One anastomosis/mini gastric bypass (OAGB/MGB) is up to date the third most performed obesity and metabolic procedure worldwide, which recently has been endorsed by ASMBS. The main criticisms are the risk of bile reflux, esophageal cancer, and malnutrition. Although IFSO has recognized this procedure, guidance is needed regarding selection criteria. To give clinicians a daily support in performing the right patient selection in OAGB/MGB, the aim of this paper is to generate clinical guidelines based on an expert modified Delphi consensus.A committee of 57 recognized bariatric surgeons from 24 countries created 69 statements. Modified Delphi consensus voting was performed in two rounds. An agreement/disagreement among ≥ 70.0% of the experts was considered to indicate a consensus.Consensus was achieved for 56 statements. Background For severely obese patients, bariatric surgery has been recommended as an effective therapy. Objectives The Bariataric Surgery and Education (BaSE) study aimed to assess the efficacy of a videoconferencing-based psychoeducational group intervention in patients after bariatric surgery. Setting The BaSE study is a randomized, controlled multicenter clinical trial involving 117 patients undergoing bariatric surgery (mean preoperative body mass index [BMI] 49.9 kg/m 2 , SD 6.4). Patients were enrolled between May 2009 and November 2012 and were randomly assigned to receive either conventional postsurgical visits or, in addition, a videoconferencing-based 1-year group program. Methods Primary outcome measures were weight in kilograms, health-related quality of life (HRQOL), and general self-efficacy (GSE). Secondary outcome measures were depression symptoms and eating behavior. Results 94% of the patients completed the study. Mean weight loss for all patients was 45.9 kg (SD 16.4) 1 year after surgery (mean excess weight loss [EWL] 63%). Intention-to-treat analyses revealed no differences in weight loss, EWL, HRQOL, or self-efficacy between study groups at 1 year after surgery. However, patients with clinically significant depression symptoms (CSD) at baseline assigned to the intervention group (n = 29) had a significantly better HRQOL ( P = .03), lower depression scores ( P = .02), and a trend for a better EWL (.06) 1 year after surgery compared with the control group (n = 20). Conclusion We could not prove the efficacy of the group program for the whole study sample. However, results indicate that the intervention is effective for the important subgroup of patients with CSD.

Every surgical problem that increases the likelihood of intraoperative and postoperative complications is considered to be a difficult surgical situation. Based on this definition, Korenkov et al. proposed to classify patients according to the following intraoperative difficulty levels (I to IV): (I) ideal situation (easy to operate, no problems), (II) fairly easy/manageable/simple (some minor difficulties may occur), (III) difficult/problematic (difficult to operate; some operative techniques are considerably more difficult than others), and (IV) very difficult (every operative step is difficult/challenging). Kaafrani et al. proposed a severity classification for intraoperative adverse events. Depending on the severity level, classes range from I (injury requiring no repair) to VI (intraoperative death). Clavien and colleagues published a globally established classification system for postoperative complications. In this classification, the severity of postoperative complications ranges from severity grade I (minimal deviation from the normal postoperative course) to severity grade V (death of patient). Based on the proposed classifications and the problems of individual surgical decision-making, we had the idea to create a Register of Difficult Intraoperative Situations (DIS register). The basic principle of such a register is the collection of an individual expert's experiences. The scientific analysis should focus on patients with apparent modifications in treatment due to difficult intraoperative situations. Registration and processing of enrolled cases will be performed anonymously based on an appropriate IT platform. The main goal of this register is to develop an accessible database for practising surgeons. This will provide an opportunity for every surgeon to find out what other surgeons did in similar situations.Als schwierige chirurgische Situation wird jedes Problem, das zu einem erhöhten Risiko für intra- oder postoperative Komplikationen führen kann, bezeichnet. Bezogen auf diese Definition wurde von Korenkov et al. eine präoperative Klassifikation erweiterter intraoperativer Schwierigkeiten entwickelt. Diese Klassifikation bezieht sich auf das Ausmaß der chirurgisch-technischen Schwierigkeiten von Schweregrad 1 – technisch einfach zu operieren, bis Schweregrad 4 – jede operationstechnische Handlung ist sehr schwierig. Für die Evaluierung des Schweregrads der intraoperativen Komplikationen schlug die Arbeitsgruppe um Kaafarani die Klassifikation der intraoperativen negativen Ereignisse vor. In Abhängigkeit vom Schweregrad wurden die intraoperativen Komplikationen auf 6 Klassen von der 1. (unproblematisch beseitigte Komplikationen) bis zur 6. Klasse (intraoperativer Exitus letalis) aufgeteilt. Für eine Systematisierung des Schweregrads der postoperativen Komplikationen hat sich weltweit die Klassifikation von Clavien-Dindo etabliert. In dieser Klassifikation sind die postoperativen Komplikationen von Schweregrad 1 (jede Abweichung vom normalen postoperativen Verlauf mit „minimaler“ Behandlung) bis zum Schweregrad 5 (Tod des Patienten) aufgeteilt. Gestützt auf die vorgeschlagenen Klassifikationen und auf die Problematik der individuellen Entscheidungen in der Chirurgie entstand die Grundidee, ein Register (SCS-Register) zu etablieren. Das Hauptprinzip solcher Studien ist die Ansammlung der individuellen Erfahrungen von operierenden Chirurgen. Die wissenschaftliche Analyse fokussiert sich auf die Sondersituationen, bei denen das Behandlungskonzept an die individuelle Patientensituation adaptiert war und manchmal von den gängigen Standards abwich. Die Registrierung und Bearbeitung der angemeldeten Fälle wird auf der Basis der entsprechenden IT-Plattform anonym geführt. Das Ziel dieses Registers ist, die schwierigen chirurgischen Fälle bundesweit zu sammeln, zu registrieren und zu analysieren, um für die operierenden Chirurgen eine zugängliche Datenbank zu gestalten. Dabei wird für jeden Chirurgen die Möglichkeit bestehen, nachzulesen, wie der eine oder andere Kollege in ähnlich schwierigen Situationen gehandelt hat.

Background Evidence regarding the efficacy of psychosocial interventions after bariatric surgery is rare and shows conflicting results. Objectives The Bariatric Surgery and Education (BaSE) study aimed to assess the efficacy of a psychoeducational group intervention in patients after bariatric surgery. Setting The BaSE study was a randomized, controlled, multicenter clinical trial involving 117 patients who underwent bariatric surgery. Patients received either conventional postsurgical visits or, in addition, a 1-year psychoeducational group program. The present study evaluated the sustained effects of the intervention program. Mean follow-up duration was 37.9 months (standard deviation [SD] 8.2 months) after surgery. Methods Outcome measures were as follows: body mass index (BMI), weight loss, self-efficacy, depression severity, and health-related quality of life (HRQOL). Groups were compared using an intention-to-treat approach with a mixed model for repeated measurements. Results A total of 74 patients (63.2%) completed the follow-up (T5) assessment. Mean weight loss for all patients was 43 kg (SD 15.5 kg) at T5 (mean BMI 35.1 kg/m 2 ). Mean excess weight loss was 60.4%. The effects of the surgery during the first postsurgical year were reflected, on average, by both decreasing weight and psychosocial burden. At the T5 time point, patients had slowly started to regain weight and to deteriorate regarding psychosocial aspects. However, at T5, patients who had participated in the intervention program (n = 39) showed significantly lower depression severity scores ( p = .03) and significantly higher self-efficacy ( p = .03) compared to the control group (n = 35). The 2 groups did not differ regarding weight loss and quality of life. Conclusion Psychoeducational intervention shows sustained effects on both depression severity scores and self-efficacy.

Roux-en-Ygastric bypass (RYGB) and sleeve gastrectomy (SG) rank among the most frequently applied bariatric procedures worldwide due to their positive risk/benefit correlation. A systematic review revealed a similar excess weight loss (EWL) 2 years postoperatively between SG and RYGB. However, there is a lack of randomized controlled multi-centre trials comparing SG and RYGB, not only concerning EWL, but also in terms of remission of obesity-related co-morbidities, gastroesophageal reflux disease (GERD) and quality of life (QoL) in the mid- and long-term. The BariSurg trial was designed as a multi-centre, randomized controlled patient and observer blind trial. The trial protocol was approved by the corresponding ethics committees of the centres. To demonstrate EWL non-inferiority of SG compared to RYGB, power calculation was performed according to a non-inferiority study design. Morbidity, mortality, remission of obesity-related co-morbidities, GERD course and QoL are major secondary endpoints. 248 patients between 18 and 70 years, with a body mass index (BMI) between 35–60 kg/m2 and indication for bariatric surgery according to the most recent German S3-guidelines will be randomized. The primary and secondary endpoints will be assessed prior to surgery and afterwards at discharge and at the time points 3–6, 12, 24, 36, 48 and 60 months postoperatively. With its five year follow-up, the BariSurg-trial will provide further evidence based data concerning the impact of SG and RYGB on EWL, remission of obesity-related co-morbidities, the course of GERD and QoL. The trial protocol has been registered in the German Clinical Trials Register DRKS00004766 .

HINTERGRUND: Der temporÄre Einsatz eines Magenballons zur Behandlung der Adipositas Grad1 und 2 nimmt weltweit zu; bei der Adipositas Grad 3 wird der Magenballon als adjuvantes Hilfsmittel zur prÄoperativen Gewichtsreduktion implantiert. Ziel dieser retrospektiven Kohorten analyse ist, die Wirksamkeit des Magenballons auf Gewichtsreduzierung und das Risikoprofil der Methode zu evaluieren. METHODEN: Retrospektive Kohortenanalysen aus 4 Adipositas-Zentren, in denen der Magenballon seit 2001 regelmÄßig implantiert wurde. ERGEBNISSE: Von Februar2001 bis April 2008 wurde bei 634 Patienten ein Magenballon implantiert (BIBTM Intragastric Balloon System; Allergan Medical, Irvine, CA, USA). Die Geschlechterverteilung war 31,5% MÄnnerzu 68,5% Frauen; das Durchschnittsalter betrug 41,5 Jahre. Das durchschnittliche Ausgangsgewicht lag bei 126 kg. Der initiale BMI bei Implantation des Magenballons war 42,5 kg/m(2). Die Implantation der Prothese war in allen FÄllen unkompliziert. Der durchschnittliche Gewichtsverlust lag bei 20,75 kg bzw. 7,05 BMI-Punkten. SUMMARY: The Gastric Balloon — a Retrospective Cohort Analysis with 634 Patients BACKGROUND: The temporary use of a gastric balloon for the treatment of obesity grade 1 and 2 is increasing worldwide, whereas in grade 3 obesity, it is implanted as a tool for preoperative adjuvant weight loss. The aim of this retrospective cohort analysis is to evaluate the effectiveness of weight reduction and to describe the risk profile of the method. METHODS: Retrospective cohort analysis of 4 obesity centers where gastric balloons had been regularly implanted since 2001. RESULTS: Between February 2001 and April 2008, the gastric balloon (BIBTM Intragastric Balloon System; Allergan Medical, Irvine, CA, USA) was implanted in 634 patients. The gender ratio was 31.5% males to 68.5% females; the average age was 41.5 years. The average initial weight was 126 kg. The initial BMI at implantation of the gastric balloon was 42.5 kg/m2. The implantation of the prosthesis was uncomplicated in all cases. Average weight loss was 20.75 kg or 7.05 BMI points, respectively.

The only effective treatment for patients with morbid obesity is surgery. Laparoscopic bariatric surgery has become quite popular in attempts to decrease the morbidity associated with laparotomy. The aim of this study was to assess the safety and efficiency by using SurgASSIST(R) for performing the gastrojejunostomy in laparoscopic Roux-en-Y gastric bypass (RYGBP). The variables were compared with the results using the standard laparoscopic circular end-to-end anastomotic stapler (CEEA(R)).After randomization, the gastro-jejuostomy of RYGBP was performed in ten patients by transabdominal introduced circular stapler (group A) and in ten patients by transorally introduced circular stapler (SurgASSIST(R); group B) via five-port laparoscopy. A prospective 12-month postoperative follow-up including documentation of minor and major complication as well as weight loss and body composition is done every 8 weeks.The average body mass index (BMI, 52 kg/m(2)) and the other baseline characteristics were equally distributed in both groups. There was no difference in reduction of BMI, excess weight loss, and fat mass in both groups. The rate of port site wound infection in group A was significantly higher (p = 0.03) when compared to group B. There was no anastomotic leak or stricture postoperatively in both groups.Performing of a gastrojejunostomy in RYGBP by SurgASSIST is a safe and feasible method in comparison to conventional circular stapler systems. The advantage of SurgASSIST is the avoidance to introduce the stapler through the abdominal wall and, by this, a possible port site wound infection. Further prospective studies have to be performed to verify the advantages of the SurgASSIST in comparison to conventional circular stapler systems.

Morbid obesity is a serious disease that is responsible for several co-morbid conditions. Patients with a body mass index of over 40 kg/m2 will require surgical intervention if a dietary program fails. In the US, patients who are morbidly obese number approximately 8 million. At present, three conventional operations, with acceptable early and late complication rates, are performed most frequently worldwide: vertical gastroplasty (VRG), vertical banded gastroplasty (VBG), and gastric bypass (GB). In this article, we describe our experience with the LAP-BAND technique, and in particular with the new LAP-BAND adjustable gastric banding system (LAGB), which further reduces postoperative complications and reoperations.

With advancing age functional and morphological changes take place within the intestinal tract. The age-dependency of intestinal absorption has been studied using a d -xylose absorption test. The absorption rate (constant k 12 ), the time of appearance of the relative maximum of the d -xylose concentrations in the blood serum after oral administration (time t m ), and the extent of the relative absorption in the small intestine as a function of time RA ( t ) were determined by graphic-mathematical evaluation of the blood level kinetics of d -xylose after oral and intravenous administration. Within the age range of 3 to 96 yr, it has been proven that a slowing down of intestinal absorption occurs with advancing age. By prolonging the period during which the food components are absorbed in the small intestine (shift of t m towards later times) the old organism is just as capable of complete absorption as in the young one, even though the absorption processes take place at a slower rate. Absorption disorders in old age are due to morbidity and not, sui generis, to involution.

To test the absorption of orally administered D-xylose in the upper small intestine its concentration in the blood plasma must be measured. Absorption is determined as a function of time by subtraction of the blood values found after intravenous administration, which would indicate the rates of metabolism and extraction of xylose by the body. It is suggested that comparison with the behavior of a proposed four compartment model may give a biological interpretation of the results. The possible dependence of amount and rate of absorption can be determined if the same time after feeding of the same diet is used in each absorption measurement. It is suggested that the time point to be used for administration be that time at which the highest concentration of xylose in the peripheral blood is found.
