CONTEXTO: Ombro doloroso é freqüente em pacientes com hemiplegia/hemiparesia (H/P) por acidente vascular encefálico (AVE), dificultando a recuperação neuromotora gerando incapacidade funcional. OBJETIVO: Estudar tratamento fisioterapêutico para a prevenção da dor no ombro com H/P em pacientes com AVE na fase sub-aguda, e analisar desfechos secundários (força muscular do ombro acometido e movimentos funcionais ativos básicos). MÉTODO: Estudaram-se 21 pacientes (12 homens, 9 mulheres; idades 26 a 87 anos) com H/P. O tratamento fisioterapêutico consistiu de 30 minutos diários de cinesioterapia, desde as 48 horas após o AVE até a alta hospitalar. Os pacientes foram avaliados antes e após o tratamento em relação à presença ou ausência de dor no ombro H/P, à força dos diversos grupos musculares do ombro e quanto aos movimentos funcionais de transferência e manutenção postural básica. PURPOSE: To assess the role of preserved bovine parietal peritoneum as a material for hernia repair in a rat ventral hernia model. METHODS: An abdominal wall defect (15mm x 25mm) was created in Wistar male rats (n=40). Control animals (n=20) had the polypropylene (PP) mesh sutured into the defect, whereas bovine preserved peritoneum (BPP) was used in experimental group (n=20). After 7 and 28 days, the abdominal wall was taken off and histological studies of the amount of collagen by Sirius Red stain and morphometric evaluation consisted in quantitative analysis of the collagen by using specific software (Imagelab®). The Mann-Whitney, Kruskal-Wallis and ANOVA tests were applied for statistical analysis (pd"0.05). RESULTS: Histological examination revealed no difference between the BPP and PP groups (p = 0.55 NS). CONCLUSION: BPP is suitable for the closure of ventral hernias in rat model as shown by its morphological properties. OBJETIVO: Estudar os aspectos histológicos do uso de uma prótese de peritônio bovino na correção de hérnia ventral em um modelo animal de doença. MÉTODOS: Utilizando 40 ratos machos Wistar, comparou-se o implante do peritônio bovino com a tela de polipropileno na correção de um defeito provocado na parede abdominal do animal. Após 7 (sub-grupo A) e 28 (sub-grupo B) dias de observação, as peças foram retiradas e procedeu-se o estudo histológico através da quantificação de colágeno pelo método de captação e processamento digital de imagens, sob a coloração do Sirius Red. Os testes de Mann-Whitney, de Kruskal-Wallis e ANOVA foram utilizados e estabeleceu-se em 0,05 o nível para rejeição da hipótese de nulidade (pd"0.05). RESULTADOS: A quantificação do colágeno na interface do implante mostrou equivalência entre os grupos Peritônio e Polipropileno (p=0,55 NS). CONCLUSÃO: O peritônio parietal bovino apresentou infiltração de tecido fibrocolágeno semelhante a da tela de polipropileno na correção de hérnia ventral em ratos.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) OBJETIVO: Verificar a possibilidade de implantação e a capacidade de resistência tênsil do peritônio parietal bovino como tela cirúrgica na correção de hérnia ventral em um modelo animal de experimentação. MÉTODO: Utilizando 57 ratos machos Wistar, comparou-se o implante do peritônio bovino com a tela de polipropileno na correção de um defeito provocado na parede abdominal do animal. Após sete (sub-grupo A) e 28 (sub-grupo B) dias de observação, as peças foram retiradas e procedeu-se a avaliação da resistência à tração em Máquina Universal de Ensaios. Um grupo sem implante de material protético foi utilizado como controle nos testes de força tênsil. Os testes de Mann-Whitney e de Kruskal-Wallis foram utilizados e estabeleceu-se em 0,05 o nível para rejeição da hipótese de nulidade. RESULTADOS: Os testes de resistência à tração, com valores expressos em Newton, não mostraram diferenças estatísticas entre os grupos estudados, tanto no 7º quanto no 28º dia de pós-operatório, e ambos foram menos resistentes que a parede abdominal normal (p = 0,003). CONCLUSÃO: O peritônio parietal bovino apresentou resistência tênsil semelhante a da tela de polipropileno em um modelo de correção de hérnia ventral em ratos. OBJECTIVE: The aim of this study is to assess the tensile strength of the preserved bovine peritoneum (PBP) as a surgical mesh in rats ventral hernia. METHODS: Using 57 rats, two mesh materials, the PBP and polypropylene (PP), were compared after implantation in a standardized ventral hernia model (full-thickness abdominal wall defects). After 7 (subgroup A) and 28 (subgroup B) days of implantation, they were removed and tensile strength assessment in vitro was performed. One group, with no procedure, just normal abdominal wall, was used as control for tensile strength. The Mann-Whitney and Kruskal- Wallis tests were applied for statistical analysis. RESULTS: All animals survived until the end point, but one (PBP group, subgroup B). No recurrence was seen during the period of observation. The mean load required to disrupt the interface were similar in PBP and PP groups, and both were weaker than normal group (p = 0,003) in all periods of observation. CONCLUSION: Preserved bovine peritoneum is a biological mesh that can be used for ventral hernia repair in rat model. Painful shoulder is considered the most common complication of hemiplegic/parethic (H/P) stroke patients. It is a negative factor for neuromotor recovery.To study the effects of kinesiotherapy treatment on shoulder pain prevention, and as a secondary endpoints, to analyze muscle strength of H/P shoulder looking at basic functional active mobility.Twenty one inpatients (12 men, 9 women) aged 26 to 87 years, with post-stroke H/P were submitted to thirty minutes daily program kinesiotherapy, started at 48 hours post-stroke up to their hospital discharge. Patients were evaluated pre and post treatment according to the presence or absence of shoulder pain, movements and shoulder strength, and for presence or absence of basic functional movements.No patient complaining of shoulder pain at the hospital discharge (p0.001). The muscle strength improved signifcantly for elevation, protusion, abduction and flexion of the shoulder (p0.001). There was improvement also for functional mobility on moving from dorsal to lateral recumb, from lateral recumb to a seated position and in keeping the seated position (p0.001).Kinesiotherapy in acute phase of stroke prevented shoulder pain.

Os autores revisam 96 procedimentos colangiográficos peroperatórios realizados no Departamento de Cirurgia da Faculdade de Medicina de Marília (FAMEMA) entre janeiro de 1993 e fevereiro de 1995. Dos exames estudados, setenta foram considerados normais, 23 alterados e três inconclusivos. Os autores apresentam casos onde a colangiografia peroperatória (CPO) não confirmou a suspeita pré-operatória, o que evitou explorações desnecessárias da via biliar (6,25%) ou não permitiu diagnóstico incorreto (9,36%). Do exposto, os autores concluem que a CPO continuará sendo rotina nas operações sobre a vesícula e vias biliares e sugerem a necessidade do estabelecimento de padrão técnico para a realização do exame. The authors revise operative cholangiographies peformed at the Department of Surgery of the “Faculdade de Medicina de Marília” (FAMEMA) from January 1993 to February 1995, with the objective of characterizing the pattern of these exams at FAMEMA. It was possible to rescue 96 findings of the registrations from Service of Radiology of FAMEMA. In this study, 70 were considered normal, 23 altered and three inconclusive. The authors present cases where the operative cholangiographies did not clinch pre-operative suspicion, which avoided unnecessary profiteering of the common bile duct (6.25%) or did not allow improper diagnosis (9.36%), considered the main advantages of the operative cholangiography. Then, they present discussion over this kind of procedure, unsuspected choledocolithiasis and about the new point of view: the cholangiography at videolaparoscopic cholecystectomy. Finally, the authors conclude that operative cholangiography will proceed as a routine on operations on gallblader and common bile duct and they propose the institution of a pattern to the accomplishment of this procedure.
