Abstract

Introduction. A classification of genetic variants’ pathogenicity associated to hereditary recurrent fevers and the novel Eurofever/PRINTO classification criteria (EPCC) have been recently developed. Objectives: to evaluate the clinical impact of EPCC criteria and new INSAID pathogenicity classification of MEFV variants in the large international Eurofever FMF cohort. Methods: baseline demographic, genetic and clinical data of FMF patients included in the Eurofever registry were analysed. Genetic and clinical EPCC criteria for FMF were applied. MEFV variants were classified according to the new INSAID classification. Results: Since November 2009, clinical information was available for 1012 FMF (532 males/480 females, 827 children/185 adults) from 119 centres. For 125 patients clinical and genetic data mandatory for the application of EPCC were missing. Among the 887 remaining patients 623 (70.2%) satisfied EPCC (EPPC+), while 264 (29.8%) did not (EPPC-). Most of the EPCC- patients (172, 65.1%) displayed negative or non-informative genetics (monoallelic or biallelic benign variants, monoallelic variant of unknown significance. At baseline, Colchicine was used in most of EPCC+ patients (88%) and in a minor percentage of EPCC- patients (69 %, p < 0.0001), who were treated in a higher proportion with steroid or NSAID on demand (p = 0.003 and 0.008, respectively). Anti-IL-1 treatment was used in 4% of patients. Conclusions: The combination of EPCC and the new classification of genetic variants’ pathogenicity captured the majority of FMF patients in the Eurofever cohort in a homogeneous group. EPPC- patients were characterized by a different phenotype and therapeutic approach.

Abstract

Objective: Musculoskeletal ultrasonography (US) has the potential to be an important tool in the assessment of disease activity in childhood arthritides. To assess pathology, clear definitions for synovitis need to be developed first. The aim of this study was to develop and validate these definitions through an international consensus process. Methods: The decision on which US techniques to use and the components to be included in the definitions, as well as the final wording, were developed by 31 US experts in a consensus process. A Likert scale of 1–5 (where 1 = complete disagreement and 5 = complete agreement) was used. A minimum of 80% of the experts scoring 4 or 5 was required for final approval. The definitions were then validated on 120 standardized US images of the wrist, metacarpophalangeal joints, and tibiotalar joints, displaying various degrees of synovitis at various ages. Results: B-mode and Doppler should be used for assessing synovitis in children. A US definition of the various components (i.e., synovial hypertrophy, effusion, and Doppler signal within the synovium) was developed. The definition was validated on still images with a median of 89% of participants (range 80–100) scoring it as 4 or 5 on a Likert scale. Conclusion: US definitions of synovitis and its elementary components covering the entire pediatric age range were successfully developed through a Delphi process and validated in a web-based still-images exercise. These results provide the basis for the standardized US assessment of synovitis in clinical practice and research.

Abstract

Le syndrome CACP (camptodactylie-arthropathie-coxa vara-péricardite) est une maladie génétique rare souvent confondue avec l'arthrite juvénile idiopathique (AJI). Il se caractérise par une arthropathie non inflammatoire, une déformation du bassin (coxa vara), une péricardite stérile, et une camptodactylie. Cependant, certains de ces signes cliniques sont souvent absents chez les enfants, ce qui peut entraîner un retard au diagnostic. La connaissance des éléments clés du syndrome CACP, notamment les caractéristiques observées à l'imagerie, peut aider à différencier cette maladie de l'AJI. L'objectif de cette étude est de mettre en évidence, à travers l'examen de trois cas (deux filles et un garçon), les signes distinctifs à l'imagerie permettant d'orienter les cliniciens vers le diagnostic de CACP. Les caractéristiques cliniques et biologiques de nos patients sont résumées dans le Tableau 1. Les trois patients, âgés en moyenne de 15 ans au moment de leur consultation dans notre service, ont tous subi un retard diagnostique important (leur maladie ayant débuté vers l'âge de 5 ans). À l'évaluation initiale, ils présentaient une atteinte polyarticulaire bilatérale étendue, avec gonflement et déformations articulaires multiples. Plusieurs traitements antirhumatismaux, dont des infiltrations articulaires et immunomodulateurs, ont été essayés sans succès ayant conduit à la réalisation d'examens d'imagerie complémentaires. Les radiographies ont révélé un aplatissement et un élargissement des épiphyses (radius, ulna, condyles fémoraux) avec un rétrécissement articulaire progressif (poignets, doigts) chez tous les patients. Bien qu'aucun patient ne présentait de coxa vara au moment du diagnostic, un aplatissement des têtes fémorales et des cols fémoraux courts a été observé. Tous les patients présentaient également une ostéopénie/ostéoporose diffuse. L'échographie articulaire a mis en évidence un épanchement avec des spots hyperéchogènes, une hypertrophie synoviale modérée sans signal Doppler, et parfois des ténosynovites (Fig. 1). L'IRM a confirmé un épanchement avec une prise de contraste synovial. Face à la suspicion d'une maladie osseuse constitutionnelle, une analyse génétique a été réalisée, identifiant une double mutation dans le gène PRG4 chez tous les patients, confirmant ainsi le diagnostic de CACP. En conclusion, chez un enfant présentant une raideur et des déformations articulaires, en particulier en cas d'échec des traitements classiques, le syndrome CACP doit être envisagé comme diagnostic différentiel. Abstract

Background: Systemic sclerosis encompasses a range of disorders characterized by vascular and connective tissue abnormalities. Although rare in pediatrics, juvenile systemic sclerosis (jSSc) is a severe and life-threatening condition that significantly impacts children's development. This study aimed to provide an overview of JSSc in France over the past decade., Methods: Patients with disease onset before the age of 16 were included following a request for observations sent via email to member practitioners of the SOFREMIP (French pediatric Rheumatology society)., Results: Our study included 18 patients from 8 different French centers. While our cohort exhibited a balanced distribution between limited and diffuse subsets of the disease, we observed a higher prevalence of the diffuse subset in children above the age of 10. Skin induration was the most reported symptom, while Raynaud's phenomenon was present in 61% of the children at initial clinical evaluation. All children tested positive for antinuclear antibodies, with anti-Scl70 being the most common specificity, even among children with limited cutaneous subsets. Interestingly, we found a high sensitivity of the ACR / EULAR criteria for diagnosing jSSc in our cohort with 83% of patients meeting these criteria, except for 3 children who presented with overlap syndromes. Despite the frequent use of corticosteroids at the onset, no deaths or renal crises were reported. Three patients received treatment with biological agents, specifically Rituximab and Tocilizumab., Conclusion: JSSc is a rare but severe disease requiring rapid, specialized, and multidisciplinary care. Further studies are needed to validate proper diagnosis criteria including overlap syndromes and evaluate the use of biotherapies in children., Competing Interests: Declarations.
