This study employed mixed methods with a participatory action research approach to explore factors currently undermining the conduction of research and to develop strategies to boost research productivity. A questionnaire was distributed to 64 staff members of the Department of Anesthesiology at a university-based hospital. Thirty-nine staff members (60.9%) gave informed consent and responses. Staff views were also collected through focus group discussions. The staff reported that limited research methodology skills, time management, and complex managerial processes were the limitations. Age, attitudes, and performance expectancy were significantly correlated with research productivity. A regression analysis demonstrated that age and performance expectancy significantly influenced research productivity. A Business Model Canvas (BMC) was implemented to gain insight into the goal of enhancing the conduct of research. Business Model Innovation (BMI) established a strategy to improve research productivity. The concept, comprising personal reinforcement (P), aiding systems (A), and a lifting-up of the value of research (L), the PAL concept, was considered key to enhancing the conduct of research, with the BMC providing details and integrating with the BMI. To upgrade the research performance, the involvement of management is imperative, and future action will involve the implementation of a BMI model to increase research productivity.

Abstract Background The time between discharge from hospital and transition to community and home is a critical period for health status among patients with a mental illness, including patients with schizophrenia. This study aimed to investigate crucial patient factors (patient-level) and hospital factors (hospital-level) affecting health status and see whether patient factor effects on health status vary with hospital factors, 30 days after hospital discharge. Methods This is a prospective study of 1255 patients with schizophrenia and their primary caregivers from 13 public mental hospitals across Thailand. Logistic regression and multi-level logistic regression was used to investigate the effects of patient and hospital factors simultaneously on health status, 30 days after hospital discharge. Results The intraclass correlation coefficient indicated that 14% of the change in health status was explained by the differences between hospital. Poor health status was identified in 14.26% of patients, 30 days after hospital discharge. The majority of participant patients were male (69.8%), single (71.87%), and the average age was 38.09 (SD = 9.74). The finding also showed that the patient factors; being female (ORadj .53, 95%CI .31,.92), perceived moderate and high levels of positive aspect of caregiving (ORadj .24, 95%CI .14,.42 and ORadj .05, 95%CI .02,.09), perceived readiness for hospital discharge (ORadj .21, 95%CI .13,.33), partial and full adherence to treatment (ORadj .24, 95%CI .14,.42 and ORadj .31, 95%CI .20,.47) showed a reduced likelihood of developing poor health status except substance use (ORadj 1.55, 95%CI .98, 2.44). Hospital factors; discharge planning process and nurse-patient ratio (ORadj 1.64, 95%CI 1.17, 2.30 and ORadj 1.16, 95%CI 1.09, 1.22) showed an increased likelihood of developing poor health status, 30 days after hospital discharge. Conclusions Findings provide relevant information on how both patient and hospital factors determine health status. These results might lead to better targeting of mental health service policy and enable more precise information gathering and allocation of resources. However, future research should be more focused and continue investigating the pathways through which hospital factors influence health status post-discharge.

Purpose – Although health-related quality of life (HRQOL) has become an important outcome, specifically in regard to the impact of illness and treatment in patients with depression, few studies have explored the HRQOL of patients from different types of hospitals. This study aimed at examining a change in HRQOL of patients from various types of hospitals Design/methodology/approach – A repeated measure was used in this study. Thirty participants in psychiatric outpatient units per center from the different types of hospitals, including a psychiatric hospital, regional hospital, general hospital and community hospital, were assessed with the Thai version of the World Health Organization Quality of Life Brief (WHOQOL-BREF-THAI) questionnaire at the first visit, and after the 6th and 12th weeks of the treatment course. Findings – The HRQOL scores for the participants were increased in each type of hospital from their first visit to the 6th week and 12th week (p < 0.001; except for the 6th week in the regional hospital, p < 0.01). Originality/value – The findings reflected HRQOL in patients with depression in terms of the resources available in different types of hospitals that could be used as baseline data for the development of Thai mental health service systems.

Abstract Background Mood swings (MS) are a widely discussed psychiatric ailment of youthful patients. However, there is a lack of research about MS in this population. Methods A school-based, cross-sectional study was conducted to investigate the prevalence and distribution pattern of mood swings due to personal and contextual determinants in Thai adolescents in the central region of Thailand. Participants were 2598 students in high schools and vocational schools in Bangkok and three provinces in the central region of Thailand. Results The prevalence of mood swings was 26.4%. It was highest among vocational students in Bangkok at 37.1%. MS were more common in adolescents who exhibited risk behaviors and who resided in hazardous situations. The probabilities of MS by characteristic in 15–24 years olds were: bullying involvement 36.9% (n = 1293), problematic social media use 55.9%(n = 127), high expressed emotion in family 36.6% (n = 1256), and studying in a vocational program 29.5% (n = 1216) and school located in Bangkok 32.4% (n = 561). Also, substance use was a risk for MS with cannabis use at 41.8%(n = 55) and heroin use at 48.0% (n = 25). Objective: To investigate the relationship of negative life events, rumination, learned resourcefulness, and currently psychiatric problems among incarcerated juveniles with a history of substance abuse. Material and Methods: Samples aged between 15 and 18 years who had a history of substance abuse at the Ban Karuna Juvenile Vocational Training Center for Boys. The data were collected by using 5 sets of research instruments: 1) demographic data questionnaires, 2) The Negative Event Scale (NES), 3) Ruminative Response Scale (RRS), 4) The Self-Control Schedule (SCS), and 5) Mini International Neuropsychiatric Interview (M.I.N.I.) Thai version. Data were analyzed by descriptive statistics and logistic regression analysis. Results: One hundred and ten juveniles completed much of the questionnaire. The mean age of juveniles was 16.92 years (S.D.=1.02). The mean age of the first time of substance abuse was 14.25 years (S.D.=1.68). The result demonstrated that 23% of variance of antisocial personality disorders was explained by learned resourcefulness (LR). While LR decreases, the likelihood of being antisocial personality disorders increases approximately 6% (OR=0.94, 95%CI=0.91-0.97, p=0.00). Additionally, approximately 22% and 9% of variance of anxiety and depressive disorders were explained by rumination. It was found that the likelihood of being anxiety disorder and depressed increase 9% (OR= 1.09, 95%CI=1.03-1.17, p=0.01) and 7% (OR=1.07, 95%CI=1.01-1.13, p=0.02), respectively. Conclusion: Based on these findings, promoting LR may decrease the risk of antisocial personality disorders, while increasing rumination may also increase the risk of depression and anxiety disorders. In other words, teaching LR and ways of thinking should be addressed in order to decrease mental health risks or likelihood of psychiatric problems.

This study explored the lived experiences of adolescents residing in a slum area in Thailand who had dropped out of school. Giorgi's phenomenological methodology was used, and data gathered through focus group discussion and semi-structured interviews with 14 participants. Data were collected from July 2011 to June 2012. This study illuminated the lived experiences of the adolescents who engaged in health-risk behaviors. Three phenomenological themes emerged: encountering life adversity, my health-risk behaviors, and bounced back effects. The participants explained their attitudes to the factors that lead them to health risk behaviors or encountering life adversity meant living with at-risk families and being forced into premature adulthood, which were all self-reported health-risk behaviors. They indicated that they had an ability to bounce back from the problems related to these behaviors. In order to encourage and promote health among adolescents who drop out from school, nurses have to concentrate on assessment and care planning of adolescents' risk behaviors and the determinants to these, including adolescents' friends and families. This study aimed to determine, based on Pearlin's Stress Process Model, factors (i.e. behavioral problems and activities of daily living dependency of individuals with dementia, caregiver burden, and coping strategies of the caregiver) that predicted depression among caregivers of persons with dementia. Using stratified sampling, 226 caregivers of persons with dementia were recruited from three memory clinics in metropolitan Bangkok, Thailand. Data were collected via seven self-report questionnaires, including: a demographic data questionnaire; a revised Memory and Behavioral Problems Checklist; a modified Barthel Activities of Daily Living Index; the Zarit Burden Interview; the Jalowiec Coping Scale; and, the Center for Epidemiologic Studies Depression Scale. Data were analyzed via descriptive statistics and path analysis. The results revealed the modified model fit the data well ( X2 = 0.990; df = 1; p = 0.443; RMSEA < 0.001; GFI = 0.998; AGFI = 0.974) and explained 51.4% of variation in caregiver depression. Behavioral problems of the persons with dementia were found to have a significant indirect effect on caregivers' depression through caregiver burden, while activities of daily living dependency of the individuals with dementia was found to have a significant indirect effect on caregivers' depression through caregiver burden. Behavioral problems and activities of daily living dependency of the individuals with dementia also had a significant direct effect on caregiver burden, while caregiver burden was found to have a positive direct effect on caregivers' depression. The findings may be helpful with development of an effective nursing intervention program for promoting good mental health and decreasing depression among persons with dementia. [ABSTRACT FROM AUTHOR]
