1. The impact on adolescents of a Transtheoretical Model-based programme on fruit and vegetable consumption

Abstract

Objective:The present study aimed to evaluate the impact of a Transtheoretical Model-based programme titled ‘Fruit & Vegetable-Friendly’ on the fruit and vegetable (F&V) consumption of adolescents.Design:A quasi-experimental study. The ‘Fruit & Vegetable-Friendly’, a multicomponent intervention based on the Transtheoretical Model, was completed in eight weeks. The data were collected one week before the intervention, one week after the completion of the intervention and six months after the post-test with an F&V intake questionnaire and the stages of change, processes of change (α = 0·91), situational self-efficacy (α = 0·91) and decisional balance (α = 0·90 for pros, α = 0·87 for cons) scales. Data were analysed with the Friedman, Wilcoxon and marginal homogeneity tests.Setting:A public secondary school in Istanbul, Turkey.Participants:Seven hundred and two adolescents.Results:The mean (sd) F&V intake of adolescents in the passive stages rose from the daily average at the time of the pre-test of 3·40 (1·79) portions to 5·45 (2·54) portions on the post-test and to 5·75 (2·70) portions on the follow-up test (P < 0·01). While the students in the passive stages represented 41·6 % of the participants prior to the programme, this rate fell to 23·7 % at the post-test and to 22·7 % at the follow-up. Students in the active stages first represented 58·5 % of the participants; this rate rose to 76·4 % at the post-test and to 77·2 % at the follow-up test.Conclusions:The programme was effective in increasing the amount of F&V the adolescents consumed on a daily basis.

Abstract

This study was conducted in order to determine the prevalence of sexual dysfunction and factors affecting sexual dysfunction (SD) in male patients with diabetes. Diabetes is a public health problem. Erectile dysfunction (ED) is seen in nearly half of all males who have had diabetes for more than 10 years. Erectile dysfunction, metabolic and psychological factors are involved in the emergence. A descriptive was conducted and questionnaire was used in this study. Data was gathered via general knowledge form which consists of socio-demographic characteristics and risk factors, International Index of Erectile Function, Quality of Life Scale (SF-36), Beck Depression Inventory, Coopersmith Self-Esteem Scale, and Body Image Scale. SD was determined in 48 % of our cases. The most affected domain among the cases was erectile function (33.1 %) and intercourse satisfaction (29.1 %), and the least affected domain was sexual desire (6.3 %). Most of the male cases with ED had mild and moderate ED (42.6 %). Factors affecting SD’s included age, education level, obesity, hypertension, retinopathy, vasculopathy, nephropathy, diabetic foot, HBA1C and LDL levels, diabetes duration, drugs used. All quality of life subgroups scores were lower in male patients with SD than patients without SD except for pain. Depression and body image scores were lower in male patients with SD. SD’s are affected by both metabolic and psychosocial factors. Sexual dysfunction in men with diabetes should be routinely evaluated.

WOS: 000386954300001, PubMed: 27809934, Background: The aim of this study was to assess health-related quality of life (HRQOL) among chronic hepatitis B (CHB) patients in Turkey and to study related factors. Methods: This multicenter study was carried out between January 01 and April 15, 2015 in Turkey in 57 centers. Adults were enrolled and studied in three groups. Group 1: Inactive HBsAg carriers, Group 2: CHB patients receiving antiviral therapy, Group 3: CHB patients who were neither receiving antiviral therapy nor were inactive HBsAg carriers. Study data was collected by face-to-face interviews using a standardized questionnaire, Short Form-36 (SF-36) and Hepatitis B Quality of Life (HBQOL). Values equivalent to p < 0.05 in analyses were accepted as statistically significant. Results: Four thousand two hundred fifty-seven patients with CHB were included in the study. Two thousand five hundred fifty-nine (60.1 %) of the patients were males. Groups 1, 2 and 3, consisted of 1529 (35.9 %), 1721 (40.4 %) and 1007 (23.7 %) patients, respectively. The highest value of HRQOL was found in inactive HBsAg carriers. We found that total HBQOL score increased when antiviral treatment was used. However, HRQOL of CHB patients varied according to their socio-demographic properties. Regarding total HBQOL score, a higher significant level of HRQOL was determined in inactive HBV patients when matched controls with the associated factors were provided. Conclusions: The HRQOL score of CHB patients was higher than expected and it can be worsen when the disease becomes active. Use of an antiviral therapy can contribute to increasing HRQOL of patients.

Abstract

Acute respiratory distress syndrome (ARDS) is a critical, life-threatening condition involving both lungs, characterized by capillary endothelial damage, diffuse pulmonary infiltration, and oxygen-resistant hypoxemia. The incidence in intensive care units is between 7.1-12.5% and its mortality can be up to 40% in severe cases. Pathological processes with exudative, proliferative and fibrotic stages in the lungs in response to different etiological factors in ARDS result in hypoxemia, hypercapnia and decreased lung compliance. There is no drug with proven efficacy in treatment. The most crucial parameter of ARDS management is protective mechanical ventilation, including low tidal volume, high positive end-expiratory pressure regulations, prone position, and recruitment maneuvering. In addition, supportive approaches such as fluid management, nutritional support, reduction of oxygen consumption, prevention of ventilator-associated pneumonia, pain management, deep vein thrombosis prophylaxis, peptic ulcer prophylaxis, blood sugar regulation, and maintaining skin/tissue integrity are applied. This review will briefly describe ARDS and related factors, then focus on treatment, care, and patient follow-up from the physician and nurse perspective.
