Abstract

Heparin-induced thrombocytopenia (HIT) is a major issue in cardiac surgery requiring cardiopulmonary bypass (CPB). HIT represents a severe adverse drug reaction after heparin administration. It consists of immune-mediated thrombocytopenia paradoxically leading to thrombotic events. Detection of antibodies against platelets factor 4/heparin (anti-PF4/H) and aggregation of platelets in the presence of heparin in functional in vitro tests confirm the diagnosis. Patients suffering from HIT and requiring cardiac surgery are at high risk of lethal complications and present specific challenges. Four distinct phases are described in the usual HIT timeline, and the anticoagulation strategy chosen for CPB depends on the phase in which the patient is categorized. In this sense, we developed an institutional protocol covering each phase. It consisted of the use of a non-heparin anticoagulant such as bivalirudin, or the association of unfractionated heparin (UFH) with a potent antiplatelet drug such as tirofiban or cangrelor. Temporary reduction of anti-PF4 with intravenous immunoglobulins (IvIg) has recently been described as a complementary strategy. In this article, we briefly described the pathophysiology of HIT and focused on the various strategies that can be applied to safely manage CPB in these patients.

Abstract

About 15 years ago, Patient Blood Management (PBM) emerged as a new paradigm in perioperative medicine and rapidly found support of all major medical societies and government bodies. Blood products are precious, scarce and expensive and their use is frequently associated with adverse short- and long-term outcomes. Recommendations and guidelines on the topic are published in an increasing rate. The concept aims at using an evidence-based approach to rationalize transfusion practices by optimizing the patient's red blood cell mass in the pre-, intra- and postoperative periods. However, elegant as a concept, the implementation of a PBM program on an institutional level or even in a single surgical discipline like cardiac surgery, can be easier said than done. Many barriers, such as dogmatic ideas, logistics and lack of support from the medical and administrative departments need to be overcome and each center must find solutions to their specific problems. In this paper we present a narrative overview of the challenges and updated recommendations for the implementation of a PBM program in cardiac surgery.

Abstract

For a decade, aortic valve replacement (AVR) surgery has been simplified thanks to the development of sutureless or rapid deployment valves (SU-RD valves), which offer an alternative to sutured valves. These SU-RD valves, inspired by the TAVI (Transcatheter Aortic Valve Implantation), allow significant operating time reduction and facilitate minimally invasive surgery. They are particularly indicated for patients that are more fragile and in combined procedures (for example AVR associated with coronary bypass surgery). Recent studies report high reliability and excellent outcomes with lower morbidity and mortality compared to other AVR modalities, including TAVI. Considering the constant technical improvement, SU-DR valves may become the new standard in AVR.

Abstract

A measurement of vector boson (V) production in conjunction with a D*(2010)(+) meson is presented. Using a data sample corresponding to 9.7 fb(-1) of proton-antiproton collisions at center-of-mass energy root s = 1.96 TeV produced by the Fermilab Tevatron, we reconstruct V + D*+ samples with the CDF II detector. The D*+ is fully reconstructed in the D*(2010)(+) -> D-0 (-> K-pi(+))pi(+) decay mode. This technique is sensitive to the associated production of vector boson plus charm or bottom mesons. We measure the ratio of production cross sections sigma(W + D*) = sigma(W) = [1.75 +/- 0.13(stat) +/- 0.09(stat)]% and sigma(Z + D*)/sigma(Z) = [1.5 +/- 0.4(stat) +/- 0.2(stat)]% and perform a differential measurement of d sigma(W + D*)/dp(T)(D*). Event properties are utilized to determine the fraction of V + D*(2010)(+) events originating from different production processes. The results are in agreement with the predictions obtained with the PYTHIA program, limiting possible contribution from non-standard-model physics processes.

Abstract

An updated measurement of the single top quark production cross section is presented using the full data set collected by the Collider Detector at Fermilab (CDF), corresponding to 9.5 fb(-1) of integrated luminosity from proton-antiproton collisions at 1.96 TeV center-of-mass energy. The events selected contain an imbalance in the total transverse momentum, jets identified as containing b quarks, and no identified leptons. The sum of the s- and t-channel single top quark cross sections is measured to be 3.53(-1.16)(+1.25) pb and a lower limit on the magnitude of the top-to-bottom quark coupling, vertical bar V-tb vertical bar of 0.63, is obtained at the 95% credibility level. These measurements are combined with previously reported CDF results obtained from events with an imbalance in total transverse momentum, jets identified as originating from b quarks, and one identified lepton. The combined cross section is measured to be 3.02(-0.48)(+0.49) pb and a lower limit on vertical bar V-tb vertical bar of 0.84 is obtained at the 95% credibility level.

Abstract

A measurement of vector boson (V) production in conjunction with a D*(2010)(+) meson is presented. Using a data sample corresponding to 9.7 fb(-1) of proton-antiproton collisions at center-of-mass energy root s = 1.96 TeV produced by the Fermilab Tevatron, we reconstruct V + D*+ samples with the CDF II detector. The D*+ is fully reconstructed in the D*(2010)(+) -> D-0 (-> K-pi(+))pi(+) decay mode. This technique is sensitive to the associated production of vector boson plus charm or bottom mesons. We measure the ratio of production cross sections sigma(W + D*) = sigma(W) = [1.75 +/- 0.13(stat) +/- 0.09(stat)]% and sigma(Z + D*)/sigma(Z) = [1.5 +/- 0.4(stat) +/- 0.2(stat)]% and perform a differential measurement of d sigma(W + D*)/dp(T)(D*). Event properties are utilized to determine the fraction of V + D*(2010)(+) events originating from different production processes. The results are in agreement with the predictions obtained with the PYTHIA program, limiting possible contribution from non-standard-model physics processes.

Abstract

An updated measurement of the single top quark production cross section is presented using the full data set collected by the Collider Detector at Fermilab (CDF), corresponding to 9.5 fb(-1) of integrated luminosity from proton-antiproton collisions at 1.96 TeV center-of-mass energy. The events selected contain an imbalance in the total transverse momentum, jets identified as containing b quarks, and no identified leptons. The sum of the s- and t-channel single top quark cross sections is measured to be 3.53(-1.16)(+1.25) pb and a lower limit on the magnitude of the top-to-bottom quark coupling, vertical bar V-tb vertical bar of 0.63, is obtained at the 95% credibility level. These measurements are combined with previously reported CDF results obtained from events with an imbalance in total transverse momentum, jets identified as originating from b quarks, and one identified lepton. The combined cross section is measured to be 3.02(-0.48)(+0.49) pb and a lower limit on vertical bar V-tb vertical bar of 0.84 is obtained at the 95% credibility level.

Abstract

Debate about Indonesian rice policy has focused on estimates of production and consumption levels, and the level of imports they imply. However, Indonesian rice production and consumption data are controversial. Rice consumption as estimated from household survey data is much lower than officially reported rice production. This suggests that Indonesia is a net rice exporter, but in fact it has generally been a net importer. Some researchers argue that rice consumption data are underestimated; others contend that production is over-estimated because of inaccuracies in 'eye estimates' of harvested area. This paper reviews how rice production and consumption are measured, notes major weaknesses, and surveys attempts to reconcile consumption and production data and examine their consistency with rice import data. It concludes that rice prices are the only accurate indicator of the balance between supply and demand, and hence of the appropriate level of imports.

Abstract

Clergy members are frequently cited as a potential source of community support to victims of sexual abuse or assault, as well as one likely avenue for disclosure. However, research and curricula on the topic are poorly developed, and studies show that victims' interactions with clergy are often negative. In the current study, a series of focus groups were conducted with clergy members of several denominations as part of a multi-faceted effort to improve the community response to victims in one state. Participating clergy reported inadequate education, support, and connections to community resources to respond adequately to the needs of sexual assault victims. Abstract

Introduction: Real-world outcomes with the HeartMate 3 left ventricular assist device (LVAD) depending on whether it's a bridge to transplantation (BTT) or destination therapy (DT) are poorly studied. We aimed to compare the profile and clinical outcomes of patients supported with HeartMate 3 according to a BTT or a DT pre-implantation strategy., Methods: All patients consecutively implanted with HeartMate 3 at our centre (University Hospital of Lausanne, Switzerland) in 2015-2022 were analysed in a retrospective observational study. Indications for HeartMate 3 implantation were advanced heart failure despite optimal medical treatment. Patients were treated with a vitamin K antagonist anticoagulant combined with antiplatelet therapy after HeartMate 3 implantation and were followed up monthly at our institution., Results: Among 71 patients implanted with HeartMate 3 between 2015 and 2022, 51 (71.8%) were implanted as a BTT and 20 (28.2%) as DT. Their median age was 58 (IQR: 52-69) years and 84% of patients were classified as INTERMACS profiles 2-4. The median follow-up duration was 18.3 (IQR: 7.5-33.9) months. Patients in the DT group were older than those in the BTT group (p <0.001) and had more chronic renal failure (p <0.001). They also had a lower 5-year survival rate (mean ± standard error: 87.3 ± 5.6% vs 49.4 ± 15.1%) and more adverse events such as renal dysfunction requiring temporary perioperative dialysis (p = 0.08) or bleeding (p = 0.06)., Conclusion: Although patients supported with HeartMate 3 have favourable survival, those with LVAD-DT have poorer outcomes. There is a need to better select patients eligible for LVAD-DT in order to limit the burden of adverse events and improve their prognosis.

Abstract

Background and Objectives : Dexmedetomidine, an alpha-2 agonist, is used as an adjunct to anesthesia in enhanced recovery after surgery (ERAS) programs. One of its advantages is the opioid-sparing effect which can facilitate early extubation and recovery. When the ERAS cardiac society was set in 2017, our facility was already using the ERAS program, in which the "fast-track Anesthesia" was facilitated by the intraoperative infusion of dexmedetomidine. Our objective is to share our experience and investigate the potential impact of intraoperative dexmedetomidine use as a part of the ERAS program on patient outcomes in elective cardiac surgery. Materials and Methods : An observational retrospective cohort study was conducted at a university hospital in Switzerland. The patients who underwent elective cardiac surgery with cardiopulmonary bypass between 1 June 2017 and 31 August 2018 were included in this analysis ( n = 327). Regardless of the surgery type, all the patients received a standardized fast-track anesthesia protocol inclusive of dexmedetomidine infusion, reduced opioid dose, and parasternal nerve block. The primary outcome was the postoperative time when the criteria for extubation were met. Three groups were identified: group 0-(extubated in the operating room), group < 6 (extubated in less than 6 h), and group > 6 (extubated in >6 h). The secondary outcomes were adverse events, length of stay in ICU and in hospital, and total hospitalization costs. Results : Dexmedetomidine was well-tolerated, with no significant adverse events reported. Early extubation was performed in 187 patients (57%). Group 3 had a significantly longer length of stay in the ICU (median: 70 h vs. 25 h) and in hospital (17 vs. 12 days), and consequently higher total hospitalization costs (CHF 62,551 vs. 38,433) compared to the net data from the other two groups ( p < 0.0001). Conclusions : Our findings suggest that dexmedetomidine can be safely used as part of the opioid-sparing anesthesia protocol in patients undergoing elective cardiac surgery with cardiopulmonary bypass with the potential to facilitate early extubation, shorter ICU and hospital stays, and reduced hospitalization costs.

Background: Multidisciplinary Heart Teams (HTs) play a central role in the management of valvular heart diseases. However, the comprehensive evaluation of patients' data can be hindered by logistical challenges, which in turn may affect the care they receive., Aims: This study aimed to explore the ability of artificial intelligence (AI), particularly large language models (LLMs), to improve clinical decision-making and enhance the efficiency of HTs., Methods: Data from patients with severe aortic stenosis presented at HT meetings were retrospectively analysed. A standardised multiple-choice questionnaire, with 14 key variables, was processed by the OpenAI Chat Generative Pre-trained Transformer (GPT)-4. AI-generated decisions were then compared to those made by the HT., Results: This study included 150 patients, with ChatGPT agreeing with the HT's decisions 77% of the time. The agreement rate varied depending on treatment modality: 90% for transcatheter valve implantation, 65% for surgical valve replacement, and 65% for medical treatment., Conclusions: The use of LLMs offers promising opportunities to improve the HT decision-making process. This study showed that ChatGPT's decisions were consistent with those of the HT in a large proportion of cases. This technology could serve as a failsafe, highlighting potential areas of discrepancy when its decisions diverge from those of the HT. Further research is necessary to solidify our understanding of how AI can be integrated to enhance the decision-making processes of HTs.

Therefore, we evaluated whether transient heat application during EVLP (thermal preconditioning [TP]) might recondition damaged lungs before LTx. TP was performed during EVLP (3 hours) of rat lungs damaged by warm ischemia by transiently heating (30 minutes, 41.5 °C) the EVLP perfusate, followed by LTx (2 hours) reperfusion. We also assessed the TP (30 minutes, 42 °C) during EVLP (4 hours) of swine lungs damaged by prolonged cold ischemia. In rat lungs, TP induced HSP expression, reduced nuclear factor κB and inflammasome activity, oxidative stress, epithelial injury, inflammatory cytokines, necroptotic death signaling, and the expression of genes involved in innate immune and cell death pathways. After LTx, heated lungs displayed reduced inflammation, edema, histologic damage, improved compliance, and unchanged oxygenation. In pig lungs, TP induced HSP expression, reduced oxidative stress, inflammation, epithelial damage, vascular resistance, and ameliorated compliance. Abstract

This study applies an emerging analytical technology, wNMR (water proton nuclear magnetic resonance), to assess the stability of aluminum adjuvants and antigen-adjuvant complexes against physical stresses, including gravitation, flow and freeze/thaw. Results from wNMR are verified by conventional analytical technologies, including static light scattering and microfluidic imaging. 46. How to Solve the Conundrum of Heparin-Induced Thrombocytopenia during Cardiopulmonary Bypass.

Abstract

Heparin-induced thrombocytopenia (HIT) is a major issue in cardiac surgery requiring cardiopulmonary bypass (CPB). HIT represents a severe adverse drug reaction after heparin administration. It consists of immune-mediated thrombocytopenia paradoxically leading to thrombotic events. Detection of antibodies against platelets factor 4/heparin (anti-PF4/H) and aggregation of platelets in the presence of heparin in functional in vitro tests confirm the diagnosis. Patients suffering from HIT and requiring cardiac surgery are at high risk of lethal complications and present specific challenges. Four distinct phases are described in the usual HIT timeline, and the anticoagulation strategy chosen for CPB depends on the phase in which the patient is categorized. In this sense, we developed an institutional protocol covering each phase. It consisted of the use of a non-heparin anticoagulant such as bivalirudin, or the association of unfractionated heparin (UFH) with a potent antiplatelet drug such as tirofiban or cangrelor. Temporary reduction of anti-PF4 with intravenous immunoglobulins (IvIg) has recently been described as a complementary strategy. In this article, we briefly described the pathophysiology of HIT and focused on the various strategies that can be applied to safely manage CPB in these patients.

Abstract

Alternative accesses for transcatheter aortic valve replacement (TAVR) are needed in a certain number of patients, who present contraindications to the standard transfemoral access. We present the transcervical approach, which allows to access the aortic valve via supra-aortic vessels, namely, the brachiocephalic trunk, carotid artery, or subclavian artery. This approach is interesting because it avoids thoracotomy, which is necessary with some other alternative approaches for TAVR, such as the transaortic or transapical ones. Although some points still need clarification, such as the best anesthesia modality (general or local) or the optimal side for vessel access, data suggest the transcervical access might yield similar periprocedural and 30-day outcomes compared with the transfemoral access. Abstract

Background: Patients with hereditary diffuse gastric cancer (HDGC) and germline mutations in the E-cadherin gene, CDH1 , have a very high cumulative lifetime risk of developing diffuse gastric cancer. In these patients, it is formally recommended to perform a prophylactic total gastrectomy (PTG). Materials and Methods: We analyzed the course of patients with HDGC who have undergone PTG in our institution. Pedigree analysis, preoperative screening results, operative course, postoperative data, and complete stomach pathologic examination were performed in all patients. Results: Seven patients with confirmed CDH1 mutation underwent PTG, five were women, and average age was 27 years (range 17-42). Signet ring cell carcinoma was found in 1 patient in the preoperative surveillance endoscopic biopsies. Laparoscopic PTG was performed in all patients. There were two complications, an intestinal obstruction that required reintervention and an asymptomatic esophagojejunal anastomosis leak that resolved with conservative treatment. In all gastrectomy specimens, intramucosal signet ring cell carcinoma foci limited to the lamina propria were found (range 1-31), 83.5% were in the body-fundus region. The mean follow-up was 28.5 months (range 8-72). The mean weight loss was 9% (range 2-18). Postoperative symptoms associated with Dumping syndrome were the most frequent. All the patients reported of being very satisfied with the procedure and of having a better quality of life than expected before the procedure. Conclusion: Laparoscopic PTG is an excellent resource to prevent the development of advanced diffuse gastric cancer (DGC) in patients with HDGC with CDH1 mutation. The procedure was well tolerated with a high satisfaction rate and very good functional results. It should be considered in these patients due to the high risk of developing advanced DGC and the lack of effective and reliable surveillance studies.
