A retrospective analysis to quantify age-related changes of the incudo-malleolar joint (IMJ) and incudo-stapedial joint (ISJ), and to analyse changes in the air-bone gap (ABG) with age, was performed. Defined histologic parameters of 153 IMJ and 106 ISJ from subjects aged from birth to 70 years were correlated to age. Additionally, audiograms of 1760 ears of 974 other subjects aged 20 to 80 years were retrospectively analysed and the ABG was correlated to age. The joint space (age group from 0 to 10 compared to 61 to 70 years) became significantly wider with age (IMJ: from a mean of 44 µm to 100 µm, p &lt, 0.001, ISJ: from a mean of 28 µm to 69 µm, p &lt, 0.009. The thickness of cartilage of the incus decreased in the first 20 years of life (IMJ, from a mean of 88 µm to 65 µm, p &lt, 0.01, ISJ: from a mean of 44 µm to 35 µm, p &lt, 0.01). The ABGs of younger ears (20–40 years) was significantly larger at 500 Hz compared to older ears (60–80 years) by 2–4 dB, while it was significantly smaller by 3–5 dB at 4000 Hz (p &lt, 0.0017). Interindividual variations in all age groups were large for both analyses. The increased joint spaces could potentially reduce the stiffness in the joints and explain the increase in ABG at 4000 Hz and the drop at 500 Hz. While the average change is small and of minimal clinical relevance, a larger increase of ABG with age is seen in some subjects.

Objective: Evaluation of the face validity of a new artificial model of an infant temporal bone (TB) suitable for surgical training, including cochlear implantation. Subject: Micro-computer-tomography images were obtained from a TB specimen of a 1-year-old normal infant available in an anatomical collection. The TB model was designed and constructed using these images and techniques known from similar models of adult TB. Intervention: Fifteen otology departments in Austria, Germany, and Switzerland rated the infant TB model and compared it with the established adult TB model manufactured commercially by the same company. Main Outcome Measure: The otologists responded to a semi-quantitative questionnaire with a rating scale ranging from 1 (strongly disagree) to 5 (strongly agree). Macroscopic and microscopic anatomic details, drilling experience, and surgical landmarks were rated. The surgical procedures included mastoidectomy, posterior tympanotomy, cochleostomy, and insertion of a cochlear electrode. Results: Overall ratings were similar (3.9) for both the infant and the adult TB models, with ranges of 3.47 to 4.47 (infant model) and 3.5 to 4.33 (adult model). Ratings of specific anatomical details differed as a function of type of model, but without preference of one model over the other. Conclusions: Infant TB models can be used similarly as adult TB models for surgical training, including cochlear implantation. They may deserve a more important role in surgical training because cadaveric human temporal bones of infants are not available.

Animals are frequently used for the development and testing of new hearing devices. Dimensions of the middle ear and cochlea differ significantly between humans and commonly used animals, such as rodents or cats. The sheep cochlea is anatomically more like the human cochlea in size and number of turns. This study investigated the middle-ear ossicular velocities and intracochlear sound pressure (ICSP) in sheep temporal bones, with the aim of characterizing the sheep as an experimental model for implantable hearing devices. Measurements were made on fresh sheep temporal bones. Velocity responses of the middle ear ossicles at the umbo, long process of the incus and stapes footplate were measured in the frequency range of 0.25-8 kHz using a laser Doppler vibrometer system. Results were normalized by the corresponding sound pressure level in the external ear canal (PEC). Sequentially, ICSPs at the scala vestibuli and tympani were then recorded with custom MEMS-based hydrophones, while presenting identical acoustic stimuli. The sheep middle ear transmitted most effectively around 4.8 kHz, with a maximum stapes velocity of 0.2 mm/s/Pa. At the same frequency, the ICSP measurements in the scala vestibuli and tympani showed the maximum gain relative to the PEC (24 dB and 5 dB, respectively). The greatest pressure difference across the cochlear partition occurred between 4 and 6 kHz. A comparison between the results of this study and human reference data showed middle-ear resonance and best cochlear sensitivity at higher frequencies in sheep. In summary, sheep can be an appropriate large animal model for research and development of implantable hearing devices.

Background Bone conduction (BC) is an alternative to air conduction (AC) for stimulation of the inner ear. Stimulation for BC can occur directly on the skull bone, on the skin covering the skull bone, or on soft tissue (i.e., eye, dura). All of these stimuli can elicit otoacoustic emissions (OAE). This study aims to compare OAEs generated by different combinations of stimuli in live humans, including direct stimulation of the intracranial contents via the dura, measured intraoperatively. Methods Measurements were performed in five normal-hearing ears of subjects undergoing a neurosurgical intervention with craniotomy in general anesthesia. Distortion product OAEs (DPOAEs) were measured for f2 at 0.7, 1, 2, 3, 4, and 6 kHz with a constant ratio of the primary frequencies (f2/f1) of 1.22. Sound pressure L1 was held constant at 65 dB SPL, while L2 was decreased in 10 dB steps from 70 to 30 dB SPL. A DPOAE was considered significant when its level was ≥6 dB above the noise floor. Emissions were generated sequentially with different modes of stimulation: 1) pre-operatively in the awake subject by two air-conducted tones (AC-AC); 2) within the same session preoperatively by one air- and one bone-conducted tone on the skin-covered temporal bone as in audiometry (AC-BC); 3) intra-operatively by one air-conducted tone and one bone-vibrator tone applied directly on the dura (AC-DC). A modified bone vibrator (Bonebridge; MED-EL, Innsbruck, Austria) was used for BC stimulation on the dura or skin-covered mastoid. Its equivalent perceived SPL was calibrated preoperatively for each individual by psychoacoustically comparing the level of a BC tone presented to the temporal region to an AC tone at the same frequency. Simultaneously with the DPOAEs, vibrations at the teeth were measured with an accelerometer attached using a custom-made holder. Results It was possible to record DPOAEs for all three stimulation modes. For AC-DC, DPOAEs were not detected above the noise floor below 2 kHz but were detectable at the higher frequencies. The best response was measured at or above 2 kHz with L2 = 60 dB SPL. The acceleration measured at the teeth for stimulation on the dura was lower than that for stimulation on the bone, especially below 3 kHz. Conclusion We demonstrate a proof-of-concept comparison of DPOAEs and teeth acceleration levels elicited by a bone vibrator placed either against the skin-covered temporal bone, as in audiometry, or directly against the dura mater in patients undergoing a craniotomy. It was demonstrated that DPOAEs could be elicited via non-osseous pathways within the skull contents and that the required measurements could be performed intra-operatively.

The aim of the study was to examine central auditory processes compromised by age, age-related hearing loss, and the presentation of a distracting cafeteria noise using auditory event-related potentials (ERPs). In addition, the relation of ERPs to behavioral measures of discrimination was investigated. Three groups of subjects participated: young normal hearing, elderly subjects with normal hearing for their age, and elderly hearing-impaired subjects. Psychoacoustic frequency discrimination thresholds for a 1000-Hz pure tone were determined in quiet and in the presence of a contralateral cafeteria noise. To elicit ERPs, small frequency contrasts were presented with and without noise under unattended and attended conditions. In the attended condition, behavioral measures of d′ detectability and reaction times were also obtained. Noise affected all measures of behavioral frequency discrimination significantly. Except N1, all ERP components in the standard and difference waveforms decreased significantly in amplitude and increased in latency to the same degree in all three subject groups, arguing against a specific age-related sensitivity to the effects of contralateral background noise. For N1 amplitude, the effect of noise was different in the three subject groups, with a complex interaction of age, hearing loss, and attention. Behavioral frequency discrimination was not affected by age but deteriorated significantly in the elderly subjects with hearing loss. In the electrophysiological test, age-related changes occurred at various levels. The most prominent finding in the response to the standard stimuli was a sustained negativity (N2) following P2 in the young subjects that was absent in the elderly, possibly indicating a deficit in the inhibition of irrelevant information processing. In the attended difference waveform, significantly larger N2b and smaller P3b amplitudes and longer N2b and P3b latencies were observed in the elderly indicating different processing strategies. The pronounced age-related changes in the later cognitive components suggest that the discrimination of difficult contrasts, although behaviorally maintained, becomes more effortful in the elderly.

Labyrinthitis with vestibulocochlear dysfunction in chronic inflammatory bowl disease is a rare but well described manifestation or complication. The diagnostic and therapeutic possibilities and limitations of this inflammatory otopathy are presented and discussed in the context of a case report. A bilateral loss of vestibular function and sensorineural hearing loss occurred in a 26-year-old male patient with previously diagnosed Crohn's disease. He was treated with immune suppressive therapy during the onset and development of cochleovestibular symptoms. Diagnostic tests included pure tone audiograms, speech audiometry, caloric tests, VEMPs, and MRI. Therapy included azathioprine, corticosteroids (systemic and intratympanic), monoclonal antibodies, and cochlear implants. Despite immunosuppressive therapy including monoclonal antibodies, the patient progressively lost his hearing. Finally, bilateral cochlear implantation was carried out with good results. The treatment of patients with systemic inflammatory or autoimmune disease affecting the cochlear-vestibular organ is challenging. An interdisciplinary approach is needed. In cases of bilateral deafness, cochlear implantation is a satisfactory treatment and should not be delayed unnecessarily.

Background The purpose of this study was to evaluate prospectively the presence and impact of the gastric carcinogen Helicobacter pylori (H. pylori) in the upper aerodigestive tract. Previous studies suggested it could represent a risk factor for head and neck squamous cell carcinoma (HNSCC). Methods Serology, rapid urease test, and quantitative polymerase chain reaction (qPCR) for H. pylori were performed in patients with head and neck cancer (N = 56) and cancer-free controls (N = 90). Comparison between groups was done using logistic regression analysis. Results Rates of positive serology and rapid urease test did not differ between the 2 groups in logistic regression analysis (p = .677 and p = .633, respectively). BACKGROUND: Postlingual single-sided deafness (SSD) is defined as normal hearing in one ear and severely impaired hearing in the other ear. A right ear advantage and dominance of the left hemisphere are well established findings in individuals with normal hearing and speech processing. Therefore, it seems plausible that a right ear advantage would exist in patients with SSD. METHODS: The audiometric database was searched to identify patients with SSD. Results from the German monosyllabic Freiburg word test and four-syllabic number test in quiet were evaluated. Results of right-sided SSD were compared with left-sided SSD. Statistical calculations were done with the Mann-Whitney U test. RESULTS: Four hundred and six patients with SSD were identified, 182 with right-sided and 224 with left-sided SSD. The two groups had similar pure-tone thresholds without significant differences. Background: Helicobacter pylori is known to cause gastric cancer. Presence and carcinogenicity in the upper aerodigestive system is doubtful. This study examined the prevalence of Helicobacter pylori and related factors in biopsies from the upper aerodigestive tract in patients with gastric colonization by Helicobacter pylori. Methods: In a case series study, 26 patients with histopathologically confirmed gastric colonization were identified. A polymerase chain reaction was performed on matched formalin-fixed and paraffin-embedded tissues of the stomach and the oral cavity, pharynx, or larynx. Results: Helicobacter pylori was found in 38% of the samples from the oral cavity, larynx and pharynx. An association with malignancies in these regions or possible risk factors such as age, smoking, or alcohol was not found. Conclusion: The upper aerodigestive system seems to be an additional reservoir in a significant percentage of patients presenting with Helicobacter pylori gastritis. Head Neck, 2013.

This study investigated the effects of long-term unilateral and bilateral amplification on central auditory processing in elderly people with symmetrical hearing loss using late auditory evoked potentials. It was hypothesized that in the unilateral setting stimulation of the aided ear would yield an acclimatization effect with larger amplitudes and shorter latencies of the components P1, N1 and P2 compared to those of the unaided ear. Auditory evoked potentials were elicited by 500, 1000 and 2000 Hz pure tones at 55, 70 and 85 dB SPL presentation level delivered either to the left or right ear. Unilaterally and bilaterally fitted experienced hearing-aid users and a control group of normally hearing adults, all aged at least 60 years, participated. The responses of the unilateral hearing-aid users did not differ significantly for any of the components P1, N1 or P2 between the aided and unaided ears, but a significant interaction between ear and frequency was present for P2 amplitudes. P2 amplitudes were significantly smaller for the 0.5- and 1-kHz stimuli and tended to be larger for the 2-kHz stimulus in the aided ear suggesting an acclimatization effect. Larger P2 amplitudes were observed in the unilaterally fitted group, which was interpreted as a correlate of more effortful auditory processing in unilaterally fitted people.

The present investigation further analysed results of a previously reported survey with a large sample of hearing aid owners (Bertoli et al, 2009) to determine the individual and technological factors related to hearing aid outcome. In particular the associations of hearing loss, level of signal processing, and fitting type (bilateral versus unilateral fitting) with hearing aid use, satisfaction with and management of the aid were evaluated. A sub-group with symmetrical hearing loss was analysed (n = 6027). Regular use was more frequent in bilateral users and in owners of devices with more complex signal processing, but the strongest determinant of regular use was severity of hearing loss. Satisfaction was higher in the group wearing simple devices, while fitting type and degree of hearing loss had no influence on satisfaction rates. Moderate and severe hearing loss was associated more frequently with poor management of the aid than mild hearing loss. It was concluded that bilateral amplification and advanced signal processing features may contribute to successful hearing aid fitting, but the resulting differences must be considered to be relatively small.

Background/Aim. Idiopathic sudden sensorineural hearing loss (ISSHL) is one of the most controversial issues in otology. The aim of this study was to determine whether a delay in treatment has any influence on hearing recovery in ISSHL. Method. This study was designed as a retrospective evaluation of an electronic patient data base of the University Hospital Z?rich from January 1995 to August 2006. Five hundred and forty one patients with a sudden hearing loss were identified. The standard treatment was carbogen inhalation (95% O2 and 5% CO2 eight times per day in the duration of 30 minutes) and prednisone orally (100 mg in one morning dose) for 7 days. Factor that was analyzed included the interval between the onset of symptoms and the beginning of the treatment. The initial hearing loss was described using the pure tone average (PTA in dB) hearing level at 4 frequencies (0.5, 1, 2 and 4 kHz). Hearing gain was expressed either as absolute hearing gain (dB values from initial PTA minus dB values from final PTA) or as relative hearing gain (absolute hearing gain divided by initial PTA minus baseline PTA) ? 100. Significant recovery of hearing was defined as the final PTA ? 30 dB (or same as PTA of the opposite ear). Results. An absolute hearing gain between the initial audiogram and the final audiogram was 15.1 dB. The mean relative hearing gain was 47%. Three hundred one (57%) patients had a significant recovery of hearing, and 228 (43%) had not. If the patients received treatment in the first 24 hours after onset of symptoms, then the rate of significant recovery was 56%, and no significant difference existed between this group and the patients who received the therapy after 24 hours, but within seven days (?2 = 0.007, DF = 1, p > 0.01). Conclusion. These results suggest that it is not critical to begin the treatment of ISSHL immediately as an emergency, but within seven days.

To clarify the contribution of the skull contents to the transmission of bone vibratory stimuli, and to examine the characteristics of such stimuli, we compared auditory thresholds and distortion-product otoacoustic emission (DPOAE) levels with a bone vibrator placed on various sites of the head, including the eye. The best audiometric thresholds and the highest DPOAE levels were obtained with the vibrator placed on the mastoid of the measuring side, or on the "ultrasound-window" of the temple. The audiometric thresholds obtained with the bone vibrator on the eye were similar to those of the forehead, and about 10 dB higher than at the best sites. DPOAEs were clearly present when elicited by a combination of air-conducted stimuli presented through an insert earphone and with the bone vibrator placed on the eye. These results indicate that vibratory sounds can be transmitted through the skull contents to the inner ear. The intracranial transmission pathway of the vibratory stimuli may play a significant role, particularly at low frequencies, and possibly also when the vibratory stimuli are applied on the skull bone.

Systemic steroids are widely used worldwide as a standard of care for primary therapy of idiopathic sudden sensorineural hearing loss (ISSHL). The German ISSHL guideline recommends high-dose steroids for primary therapy of ISSHL, without evidence from randomized controlled trials (RCTs). The rationale for the treatment of ISSHL using high dose steroids is only based on retrospective cohort studies.This article describes the planning and initiation of a multicenter, national, randomized, controlled clinical trial entitled Efficacy and safety of high dose glucocorticosteroid treatment for idiopathic sudden sensorineural hearing loss - a three-armed, randomized, triple-blind, multicenter trial (HODOKORT). This clinical trial aims to compare standard dose with two types of high-dose steroids for primary systemic therapy with respect to their efficacy in improving hearing, and thus communication ability, in patients with idiopathic sudden sensorineural hearing loss.This study is funded by the "Clinical Trials with High Patient Relevance" research program in the health research framework of the German Federal Ministry of Education and Research. It is one of two studies by the German Study Center of Clinical Trials of the German Society of Otorhinolaryngology, Head and Neck Surgery (DSZ-HNO). Planning and initiation was done in cooperation with the DSZ-HNO, the Coordination Center of Clinical Trials of the Martin-Luther-University Halle-Wittenberg, and the Study Center of the University Hospital Freiburg.

A very high signal-to-noise ratio is required for equipment designed for extended high-frequency audiometry because listeners with almost no hearing ability in the extended high-frequency range may have normal hearing sensitivity in the lower frequencies. Two commercially available systems designed for pure-tone audiometry were evaluated both in the conventional and extended high-frequency range. Unwanted lower frequency signals greater than the noise floor occurred predominantly at presentation levels of approximately 110 dB SPL or higher. Test tones in the extended high-frequency range should be restricted to levels that are not associated with lower frequency noise.

To introduce a new method of measuring sound localization ability based on eye-tracking and to test this method by analysing the influence of mild induced conductive hearing loss on sound localization.Sound signals were presented from different angles, and the participant's responses were measured using an eye-tracking device. For validation, a comparison of responses to visual stimuli was performed. To test the clinical application of this method, a mild conductive hearing loss was simulated, and the impact of this change on sound localization was measured.Fifteen participants.The system provided repeatable measurements, and there was a good correlation of sound and visual signals. A large number of trials could be completed fairly rapidly. Following the induced conductive hearing loss, a decline of 5.5° in the accuracy of sound localization in the horizontal plane was found towards the side of the non-impaired ear for frontal presentations.Quantifying sound localization by eye-tracking was found to be feasible, fast and accurate. A mild conductive hearing loss caused a slight degradation of sound localization accuracy within the 30° frontal sector, which is in good agreement with results found using methods requiring more extensive instrumentation.

A screening device for otoacoustic emission (OAE) testing was evaluated via measurements of click evoked OAEs (CEOAEs) and/or distortion product OAEs (DPOAEs) obtained in normally hearing young adults and newborns. Moreover, measurements were performed in a passive cavity and in ears with severe sensorineural hearing loss, in which the occurrence of a pass result was assumed to indicate artifacts. Different parameter settings were investigated. In comparison to normally hearing young adults, overall pass rates in newborns were similar using CEOAEs (93 to 100%), but lower using DPOAEs (65 to 95% vs. 83 to 100%). Pass rates in ears with severe hearing loss were 10% at 2 kHz, 13% at 3 kHz, and lower at other frequencies. Pass rates in the test cavity were generally low. The influence of test frequency on pass rates and calculations using computer simulation indicated the presence of artifacts. The discrimination of such artifacts from biological signals is difficult or impossible, and they may represent a serious problem for efficient OAE screening.

We examined the long-term sequelae in both ears of 42 patients who reported the occurrence of auditory changes resulting from a single exposure to intense sound levels during non-occupational activities. We divided these patients into two groups, based upon noise exposures of either continuous duration or single high-energy impulse. Audiometric data were available for each of these subjects shortly after their noise-exposure events and follow-up examinations took place more than one year after the noise occurrence (range: 1-16 years). The initial median hearing loss for the continuous-type noise exposure group at 3-8 kHz was found to be 9 dB, relative to the age-appropriate norms, in the more affected ears, and hearing function was found to have returned to normal levels at follow-up. The same initial hearing loss was measured for the impulse-type noise group, but a residual hearing loss of 4 dB was measured at follow-up. Furthermore, the majority of the subjects from both groups reported tinnitus and hypersensitivity to sound at follow-up, but with minimal impact on their lives.

We analyzed the cortical responses to standard stimuli presented in an auditory oddball paradigm from two groups of young and elderly participants in unattended and attended conditions. A sustained negativity (N2) followed NI and P2 in the responses of the young participants. This negativity was reduced or absent in the elderly.With attention, the early portion of N2 (N2 early ) decreased in both groups, whereas the late portion (N2 late ) increased only in young participants. We suggest inhibitory processes at the origin of this component. The lack of N2 in the elderly participants is consistent with structural changes in the aging brain and supports the hypothesis of a specific age-related deficit in the inhibition of irrelevant information processing.

Objective: Mutations in the GJB2 gene encoding connexin 26 (Cx26) protein are a major cause for nonsyndromic autosomal recessive and sporadic deafness. However, its contribution to hearing impairment in Switzerland remains undefined. To determine the frequency and type of GJB2 mutations in the Swiss hearing-impaired population diagnosed under the age of 2 yr and at 2 yr and older and to assess the effectiveness of denaturing high-performance liquid chromatography (DHPLC) in screening for mutation in GJB2. Methods: Thirty-four patients with hearing impairment underwent mutation screening of the single coding exon of GJB2 with DHPLC followed by bidirectional sequencing to identify sequence alterations. Results: GJB2 mutations were more common in children diagnosed with hearing impairment under the age of 2 yr compared to the group 2 yr and older. In patients under age 2 yr, 9 of 20 (45%) harbored 13 GJB2 mutations including a common 313del14nt mutation; four of these patients were homozygous or compound heterozygous for GJB2 mutations. In contrast, 2 of 14 patients in the 2 yr and older group (14%) had a single mutation in GJB2. The 35delG mutation was exclusively found in 5 patients under the age of 2 yr. DHPLC for mutation screening was 100% sensitive and 83% specific for detecting sequence alterations in GJB2. Conclusions: In Switzerland, GJB2 mutations are a major cause of nonsyndromic hearing impairment in children under the age of 2. Similar to other populations, GJB2 mutations are uncommon in the affected Swiss patients identified after 2 yr. Although 35delG mutation is common in the hearing-impaired children under the age of 2, it was absent in patients diagnosed with hearing impairment after the age of 2. DHPLC is a highly sensitive tool for detection of GJB2 mutations.

Objectives : The speech understanding difficulties of elderly persons with age-related hearing loss may be related in part to reduced auditory temporal resolution. To investigate the effects of aging on temporal resolution, the electrophysiological and psychoacoustic detection thresholds for a very short silent gap within a pure tone were determined, and the relation between the two test results was examined. Methods : Behavioral gap detection thresholds were determined in 10 young and 10 elderly normally hearing subjects using an adaptive test procedure. To elicit mismatch negativity (MMN), deviant stimuli with gap durations varying from 6 to 24ms in 3ms steps were presented in separate test blocks. Results : There were no significant differences in the psychoacoustic gap detection thresholds between young and elderly subjects. In contrast, longer gaps were needed to elicit MMN in elderly subjects. They also had significantly reduced MMN peak amplitudes, increased MMN peak latencies, a significantly smaller P2 amplitude and longer P2 latency in their responses to the standard stimulus when compared to the same measures in young subjects. Conclusions : Processing of basic temporal stimulus features in elderly subjects is considerably more reduced at the preattentive level (as indicated by MMN), than when attention is directed to the task (as indicated by the psychoacoustic results).

In order to test the frequency specificity of the efferent suppressive effect on otoacoustic emissions, changes in the 2f1-f2 distortion product otoacoustic emission (DPOAE) levels induced by contralateral stimuli of different spectra were measured in 10 normally hearing adults. Three types of contralateral stimuli were used: (i) a set of 6 pairs of pure tones with the same frequencies as used for DPOAE stimulation; (ii) 6 narrow-band noise signals with cut-off frequencies equal to the frequencies of the primary tones used for DPOAE stimulation; and (iii) broad-band noise with a bandwidth of 840-6,000 Hz. A small suppressive effect was observed mainly in the mid-frequency region. Broad-band noise was more effective at suppressing DPOAEs than narrow-band noises and two-tone complexes. Occasionally, small enhancements in DPOAE amplitudes were observed. Based on the results of this study, it is concluded that DPOAE changes induced by contralateral stimuli are not frequency-specific, and are too small to have routine clinical value.

The purpose of this study was to evaluate prospectively the presence and impact of the gastric carcinogen Helicobacter pylori (H. pylori) in the upper aerodigestive tract. Previous studies suggested it could represent a risk factor for head and neck squamous cell carcinoma (HNSCC).Serology, rapid urease test, and quantitative polymerase chain reaction (qPCR) for H. pylori were performed in patients with head and neck cancer (N = 56) and cancer-free controls (N = 90). Comparison between groups was done using logistic regression analysis.Rates of positive serology and rapid urease test did not differ between the 2 groups in logistic regression analysis (p = .677 and p = .633, respectively). Birth in a developing country and age above 50 years old were predictors of positive serology (p .001 and p = .040, respectively). Using qPCR, no biopsy showed the presence of H. pylori.This study challenges the concept that H. pylori may be a risk factor for HNSCC.

Background/Importance Supraglottoplasty is the surgical procedure of choice for severe laryngomalacia and has shown to be successful in most cases; however, patients with medical comorbidities present a higher rate of failure. To date, the best management of laryngomalacia in children with congenital syndrome remains unclear. Purpose To study the outcome of supraglottoplasty in children with severe laryngomalacia, and to analyze the management and outcome in infants with a congenital syndrome. Methods Retrospective medical records review from January 2003 to October 2012 of all patients who underwent laser supraglottoplasty for severe laryngomalacia at the University Children's Hospital Zurich, Switzerland. Results Thirty-one patients were included; median age at time of surgery was 3.5months. Three patients (10%) had a genetically proven congenital syndrome with associated neurologic anomalies. Overall success rate was 87%. Failures were observed in four (13%) of 31 cases; including all three patients presenting a congenital syndrome. Conclusions Supraglottoplasty is an effective and safe treatment for laryngomalacia in otherwise healthy children. Signs of a possible underlying predominant neurologic origin and discrepancy between the clinical presentation and the endoscopic findings have to be taken into account, as in children with congenital syndrome with neurologic anomalies the risk of failure is higher.

Actinomyces is a bacterial rod found in the normal oral flora. It can gain entry to the submucosa via trivial wounds and ultimately lead to slow growing lesions which may mimic cancerous lesions.We present the case of an elderly, immunosuppressed woman who presented with a herald bleed from a tonsillar lesion. Despite initial operative arrest of the hemorrhage, she died of a carotid blowout. At autopsy, no sign of cancer was found, but rather an invasive actinomycosis.Although actinomycosis of the tonsil is well known and has even been described as mimicking tonsillar cancer, this is, to our knowledge, the first report of a carotid blowout secondary to actinomycosis.

Direct intracochlear acoustic pressure recordings (from 20 to 20,000 Hz) are used to measure the middle-ear transfer functions (forward and reverse) and to better understand the cochlear mechanics in the guinea pig. In the forward direction, the middle-ear transfer function is strongly dependent on the frequency and presents a maximum of +30 dB at 1,000 Hz (bulla open). In the reverse direction, the middle-ear transfer function looks like an ideal reverse middle-ear pressure transformer with –35 dB gain and 0° phase lag from 20 to 8,000 Hz (bulla open, closed ear canal). Passive cochlear mechanics is studied with the help of intracochlear pressure measurements and differential cochlear microphonic potential recordings in the different turns.

The fine tuning mechanisms involved in the normal processing of sound in the cochlea are non-linear, hence combination tones are generated inside the cochlea when a pair of low-level pure tones with neighbouring frequencies f1 and f2 is used as a stimulus. Their detection as sounds in the ear canal proves that they undergo backward propagation in the cochlea and through the middle ear, and the non-invasive measurement of the combination tone at 2f1–f2, called the cubic difference tone (CDT), has become a routine method of monitoring cochlear function. In order to gain information on the hypothetical places where CDTs are generated, on their intracochlear levels and propagation velocities, direct measurements of CDT pressure waves were carried out in scala vestibuli and tympani of the first and second turn of the guinea-pig cochlea. Cubic difference tones at 2f1–f2 varied from 0.75 to 9 kHz and were measured with a miniature piezoresistive transducer. Its high sensitivity allowed the detection of CDTs whenever their levels exceeded 5 dB SPL in the ear canal, i.e. 40 dB SPL (re: 20 μPa) inside the cochlea. The levels of CDTs were similar in scala vestibuli of the first and second turn. Phase comparisons between measurements at 2f1–f2 in the first and second turn allowed determination of the place where the CDT phase was minimum. It provided an estimation of the generation site of the CDT, which appeared to be close to the place tuned to f2 for stimulus levels lower than 70 dB SPL. Forward and backward travel times from one turn to the other were assessed at several frequencies, and both values were shorter than 0.2 ms. In contrast, the overall ‘round-trip’ delay of CDTs, measured in the ear canal, was about five times larger, suggesting that local filtering processes rather than propagation delays account for the overall CDT delay.

The effects of a consistently disappearing and reappearing spontaneous otoacoustic emission (SOAE) at around 2280 Hz on microstructure for pure tones of varying durations in a 33 year-old woman with normal hearing was studied. The SOAE began to appear after 10–15 min in a quiet test room and increased in level by up to 22 dB over a 30–40-min period. The SOAE was measured every 12 to 15 min. Between measurements, the subject performed a signal detection task for pure tones with total durations varying from 20 to 320 ms. The signal frequencies were within a ±30-Hz range relative to the SOAE frequency. For signal durations of 40–320 ms, there was a local dip at the target SOAE frequency when it was either not detectable or its level was lower than −14 dB SPL. Subjective threshold levels were as much as 12 dB better than those obtained when the SOAE was −6 dB SPL or greater. The results suggest that a region of the cochlea with high sensitivity and instability can be put into self-oscillation producing an SOAE, possibly by a change of efferent activity. Hearing threshold is affected possibly due to adaptation or masking.

Otoacoustic emissions are increasingly useful for determining cochlear function noninvasively. It is widely agreed that these acoustic signals reflect micromechanical processes in the cochlea. However, their quantitative interpretation requires knowledge of the ways in which vibrations travelling back to the ear canal from the cochlea are shaped by the middle ear. An intracochlear source is needed to derive the reverse middle-ear transfer function (rMETF) by comparing pressure in the external ear canal to the corresponding pressure in scala vestibuli. In the present study, the rMETF was obtained in vivo in the guinea pig using as intracochlear sound source the cubic difference tones (CDTs) generated by a pair of external pure tones. With a closed ear canal and open bulla, the rMETF was found to be flat (−35 dB) over a broad frequency range (1.5–8 kHz). The differences between forward and reverse METF could be explained by different loads acting on the middle ear network, which depends on the direction of signal transmission. With knowledge of the rMETF, it becomes possible to quantify CDTs within the cochlea by measuring them noninvasively in the ear canal.

Helicobacter pylori is known to cause gastric cancer. Presence and carcinogenicity in the upper aerodigestive system is doubtful. This study examined the prevalence of Helicobacter pylori and related factors in biopsies from the upper aerodigestive tract (UADT) in patients with gastric colonization by Helicobacter pylori.In a case series study, 26 patients with histopathologically confirmed gastric colonization were identified. A polymerase chain reaction (PCR) was performed on matched formalin-fixed and paraffin-embedded tissues of the stomach and the oral cavity, pharynx, or larynx.Helicobacter pylori was found in 38% of the samples from the oral cavity, pharynx, and larynx. An association with malignancies in these regions or possible risk factors, such as age, smoking, or alcohol, was not found.The upper aerodigestive system seems to be an additional reservoir in a significant percentage of patients presenting with Helicobacter pylori gastritis.
