1. Non-invasive and invasive assessment of carbohydrate intakes and muscle glycogen utilisation in Rugby League and AFL

Abstract

The importance of muscle glycogen availability for both endurance and high-intensity intermittent exercise performance has been recognised for several decades. The measurement of muscle glycogen in human skeletal muscle has traditionally been performed through the invasive muscle biopsy technique. However, recent data from laboratory-based cycling studies suggests that ultrasound technology (using commercially based software known as MuscleSound) provides a valid and non-invasive assessment tool for which to measure exercise-induced glycogen utilization. Nonetheless, no data currently exist to evaluate the validity of the MuscleSound software in team sport athletes in response to field-based training or match play situations. Accordingly, the aim of the present thesis was to therefore examine the reliability and validity of ultrasound technology (via the MuscleSound system) to non-invasively assess muscle glycogen concentration in skeletal muscle of team sport athletes. The aim of Study 1 (Chapter 3) was to quantify the test re-test reproducibility of the MuscleSound scoring system between (vastus lateralis, VL; vastus medalis, VM; rectus femoris RF) and within (25, 50 and 75% of the muscle length) muscles. Using a test-retest (60 minutes apart) scanning protocol in 100 recreationally active subjects (78 males, 22 females), it was identified that 50% of the VL muscle provided the most reliable anatomical site for which to obtain reproducible MuscleSound scores, as evidenced from the coefficient of variation (CV) - 5.8, intraclass correlation (ICC) - 0.89 (0.85 - 0.93), and ratio limits of agreement 1.02 (1.17) reliability statistics. Having identified the optimal anatomical location for which to obtain reproducible MuscleSound scores, the aim of Study 2 (Chapter 4) was to evaluate the validity of the MuscleSound scoring system to non-invasively assess muscle glycogen utilisation in elite male Rugby League players (n=16) during competitive match play. Although match play depleted absolute muscle glycogen in the VL muscle by approximately 200-250 mmol·kg-1 dw, there were no significant differences in MuscleSound scores obtained before and after the game. As such, these data suggest that MuscleSound is not a valid tool to assess muscle glycogen utilisation during the high-intensity intermittent activity profiles that are inherent to team sports. Given that these data are in contrast to previous validation studies conducted with cycling-based protocols, it was suggested that differences in mechanical loads and fluids shifts between exercise protocols may explain the discrepancies between studies. To eliminate the potential confounding effects of acute changes in fluid shifts associated with differing exercise stimuli, the aim of Study 3 (Chapter 5) was to assess the validity of the MuscleSound system to detect changes in muscle glycogen re-synthesis across a time-scale in which acute changes in fluid shifts have been restored to pre-exercise values. Using a combination of glycogen depleting cycling-based exercise followed by 36 h of an isonenergetic high CHO or low CHO diet, resting muscle glycogen of the VL muscle (from 16 recreationally active males) was approximately 500 versus 250 mmol·kg-1 dw in the high and low CHO trials, respectively. However, no differences were observed in the MuscleSound scores obtained between the high and low trials. When considered with the results of Chapter 4, these data collectively suggest that the MuscleSound system is unable to detect differences in muscle glycogen concentration within the physiological range that typically occurs as a result of exercise-induced glycogen utilisation and/or post-exercise muscle glycogen re-synthesis. Given that MuscleSound was not deemed a valid measurement tool, it was recommended that evaluations of the daily CHO and muscle glycogen requirements of team sport athletes should therefore be undertaken using traditional assessments of energy intake and muscle biopsies, respectively. Accordingly, the habitual CHO intakes of 44 elite professional male Australian Rules Football (AFL) players were quantified across the weekly micro-cycle in Study 4 (Chapter 6). These data demonstrate that elite AFL players practice elements of daily CHO periodisation in accordance with fluctuations in daily training load and proximity to the match day itself. Nonetheless, such data also highlighted that players do not consume CHO at the recommended dose to achieve glycogen storage that is likely required to facilitate optimal match day physical performance. In Study 5 (Chapter 7), muscle glycogen utilisation in the VL muscle was subsequently quantified using the muscle biopsy technique in two elite professional males during competitive match play. Importantly, players commenced the match after adhering to a 24 h CHO loading protocol of 8 g·kg-1 and after consuming a pre-match meal of 2 g·kg-1. Data demonstrate that such a dietary protocol achieves pre-match muscle glycogen concentration >500 mmol·kg-1 dw and that match play can induce an absolute glycogen utilisation >400 mmol·kg-1 dw. However, with the different demands observed between positions, total glycogen utlisation may vary. As such, it appears that the metabolic demands of AFL maybe more CHO dependent than other invasive team sports such as soccer and rugby. In summary, the work undertaken in this thesis has demonstrated that the MuscleSound software is not valid to detect differences in muscle glycogen concentration within the physiological range (i.e. 200-300 mmol·kg-1 dw) that typically occurs as a result of exercise-induced glycogen utilisation and/or post-exercise muscle glycogen re-synthesis. For this reason, assessment of the CHO requirements of team sport athletes is likely dependent on the traditional approaches of dietary assessment and muscle biopsy. Using the latter methods, the present data also demonstrate that the CHO requirements of AFL match play is potentially different (though larger sample sizes are required) than other team sports such as soccer and rugby. Further studies are also required to quantify the glycogen requirement of the typical training sessions undertaken by professional players of specific team sports.

Abstract

Introduction: Today, endovascular treatment (EVT) is the therapy of choice for strokes due to acute large vessel occlusion, irrespective of prior thrombolysis. This necessitates fast, coordinated multi-specialty collaboration. Currently, in most countries, the number of physicians and centres with expertise in EVT is limited. Thus, only a small proportion of eligible patients receive this potentially life-saving therapy, often after significant delays. Hence, there is an unmet need to train a sufficient number of physicians and centres in acute stroke intervention in order to allow widespread and timely access to EVT. Aim: To provide multi-specialty training guidelines for competency, accreditation and certification of centres and physicians in EVT for acute large vessel occlusion strokes. Material and methods: The World Federation for Interventional Stroke Treatment (WIST) consists of experts in the field of endovascular stroke treatment. This interdisciplinary working group developed competency – rather than time-based – guidelines for operator training, taking into consideration trainees’ previous skillsets and experience. Existing training concepts from mostly single specialty organizations were analysed and incorporated. Results: The WIST establishes an individualized approach to acquiring clinical knowledge and procedural skills to meet the competency requirements for certification of interventionalists of various disciplines and stroke centres in EVT. WIST guidelines encourage acquisition of skills using innovative training methods such as structured supervised high-fidelity simulation and procedural performance on human perfused cadaveric models. Conclusions: WIST multispecialty guidelines outline competency and quality standards for physicians and centres to perform safe and effective EVT. 4. Clinical Outcomes of Percutaneous Coronary Intervention for Bifurcation Lesions According to Medina Classification

Abstract

Background Coronary bifurcation lesions (CBLs) are frequently encountered in clinical practice and are associated with worse outcomes after percutaneous coronary intervention. However, there are limited data around the prognostic impact of different CBL distributions. Methods and Results All CBL percutaneous coronary intervention procedures from the prospective e‐Ultimaster (Prospective, Single‐Arm, Multi Centre Observations Ultimaster Des Registry) multicenter international registry were analyzed according to CBL distribution as defined by the Medina classification. Cox proportional hazards models were used to compare the hazard ratio (HR) of the primary outcome, 1‐year target lesion failure (composite of cardiac death, target vessel–related myocardial infarction, and clinically driven target lesion revascularization), and its individual components between Medina subtypes using Medina 1.0.0 as the reference category. A total of 4003 CBL procedures were included. The most prevalent Medina subtypes were 1.1.1 (35.5%) and 1.1.0 (26.8%), whereas the least prevalent was 0.0.1 (3.5%). Overall, there were no significant differences in patient and procedural characteristics among Medina subtypes. Only Medina 1.1.1 and 0.0.1 subtypes were associated with increased target lesion failure (HR, 2.6 [95% CI, 1.3–5.5] and HR, 4.0 [95% CI, 1.6–9.0], respectively) at 1 year, compared with Medina 1.0.0, prompted by clinically driven target lesion revascularization (HR, 3.1 [95% CI, 1.1–8.6] and HR, 4.6 [95% CI, 1.3–16.0], respectively) as well as cardiac death in Medina 0.0.1 (HR, 4.7 [95% CI, 1.0–21.6]). No differences in secondary outcomes were observed between Medina subtypes. Conclusions In a large multicenter registry analysis of coronary bifurcation percutaneous coronary intervention procedures, we demonstrate prognostic differences in 1‐year outcomes between different CBL distributions, with Medina 1.1.1 and 0.0.1 subtypes associated with an increased risk of target lesion failure.

Abstract

AIMS Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) collecting the opinion of the cardiology community on the invasive management of acute coronary syndrome (ACS), before and after the MATRIX trial presentation at the American College of Cardiology (ACC) 2015 Scientific Sessions. METHODS AND RESULTS A web-based survey was distributed to all individuals registered on the EuroIntervention mailing list (n=15,200). A total of 572 and 763 physicians responded to the pre- and post-ACC survey, respectively. The radial approach emerged as the preferable access site for ACS patients undergoing invasive management with roughly every other responder interpreting the evidence for mortality benefit as definitive and calling for a guidelines upgrade to class I. The most frequently preferred anticoagulant in ACS patients remains unfractionated heparin (UFH), due to higher costs and greater perceived thrombotic risks associated with bivalirudin. However, more than a quarter of participants declared the use of bivalirudin would increase after MATRIX. CONCLUSIONS The MATRIX trial reinforced the evidence for a causal association between bleeding and mortality and triggered consensus on the superiority of the radial versus femoral approach. The belief that bivalirudin mitigates bleeding risk is common, but UFH still remains the preferred anticoagulant based on lower costs and thrombotic risks.

Abstract

AIMS Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) concerning opinion on the evidence relating to dual antiplatelet therapy (DAPT) duration after coronary stenting. METHODS AND RESULTS Results from three randomised clinical trials were scheduled to be presented at the American Heart Association Scientific Sessions 2014 (AHA 2014). A web-based survey was distributed to all individuals registered in the EuroIntervention mailing list (n=15,200) both before and after AHA 2014. A total of 1,134 physicians responded to the first (i.e., before AHA 2014) and 542 to the second (i.e., after AHA 2014) survey. The majority of respondents interpreted trial results consistent with a substantial equipoise regarding the benefits and risks of an extended versus a standard DAPT strategy. Two respondents out of ten believed extended DAPT should be implemented in selected patients. After AHA 2014, 46.1% of participants expressed uncertainty about the available evidence on DAPT duration, and 40.0% the need for clinical guidance. CONCLUSIONS This EAPCI survey highlights considerable uncertainty within the medical community with regard to the optimal duration of DAPT after coronary stenting in the light of recent reported trial results. Updated recommendations for practising physicians to guide treatment decisions in routine clinical practice should be provided by international societies.

20. Second-Generation Drug-Eluting Balloon for Ostial Side Branch Lesions (001-Bifurcations): Mid-Term Clinical and Angiographic Results

Abstract

Background: In the drug-eluting stent era, the best strategy to treat Medina 001 lesion remains unestablished. This is the first prospective registry assessing the efficacy and safety of the second-generation drug-coated balloon in patients with side-branch ostial lesion. Methods: Forty-nine patients with de novo Medina 001 lesion and associated myocardial ischemia were treated with second-generation drug-coated balloon-Dior balloon catheter (Eurocor GmbH, Bonn Germany), and prospectively included in this study. After mandatory pre-dilatation, a paclitaxel-eluting balloon was inflated for a minimum of 45 seconds. Left main bifurcation, severely calcified lesions and cardiogenic shock, were the only exclusion criteria. Results: The inclusion period was 2.7 years. Mean age was 62 +/- 12 years old, 41% diabetic, 65% presented with acute coronary syndrome. The most common vessel treated was the first diagonal (50%). Pre-dilatation with a cutting balloon was used in 59%. Angiographic success was 86% (in 14% a bare metal stent was implanted because of acute recoil [n = 5] or coronary dissection more than type B [n = 2]). At a mean of 12.2 +/- 2.2 months, major cardiac adverse events rate was 14.3% (1 myocardial infarction, 0 cardiac deaths, 7 target lesion revascularization). There was no thrombosis or occlusion. At a mean of 7.2 +/- 1.1 months, binary restenosis was 22.5% (n = 7) with a late loss of 0.32 +/- 0.73 mm. Conclusion: Medina 001 lesion is an infrequent type of coronary lesion. Drug-coated balloon-Dior is a safe and technically easy therapeutic option, associated with acceptable mid-term clinical outcomes.

Abstract

Aims: Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) concerning opinion on the evidence relating to dual antiplatelet therapy (DAPT) duration after coronary stenting. Methods and results: Results from three randomised clinical trials were scheduled to be presented at the American Heart Association Scientific Sessions 2014 (AHA 2014). A web-based survey was distributed to all individuals registered in the EuroIntervention mailing list (n=15,200) both before and after AHA 2014. A total of 1,134 physicians responded to the first (i.e., before AHA 2014) and 542 to the second (i.e., after AHA 2014) survey. The majority of respondents interpreted trial results consistent with a substantial equipoise regarding the benefits and risks of an extended versus a standard DAPT strategy. Two respondents out of ten believed extended DAPT should be implemented in selected patients. After AHA 2014, 46.1% of participants expressed uncertainty about the available evidence on DAPT duration, and 40.0% the need for clinical guidance. Conclusions: This EAPCI survey highlights considerable uncertainty within the medical community with regard to the optimal duration of DAPT after coronary stenting in the light of recent reported trial results. Updated recommendations for practising physicians to guide treatment decisions in routine clinical practice should be provided by international societies.

Abstract

While the field of leadership studies includes a large corpus of literature and studies, the literature and scientific research in the field of e-leadership and soft skills used in learning game environments are at present small in scale. Towards contributing to this newly emerging field of literature and study, this research paper presents a new model, the e-Leadership and Soft Skills Educational Design Model (ELESS) for assessing the use and constructing the effective design of soft skills and leadership skills in multiplayer learning game environments. The central research question considers: what is the role of e-leadership and how can it be designed and tested in the context of educational games? To address this question, the research paper includes a review of the current literature around distributed (shared) leadership models and presents some examples of studies assessing how leadership is designed, used and tested in available game environments. The paper then presents the ELESS model and validates it within the context of a case study of the Leadership Game. As a result, the ELESS model can be used to test the efficacy of existing games and to inform the effective design of new games that focus upon developing leadership and soft skills.

Abstract

Summary Objective: To determine the relative β1 -selectivity of three β-blockers (nebivolol, bisoprolol and atenolol), administered orally at normal therapeutic doses, by assessing their impact on the β2 -mediated, haemodynamic and biochemical responses to a terbutaline infusion, which decreases serum potassium and increases serum glucose and insulin. Methods: Twenty-four healthy volunteers (14 men, 10 women), with no history of respiratory disease, attended on five separate occasions; β-blockers (nebivolol 5 mg, bisoprolol 10 mg, atenolol 50 and 100 mg) or placebo were supplied in random order. Three baseline blood samples were collected at 65–85 min post-β-blocker. A 60-min terbutaline infusion was started 90 min after taking the β-blocker. Blood samples were taken and blood pressure and heart rate recorded at 15 min intervals up to 30-min post-infusion. Blood samples were analysed for serum potassium, glucose and insulin concentrations. Results: Terbutaline increased heart rate. Pretreatment with nebivolol caused a modest and non-significant reduction in terbutaline-induced tachycardia whilst bisoprolol produced a more marked effect. Atenolol at both 50 and 100 mg doses caused a highly significant reduction in terbutaline-induced tachycardia. All active preparations had a comparable impact on the terbutaline-induced increase in systolic blood pressure, but the drugs had no impact on the changes produced in diastolic blood pressure. After pretreatment with placebo, the terbutaline infusion caused a significant decrease in serum potassium and increases in serum glucose and insulin. Pretreatment with nebivolol had no discernible effect on potassium compared with placebo. In contrast, when compared with either placebo or nebivolol, bisoprolol (P < 0·01) and both doses of atenolol (P < 0·001) significantly attenuated the hypokalaemic effect of terbutaline. Abstract

Conventionally angiotensin-converting enzyme (ACE) inhibitors are contraindicated in patients with aortic stenosis. Abundant evidence is now available showing that angiotensin II has a central role in the development of left ventricular hypertrophy (LVH), myocardial contractile failure and diastolic dysfunction in response to pressure overload. In animal models, ACE inhibitors have been shown to attenuate these pathological responses. In humans there is no such evidence available, however uncontrolled studies have shown that these agents are not only tolerated but are associated with acute improvements in haemodynamics and diastolic function. Further studies are merited to assess the possible role of ACE inhibitors in aortic stenosis both before and after valve replacement. Potential benefits may include prevention of LVH, improved diastolic function, reduction of arrhythmias and preservation of left ventricular function. [ABSTRACT FROM AUTHOR]

Abstract

Introduction This study investigates the relation between changes in pulse oximeter oxygen saturation (SpO2) and changes in arterial oxygen saturation (SaO2) in the critically ill, and the effects of acidosis and anaemia on precision of using pulse oximetry to predict SaO2. Patients and methods Forty-one consecutive patients were recruited from a nine-bed general intensive care unit into a 2-month study. Patients with significant jaundice (bilirubin >40 μmol/l) or inadequate pulse oximetry tracing were excluded. Results A total of 1085 paired readings demonstrated only moderate correlation (r= 0.606; P < 0.01) between changes in SpO2 and those in SaO2, and the pulse oximeter tended to overestimate actual changes in SaO2. Anaemia increased the degree of positive bias whereas acidosis reduced it. However, the magnitude of these changes was small. Conclusion Changes in SpO2 do not reliably predict equivalent changes in SaO2 in the critically ill. Neither anaemia nor acidosis alters the relation between SpO2 and SaO2 to any clinically important extent.

Abstract

The importance of muscle glycogen availability for both endurance and high-intensity intermittent exercise performance has been recognised for several decades. The measurement of muscle glycogen in human skeletal muscle has traditionally been performed through the invasive muscle biopsy technique. However, recent data from laboratory-based cycling studies suggests that ultrasound technology (using commercially based software known as MuscleSound) provides a valid and non-invasive assessment tool for which to measure exercise-induced glycogen utilization. Nonetheless, no data currently exist to evaluate the validity of the MuscleSound software in team sport athletes in response to field-based training or match play situations. Accordingly, the aim of the present thesis was to therefore examine the reliability and validity of ultrasound technology (via the MuscleSound system) to non-invasively assess muscle glycogen concentration in skeletal muscle of team sport athletes. The aim of Study 1 (Chapter 3) was to quantify the test re-test reproducibility of the MuscleSound scoring system between (vastus lateralis, VL; vastus medalis, VM; rectus femoris RF) and within (25, 50 and 75% of the muscle length) muscles. Using a test-retest (60 minutes apart) scanning protocol in 100 recreationally active subjects (78 males, 22 females), it was identified that 50% of the VL muscle provided the most reliable anatomical site for which to obtain reproducible MuscleSound scores, as evidenced from the coefficient of variation (CV) – 5.8, intraclass correlation (ICC) – 0.89 (0.85 - 0.93), and ratio limits of agreement 1.02 (1.17) reliability statistics. Having identified the optimal anatomical location for which to obtain reproducible MuscleSound scores, the aim of Study 2 (Chapter 4) was to evaluate the validity of the MuscleSound scoring system to non-invasively assess muscle glycogen utilisation in elite male Rugby League players (n=16) during competitive match play. Although match play depleted absolute muscle glycogen in the VL muscle by approximately 200-250 mmol·kg-1 dw, there were no significant differences in MuscleSound scores obtained before and after the game. As such, these data suggest that MuscleSound is not a valid tool to assess muscle glycogen utilisation during the high-intensity intermittent activity profiles that are inherent to team sports. Given that these data are in contrast to previous validation studies conducted with cycling-based protocols, it was suggested that differences in mechanical loads and fluids shifts between exercise protocols may explain the discrepancies between studies. To eliminate the potential confounding effects of acute changes in fluid shifts associated with differing exercise stimuli, the aim of Study 3 (Chapter 5) was to assess the validity of the MuscleSound system to detect changes in muscle glycogen re-synthesis across a time-scale in which acute changes in fluid shifts have been restored to pre-exercise values. Using a combination of glycogen depleting cycling-based exercise followed by 36 h of an isonenergetic high CHO or low CHO diet, resting muscle glycogen of the VL muscle (from 16 recreationally active males) was approximately 500 versus 250 mmol·kg-1 dw in the high and low CHO trials, respectively. However, no differences were observed in the MuscleSound scores obtained between the high and low trials. When considered with the results of Chapter 4, these data collectively suggest that the MuscleSound system is unable to detect differences in muscle glycogen concentration within the physiological range that typically occurs as a result of exercise-induced glycogen utilisation and/or post-exercise muscle glycogen re-synthesis. Given that MuscleSound was not deemed a valid measurement tool, it was recommended that evaluations of the daily CHO and muscle glycogen requirements of team sport athletes should therefore be undertaken using traditional assessments of energy intake and muscle biopsies, respectively. Accordingly, the habitual CHO intakes of 44 elite professional male Australian Rules Football (AFL) players were quantified across the weekly micro-cycle in Study 4 (Chapter 6). These data demonstrate that elite AFL players practice elements of daily CHO periodisation in accordance with fluctuations in daily training load and proximity to the match day itself. Nonetheless, such data also highlighted that players do not consume CHO at the recommended dose to achieve glycogen storage that is likely required to facilitate optimal match day physical performance. In Study 5 (Chapter 7), muscle glycogen utilisation in the VL muscle was subsequently quantified using the muscle biopsy technique in two elite professional males during competitive match play. Importantly, players commenced the match after adhering to a 24 h CHO loading protocol of 8 g·kg-1 and after consuming a pre-match meal of 2 g·kg-1. Data demonstrate that such a dietary protocol achieves pre-match muscle glycogen concentration >500 mmol·kg-1 dw and that match play can induce an absolute glycogen utilisation >400 mmol·kg-1 dw. However, with the different demands observed between positions, total glycogen utlisation may vary. As such, it appears that the metabolic demands of AFL maybe more CHO dependent than other invasive team sports such as soccer and rugby. In summary, the work undertaken in this thesis has demonstrated that the MuscleSound software is not valid to detect differences in muscle glycogen concentration within the physiological range (i.e. 200-300 mmol·kg-1 dw) that typically occurs as a result of exercise-induced glycogen utilisation and/or post-exercise muscle glycogen re-synthesis. For this reason, assessment of the CHO requirements of team sport athletes is likely dependent on the traditional approaches of dietary assessment and muscle biopsy. Using the latter methods, the present data also demonstrate that the CHO requirements of AFL match play is potentially different (though larger sample sizes are required) than other team sports such as soccer and rugby. Further studies are also required to quantify the glycogen requirement of the typical training sessions undertaken by professional players of specific team sports.

Abstract

BACKGROUND Percutaneous coronary intervention (PCI) is frequently performed to reduce the symptoms of stable angina. Whether PCI relieves angina more than a placebo procedure in patients who are not receiving antianginal medication remains unknown. METHODS We conducted a double-blind, randomized, placebo-controlled trial of PCI in patients with stable angina. Patients stopped all antianginal medications and underwent a 2-week symptom assessment phase before randomization. Patients were then randomly assigned in a 1:1 ratio to undergo PCI or a placebo procedure and were followed for 12 weeks. The primary end point was the angina symptom score, which was calculated daily on the basis of the number of angina episodes that occurred on a given day, the number of antianginal medications prescribed on that day, and clinical events, including the occurrence of unblinding owing to unacceptable angina or acute coronary syndrome or death. Scores range from 0 to 79, with higher scores indicating worse health status with respect to angina. RESULTS A total of 301 patients underwent randomization: 151 to the PCI group and 150 to the placebo group. The mean (±SD) age was 64±9 years, and 79% were men. Ischemia was present in one cardiac territory in 242 patients (80%), in two territories in 52 patients (17%), and in three territories in 7 patients (2%). In the target vessels, the median fractional flow reserve was 0.63 (interquartile range, 0.49 to 0.75), and the median instantaneous wave-free ratio was 0.78 (interquartile range, 0.55 to 0.87). At the 12-week follow-up, the mean angina symptom score was 2.9 in the PCI group and 5.6 in the placebo group (odds ratio, 2.21; 95% confidence interval, 1.41 to 3.47; P<0.001). One patient in the placebo group had unacceptable angina leading to unblinding. Acute coronary syndromes occurred in 4 patients in the PCI group and in 6 patients in the placebo group. CONCLUSIONS Among patients with stable angina who were receiving little or no antianginal medication and had objective evidence of ischemia, PCI resulted in a lower angina symptom score than a placebo procedure, indicating a better health status with respect to angina. This article focuses on a study related to heart rate variability and turbulence as predictors of implantable cardioverter defibrillator discharge. 24 hour ECG markers of cardiac autonomic function, heart rate variability (HRV), and heart rate turbulence (HRT) are known to stratify the risk of arrhythmic death in cardiovascular disease. Their utility in selecting candidates who might benefit from an implantable cardioverter defibrillator (ICD) has not been studied. HRT was easily obtained from all patients and unlike conventional 24 hour HRV, distinguished those who benefited from ICD implantation.

Abstract

Aim To assess the effects of 1- or ≥3-month dual antiplatelet therapy (DAPT) in high bleeding risk (HBR) patients who received biodegradable-polymer sirolimus-eluting stents for complex percutaneous coronary intervention (PCI) and/or acute coronary syndrome (ACS). Methods and results In the MASTER DAPT trial, 3383 patients underwent non-complex (abbreviated DAPT, n = 1707; standard DAPT, n = 1676) and 1196 complex (abbreviated DAPT, n = 588; standard DAPT, n = 608) PCI. Co-primary outcomes at 335 days were net adverse clinical events [NACE; composite of all-cause death, myocardial infarction, stroke, and bleeding academic research consortium (BARC) 3 or 5 bleeding events]; major adverse cardiac or cerebral events (MACCE; all-cause death, myocardial infarction, and stroke); and Types 2, 3, or 5 BARC bleeding. Net adverse clinical events and MACCE did not differ with abbreviated vs. standard DAPT among patients with complex [hazard ratio (HR): 1.03, 95% confidence interval (CI): 0.69–1.52, and HR: 1.24, 95% CI: 0.79–1.92, respectively] and non-complex PCI (HR: 0.90, 95% CI: 0.71–1.15, and HR: 0.91, 95% CI: 0.69–1.21; Pinteraction = 0.60 and 0.26, respectively). BARC 2, 3, or 5 was reduced with abbreviated DAPT in patients with and without complex PCI (HR: 0.64; 95% CI: 0.42–0.98, and HR: 0.70; 95% CI: 0.55–0.89; Pinteraction = 0.72). Among the 2816 patients with complex PCI and/or ACS, NACE and MACCE did not differ and BARC 2, 3, or 5 was lower with abbreviated DAPT. Conclusion In HBR patients free from recurrent ischaemic events at 1 month, DAPT discontinuation was associated with similar NACE and MACCE and lower bleeding rates compared with standard DAPT, regardless of PCI or patient complexity. Abstract

Background: ORBITA-2 (the Placebo-Controlled Trial of Percutaneous Coronary Intervention for the Relief of Stable Angina) provided evidence for the role of percutaneous coronary intervention (PCI) for angina relief in stable coronary artery disease. Fractional flow reserve (FFR) and instantaneous wave-free ratio (iFR) are often used to guide PCI; however, their ability to predict placebo-controlled angina improvement is unknown., Methods: Participants with angina, ischemia, and stable coronary artery disease were enrolled, and anti-anginal medications were stopped. Participants reported angina episodes daily for 2 weeks using the ORBITA smartphone symptom application (ORBITA-app). At the research angiogram, FFR and iFR were measured. After sedation and auditory isolation, participants were randomized to PCI or placebo before entering a 12-week blinded follow-up phase with daily angina reporting. The ability of FFR and iFR, analyzed as continuous variables, to predict the placebo-controlled effect of PCI was tested using Bayesian proportional odds modeling., Results: Invasive physiology data were available for 279 patients (140 PCI and 139 placebo). The median (interquartile range) age was 65 years (59.0-70.5), and 223 (79.9%) were male. Median FFR was 0.60 (0.46-0.73), and median iFR was 0.76 (0.50-0.86). The lower the FFR or iFR, the greater the placebo-controlled improvement with PCI across all end points. There was strong evidence that a patient with an FFR at the lower quartile would have a greater placebo-controlled improvement in angina symptom score with PCI than a patient at the upper quartile (FFR, 0.46 versus 0.73: odds ratio, 2.01; 95% credible interval, 1.79-2.26; probability of interaction, >99.9%). Similarly, there was strong evidence that a patient with an iFR at the lower quartile would have greater placebo-controlled improvement in angina symptom score with PCI than a patient with an iFR at the upper quartile (iFR, 0.50 versus 0.86: odds ratio, 2.13; 95% credible interval, 1.87-2.45; probability of interaction, >99.9%). The relationship between benefit and physiology was seen in both Rose angina and Rose nonangina., Conclusions: Physiological stenosis severity, as measured by FFR and iFR, predicts placebo-controlled angina relief from PCI. 43. Complete versus incomplete revascularization in patients with a non-ST-elevation myocardial infarction: Analysis from the e-ULTIMASTER registry.

Abstract

Background: Incomplete revascularization (ICR) has been associated with a worse prognosis after a percutaneous coronary intervention (PCI). In NSTEMI patients with multivessel disease (MVD) however, the benefit of a complete revascularization (CR) remains unclear., Methods: Patients presenting with an NSTEMI and MVD were selected from the global e-ULTIMASTER registry and grouped according to completeness of revascularization at index hospitalization discharge. The primary endpoint was the patient oriented composite endpoint (POCE) defined as all death, any myocardial infarction, and any revascularization at 1 year. Target lesion failure (TLF) was defined as the composite of cardiac death, target vessel related myocardial infarction and clinically driven target lesion revascularization. Inverse propensity score weighting (IPSW) was performed to harmonize the patient's baseline characteristics between the groups., Results: CR was achieved in 1800 patients (47.0 %) and ICR in 2032 patients (53.0 %). The incidence of POCE at 1 year was lower in the CR group compared to the ICR group: 7.0 % vs. 12.9 %, p < 0.0001. Similarly for TLF at 1 year: 3.6 % vs. 5.5 %, p < 0.01. After IPSW, the incidence of POCE was 7.7 % vs. 12.0 %, p < 0.0001, due to a lower all-cause mortality: 2.7 % vs. 4.2 %, p = 0.02 and less revascularizations: 4.9 % vs. 7.9 %, p < 0.001. The incidence of TLF was no longer statistically significant: CR 3.9 % vs. IR 5.0 %, p = 0.10., Conclusions: Patients with a NSTEMI and multi vessel disease undergoing a percutaneous coronary revascularization with a complete revascularization during index hospitalization have better 1-year clinical outcomes. Background: Placebo-controlled evidence from ORBITA-2 (Objective Randomised Blinded Investigation with Optimal Medical Therapy of Angioplasty in Stable Angina-2) found that percutaneous coronary intervention (PCI) in stable coronary artery disease with little or no antianginal medication relieved angina, but residual symptoms persisted in many patients. The reason for this was unclear., Objectives: This ORBITA-2 secondary analysis investigates the relationship between presenting symptoms and disease severity (anatomic, noninvasive, and invasive ischemia) and the ability of symptoms to predict the placebo-controlled efficacy of PCI., Methods: Prerandomization symptom severity and nature were assessed using the ORBITA smartphone application and symptom and quality of life questionnaires including the World Health Organization Rose angina questionnaire (Rose). Disease severity was assessed using quantitative coronary angiography, stress echocardiography, fractional flow reserve, and instantaneous wave-free ratio. Bayesian ordinal regression was used., Results: At prerandomization, the median number of daily angina episodes was 0.8 (Q1-Q3: 0.4-1.6), 64% had Rose angina, quantitative coronary angiography diameter stenosis was 61% (Q1-Q3: 49%-74%), stress echocardiography score was 1.0 (Q1-Q3: 0.0-2.7), fractional flow reserve was 0.63 (Q1-Q3: 0.49-0.75), and instantaneous wave-free ratio was 0.78 (Q1-Q3: 0.55-0.87). There was little relationship between symptom severity and nature and disease severity: angina symptom score with quantitative coronary angiography ordinal correlation coefficient: 0.06 (95% credible interval [CrI]: 0.00-0.08); stress echocardiography: 0.09 (95% CrI: 0.02-0.10); fractional flow reserve: 0.04 (95% CrI: -0.03 to 0.07); and instantaneous wave-free ratio: 0.04 (95% CrI: -0.01 to 0.07). 45. Complex percutaneous coronary intervention in patients unable to undergo coronary artery bypass grafting during the COVID-19 pandemic: insights from the UK-ReVasc Registry.

Abstract

Objectives: Cardiac surgery for coronary artery disease was dramatically reduced during the first wave of the COVID-19 pandemic. Many patients with disease ordinarily treated with coronary artery bypass grafting (CABG) instead underwent percutaneous coronary intervention (PCI). We sought to describe 12-month outcomes following PCI in patients who would typically have undergone CABG., Methods: Between March 1 and July 31, 2020, patients who received revascularization with PCI when CABG would have been the primary choice of revascularization were enrolled in the prospective, multicenter UK-ReVasc Registry. We evaluated the following major adverse cardiovascular events at 12 months: all-cause mortality, myocardial infarction, repeat revascularization, stroke, major bleeding, and stent thrombosis., Results: A total of 215 patients were enrolled across 45 PCI centers in the United Kingdom. Twelve-month follow up data were obtained for 97% of the cases. There were 9 deaths (4.3%), 5 myocardial infarctions (2.4%), 12 repeat revascularizations (5.7%), 1 stroke (0.5%), 3 major bleeds (1.4%), and no cases of stent thrombosis. No difference in the primary endpoint was observed between patients who received complete vs incomplete revascularization (residual SYNTAX score £ 8 vs > 8) (P = .22)., Conclusions: In patients with patterns of coronary disease in whom CABG would have been the primary therapeutic choice outside of the pandemic, PCI was associated with acceptable outcomes at 12 months of follow-up. Contemporary randomized trials that compare PCI to CABG in such patient cohorts may be warranted.

Abstract

Importance: Abbreviated dual antiplatelet therapy (DAPT) reduces bleeding with no increase in ischemic events in patients at high bleeding risk (HBR) undergoing percutaneous coronary intervention (PCI)., Objectives: To evaluate the association of sex with the comparative effectiveness of abbreviated vs standard DAPT in patients with HBR., Design, Setting, and Patients: This prespecified subgroup comparative effectiveness analysis followed the Management of High Bleeding Risk Patients Post Bioresorbable Polymer Coated Stent Implantation With an Abbreviated vs Standard DAPT Regimen (MASTER DAPT) trial, a multicenter, randomized, open-label clinical trial conducted at 140 sites in 30 countries and performed from February 28, 2017, to December 5, 2019. A total of 4579 patients with HBR were randomized at 1 month after PCI to abbreviated or standard DAPT. Data were analyzed from July 1 to October 31, 2022., Interventions: Abbreviated (immediate DAPT discontinuation, followed by single APT for ≥6 months) or standard (DAPT for ≥2 additional months, followed by single APT for 11 months) treatment groups., Main Outcomes and Measures: One-year net adverse clinical events (NACEs) (a composite of death due to any cause, myocardial infarction, stroke, or major bleeding), major adverse cardiac or cerebral events (MACCEs) (a composite of death due to any cause, myocardial infarction, or stroke), and major or clinically relevant nonmajor bleeding (MCB)., Results: Of the 4579 patients included in the analysis, 1408 (30.7%) were women and 3171 (69.3%) were men (mean [SD] age, 76.0 [8.7] years). Ischemic and bleeding events were similar between sexes. Abbreviated DAPT was associated with comparable NACE rates in men (hazard ratio [HR], 0.97 [95% CI, 0.75-1.24]) and women (HR, 0.87 [95% CI, 0.60-1.26]; P = .65 for interaction). There was evidence of heterogeneity of treatment effect by sex for MACCEs, with a trend toward benefit in women (HR, 0.68 [95% CI, 0.44-1.05]) but not in men (HR, 1.17 [95% CI, 0.88-1.55]; P = .04 for interaction). There was no significant interaction for MCB across sex, although the benefit with abbreviated DAPT was relatively greater in men (HR, 0.65 [95% CI, 0.50-0.84]) than in women (HR, 0.77 [95% CI, 0.53-1.12]; P = .46 for interaction). Results remained consistent in patients with acute coronary syndrome and/or complex PCI., Conclusions and Relevance: These findings suggest that women with HBR did not experience higher rates of ischemic or bleeding events compared with men and may derive particular benefit from abbreviated compared with standard DAPT owing to these numerically lower rates of events., Trial Registration: ClinicalTrials.gov Identifier: NCT03023020.

Abstract

Background: Percutaneous coronary intervention (PCI) for bifurcation lesions can be technically challenging and is associated with higher risk. There is little data on sex-based differences in strategy and outcomes in bifurcation PCI., Aims: We sought to assess whether differences exist between women and men in the treatment and outcomes of bifurcation PCI., Methods: We collected data on 4006 patients undergoing bifurcation PCI, from the e-ULTIMASTER study, a prospective, multicentre study enrolling patients from 2014 to 2018. We divided the bifurcation cohort according to sex, with 1-year follow-up of outcomes (target lesion failure [TLF], target vessel failure [TVF], and patient-oriented composite endpoint [POCE])., Findings: Women were older (69.2 ± 10.9 years vs. 64.4 ± 11.0 years), with a greater burden of cardiovascular comorbidities. For true and non-true bifurcation lesions, women and men were equally likely to undergo a single stent approach (true: 63.2% vs. 63.6%, p = 0.79, non-true: 95.4% vs. 94.3%, p = 0.32), with similar rates of final kissing balloon (FKB) (37.2% vs. 35.5%, p = 0.36) and proximal optimization (POT) (34.4% vs. 34.2%, p = 0.93) in cases where two stents were used. Lastly, after propensity score matching, there was no difference between women and men in the incidence of the composite endpoints of TLF (5.5% vs. 5.2%, RR 1.05 [95% CI 0.77-1.44], p = 0.75), TVF (6.2% vs. 6.3%, RR 0.99 [95% CI 0.74-1.32], p = 0.96), and POCE (9.9% vs. 9.5%, RR 1.05 [95% CI 0.83-1.31], p = 0.70)., Conclusion: In this contemporary, real-world study of bifurcation PCI, we report no difference in stent strategy between women and men, with similar outcomes at 1-year., (© 2023 The Authors. Abstract

Introduction: Today, endovascular treatment (EVT) is the therapy of choice for strokes due to acute large vessel occlusion, irrespective of prior thrombolysis. This necessitates fast, coordinated multi-specialty collaboration. Currently, in most countries, the number of physicians and centres with expertise in EVT is limited. Thus, only a small proportion of eligible patients receive this potentially life-saving therapy, often after significant delays. Hence, there is an unmet need to train a sufficient number of physicians and centres in acute stroke intervention in order to allow widespread and timely access to EVT., Aim: To provide multi-specialty training guidelines for competency, accreditation and certification of centres and physicians in EVT for acute large vessel occlusion strokes., Material and Methods: The World Federation for Interventional Stroke Treatment (WIST) consists of experts in the field of endovascular stroke treatment. This interdisciplinary working group developed competency - rather than time-based - guidelines for operator training, taking into consideration trainees' previous skillsets and experience. Existing training concepts from mostly single specialty organizations were analysed and incorporated., Results: The WIST establishes an individualized approach to acquiring clinical knowledge and procedural skills to meet the competency requirements for certification of interventionalists of various disciplines and stroke centres in EVT. WIST guidelines encourage acquisition of skills using innovative training methods such as structured supervised high-fidelity simulation and procedural performance on human perfused cadaveric models., Conclusions: WIST multispecialty guidelines outline competency and quality standards for physicians and centres to perform safe and effective EVT. Despite this, very few patients in the UK receive mechanical thrombectomy. This editorial explores the main barriers to mechanical thrombectomy use and mechanisms to improve uptake.
