Abstract

Este artigo tem como objetivo avaliar a existência de gerenciamento de resultados (GR) em planos previdenciários das entidades fechadas de previdência complementar (EFPC) brasileiras. A hipótese de GR foi testada por meio de um modelo de accruals específicos, usando, dentre outras, a variável “resultado anual excluído as contingências judiciais”, para explicar a variável dependente “provisões para contingências judiciais”, em uma amostra com 5.892 observações. Foi possível verificar que a primeira explica a segunda e esse relacionamento é positivo, o que é um argumento favorável à hipótese de GR. Os achados sugerem que as EFPC escolhem fazer provisões quando os planos tendem a ser superavitários antes do reconhecimento dessas provisões e realizam reversões quando uma situação de déficit pode ser divulgada. Ou seja, existem indícios de que os gestores atuam de maneira oportunística para melhorarem artificialmente a saúde econômica dos planos, suavizando ou ocultando eventuais resultados negativos de seus participantes.

2. Resurfacing of Two Separate Digital Defects Using a Single Fascial Free Flap with Neosyndactylization

Abstract

We report a case of a 20-year-old patient who sustained a mutilating crush injury to the left-hand index and ring finger volar surface measuring 5 × 6 cm from the distal end of the proximal phalanx to the fingertips. After thorough debridement and stabilization of the skeletal injury, a radial forearm fascia only free flap measuring 6 × 7 cm was used to resurface the distal soft tissue volar defect of two adjacent fingers. Digital nerve grafting was also required, and this was done using autologous nerve graft from the lateral antebrachial cutaneous nerve. This thin fascia only flap allows for stable soft tissue coverage and provides a gliding surface for the underlying tendons. The neosyndactylized digits were safely divided at 3-month follow-up, and excellent functional and aesthetic results were achieved. The radial forearm fascia is a thin, durable, and pliable tissue that is based on the radial artery as a vascular pedicle. We consider this free fascial flap as a valuable option for coverage of multiple complex distal digit injuries using a single flap and highly recommend its use.

Abstract

This article aims to contribute to the understanding of the process of import substitution in Sub-Saharan Africa. The process of industrialization in Sub-Saharan Africa occurred in two phases: a first step, even very early during the colonial regime began around the 1920s and ended in the late forties; a second phase of industrialization began in the late fifties and gained momentum in the sixties, when import substitution was implemented more widely. Although these countries were the last to embark on the strategy of import substitution, they followed the same steps of Latin American countries, and as the structural domestic and external constraints were too strong, the failure of the policy of import substitution arrived early and the negative impact on these economies had a greater magnitude.

Abstract

Emerging infectious diseases with zoonotic potential often have complex socioecological dynamics and limited ecological data, requiring integration of epidemiological modeling with surveillance. Although our understanding of SARS-CoV-2 has advanced considerably since its detection in late 2019, the factors influencing its introduction and transmission in wildlife hosts, particularly white-tailed deer (Odocoileus virginianus), remain poorly understood. We use a Susceptible-Infected-Recovered-Susceptible epidemiological model to investigate the spillover risk and transmission dynamics of SARS-CoV-2 in wild and captive white-tailed deer populations across various simulated scenarios. We found that captive scenarios pose a higher risk of SARS-CoV-2 introduction from humans into deer herds and subsequent transmission among deer, compared to wild herds. However, even in wild herds, the transmission risk is often substantial enough to sustain infections. Furthermore, we demonstrate that the strength of introduction from humans influences outbreak characteristics only to a certain extent. Transmission among deer was frequently sufficient for widespread outbreaks in deer populations, regardless of the initial level of introduction. We also explore the potential for fence line interactions between captive and wild deer to elevate outbreak metrics in wild herds that have the lowest risk of introduction and sustained transmission. Our results indicate that SARS-CoV-2 could be introduced and maintained in deer herds across a range of circumstances based on testing a range of introduction and transmission risks in various captive and wild scenarios. Our approach and findings will aid One Health strategies that mitigate persistent SARS-CoV-2 outbreaks in white-tailed deer populations and potential spillback to humans. Author summary: Novel zoonotic diseases persist and evolve in both human and non-human hosts, posing challenges for human and animal health. SARS-CoV-2, the virus responsible for the COVID-19 pandemic in humans, has been detected in white-tailed deer (Odocoileus virginianus) yet we have limited understanding of its introduction and transmission within deer populations. Here, we use epidemiological models to describe SARS-CoV-2 introduction and transmission patterns within wild and captive white-tailed deer populations. We found that captive deer herds faced a higher risk of spillover of SARS-CoV-2 from humans and higher transmission rate compared to wild deer. Despite these differences in wild and captive contexts, we found that transmission in both wild and captive contexts often resulted in persisting circulation of SARS-CoV-2. This circulation was determined by deer-deer transmission in both wild and captive contexts, rather than by high rates of introduction from humans. Outbreaks in wild populations were greater when these animals could interact with captive deer populations circulating SARS-CoV-2. SARS-CoV-2 appears to circulate in both captive and wild deer across a range of circumstances. Our findings help to inform how to best mitigate the introduction and spread of SARS-CoV-2 in deer, with benefits to protecting human health. [ABSTRACT FROM AUTHOR]

12. The construction briefing process : evaluating the influence of personality traits on the performance of the consultant project manager

Abstract

The briefing process is critical for achieving project success and client satisfaction. It typically involves the consultant project manager co-ordinating the efforts of the demand and supply side stakeholders at the conceptual stage of a project, to identify and define the client requirements. The importance of such early intervention has been highlighted by public sector agencies and professional bodies, leading to the publication of numerous guidance notes and best practice manuals for briefing. Despite such extensive guidance literatures and efforts research shows that current briefing practice is inadequate and a weakness in the construction process. Broadly, the briefing process involves consultant project managers from different background and personality traits. Personality traits are thought of as a pervasive style of thinking, feeling and behaving. The literature demonstrate effective performance relates to an individual`s personality traits. The study attempts to evaluate the influence of personality traits of the consultant project manager on the briefing process. A review of the literature is structured under three chapters; (1) briefing process; (2) consultant project manager role in briefing and; (3) personality traits relationship and influence on performance to introduce a new perspective. It is found that there are correlations between personality traits and effective performance. From the review sixteen traits have been extracted and assessed to be related to the consultant project manager performance in construction briefing. The investigation attempts to establish the relationships between the 16 traits and the consultant project manager performance, and seeks to determine how each trait contributes to effective briefing. This is explored through observing the consultant project manager in briefing and an industry wide questionnaire survey. As the research focuses on the influence of sixteen personality traits on the consultant project manager performance in the briefing process the data was analysed by the Pearson product moment (correlation) coefficient method to evaluate relationships. The research concludes that the consultant project manager performance is connected to the iii sixteen personality traits dimensions tested and these traits are associated with effective briefing performance. The findings have implications for selecting consultant project managers. The implications of the results of this research, and recommendations for future investigations are discussed.

14. Role Perception of Minority Admission Professionals: History of Minority Admission Administrators on Predominantly White Campuses.

Abstract

In order to evaluate the role of race in the experience of African-American and other non-white admissions professionals at institutions of higher education, a survey was conducted. The study subjects were 130 minority admissions professionals who participated in a staff development program for non-white admissions professionals. The study sought to explore the following broad questions: (1) how do minority admissions workers view their role and status; (2) what are the advantages and disadvantages of being a black admissions professional on a predominantly white campus; (3) what role does affirmative action play in hiring of admissions professionals; (4) how do these admissions workers view their role as mentors for minority students; (5) and what is the minority admissions worker's role in institutional decision making. Results suggest that minority admissions professionals want to participate more actively and significantly in the policy-decision making of admissions offices and that these admissions workers are satisfied working with minority students and do not feel their employment is tokenism. However, they do perceive a different set of criteria used to hire minority admissions counselors than that used to hire white ones. In addition, most feel that their programs are not sufficiently funded. The report includes 12 references and a copy of the survey instrument. (JB)

Abstract

Background: Despite advances in surgery and pharmacotherapy, there remains significant residual ischemic risk after coronary artery bypass grafting surgery. Methods: In REDUCE-IT (Reduction of Cardiovascular Events With Icosapent Ethyl–Intervention Trial), a multicenter, placebo-controlled, double-blind trial, statin-treated patients with controlled low-density lipoprotein cholesterol and mild to moderate hypertriglyceridemia were randomized to 4 g daily of icosapent ethyl or placebo. They experienced a 25% reduction in risk of a primary efficacy end point (composite of cardiovascular death, myocardial infarction, stroke, coronary revascularization, or hospitalization for unstable angina) and a 26% reduction in risk of a key secondary efficacy end point (composite of cardiovascular death, myocardial infarction, or stroke) when compared with placebo. The current analysis reports on the subgroup of patients from the trial with a history of coronary artery bypass grafting. Results: Of the 8179 patients randomized in REDUCE-IT, a total of 1837 (22.5%) had a history of coronary artery bypass grafting, with 897 patients randomized to icosapent ethyl and 940 to placebo. Baseline characteristics were similar between treatment groups. Randomization to icosapent ethyl was associated with a significant reduction in the primary end point (hazard ratio [HR], 0.76 [95% CI, 0.63–0.92]; P =0.004), in the key secondary end point (HR, 0.69 [95% CI, 0.56–0.87]; P =0.001), and in total (first plus subsequent or recurrent) ischemic events (rate ratio, 0.64 [95% CI, 0.50–0.81]; P =0.0002) compared with placebo. This yielded an absolute risk reduction of 6.2% (95% CI, 2.3%–10.2%) in first events, with a number needed to treat of 16 (95% CI, 10–44) during a median follow-up time of 4.8 years. Safety findings were similar to the overall study: beyond an increased rate of atrial fibrillation/flutter requiring hospitalization for at least 24 hours (5.0% vs 3.1%; P =0.03) and a nonsignificant increase in bleeding, occurrences of adverse events were comparable between groups. Conclusions: In REDUCE-IT patients with a history of coronary artery bypass grafting, treatment with icosapent ethyl was associated with significant reductions in first and recurrent ischemic events. Abstract

Single-point incremental forming (SPIF) was conducted on a 1-mm-thick commercially pure titanium grade 2 (Ti-G2) sheet metal in a CNC vertical milling unit. A hardened steel ball of 12 mm diameter was used as forming tool. Frustum cups were formed with varying spindle speeds between 300, 450, and 600 RPM. Other process parameters including the vertical step down and feed rate were kept as 0.2 mm and 300 mm/min, respectively. The metallurgical and mechanical properties of the formed material were investigated by cutting samples from the frustum cup walls. Electron-backscattered diffraction (EBSD) investigation revealed limited change in grain size with an increase in spindle speed. Dislocation density was measured by x-ray diffraction peak broadening analysis. The results indicate that an increase in spindle speed resulted in an increased dislocation density. The EBSD-based textural studies revealed a strong basal texture with near P and B type orientations visible at the maximum spindle speed. The tensile tests demonstrated a proportional increase in tensile strength with an increase in spindle speed along with a significant reduction in total ductility. The enhanced dislocation density and the formation of a strong basal texture were considered as the main drivers for the improvement in the tensile strength. A maximum tensile strength of nearly 550 MPa was obtained for samples extracted from the walls of the frustum cup at the maximum spindle speed of 600 RPM. This translates to an 80% enhancement of the tensile strength when compared to the base metal .

Abstract

Fatigue life is an important design criterion for many structural parts. The surface integrity induced by a typical machining operation may have a marked effect on fatigue life. Literature evidence suggests that the compressive surface residual stress state may change appreciably from the state obtained at the recommended conventional cutting speeds when intermediate cutting speeds are utilized. Titanium alloys typically show an increased compressive residual stress state at intermediate speeds before eventually tending towards tensile stresses at elevated cutting speeds. This paper investigates the effect of an optimised cutting strategy to control the residual stress state of a machined Ti-6Al-4V ELI (extra low interstitial) component to thereby control the fatigue life. An experimental program is conducted whereby stepped shafts are machined at various cutting speeds before being subjected to a full cyclic in-plane bending moment fatigue test. Cutting speed is varied between the typically recommended speed of 40 m/min to an intermediate speed of 110 m/min. Residual stresses are measured by XRD and compared to the fatigue life achieved. Other surface integrity descriptors including surface roughness, sub surface microstructure and selected surface damage effects are presented and discussed in relation to the fatigue performance. In essence the paper reports that fatigue life can be improved by machining at the optimum cutting speeds. These optimum intermediate speeds do however introduce other less desirable effects such as an increase in surface damage including the presence of weldments and ploughing grooves that may have a negative effect on the reliability of the improvements obtained.

Abstract

All fabrication techniques utilized to manufacture metallic parts modify the surface integrity of the part. Complementary machining is a relatively recent machining strategy characterized by combining metal cutting and mechanical surface treatment. Typically, it implies that after conventional machining, the cutting insert is used reversely to modify the surface by local plastic deformation. To improve product performance, mechanical surface treatment is an additional phase in the manufacturing process chain that usually results in longer production times and higher costs. As a result, a variety of hybrid techniques have been created, such as complementary machining, which has the benefit of using conventional machine tools and their associated cutting tools. The study on complementary machining is reviewed in the article. The main focus is to assess the viability of complementary machining to modify surface integrity for enhanced properties by specifically establishing its effect on tool wear, surface roughness, microhardness, fatigue, microstructure, and residual stress state.

Im Entwurf innovativer Flugzeugkabinenkonzepte für die Zukunft zählt die Passagierakzeptanz zu den wichtigsten Kriterien für Hersteller, Fluggesellschaften und Forschungseinrichtungen. Im Zuge wachsender Passagierzahlen sowie weltweiter gesellschaftlicher Veränderungen besteht die zentrale Herausforderung darin, dass sich veränderte Anforderungsprofil eines künftigen Nutzerspektrums bereits im frühen Entwurfsstadium zu identifizieren und in den Entwurfsprozess zu übertragen. Im Zuge dessen zeigen nutzerorientierte Entwurfsansätze wie die Design Thinking Methode ein hohes Potential, was die stetig wachsende Popularität in industriellen Anwendungsfällen aus den Bereichen der Konsumgüter und der Automobilindustrie unterstreicht. In diesem Paper wird die Applikation der Design Thinking Methode auf den Kabinenentwurfsprozess vorgestellt. Als Basis für ein Praxisbeispiel wird die Nutzergruppe beruflich-reisender Fluggäste mithilfe einer Persona Definition identifiziert und für den nutzerorientierten Kabinenentwurf verwendet. Einen weiteren Baustein bildet der Ideen- und Prototypen-Entwurfsprozess mittels digitaler Design- und Modellierungsanwendungen. Ein iterativer Bewertungs- und Anpassungsprozess erfolgt dabei durch eine Konzeptentwurfsabbildung in der virtuellen Realität sowie einer agentenbasierten Passagierflusssimulation. Appliziert auf den Entwurfsprozess von Flugzeugkabinenkonzepten liefert der Design Thinking Ansatz eine zeit- und qualitätseffiziente Alternative zu bestehenden Design Methoden, was durch den digitalen Prototypen- und Bewertungsprozess im frühen Entwicklungsstadium gewährleistet wird. Zudem wird im Rahmen dieser Arbeit ein möglicher Ansatz zur Schließung der Lücke zwischen der technologisch komplexen Umgebung einer Flugzeugkabine und den sich ändernden Bedürfnissen des zukünftigen Passagiers skizziert.

Supplemental Digital Content is available in the text., Background: In the WRAP-IT trial (Worldwide Randomized Antibiotic Envelope Infection Prevention), adjunctive use of an absorbable antibacterial envelope resulted in a 40% reduction of major cardiac implantable electronic device infection without increased risk of complication in 6983 patients undergoing cardiac implantable electronic device revision, replacement, upgrade, or initial cardiac resynchronization therapy defibrillator implant. There is limited information on the cost-effectiveness of this strategy. As a prespecified objective, we evaluated antibacterial envelope cost-effectiveness compared with standard-of-care infection prevention strategies in the US healthcare system. Methods: A decision tree model was used to compare costs and outcomes of antibacterial envelope (TYRX) use adjunctive to standard-of-care infection prevention versus standard-of-care alone over a lifelong time horizon. The analysis was performed from an integrated payer-provider network perspective. Infection rates, antibacterial envelope effectiveness, infection treatment costs and patterns, infection-related mortality, and utility estimates were obtained from the WRAP-IT trial. Life expectancy and long-term costs associated with device replacement, follow-up, and healthcare utilization were sourced from the literature. Costs and quality-adjusted life years were discounted at 3%. An upper willingness-to-pay threshold of $150 000 per quality-adjusted life year was used to determine cost-effectiveness, in alignment with the American College of Cardiology/American Heart Association practice guidelines and as supported by the World Health Organization and contemporary literature. Results: The base case incremental cost-effectiveness ratio of the antibacterial envelope compared with standard-of-care was $112 603/quality-adjusted life year. The incremental cost-effectiveness ratio remained lower than the willingness-to-pay threshold in 74% of iterations in the probabilistic sensitivity analysis and was most sensitive to the following model inputs: infection-related mortality, life expectancy, and infection cost. Conclusions: The absorbable antibacterial envelope was associated with a cost-effectiveness ratio below contemporary benchmarks in the WRAP-IT patient population, suggesting that the envelope provides value for the US healthcare system by reducing the incidence of cardiac implantable electronic device infection. Abstract

Proximity proteomics has greatly advanced the analysis of native protein complexes and subcellular structures in culture, but has not been amenable to study development and disease in vivo. Here, we have generated a knock-in mouse with the biotin ligase (BioID) inserted at titin's Z-disc region to identify protein networks that connect the sarcomere to signal transduction and metabolism. Our census of the sarcomeric proteome from neonatal to adult heart and quadriceps reveals how perinatal signaling, protein homeostasis and the shift to adult energy metabolism shape the properties of striated muscle cells. Mapping biotinylation sites to sarcomere structures refines our understanding of myofilament dynamics and supports the hypothesis that myosin filaments penetrate Z-discs to dampen contraction. Extending this proof of concept study to BioID fusion proteins generated with Crispr/CAS9 in animal models recapitulating human pathology will facilitate the future analysis of molecular machines and signaling hubs in physiological, pharmacological, and disease context.

Abstract

The microstructure and mechanical properties of friction and laser beam welded Grade 2 titanium tubes (wall thickness of 3.9 mm and outer diameter of 60 mm) are compared in the present investigation. Friction welding (FW) was conducted on a vertical friction processing system. Laser beam welding (LBW) was accomplished by utilizing a 4 KW Nd:YAGlaser. The microstructure evolution was observed by optical microscopy and advanced techniques such as TEM and EBSD. The friction welded joints showed fine grains due to dynamic recrystallization. A variation in grain size towards the outer circumference of the tube was observed due to flash formation. The laser beam welded joints displayed large grains with irregular boundaries. Both joints exhibited an elevated dislocation density due to severe plastic deformation and thermal stresses respectively. Strengthening of the joint area was recorded in both joints albeit due to different strengthening mechanisms. The failure location, during mechanical testing, occurred in the base metal for both processes. Overall when compared, LBW is probably the preferred process to join these tubes due to its higher welding speed and therefore reduced production cycle.
