Abstract

OBIETTIVI I Centri di Radioterapia UPMC San Pietro FBF di Roma (CC#1) e UPMC Villa Maria di Mirabella Eclano (CC#2) hanno condotto uno studio con l���obiettivo di analizzare le variazioni avvenute durante la pandemia sui workload e i workflow di reparto. �� stato, inoltre, ricercato l���eventuale nesso tra queste e l���introduzione di nuove procedure per la prevenzione e il contenimento del contagio da Covid-19. MATERIALI E METODI I dati utilizzati sono stati ricavati da reports presenti nel sistema ARIA (V.15.1 Varian Medical System, CA, Palo Alto, USA). Per esaminare i workload �� stato utilizzato il Downtime, un indicatore che quantifica direttamente l'inattivit�� del reparto, ricavato dal rapporto tra il tempo di standby giornaliero dei LINAC (TrueBeam STx��, Varian Medical System, CA, Palo Alto, USA) e la media di trattamenti giornalieri effettuati. Per esaminare workflow ed eventuali ritardi tra le attivit��, sono stati valutati gli intervalli di tempo tra una terapia e la successiva (Intervalli di terapia). RISULTATI Il Downtime nel CC#1 ha sub��to un leggero aumento del valore medio dal 3.1% del 2019 al 3.8% del 2020, tuttavia l���analisi mensile mostra consistenti picchi di riduzione (marzo-aprile-maggio) e di incremento (novembre-dicembre). Per il CC#2 il trend del Downtime nel 2020 �� abbastanza regolare (valore medio del 3,3%), con un incremento durante la prima ondata della pandemia. Gli Intervalli di terapia di ���5-10 min��� nel CC#1, analizzati confrontando il trimestre marzo-aprile-maggio 2020 col 2019, risultano maggiori per i primi mesi e ridotti a maggio; quelli di ���10-15��� min risultano stabili; quelli ���>20 min��� sono leggermente aumentati a marzo 2020. Per il CC#2 il trend nel 2020 decresce nei mesi di maggiore emergenza sanitaria e incrementa nei mesi estivi. CONCLUSIONI Il fatto che i trend degli indicatori utilizzati abbiano dei picchi esclusivamente in corrispondenza dei periodi di maggiore emergenza sanitaria, �� indice di un certo impatto ��� sia in termini di workload che di workflow ��� della pandemia, ma anche della capacit�� del personale di adattarsi in breve tempo alle nuove procedure da eseguire, senza inficiare sul rendimento generale dei Centri., Journal of Biomedical Practitioners, V. 5 N. 2 (2021)

Abstract

PurposeImmunotherapy has shown activity in patients with brain metastases (BM) and leptomeningeal disease (LMD). We have evaluated LMD and intraparenchymal control rates for patients with resected BM receiving postoperative stereotactic radiosurgery (SRS) and immunotherapy or postoperative SRS alone. We hypothesize that postoperative SRS and immunotherapy will result in a lower rate of LMD with acceptable toxicity compared with postoperative SRS.Patients and methodsOne hundred and twenty-nine patients with non-small-cell lung cancer (NSCLC) and melanoma BM who received postoperative fractionated SRS (fSRS; 3×9 Gy) in combination with immunotherapy or postoperative fSRS alone for completely resected BM were retrospectively evaluated. The primary endpoint of the study was the rate of LMD after treatments. The secondary endpoints were local failure, distant brain parenchymal failure (DBF), overall survival (OS), and treatment-related toxicity.ResultsSixty-three patients received postoperative SRS and immunotherapy, either nivolumab or pembrolizumab, and 66 patients received postoperative SRS alone to the resection cavity. With a median follow-up of 15 months, LMD occurred in 19 patients: fSRS group, 14; fSRS and immunotherapy, 5. The 12-month LMD cumulative rates were 22% (95% CI 14% to 37%) in the fSRS group and 6% (95% CI 2% to 17%) in the combined treatment group (p=0.007). Resection cavity control was similar between the groups, whereas DBF and OS were significantly different; the 1-year DBF rates were 31% (95% CI 20% to 46%) in the fSRS and immunotherapy group and 52% (95% CI 39% to 68%) in the fSRS group; respective OS rates were 78% (95% CI 67% to 88%) and 58.7% (95% CI 47% to 70%). Twenty-two patients undergoing postoperative fSRS and immunotherapy and nine subjected to postoperative fSRS experienced treatment-related imaging changes suggestive of radiation-induced brain necrosis (p=0.02).ConclusionsPostoperative fSRS in combination with immunotherapy decreases the incidence of LMD and DBF in patients with resected BM from NSCLC and melanoma as compared with fSRS alone, reducing the rate of neurological death and prolonging survival.

Abstract

A thermodynamic study using differential scanning calorimetry of the reconstitution of H1-deficient calf thymus chromatin suggests that H1 removal causes a thermal transition corresponding to chromatin decondensation. Binding of H1 regains the thermal profile of the native chromatin. H1 binds at independent sites containing 180 plus or minus 10 bp. H1 inhibits repulsive electrostatic forces and is crucial for the spatial localization of nucleosomes during initial condensation.

22. Solvent regeneration of activated carbon mixed-matrix membranes for the removal of micropollutants from wastewater

Abstract

The constantly increasing demand of clean water has become challenging to deal with over the past years, water being an ever more precious resource. In recent times, the existing wastewater treatments had to be integrated with new steps, due to the detection of so-called organic micropollutants (OMPs). These compounds have been shown to adversely affect the environment and possibly human health, even when found in very low concentrations. In order to remove OMPs from wastewater, one possible technique is a hybrid process combining filtration and adsorption. In this work, polyethersulfone multi-channel mixed-matrix membranes with embedded powdered activated carbon (PAC) were tested to investigate the membrane’s adsorption and desorption performance. Micropollutants retention was analyzed using the pharmaceutical compounds diclofenac (DCF), paracetamol (PARA) and carbamazepine (CBZ) in filtration mode, combining the PAC adsorption process with the membrane’s ultrafiltration. Desorption performance was studied through solvent regeneration, using seven different solvents: pure water, pure ethanol, mixture of ethanol and water in different concentration, sodium hydroxide and a mixture of ethanol and sodium hydroxide. Regeneration experiments were carried out in forward-flushing. At first regeneration efficiency was investigated using a single-solute solution (diclofenac in water). The mixture Ethanol/Water (50:50) was found to be the most efficient with long-term retention of 59% after one desorption cycle. It was, therefore, later tested on a membrane previously loaded with a multi-solute solution. Three desorption cycles were performed after which, retention (after 30 min) reached values of 87% for PARA and 72% for CBZ and 55% for DCF, which indicates decent regenerability. A morphological analysis on the membranes confirmed that, in any case, the regeneration cycles did not affect either the membranes’ structure, or the content and distribution of PAC in the matrix.

Abstract

Objective: In amino acid positron emission tomography brain tumour imaging, tumour-to-background uptake parameters are often used for treatment monitoring. We studied the effects of patients' characteristics and anticancer treatments on 18 F-fluoro-l-phenylalanine uptake of normal brain and tumour lesions, with particular emphasis on temozolomide (TMZ) chemotherapy., Methods: 155 studies from 120 patients with glioma were analysed. Average uptake of normal background (standardized uptake value, SUVbckgr) and basal ganglia (SUVbg), as well as tumour-to-brain ratios (TBR) were compared between positron emission tomography/CT studies acquired before (Group A, n = 48), after (Group B, n = 50) or during (Group C, n = 57) TMZ treatment, using analysis of variance., Results: Overall, mean SUVbckgr and mean SUVbg were 1.06 ± 0.26 and 2.12 ± 0.47, respectively. Female had significantly higher SUVbckgr (p = 0.002) and SUVbg (p = 0.012) than male patients. Age showed a positive correlation with SUVbg (p = 0.001). In the overall cohort, there were significant effects of TMZ on SUVbckgr (p = 0.0237) and TBR (p = 0.0138). In particular, SUVbckgr was lower in Group C than in Group B (1.00 ± 0.25 vs 1.14 ± 0.31, p = 0.0173). Significant variations of SUVbckr could be observed in female only. TBR was significantly higher in Group C than in Group B (2.37 ± 0.54 vs 2.06 ± 0.38, p = 0.010). Variations of SUVbg between groups slightly missed significance (p = 0.0504)., Conclusion: Temozolomide chemotherapy and patients' characteristics, including gender and age, affect physiological [ 18 F]-fluoro-l-phenylalanine uptake and, consequently, the calculation of TBRs. Advances in knowledge: For the first time, the effects of past or concurrent temozolomide chemotherapy on brain physiological amino acid uptake have been investigated. Such effects are relevant and should be taken into account when evaluating tumour-to-background ratios.

Abstract

Background/aim: The aim of this study was to investigate the treatment outcomes and toxicities in patients with liver disease treated by Stereotactic Body Radiation Therapy (SBRT)., Patients and Methods: From 2007 to 2016, 43 patients with 58 lesions (6 primary and 37 metastatic liver tumors) were treated with SBRT., Results: Local Control was reached in 47 out of 58 (81%) treated lesions with 12 and 24-month rates of 81% and 74% respectively. The progression-free survival at 12 and 24 months was 42% and 36%, respectively. The disease specific survival at 12 and 24 months was 74% and 46% respectively. Median overall survival (OS) was 20 months and the rates of OS were 74% and 46% at 12 and 24 months respectively. 26. Stereotactic Body Radiation Therapy for Liver Lesions. A Single-institution Experience.

Abstract

Aim: To evaluate survival and toxicity in a cohort of patients treated with stereotactic body radiation therapy (SBRT) for unresectable intrahepatic malignancies., Patients and Methods: From 2007 to 2014, 23 patients with 34 lesions (three primary and 31 metastatic liver tumors) were treated with SBRT., Results: The median follow-up was 9 months (range=1-76) for all patients. Local control was reached in 27 out of 34 (79%) treated lesions, with 1 and 2 years rates of 93% and 73%, respectively. The progression-free survival at 1-year and 2-year was 50% and 25%, respectively. Median overall survival was 16 months (95% confidence interval=8-24 months), with 1-year and 2-year rates of 58% and 41%, respectively.
