encurtamento da neovagina, corrigido com o uso de exercícios com molde vaginal, três (10,7%) tiveram fístula retovaginal, uma (3,6%) fístula uterovesical e uma (3,6%) teve excesso de pele no intróito vaginal - todas corrigidas com êxito com nova cirurgia. Quatro pacientes (14,3%) apresentaram infecção do trato urinário. Dois meses após a cirurgia, 11 de 19 pacientes (57,8%) apresentaram atividade sexual satisfatória e 42% relataram dispareunia e, no período máximo de quatro anos, 20/21 pacientes (95,2%) tiveram atividade sexual satisfatória e 4,8% dispareunia. CONCLUSÕES: o enxerto de membrana amniótica é uma boa opção no tratamento da agenesia vaginal. O acompanhamento perioperatório envolve questões educacionais, de orientação quanto ao uso do molde e em relação à sexualidade da paciente, com vistas à redução das queixas de coito disfuncional na presença de evolução cirúrgica favorável e neovagina de aspecto adequado.PURPOSE: to evaluate the results of neovaginoplasty with the use of a human amniotic graft in patients with the Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome. METHODS: the study was a retrospective analysis of a series of 28 patients with the MRKH syndrome conducted from 1990 to 2003. The patients were attended and treated at the Ambulatório de Ginecologia Infanto-Puberal (AGIP) of the Hospital Universitário of the Faculdade de Medicina de Ribeirão Preto of the Universidade de São Paulo (FMRP-USP), being submitted to neovaginoplasty by the technique of McIndoe and Bannister, modified by the use of a human amniotic membrane graft. Epithelization, amplitude and depth of the neovaginas were evaluated 7 and 40 days after the procedure. Patient satisfaction was determined during the late postoperative period in terms of the presence of discomfort and dyspareunia during sexual relations. RESULTS: postoperatively, seven patients (25%) presented vaginal stenosis and six of them were submitted to a new surgical intervention, one had shortening of the neovagina, corrected with the use of exercises with a vaginal mold, three (10.7%) developed a rectovaginal fistula, one (3.6%) a uterovesical fistula, and one (3.6%) excess skin in the vaginal introitus - all successfully corrected with surgery. Four patients (14.3%) presented urinary tract infection. Two months after surgery, 11/19 patients (57.8%) presented satisfactory sexual activity and 42% dyspareunia, and within a maximum period of four years, 20/21 patients (95.2%) had satisfactory sexual activity and 4.8% dyspareunia. CONCLUSIONS: an amniotic membrane graft is a good option for the treatment of vaginal agenesis. Perioperative follow-up involves educational guidance regarding the use of the mold and regarding patient sexuality in order to reduce the complaints of dysfunctional coitus in the presence of a favorable surgical evolution and a neovagina of adequate aspect.

OBJETIVO: avaliar o nível de peroxidação lipídica e vitamina E no fluido folicular e soro de pacientes inférteis, com ou sem endometriose, submetidas à indução da ovulação para procedimentos de reprodução assistida. MÉTODOS: estudo prospectivo envolvendo, consecutivamente, 36 pacientes inférteis com idade entre 20 e 38 anos, divididas em dois grupos: Endometriose (n=17) e Controle (n=19, laqueadura tubária prévia ou fator masculino). Amostras sanguíneas foram coletadas em: D1 (imediatamente antes do início do uso de gonadotrofinas), D2 (dia da aplicação da gonadotrofina coriônica humana) e D3 (dia da aspiração folicular). Em D3, amostras de fluido folicular livres de contaminação sanguínea foram coletadas e armazenadas. O nível de eroxidação lipídica foi mensurado pela quantificação de malondialdeído (MDA) por espectrofotometria, e o status antioxidante por meio da medida da vitamina E por cromatografia líquida de alta pressão. RESULTADOS: em D1, nenhuma diferença significante foi observada entre os Grupos Endometriose (2,4 nmol/mL) e Controle (1,9 nmol/mL) no nível de MDA. Contudo, os níveis de vitamina E foram significativamente mais elevados no Grupo Controle (24 mimol/L). Em D2, os níveis de MDA foram significantemente maiores no Grupo Endometriose (2,3 nmol/mL) quando comparados com o Controle (1,4 nmol/mL), enquanto os níveis de vitamina E permaneceram significativamente mais elevados no Controle (23,4 mimol/L). Em D3 não houve diferença significante nos níveis de MDA e de vitamina E do soro e fluido folicular entre os grupos. Contudo, em D3, os níveis de vitamina E foram significativamente mais elevados no soro do que no fluido folicular em ambos os grupos, e os níveis de MDA foram significativamente menores no fluido folicular do que no soro apenas no Grupo Controle. CONCLUSÃO: antes do início da indução da ovulação, uma diminuição significativa nos níveis séricos de vitamina E foi observada em pacientes com endometriose, cujo consumo poderia estar contribuindo para a manutenção de níveis séricos de MDA similares ao Grupo Controle. Depois da indução da ovulação com gonadotrofinas exógenas, o grupo de pacientes com endometriose apresentou não somente aumento nos níveis séricos de MDA, mas também manteve status antioxidante inferior ao Grupo Controle. Contudo, no dia da captação oocitária, ambos os níveis séricos de MDA e de vitamina E foram semelhantes nos dois grupos.PURPOSE: to assess the level of lipid peroxidation (LP) and vitamin E in the follicular fluid and serum of infertile patients, with or without endometriosis, submitted to induction of ovulation for assisted reproduction procedures. METHODS: infertile patients aged 20 to 38 years old were selected prospectively and consecutively and divided into Endometriosis Group (17 patients with pelvic endometriosis) and Control Group (19 patients with previous tubal ligation or with male factor). Blood samples were collected on: D1 (before the beginning of the use of gonadotrophins), D2 (day of human chorionic gonadotrofin application) and D3 (day of oocyte retrieval). On D3, follicular fluid samples free from blood contamination were also collected and stored. LP was assessed for malondialdehyde (MDA) quantification by spectrophotometry, and antioxidant status by measurement of vitamin E by HLPC. RESULTS: on D1, no significant difference in LP was observed between groups. However, vitamin E levels were significantly higher in the Control Group. On D2, LP levels were significantly higher in the Endometriosis Group compared to Control and vitamin E levels continued to be significantly higher in the Control Group. On D3, there was no significant difference in both serum and follicular fluid levels of LP or vitamin E between groups. However, on D3, vitamin E levels were found to be significantly higher in serum than in follicular fluid in both groups, whereas MDA levels were significantly lower in follicular fluid than in serum only in the Control Group. CONCLUSION: before the beginning of the induction of ovulation, a significant decrease in antioxidant status was observed in patients with endometriosis, perhaps because antioxidants are consumed during oxidation reactions. After the induction of ovulation with exogenous gonadotrophins, the group of patients with endometriosis presented not only increased lipid peroxidation compared to Control, but also maintained a lower antioxidant status than the Control Group. However, on the day of oocyte retrieval, both serum LP potential and the levels of vitamin E were found to be similar in both groups.

OBJETIVO: avaliar o efeito da obesidade sobre a função das célulasbeta pancreáticas de pacientes portadoras de síndrome dos ovários policísticos (SOP). MÉTODOS: estudo transversal no qual foram avaliadas 82 pacientes portadoras de SOP, selecionadas de forma consecutiva, no momento do diagnóstico de SOP. Pacientes com índice de massa corporal (IMC) maior ou igual a 30 kg/m² foram consideradas SOP obesas (n=31). Valores de índice de massa corporal menores que este limite foram consideradas SOP não-obesas, o que correspondeu a 51 mulheres. Foram utilizadas a glicemia e a insulina de jejum para cálculo da função das células beta pancreáticas (HOMA-%beta-Cell) e da resistência à insulina (HOMA-IR e QUICKI) entre os grupos. Analisaram-se, também, variáveis secundárias como idade, idade da menarca, níveis séricos hormonais (testosterona, prolactina, LH e FSH) e de colesterol total, triglicerídeos, HDL colesterol e LDL-colesterol. RESULTADOS: a idade da menarca das pacientes obesas com SOP (11,7±1,8 anos) foi menor que as não-obesas (12,7±1,9) (pPURPOSE: to evaluate the effect of obesity on beta-cell function in patients with polycystic ovary syndrome (PCOS). OBJETIVO: avaliar os casos com suspeita de puberdade precoce, em relação à classificação diagnóstica e etiológica, atendidos no período compreendido entre os anos de 2000 e 2005. MÉTODOS: foram revisados os prontuários de 58 pacientes com suspeita diagnóstica de puberdade precoce atendidas no período compreendido entre os anos de 2000 e 2005 para análise de dados relevantes. Os critérios de inclusão foram desenvolvimento de mamas e/ou pêlos pubianos antes dos oito anos de idade. As pacientes foram classificadas de acordo com o quadro clínico e os exames complementares em uma das seguintes categorias: puberdade precoce central (PPC), pseudopuberdade precoce, telarca precoce e pubarca precoce. RESULTADOS: dos 58 casos revisados, 28 tiveram diagnóstico de PPC, um de pseudopuberdade precoce, dez de telarca precoce e 19 de pubarca precoce. Todos os casos de PPC foram de origem idiopática, com exceção de uma paciente em que houve ativação do eixo hipotálamo-hipófise-ovariano secundária a hiperplasia adrenal congênita. Houve um caso com suspeita diagnóstica de pseudopuberdade precoce devido à síndrome de McCune-Albright. Todos os casos de telarca precoce foram de origem idiopática, exceto um caso que foi associado a hipotireoidismo primário. Todos os casos de pubarca precoce foram de origem idiopática. CONCLUSÕES: dentre os casos atendidos com diagnóstico de puberdade precoce no período de cinco anos, o diagnóstico final predominante foi PPC e a grande maioria dos casos foi de origem idiopática, havendo baixa incidência de patologias orgânicas.PURPOSE: to evaluate the classification and the etiology of girls attended in a Pediatric and Adolescent Gynecology Clinic. METHODS: The hospital charts of 58 female patients attended from 2000 to 2005 with diagnosis of probable precocious puberty were reviewed and relevant data analyzed. Inclusion criteria were breast and/or pubic hair growth before eight years old. The girls were classified according to the clinic aspects and the supplementary exams they had been submitted to, into one of the categories: central precocious puberty (CPP), precocious pseudopuberty, premature thelarche and premature pubarche. RESULTS: from the 58 reviewed cases, 28 girls were diagnosed as CPP, one as precocious pseudopuberty, ten as premature thelarche and 19 as premature pubarche. All the cases of CPP had an idiopathic etiology, except for one girl whose activation of the ypothalamic-pituitary-gonadal axis was secondary to congenital adrenal hyperplasia. There was one case of precocious pseudopuberty due to McCune-Albright syndrome. All the cases of premature thelarche had an idiopathic etiology, except for one girl who had primary hypothyroidism. All the cases of premature pubarche had an idiopathic etiology. CONCLUSIONS: among the cases diagnosed as precocious puberty, CPP was the leading diagnosis and most cases had an idiopathic etiology. Organic causes leading to precocious puberty were infrequent.

OBJETIVOS: avaliar os efeitos da laqueadura tubária sobre os resultados de fertilização in vitro com transferência de embrião. MÉTODOS: estudo retrospectivo, no qual foram analisadas 98 mulheres submetidas a ciclos consecutivos de fertilização in vitro. Grupo LT, 55 mulheres com antecedente de laqueadura tubária bilateral, sem outros fatores de infertilidade associados, e grupo IM, 43 mulheres com infertilidade devido a fator masculino leve. As variáveis analisadas foram: idade, taxa de suspensão do ciclo de indução da ovulação, resposta à indução da ovulação (dias de indução, unidades de gonadotrofinas, número de folículos recrutados e oócitos captados), taxas de fertilização e de clivagem, número de embriões transferidos e gravidez clínica por ciclo de transferência. RESULTADOS: não houve diferença entre as taxas de suspensão de ciclo de indução da ovulação, as respostas à indução da ovulação, as taxas de fertilização e de clivagem, o número de embriões transferidos e as taxas de gravidez clínica por ciclo transferido, entre os grupos LT e IM. Considerando-se isoladamente a variável idade no grupo LT, evidenciou-se que pacientes com idade superior a 35 anos utilizaram mais unidades de gonadotrofinas (2445 versus 2122 UI), tiveram menor número de folículos recrutados (11,3 versus 15,8) e menos oócitos foram captados (6,1 versus 8,5), quando comparadas às mulheres com idade de 34 anos ou menos. CONCLUSÕES: a laqueadura tubária não interferiu nos resultados da fertilização in vitro. Observamos uma piora da resposta à indução da ovulação nas pacientes submetidas à laqueadura tubária com idade superior a 35 anos.PURPOSE: to investigate the effects of previous bilateral tubal sterilization on the outcome of in vitro fertilization. METHODS: retrospective study of 98 consecutive in vitro fertilization cycles. Fifty-five women with previous tubal sterilization without any other infertility factor (TL group) were compared with 43 women with infertility due only to mild male factor (MI group. Age, cancellation rate per induction cycle, response to ovulation induction (number of days of ovulation induction, total amount of gonadotrophin units used, number of follicles and oocytes retrieved), fertilization and cleavage rates, number of transferred embryos and clinical pregnancy per transfer cycle were the variables considered. RESULTS: the cycle discontinuation rate due to poor response, results of ovulation induction, fertilization and cleavage rates, number of transferred embryos and the occurrence of clinical pregnancy were similar in both groups. Considering solely the variable age in TL group, we observed that patients older than 35 years required higher gonadotrophin doses during ovulation induction (2445 versus 2122 IU), presented lower response with fewer follicular growth (11.3 versus 15.8) and less oocytes retrieved (6.1 versus 8.5) compared to younger women (34 years old or less). CONCLUSIONS: tubal sterilization did not interfere with in vitro fertilization outcomes. We observed a worse response to ovulation induction in women older than 35 years, who had previous tubal sterilization.

OBJETIVOS: avaliar a reprodutibilidade intra-observador e interobservador da medida do volume endometrial utilizando ultra-sonografia tridimensional (3D) e o programa VOCAL® (Virtual Organ Computer-aided AnaLysis). MÉTODOS: um bloco ultra-sonográfico 3D do endométrio foi obtido de cinco pacientes inférteis voluntárias que apresentavam diferentes volumes endometriais. Para cada bloco 3D, o volume endometrial foi calculado utilizando o modo manual em quatro diferentes passos de rotação (30º, 15º, 9º e 6º) por dois diferentes observadores. Dez medidas foram obtidas para cada rotação e por cada observador. Testamos a diferença entre as médias com o one-way ANOVA e o pós-teste de Tukey e a reprodutibilidade com os coeficientes de correlação intraclasse. RESULTADOS: as medidas realizadas com passo de rotação de 30º foram associadas a médias significativamente menores em 3 das 5 pacientes. Não houve diferença entre as médias obtidas pelos passos de rotação de 15º, 9º ou 6º. Em nenhuma das avaliações foi notada diferença entre as médias obtidas pelos dois observadores. Os coeficientes de correlação intraclasse foram significativamente menores com o passo de rotação de 30º (todos abaixo de 0,984) que com os outros passos de rotação (todos acima de 0,996). CONCLUSÕES: o uso de passos de rotação de 15º ou menos apresentou medidas reprodutíveis do volume endometrial: não houve diferença significativa entre as médias obtidas pelos dois observadores, associado ao alto coeficiente de correlação intraclasse (>0,996). É recomendável o uso do passo de rotação de 15º, pois demora menos para ser obtido em comparação com 6º e 9º.PURPOSE: to determine the intraobserver and interobserver reproducibility of endometrial volume measurements using the VOCAL®-imaging program (Virtual Organ Computer-aided AnaLysis). METHODS: one three-dimensional (3-D) ultrasound dataset of the endometrium was obtained from each of five infertile women with different endometrial volumes. For each 3-D dataset, the endometrial volume was calculated by two different observers using the manual mode in four different rotational steps (30º, 15º, 9º and 6º). Ten measurements were obtained with each method and observer from each 3-D dataset. We have used one-way ANOVA and the Tukey post-test to verify the differences among means and the intraclass correlation coefficient to test reliability. RESULTS: rotational methods employing a rotation step of 30º were associated with lower endometrial volume readings in 3 of the 5 patients. There were no significant differences between the means obtained by the 15º, 9º or 6º step rotation. No significant difference was found between the means obtained by the two different observers. The intraclass correlation coefficients were significantly lower with 30º (all under 0.984) than with the other step rotations (all above 0.996). CONCLUSIONS: the use of a rotational step of 15º or less provides reliable readings of endometrial volume: there were no significant differences between the means calculated by the two observers, associated with highintraclass correlation coefficient (>0.996). We recommend the 15º step rotation because it is quicker to be performed than 6º and 9º.

OBJETIVO: avaliar as taxas de gestação diagnosticada química (beta-HCG > 25 mUI/mL sérica obtida 14 dias após transferência embrionária) ou clinicamente (saco gestacional visualizado por ultra-sonografia quatro a seis semanas após transferência embrionária) e aborto em mulheres submetidas a fertilização in vitro (FIV) ou injeção intracitoplasmática de espermatozóide (ICSI) em serviço terciário de reprodução assistida e correlacionar estas taxas com a idade. MÉTODOS: estudo transversal retrospectivo, com análise de 1016 ciclos de hiperestimulação ovariana controlada de 932 mulheres inférteis com indicação para FIV (370 ciclos) ou ICSI (646 ciclos). A idade das pacientes variou entre 22 e 46 anos. Todas as mulheres com idade superior a 35 anos incluídas no estudo apresentavam FSHPURPOSE: to evaluate the rate of chemically diagnosed pregnancy (serum beta-HCG>25 mIU/mL levels obtained fourteen days after the embryonic transfer) clinically diagnosed pregnancy (gestational sac visualized by ultrasound four to six weeks after the embryonic transfer) and miscarriage rates in women submitted in vitro fertilization to (IVF) and intracytoplasmic sperm injection (ICSI) in a tertiary center of assisted reproduction and to correlate these rates with age. OBJETIVOS: determinar a freqüência de infecção pelo Mycoplasma hominis e Ureaplasma urealyticum e relacioná-la a variáveis clínicas de mulheres inférteis. MÉTODOS: estudo transversal com 322 pacientes inférteis submetidas à coleta de swab endocervical para pesquisa de Mycoplasma hominis e Ureaplasma urealyticum, de outubro de 2002 a maio de 2004. Todas as pacientes foram submetidas a protocolo básico de investigação clínica e laboratorial da infertilidade. Como controle, utilizou-se série histórica de 51 mulheres não gestantes, previamente pesquisadas quanto aos agentes infecciosos estudados. RESULTADOS: a freqüência de infecção pelo Mycoplasma hominis e Ureaplasma urealyticum foi de 4,9% nas pacientes inférteis e 13,8% no grupo controle. Entre as pacientes inférteis observou-se relação entre a presença dos dois patógenos e alterações no resultado da histerossalpingografia (OR: 3,20; IC 95%: 1,05-9,73), presença de dispareunia (OR: 10,72; IC 95%: 3,21-35,77) e corrimento vaginal (OR: 8,5; IC 95%: 2,83-26,02), além de cultura endocervical positiva para Escherichia coli (OR: 16,09; IC 95%: 4,95-52,25). CONCLUSÃO: a taxa de infecção pelo Mycoplasma hominis e Ureaplasma urealyticum é baixa em pacientes inférteis e está associada a seqüelas reprodutivas tardias.PURPOSE: to determine the frequency of Mycoplasma hominis and Ureaplasma urealyticum infection, and relate it to the associated clinical variables of infertile women. METHODS: transversal study involving 322 infertile women, submitted to collection of endocervix swab for research of Mycoplasma hominis and Ureaplasma urealyticum infecction, from October 2002 to May 2004. All patients were submitted to a basic infertility investigation protocol. As control, a historical series of 51 non-pregnant women previously investigated as for the studied infectious agents, was used. RESULTS: the frequency of Mycoplasma hominis and Ureaplasma urealyticum infection was 4.9% in the infertile women and 13.8% in the control group. Among the infertile patients, a relationship between the presence of the two agents and changes in the histerosalpingography result (OR: 3.20; IC 95%: 1.05-9.73), presence of dyspareunia (OR: 10.72; IC 95%: 3.21-35.77) and vaginal discharge (OR: 8.5; IC 95%: 2.83-26.02), besides endocervical culture positive for Escherichia coli (OR: 6.09; IC 95%: 4.95-52.25) was observed. CONCLUSION: Mycoplasma hominis and Ureaplasma urealyticum infection rate is low in infertile patients and is associated with reproductive sequels.

OBJETIVO: avaliar os resultados da superindução de ovulação seguida de fertilização in vitro (FIV) em pacientes com síndrome dos ovários policísticos (SOP), comparado-as a mulheres com ovários normais. MÉTODOS: estudo retrospectivo, controlado, no qual foram incluídas 36 mulheres com SOP (grupo SOP) e 44 mulheres que apresentavam infertilidade por fator masculino leve (grupo controle), submetidas à FIV no período de 1997 a 2003. A idade variou de 18 a 36 anos. A superindução da ovulação foi realizada com hormônio folículo-estimulante recombinante e agonista do hormônio liberador de gonadotrofinas. As variáveis analisadas foram os folículos com diâmetros médios entre 14 mm e 17 mm e folículos com diâmetros maiores ou iguais a 18 mm no dia da administração de gonadotrofina coriônica humana, porcentagem de folículos >18 mm, número de oócitos captados, taxa de fertilização, taxa de clivagem, incidência de síndrome de hiperestimulação ovariana (SHO), taxa de gravidez clínica e taxa de abortamento. Estas variáveis foram analisadas pelo testes t não pareado, exato de Fisher e Mann-Whitney. RESULTADOS: o grupo SOP apresentou maior número de folículos recrutados, a maioria com diâmetro entre 14 e 17 mm, quando comparado ao grupo controle (64,8 versus 53,9%), menor taxa de fertilização (59,4 versus 79,6%) e maior incidência de SHO (38,9 versus 9,1%) (p < 0,05). O número de oócitos captados, as taxas de clivagem, de gestação por transferência de embriões, de aborto e de recém-nascido vivo não diferiram entre os grupos. CONCLUSÃO: o sucesso da FIV está comprometido em mulheres com SOP por apresentarem recrutamento de maior número de folículos com diâmetros reduzidos, taxa reduzida de fertilização e elevada taxa de SHO.PURPOSE: to evaluate the results of ovulation hyperinduction followed by in vitro fertilization (IVF) in women with polycystic ovary syndrome (POS), as compared to normal cycle women. Os macroprolactinomas são tumores benignos hipofisários, secretores de prolactina, manifestando-se com amenorréia, galactorréia e disfunção gonadal. O tratamento de primeira escolha é clínico, com agonistas dopaminérgicos. O tratamento cirúrgico é reservado para os raros casos que não respondem ao tratamento clínico, intolerância à medicação ou nos casos agudos quando ocorre a hemorragia intra-tumoral. Descrevemos o caso de uma paciente portadora de macroprolactinoma submetida a duas ressecções cirúrgicas e resistente ao tratamento clínico, com evolução não usual.Macroprolactinomas are benign prolactin-secreting pituitary tumors, causing amenorrhea, galactorrhea and gonadal dysfunction. Clinical treatment with dopamine agonists is the first-choice therapy. Surgery is indicated for the rare cases that are resistant to clinical treatment, when there is intolerance to the medication, or intratumoral hemorrhage is detected. We describe the case of a female patient with macroprolactinoma submitted to two surgical procedures and resistant to clinical treatment, with unusual evolution.

OBJETIVOS: avaliar os efeitos do raloxifeno sobre a concentração plasmática de homocisteína e o perfil lipídico em mulheres na pós-menopausa. MÉTODOS: foram estudadas 24 mulheres saudáveis na pós-menopausa, com idade entre 50 e 70 anos e diagnóstico de osteopenia e/ou osteoporose, submetidas à terapia com raloxifeno, 60 mg/dia, durante seis meses. Foram dosados a homocisteína plasmática antes do início e após três e seis meses de tratamento, além do colesterol total, HDL-colesterol, LDL-colesterol e triglicérides. A concentração de homocisteína foi determinada por imunoensaio de fluorescência polarizada e o lipidograma pelo método colorimétrico e enzimático. Para a análise estatística foram utilizados os testes ANOVA, de Newman-Keuls e de correlação de Pearson. RESULTADOS: observamos redução significativa do colesterol total de 15,3% (227,6±56,3 vs 200,6±29,8 vs 192,8±32,1 mg/dl; p0,05). Embora não significativa, foi observada redução da homocisteína de 4.5% (11,7±3,0 vs 11,0±2,9 vs 11,2±2,1 miM/l; p>0,05) entre os períodos pré e pós-tratamento, com correlação negativa significativa entre os níveis basais e os percentuais de redução após o tratamento (r=-0,71; pPURPOSE: to evaluate the effects of raloxifene on plasma homocysteine concentration and lipid profile in postmenopausal women. METHODS: twenty-four healthy postmenopausal women, aged 50 to 70 years, with osteopenia and/or osteoporosis, were submitted to raloxifene therapy, 60 mg/day, for six months. Plasma homocysteine concentration was determined before and after three and six months of therapy, as well as total cholesterol, HDL-cholesterol LDL-cholesterol and triglyceride levels. Plasma homocysteine was measured by a polarized immunofluorescence assay and serum lipids by the enzymatic and colorimetric method. Data were analyzed statistically by ANOVA, Newman-Keuls test and Pearson's correlation test. RESULTS: a significant decrease in total cholesterol of 15.3% (227.6±56.3 vs 200.6±29.8 vs 192.8±32.1 mg/dl; p0.05). OBJETIVO: verificar a eficácia de uma dose única subcutânea de acetato de cetrorelix em evitar a ovulação prematura em ciclos de fertilização assistida. MÉTODOS: estudo prospectivo, randomizado e controlado, pelo qual foram avaliados 20 ciclos de estimulação ovariana em mulheres submetidas a fertilização assistida, 10 das quais utilizaram o esquema tradicional de bloqueio hipofisário com análogos de GnRH em doses diárias (grupo controle) e 10 utilizaram antagonista de GnRH em dose única de 3 mg no 7º dia de estimulação ovariana (grupo cetrorelix). Foram dosados FSH, LH, estradiol e progesterona no soro no primeiro e sétimo dia da estimulação, no dia da injeção de HCG e no dia da captação de oócitos. Os grupos foram comparados entre si quanto a eficácia do bloqueio hipofisário (nível de progesterona no dia da aplicação do HCG) e desempenho nos ciclos de fertilização assistida (ampolas de gonadotrofinas utilizadas, folículos maiores que 18 mm, oócitos captados, taxas de fertilização, implantação e gravidez) utilizando os testes de Mann-Whitney e exato de Fisher. RESULTADOS: não houve diferença significativa entre os grupos controle e cetrorelix, respectivamente, para a mediana da idade (31,5 e 34 anos), índice de massa corpórea (24 e 22), ampolas de gonadotrofinas utilizadas (34 e 32), folículos recrutados (3,5 e 3,0), oócitos captados (11 e 5), embriões obtidos (4 e 3), taxas de fertilização (93,7 e 60%, p = 0,07) e gravidez (50 e 60%, p = 0,7). Em ambos os grupos observou-se bloqueio hipofisário eficaz durante o período de estimulação ovariana. CONCLUSÕES: estes resultados confirmam a eficácia da dose única de 3 mg de acetato de cetrorelix em prevenir ovulações prematuras em pacientes submetidas a fertilização assistida, mostrando tendência a obtenção de menor número de embriões e menores taxas de fertilização no grupo cetrorelix em relação ao grupo controle. As taxas de implantação e gravidez foram semelhantes entre os dois grupos. Estudos prospectivos com maior número de pacientes são necessários para confirmar estes achados.PURPOSE: to verify the efficacy of a single subcutaneous dose of cetrorelix acetate to avoid premature ovulation in assisted fertilization cycles. METHODS: this is a prospective, controlled and randomized study, with 20 women undergoing ovarian stimulation for assisted fertilization, 10 of whom were submitted to classical GnRH agonist protocol (control group) while 10 utilized a 3-mg subcutaneous dose of the GnRH antagonist on the 7th day of ovarian stimulation (cetrorelix group). Serum FSH, LH, estradiol and progesterone concentrations were assessed on the first, seventh, HCG administration and oocyte retrieval days. Both groups were compared for pituitary suppression (progesterone concentration on HCG day) and assisted fertilization cycle performance (gonadotropin ampoules utilized, follicles over 18 mm, retrieved oocytes, fertilization, implantation and pregnancy rates), utilizing Mann-Whitney and Fisher exact tests. RESULTS: no significant difference was observed between control and cetrorelix groups, respectively, for medians of age (31.5 and 34 years), body mass index (24 and 22), gonadotropin ampoles utilized (34 and 32), follicles over 18 mm (3.5 and 3.3), retrieved oocytes (11 and 5), obtained embryos (4 and 3), fertilization (93,7 and 60%, p=0.07) and pregnancy rates (50 and 60%, p=0.07). Efficient pituitary blockade through the ovarian stimulation period was observed for both groups. CONCLUSIONS: these results confirm the efficacy of a 3-mg dose of cetrorelix acetate to prevent premature ovulation in patients undergoing assisted fertilization and suggest a tendency towards a smaller number of embryos and fertilization rates in the cetrorelix group than in the control group. Implantation and pregnancy rates were similar between groups. Other prospective studies with a greater number of patients should be performed to confirm these results.

OBJETIVO: avaliar três sistemas de escore embrionário para embriões de 3º dia e correlacioná-los com resultados positivos da técnica de fertilização in vitro. MÉTODO: estudo retrospectivo desenvolvido pelo programa de fertilização in vitro do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto - USP. Foram incluídas 137 pacientes submetidas a fertilização in vitro e transferência de 439 embriões. Os principais resultados avaliados foram taxa de gravidez e taxa de implantação. Objetivo: avaliar a reserva ovariana por meio da dosagem do FSH no 3º dia do ciclo menstrual, comparando-o com o teste do clomifeno, e correlacionar os resultados com a resposta ovariana à hiperestimulação controlada com gonadotrofinas para a fertilização in vitro. Métodos: foram selecionadas 49 pacientes com idade superior a 30 anos que apresentavam quadro clínico de infertilidade há pelo menos 1 ano. Foi realizada avaliação da reserva ovariana destas pacientes pelo teste do citrato de clomifeno. Posteriormente, 26 das 49 pacientes foram submetidas à hiperestimulação ovariana controlada com gonadotrofinas. Destas 26 pacientes, 18 tiveram boa resposta à hiperestimulação ovariana e 8, má resposta. Foram calculadas as médias e os desvios-padrão referentes aos valores do FSH do 3º dia, do 10º dia e do somatório de ambos, no grupo das pacientes que responderam favoravelmente à estimulação ovariana. Posteriormente foi realizada a correlação dos valores obtidos com a resposta ovariana após a estimulação dos ovários com gonadotrofinas. Resultados: empregando-se a dosagem do FSH no 10º dia (média somada a 2 desvios-padrão) com valor >16,1 UI/mL para predizer a má resposta ovariana ao teste do clomifeno, observaram-se sensibilidade, especificidade e valores preditivos positivo e negativo de 50, 100, 100 e 81,8%, respectivamente. Considerando-se o teste do clomifeno positivo quando o valor do somatório das dosagens do FSH do 3º e do 10º dia somado a dois desvios-padrão foi > 22,6 UI/mL, obtiveram-se sensibilidade de 62,5%, especificidade de 100%, valor preditivo positivo de 100% e valor preditivo negativo de 85,7% . O uso da dosagem única do FSH no 3º dia do ciclo acima de 10 UI/mL para predizer a má resposta ovariana mostrou sensibilidade de 87%, especificidade de 100%, valor preditivo positivo de 100% e valor preditivo negativo de 94,7%. Conclusão: no presente estudo, a dosagem única do FSH no 3º dia do ciclo apresentou maior sensibilidade quando comparada ao teste do clomifeno para a avaliação da reserva ovariana.Purpose: to assess ovarian reserve by FSH determination on the 3rd day of the menstrual cycle compared to the clomiphene test and to correlate the results with the ovarian response to controlled hyperstimulation with gonadotrophins for in vitro fertilization. Methods: a total of 49 patients older than 30 years who had been presenting a clinical picture of infertility for at least 1 year were selected. All patients were evaluated for ovarian reserve by the clomiphene citrate test and 26 of them were later submitted to controlled ovarian hyperstimulation with gonadotrophins. Of these 26 patients, 18 showed a good response to ovarian hyperstimulation and 8 showed a poor response. Mean (+ SD) FSH values were calculated for the determinations on the 3rd and on the 10th day and for their sum in the group of patients who responded favorably to ovarian stimulation, and were later correlated with the ovarian response after gonadotrophin stimulation. Results: employing a FSH value > 16.1 IU/mL on the 10th day (mean plus 2 SD) for the prediction of a poor ovarian response in the clomiphene test, the sensitivity, specificity, and positive and negative predictive values of this parameter were 50, 100, 100 and 81.8%, respectively. Considering the clomiphene test to be positive when the sum of the FSH values determined on the 3rd and 10th day plus 2 SD was > 22.6 IU/mL, we obtained 62.5% sensitivity 100% specificity, 100% positive predictive value, and 85.7% negative predictive value. A single FSH determination of 10 IU/mL on the 3rd day of the cycle for the prediction of a poor ovarian response showed 87% sensitivity, 100% specificity, 100% positive predictive value and 94.7% negative predictive value. Conclusion: in the present study, a single FSH determination on the 3rd day of the cycle showed to be more sensitive than the clomiphene test for the evaluation of ovarian reserve.

Objetivo: avaliar o perfil lipídico (colesterol, triglicerídeos, HDL e LDL) em mulheres portadoras de síndrome dos ovários policísticos (SOP) comparado a mulheres com ciclos menstruais ovulatórios. Métodos: as pacientes foram divididas em grupos, tomando como base o índice de massa corporal, em obesas e não-obesas, permitindo assim observar a influência conjunta da SOP e da obesidade no metabolismo lipídico das mulheres estudadas. Estudamos 117 mulheres, divididas em 4 grupos: grupo I (SOP-obesas) -- n = 33, grupo II (SOP--não-obesas) -- n = 27, grupo III (controle-obesas) -- n = 28 e grupo IV (controle-não obesas) -- n = 29. Resultados: evidenciou-se nas pacientes obesas com ciclos menstruais ovulatórios (grupo III) uma elevação dos níveis de colesterol (179 mg/dl) em relação aos grupos I (147 mg/dl) e II (149 mg/dl) e nos níveis de triglicerídeos (117 mg/dl) em relação ao grupo IV (77 mg/dl) e de LDL (117 mg/dl) em relação ao grupo I (82 mg/dl). Conclusão: esses dados sugerem que as alterações no perfil lipídico, nas pacientes com SOP, estão relacionadas apenas à obesidade.Purpose: to evaluate the lipid profile (cholesterol, triglycerides, HDL and LDL) of women with polycystic ovary syndrome (PCO) and compare it to that of women with ovulatory menstrual cycles. Methods: the patients were divided into two groups, obese and nonobese, based on body mass index, so that it would be possible to determine the joint effect of PCO and obesity on the lipid metabolism of the studied women. We studied 117 women divided into 4 groups: group I (PCO--obese), n = 33; group II (PCO--nonobese), n = 27; group III (control--obese), n = 28; group IV (control--nonobese), n = 29. Results: cholesterol levels were elevated (179 mg/dl) in obese patients with ovulatory cycles (group III) compared to group I (147 mg/dl) and group II (149 mg/dl), as also were triglyceride levels (117 mg/dl) compared to group IV (77 mg/dl) and LDL levels (117 mg/dl) compared to group I (82 mg/dl). Conclusion: these data suggest that alterations in lipid profile are related to obesity only.

Objetivo: o objetivo do presente trabalho é apresentar um perfil das pacientes com endometriose, estadio e resultados do tratamento entre as pacientes atendidas no nosso Serviço. Métodos: foram revistos 155 prontuários médicos de pacientes portadoras de endometriose atendidas no período de 1991 a 1996 segundo um protocolo preestabelecido. Resultados: a média de idade das pacientes foi de 31 anos, sendo que a maioria era branca e com ciclos menstruais regulares. Os sintomas mais observados foram a dismenorréia, dor pélvica e infertilidade, sendo esses sinais e sintomas de grau leve na maioria dos casos. Os métodos diagnósticos utilizados foram a laparoscopia ou ultra-sonografia. Houve concordância entre a ultra-sonografia e a laparoscopia em 96% das vezes. Em 28% dos casos houve necessidade de realização de laparotomia para elucidação diagnóstica e/ou tratamento. O achado de endometrioma ocorreu em 37% das vezes e em apenas 74% das biópsias das lesões houve a confirmação de endometriose pelo estudo anatomopatológico. Houve melhora significativa com o tratamento clínico independente da droga utilizada, com melhora dos sintomas em cerca de 50% delas. Foram realizadas técnicas de fertilização assistida em 34 pacientes, das quais 80% foram fertilização in vitro (FIV), com taxa de 27% de gravidez/paciente. Conclusões: o método diagnóstico de escolha deve ser a laparoscopia, embora a ultra-sonografia apresente um alto índice de concordância. O estudo anatomopatológico das lesões deve ser utilizado como método auxiliar no diagnóstico, pois ele não é positivo em todos os casos. O tratamento clínico com reprodução assistida é uma boa opção terapêutica, principalmente com o uso de FIV.Purpose: to present a profile of endometriosis, stages and results of treatment among the patients seen in our Service. Methods: the medical records of 155 patients with endometriosis treated during the period from 1991 to 1996 according to a preestablished protocol were examined. Results: mean patient age was 31 years, most patients were white, with regular menstrual cycles. The most frequently observed symptoms were dysmenorrhea, pelvic pain and infertility and were mild in most of the cases. The diagnostic methods utilized were laparoscopy or echography. There was agreement between echography and laparoscopy in 96% of cases. Laparotomy was required in 28% of cases for diagnostic elucidation and/or treatment. Endometrioma was detected in 37% of the cases and endometriosis was confirmed in only 74% of the biopsies from the lesion by anatomopathological examination. There was a significant improvement with clinical treatment regardless of the drug used, with improved symptoms in approximately 50% of the patients. Assisted fertilization was performed in 34 patients, consisting of in vitro fertilization (IVF) for 80% of them, with a 27% pregnancy/transfer rate. Conclusions: the diagnostic method of choice should be laparoscopy, although echography presented a high rate of agreement. Anatomopathological examination of the lesions should be used as an adjuvant method in the diagnosis since it is not confirmatory in all of the cases. Clinical treatment with assisted reproductive technologies is a good therapeutic option, especially with the use of IVF.

ABSTRACT Objective To investigate the associations among visceral adiposity index (VAI), lipid accumulation product (LAP), body fat percentage (%), and android/gynoid ratio (A/G ratio) in women with polycystic ovary syndrome (PCOS) and verify if the parameters representative of visceral obesity correlate with and exhibit the same frequency as body composition variables; anthropometric indices; and metabolic, hormonal, and inflammatory parameters. Subjects and methods This was a cross-sectional study that included 94 women with PCOS. Hormonal, metabolic, and inflammatory parameters were analyzed in all women. Free androgen index (FAI) and homeostasis model assessment (HOMA-IR), as well as LAP, VAI, and anthropometric indices, were calculated. The regions of interest (ROIs) in body composition and body composition indices were evaluated using a dual X-ray absorptiometry (DXA). Overall, 32 variables were selected as markers of body fat distribution. Results Among the 32 markers evaluated, 29 correlated with LAP, whereas 25 correlated with VAI, 19 with body fat (%), and 30 with A/G ratio. Additionally, some markers correlated with the four adiposity indices evaluated: ROIs, except for total mass and leg fat (%); body composition (body mass index, waist circumference, and hip circumference) indices; fasting insulin; and C-reactive protein. Conclusion LAP and VAI may be sensitive measures for screening and preventing metabolic syndrome and insulin resistance in PCOS, with LAP being more sensitive than VAI, and the A/G ratio may be more sensitive than body fat percentage.
