1. In utero origin of myelofibrosis presenting in adult monozygotic twins

Abstract

The latency between acquisition of an initiating somatic driver mutation by a single-cell and clinical presentation with cancer is largely unknown. We describe a remarkable case of monozygotic twins presenting with CALR mutation-positive myeloproliferative neoplasms (MPNs) (aged 37 and 38 years), with a clinical phenotype of primary myelofibrosis. The CALR mutation was absent in T cells and dermal fibroblasts, confirming somatic acquisition. Whole-genome sequencing lineage tracing revealed a common clonal origin of the CALR-mutant MPN clone, which occurred in utero followed by twin-to-twin transplacental transmission and subsequent similar disease latency. Index sorting and single-colony genotyping revealed phenotypic hematopoietic stem cells (HSCs) as the likely MPN-propagating cell. Furthermore, neonatal blood spot analysis confirmed in utero origin of the JAK2V617F mutation in a patient presenting with polycythemia vera (aged 34 years). These findings provide a unique window into the prolonged evolutionary dynamics of MPNs and fitness advantage exerted by MPN-associated driver mutations in HSCs.

Abstract

Harassment is an issue that occurs in all workplaces and institutions. Due to increased exposure to patients, higher rates of harassment are experienced in health care systems. Health care workers need to form a professional relationship with patients to attend to their health care needs; however, harassment by patients can disrupt this relationship and have detrimental physical, mental, and emotional impacts on the health practitioner.The purpose of this study was to examine the prevalence and impact of harassment by patients toward clinical students at a prominent Australasian dental school.A 14-item survey was distributed to clinical students. The survey collected basic demographic information and contained Likert-scale closed questions on harassment prevalence and experience and a free text question asking for details of a significant harassment experience. Descriptive statistics and analysis of free text data were performed.The response rate was 67%. About 20% of respondents reported experiencing at least one incident of harassment. Verbal harassment was most commonly reported, followed by sexual and racial harassment. All incidences of sexual harassment were reported by female students, while racial harassment was most frequently reported by students of Asian ethnicity. Most students indicated they were unsure of how to respond to harassment by patients.Dental students were exposed to harassment by patients.Appropriate policies and education on responding to harassment, alongside support for the same, could help ensure the safety and wellbeing of students.

3. Multi-Scale Graphical Representation of Cell Environment

Abstract

We present a multi-scale graphical network that can capture the relevant representations of individual cell morphology, topological structure of cell communities in a tissue image, as well as whole slide level attributes. This helps to effectively merge the disease relevant cell morphology to the overall topological context within the sample, within one unified deep framework. From the explainability point of view, instead of empirical design, the graphs are designed with biomedical considerations in mind in order to have translational validity. We also provide a clinically interpretable visualisation of the cells and their micro- and macro-environment by leveraging label noise reduction. We demonstrate the efficacy of our methodology on myeloproliferative neoplasms (MPN), a haematopoietic stem cell disorder as an exemplar test case. The proposed method achieves an encouraging performance in the robust separation of different MPN subtypes in this exciting new dataset as part of this work.

Abstract

The endovascular coil embolization of intracranial saccular aneurysms requires a set of specific X-ray images with which to view the aneurysm during coiling. These two-dimensional (2-D) images, known as working projections, should be optimal for measuring the aneurysm sac diameter, inserting the first coil, and checking coil overhang into the surrounding vessels. At present the gantry tilt that produces these images is found by the radiologist by trial and error. In this pape, we present a method for automatically finding the angles that will produce the desired X-ray projections. Our method consists of four steps: 1) finding the location and orientation of the aneurysm neck; (2) labeling the aneurysm sac; 3) determining the optimal tilts for viewing the aneurysm during coiling; and 4) adjusting the optimal tilts for change in the patient orientation between pre-Guglielmi detachable coil (GDC) scanning and the coiling treatment. We discuss these steps and present results of the algorithm applied to pathological examples in the form of simulated X-ray images. A final discussion is given for one example where our results have been applied in a clinical situation. Abstract

We report the case of a well-controlled female asthmatic who developed 'multiple pulmonary hamartomas' on three separate occasions over a period of 25 years that necessitated surgical resection. To our knowledge, this is the first report of recurrent hamartomas in a single individual necessitating multiple thoracotomies.

Abstract

O artigo objetiva compreender significações produzidas por uma professora alfabetizadora na experiência formativa vivenciada no Programa de Bolsa de Iniciação à Docência (PIBID) e as implicações no ensino de alunos com deficiência. No PIBID, o professor da Educação Básica atua como coformador dos licenciandos e também realiza sua formação continuada. A pesquisa contou com uma professora alfabetizadora de escola pública cearense, e utilizou questionário e entrevista como instrumentos. A análise baseou-se na proposta dos Núcleos de Significação e evidenciou que a experiência formativa emergiu nas significações da professora como contextualizada e colaborativa, propiciou acesso a novos saberes e gerou implicações favoráveis ao ensino de alunos com deficiência.
